A transcranial magnetic stimulation (TMS) session is an outpatient treatment: you sit or recline, remain awake, and receive magnetic pulses through a coil positioned against your scalp. The first appointment commonly includes mapping to identify the treatment location and stimulation intensity; later visits generally use the recorded position and prescribed settings. You may hear clicking and feel tapping, and the clinic should provide hearing protection. The exact visit length and schedule depend on your treatment protocol and clinic.

If you are wondering, “What does TMS treatment look like?”, the short answer is that the operator positions the coil, delivers a series of pulses, and follows the treatment plan established for you. Mapping, sensations, timing, and preparation can vary by device, condition, protocol, and clinic, so use your treating team’s instructions for your own appointments.

What happens during a TMS session?

TMS uses a magnetic coil placed against the scalp to deliver pulses. You stay awake while an operator positions the coil and administers treatment. It is generally performed without anesthesia or sedation, according to the American Psychiatric Association (APA). The procedure is outpatient, so you do not typically need to stay at the clinic after the session.

The experience is not identical for everyone: the device, condition being treated, stimulation protocol, and clinic procedures can affect how a visit is conducted. Ask your clinic for its specific instructions rather than assuming every TMS appointment follows the same timing or sequence. In particular, distinguish the first appointment—which commonly includes mapping—from routine follow-up visits.

At a high level, expect to be seated or reclined, have the coil positioned against your scalp, and hear the device click as pulses are delivered. You remain awake during the procedure. The operator uses the prescribed settings and positioning for your treatment; if you have questions or discomfort, follow the clinic’s directions for communicating with the treatment team.

What happens at the first appointment?

Screening and setup

Before treatment, a clinician evaluates whether TMS is appropriate and reviews your medical history. Be ready to disclose implants, implanted or active medical devices, seizure or epilepsy history, medication changes, and other relevant health changes. The APA notes that some head implants, aneurysm clips or coils, cochlear implants, cardiac defibrillators, and epilepsy history may affect eligibility. FDA guidance also identifies possible interactions between pulsed repetitive TMS (rTMS) and pacemakers or other active implanted devices.

These considerations do not determine eligibility by themselves: screening is individual, and the treating clinician should assess your circumstances. Do not decide you are eligible—or ineligible—based only on a general list. Mention the precise device or medical history in question and ask the clinician to explain how it affects your treatment plan. The FDA also discusses seizure precautions and patient labeling that explains warnings and risks, so ask the team to review the information relevant to your device and protocol.

The operator will help you sit or recline and position the coil against your scalp. Cleveland Clinic advises removing metal items and describes the use of hearing protection and test pulses. Follow the clinic’s preparation directions, including any instructions about personal items. If you are unsure whether a particular item should be removed, ask rather than guessing.

Mapping the treatment position and intensity

The first session commonly includes mapping. The operator uses test pulses and may adjust or move the coil to establish a treatment location and motor threshold—the stimulation intensity reference used to guide treatment. During this process, a brief twitch in a hand or finger can occur. The twitch is a possible part of mapping, not something to assume will happen throughout every routine treatment.

Mayo Clinic estimates that a first appointment typically takes about an hour; that is its general estimate, not a guaranteed duration for every clinic or protocol. Mapping is one reason the first appointment may differ from later sessions. Ask whether mapping is scheduled as part of your first treatment appointment or handled at a separate visit, and how much time to allow. Avoid using one clinic’s estimate as a promise about your own appointment.

Mapping establishes the treatment location and intensity reference that guide the prescribed treatment. The operator can record the location and settings so they can be used in subsequent visits. Ask what parts of the process will occur at your initial appointment, whether you should allow extra time for it, and whom to tell if the test pulses are uncomfortable.

What happens during a routine follow-up session?

At a later visit, the operator generally returns the coil to the recorded treatment position and applies the prescribed settings. FDA guidance says motor threshold and treatment location are not typically repeated at every session. This means follow-up appointments generally do not repeat the full initial mapping process, although the clinic may reassess under some protocols.

Reassessment may be appropriate after changes in medication or health. Tell the clinic if something relevant has changed rather than assuming previously recorded settings must remain unchanged. Your clinician can decide whether the treatment plan or measurements need review. The same principle applies if you have a new concern about an implant, device, or seizure history: raise it with the treatment team.

The coil delivers pulses while held against the scalp. You remain awake, and the operator can monitor the session. Follow the clinic’s instructions if you need to report discomfort or ask for an adjustment. Routine treatment is not a single universal sequence or duration: the prescribed protocol and device determine what the clinic does and how long it takes.

What does TMS feel and sound like?

  • Clicking: The device can make loud clicking sounds as it delivers pulses. Hearing protection is important even if you are focused on the sensation at the scalp.
  • Tapping: You may feel a tapping sensation on the scalp where the coil is positioned. The sensation is associated with the coil’s placement and pulses.
  • Brief twitch during mapping: Test pulses used to establish motor threshold can cause a short hand or finger twitch.
  • Discomfort or headache: Scalp discomfort and mild headache are commonly described. FDA guidance also identifies headache and pain at the application site among common adverse events.

Experiences vary, so do not assume that every person will feel the same thing or that treatment will be painless. Tell the operator if you have discomfort and ask what the clinic recommends if a headache or application-site pain occurs during or after treatment. FDA guidance discusses seizure precautions as well as common adverse events; ask your clinician to explain the risks and warnings that apply to your particular treatment.

Hearing protection is part of safe session practice. FDA device guidance states, “Subjects should be required to wear earplugs during the treatment sessions.” Your clinic should explain what protection it supplies and how to use it. Do not assume that treatment will be painless or that the sound is too quiet to require protection. Mayo Clinic describes patients receiving earplugs, but confirm with your own clinic what is provided. Buying personal earplugs is not necessarily required if the clinic supplies suitable protection.

How long are visits, and how many treatments are there?

There is no single visit duration or course schedule that applies to every TMS protocol. The first appointment may take longer if it includes mapping. Mayo Clinic’s estimate of about an hour applies to its description of a first appointment, not a universal appointment length. Routine-visit duration depends on the protocol and clinic; ask for the expected time for your own treatment, including whether the time quoted includes setup or other appointment steps.

The APA describes a standard course as five treatments per week for four to six weeks. FDA guidance describes a course as traditionally involving multiple treatments. Shorter-session and accelerated regimens also exist, so the schedule your clinic prescribes is the relevant one. Do not assume that the standard schedule applies to every device, condition, or treatment plan, and do not infer a total course length from a single visit estimate.

When comparing schedules or planning your calendar, ask about several separate details: time per visit, treatments per day or week, total course length, the condition and device being treated, and whether the first appointment includes mapping. A course with frequent visits can involve a different weekly routine than a shorter-session or accelerated regimen. Your clinic can explain which schedule it is recommending and what to expect for both the first appointment and later visits.

How should you prepare?

  1. Share relevant health information. Tell the clinician about implants, pacemakers or other active implanted devices, seizure or epilepsy history, medication changes, and other changes in your health. The APA and FDA identify some of these histories and devices as important to screening. Ask how any specific condition or device affects your treatment plan; general information cannot determine your personal eligibility.
  2. Follow the clinic’s instructions about metal. Cleveland Clinic advises removing metal items. Ask what to remove or leave outside the treatment area rather than relying on a general checklist. Bring up any item or implant you are unsure about before treatment.
  3. Ask about hearing protection. Confirm that the clinic provides earplugs or other protection and explains how to wear it during treatment. FDA guidance calls for earplugs during treatment sessions, so do not skip protection because the clicking seems manageable.
  4. Confirm timing and logistics. Ask how long mapping and routine visits are expected to take, whether mapping occurs at your first appointment, and whether the clinic has any additional preparation instructions. If you are arranging transportation or work around visits, confirm the clinic’s own appointment estimate and schedule rather than relying on a general example.

These steps do not replace the clinic’s screening. FDA patient labeling and guidance discuss warnings and risks, so ask the treating team to explain the information that applies to your device and protocol. Tell the clinic about relevant changes throughout the treatment course, not only at the first appointment.

What should you expect after the appointment?

Because TMS generally does not require anesthesia or sedation, the APA says patients can drive to and from appointments and continue most usual activities. That is general guidance, not a guarantee for every person or appointment. Follow your clinic’s specific advice, particularly if your first visit includes additional assessments or you have individual concerns about driving. If you are uncertain, ask the clinician rather than treating the general guidance as an individual clearance.

Ask what to do if you notice a headache or scalp discomfort after leaving, and how to contact the clinic with questions about symptoms or a change in health. FDA guidance identifies headache and application-site pain among common adverse events, but your treating team is the right source for advice about your specific experience and whether follow-up is needed.

How often will I have TMS treatments?

The APA describes a standard course as five treatments per week for four to six weeks. Protocols vary, and shorter-session or accelerated regimens exist, so follow the schedule prescribed by your clinic.

Can I drive after TMS?

The APA says patients can drive to and from appointments because TMS generally does not require anesthesia or sedation. This is general guidance, not an individual guarantee; follow your clinic’s advice, especially if your first visit includes other assessments.

What medical information should I tell the clinic?

Disclose implants, pacemakers or other active implanted devices, seizure or epilepsy history, medication changes, and other relevant health changes. Screening is individualized, so ask the treating clinician how your specific history affects eligibility and treatment.

The Bottom Line

A TMS visit usually involves staying awake while an operator positions a coil against your scalp and delivers pulses. The first appointment commonly includes mapping; later sessions generally use recorded positioning and prescribed settings. Expect clicking and possible tapping, wear the hearing protection the clinic provides, and ask about your own visit length, course schedule, and preparation. Disclose relevant devices, implants, seizure history, and health or medication changes to your clinician.
