During an inflammatory bowel disease (IBD) flare, there is no universal food list. Many people find that fluids, small frequent meals, adequate protein, and soft or well-cooked foods are easier to manage, but your symptoms, diagnosis, medications, and any bowel narrowing should determine what you try. These measures can support hydration and nutrition; they do not treat intestinal inflammation or replace advice from your IBD team.

Start with the goals: fluids, energy and protein

A flare can involve diarrhea, urgency, abdominal pain, nausea, bleeding, fatigue and reduced appetite. Those symptoms can make it difficult to eat and drink enough even when your body needs more support. Build meals around three priorities rather than around a rigid “flare diet.”

  • Fluids: Replace water and electrolytes lost through frequent diarrhea or bleeding.
  • Energy: Get enough calories to avoid unintended weight loss and help your body cope with active disease.
  • Protein: Include a tolerable protein source regularly; protein needs may rise during active disease.

Crohn’s & Colitis Foundation guidance recommends nutrient-dense meals and snacks, while Crohn’s & Colitis UK advises seeking dietitian input if symptoms or food restrictions make adequate intake difficult. An IBD-focused registered dietitian can adapt portions and textures to your symptoms, laboratory results, medications and cultural diet.

Fluids and oral rehydration when diarrhea is heavy

Take frequent sips rather than waiting until you feel thirsty. Water, broth, milk or a tolerated non-dairy drink can all contribute to intake. Soups and smoothies may be easier than a large glass of liquid when appetite is poor. If a drink clearly increases diarrhea, urgency or pain, stop it and discuss the pattern with your care team.

Crohn’s & Colitis UK notes that an oral rehydration solution (ORS) may help when substantial fluid is lost through diarrhea or bleeding. ORS supplies a measured combination of water and salts; it is fluid-replacement support, not a treatment for Crohn’s disease or ulcerative colitis. Follow the packet’s mixing instructions exactly and ask a pharmacist or clinician which product is appropriate for you, particularly if you have kidney disease, heart failure, diabetes or a prescribed fluid restriction.

Contact your IBD team if you cannot keep fluids down, diarrhea or bleeding is substantial or persistent, or you are becoming weak, dizzy or unable to maintain intake. Do not use drinks marketed as “detox,” laxative teas or very concentrated homemade mixtures as substitutes for a correctly prepared ORS.

Protein and calorie-dense choices when appetite is low

Small portions every few hours can be more realistic than three full meals. Add energy to foods you already tolerate instead of forcing a large plate. The examples below are options to test, not requirements.

Food group Examples to try if tolerated Ways to make intake easier
Eggs and poultry Soft scrambled eggs, omelet, tender chicken or turkey Choose moist preparations; shred meat and combine with rice, potatoes or soup.
Fish Poached, baked or steamed fish Use mild seasoning and a soft side dish; avoid tough, dry pieces.
Dairy or alternatives Yogurt, milk, lactose-free milk or a fortified alternative Select smooth products without seeds or large fruit pieces if texture is bothersome.
Plant proteins Tofu, smooth hummus, beans or pulses if tolerated Blend or puree; stop if the portion increases gas, pain or diarrhea.
Nut products Smooth nut butter Stir a small amount into oatmeal, a smoothie or yogurt rather than eating a large spoonful alone.

When chewing or digestion feels difficult, a smoothie, blended soup or yogurt may be more manageable than an equivalent solid meal. A smoothie should still contain a protein source and enough energy; fruit juice alone does not provide a balanced replacement. If dairy seems to worsen symptoms, try a lactose-free option or another fortified alternative and ask your clinician or dietitian how to maintain calcium and vitamin D intake.

Adjust texture before eliminating whole food groups

Texture, portion size and preparation can change tolerability without permanently narrowing your diet. Some people report that peeled, well-cooked vegetables and soft fruits such as bananas or melons pass more comfortably during a flare. Cooking breaks down plant structure, while peeling removes some insoluble material. These changes may reduce the work of chewing and digestion, but they do not make a food universally safe or reduce inflammation.

If a food feels difficult Try this preparation What to monitor
Raw vegetables Peel, remove tough parts and cook until soft; puree into soup if desired. Urgency, cramping, bloating or increased stool frequency.
Firm fruit Use ripe banana or melon; peel, cook or blend fruit. Pain, loose stools and whether seeds or skins appear to irritate you.
Large solid meals Divide the same food into smaller meals or snacks. Total intake over the day, not just the size of one meal.
Dry or tough protein Choose minced, shredded, poached or slow-cooked versions with sauce or broth. Chewing difficulty, abdominal discomfort or a feeling that food is sticking.

Avoid assuming that “bland” means nutritionally adequate. White toast or plain crackers may be useful alongside other foods, but relying on them for days can leave you short of protein, calories and micronutrients.

Should you avoid fiber during a flare?

Fiber is not automatically harmful, and the right amount depends on your symptoms and whether a clinician has identified a narrowing. Crohn’s & Colitis UK states that there is no high-quality evidence that a low-residue diet reliably relieves flare symptoms. If you temporarily reduce rough or bulky foods because they worsen symptoms, the guidance recommends increasing fiber gradually as you improve rather than staying on a highly restricted plan indefinitely.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a symptom-led approach:

  • If a particular high-fiber food repeatedly causes pain, urgency or visible worsening, pause it and record the response.
  • Keep easier sources of energy and protein in the diet while you adjust texture or portion size.
  • When stools and appetite improve, reintroduce one food or texture at a time, starting with a small portion.
  • Ask your dietitian how to replace nutrients if several fiber-rich foods are excluded.

Do not start a restrictive protocol such as the specific-carbohydrate diet, CDED or CD-TREAT on your own as a flare cure. Crohn’s & Colitis UK describes evidence as insufficient for routine general recommendation, and restrictive eating can make nutritional adequacy harder.

When a stricture changes the advice

A bowel stricture is a narrowing that can make bulky or poorly chewed food more difficult to pass. If you have a known or suspected stricture, follow the individualized plan from your gastroenterologist and dietitian rather than general flare advice. Crohn’s & Colitis UK identifies fruit and vegetable skins, seeds or stalks, wholegrain foods, nuts and seeds, and tough or gristly meat as foods that may need particular attention for some people with a narrowing.

That list does not apply automatically to everyone with IBD. Do not use it to diagnose yourself or to impose a long-term low-fiber diet without supervision. New colicky pain, vomiting, marked bloating or inability to pass stool or gas warrants prompt medical advice, especially if a narrowing is known.

Identify personal triggers without creating an unnecessarily narrow diet

NIDDK states that researchers have not found specific foods that cause or worsen Crohn’s disease symptoms, and its ulcerative-colitis guidance makes the same point. This refers to population-level evidence about causing or worsening symptoms; it does not mean every food feels the same for every person. A food can be uncomfortable because of lactose intolerance, a large portion, caffeine, alcohol, an artificial sweetener or another individual factor without causing the underlying inflammation.

Crohn’s & Colitis UK notes that caffeine, alcohol and artificial sweeteners may worsen diarrhea for some people. Instead of banning all three permanently, keep a short record for several days:

  1. Write down the food or drink, approximate amount and preparation.
  2. Record stool frequency and consistency, pain, urgency, nausea and bleeding separately.
  3. Note medications, supplements, stress, sleep and whether the symptom was already worsening.
  4. Discuss a repeated pattern with your IBD team before making a major exclusion.

Change one variable at a time when possible. Otherwise, removing dairy, wheat, vegetables and several other foods together makes it impossible to know what helped and increases the risk of nutrient gaps.

A practical one-day framework

The following is an illustration of how to combine small portions, soft texture and protein. It is not a prescription or a universal menu; substitute foods that you know you tolerate.

Time Example Purpose
Breakfast Soft eggs with a tolerated starch, or yogurt blended with banana and smooth nut butter Protein and energy in a small volume
Mid-morning ORS when substantial losses are occurring, or another tolerated drink; add a small snack if able Replace fluid and salts; prevent a long gap without food
Lunch Blended chicken, fish or tofu soup with soft rice or potatoes Fluid, protein and a soft texture
Afternoon Smooth yogurt, fortified alternative or a small smoothie Additional calories and protein when appetite is limited
Dinner Tender fish, poultry or tofu with peeled, well-cooked vegetables and a tolerated starch Nutrient-dense meal without a large bulky portion
Evening A small snack or drink if your total intake is still low Spread intake across the day rather than forcing one large meal

Adjust the plan for diabetes, kidney disease, food allergies, pregnancy, tube feeding or other medical conditions with your clinician. If you cannot meet needs with ordinary foods, ask whether a medically appropriate oral nutrition supplement is suitable; choose one with professional guidance rather than assuming any commercial shake is appropriate.

Diet supports comfort; it does not control a flare by itself

The American Gastroenterological Association’s clinical practice update, published January 23, 2024, advises a Mediterranean-style pattern for general health when it is not medically contraindicated, but reports that no diet has consistently reduced adult IBD flare rates. That advice concerns long-term eating, not a requirement to eat raw salads or high-fiber grains while you are acutely unwell.

During a flare, prioritize foods and textures you can actually tolerate, then work toward greater variety as symptoms settle. Medical treatment remains essential. Do not stop, reduce or replace prescribed medication because a food change seems helpful.

When to contact your IBD team

Arrange clinical advice for worsening or persistent symptoms, difficulty maintaining fluids, substantial diarrhea or bleeding, unintended weight loss, a known stricture, or difficulty eating enough. A clinician may need to assess inflammation, infection, medication response, anemia, dehydration or obstruction; changing food alone cannot answer those questions.

Ask for a dietitian referral if you are repeatedly eliminating foods, losing weight, avoiding protein, relying on a very small menu, or unsure how to reintroduce fiber. Bring your food-and-symptom record, current medications and any recent weight change to the appointment.

Key takeaways for the next meal

  • Choose a tolerable drink and sip regularly; consider correctly prepared ORS when losses are substantial.
  • Eat a small protein-containing portion rather than waiting for appetite to return fully.
  • Use peeling, cooking, blending and smaller portions to change texture before excluding an entire food group.
  • Treat fiber reduction as a temporary, individualized adjustment—not a proven universal flare treatment—and reintroduce gradually when improving.
  • Follow a specialist plan if you have a stricture, ongoing bleeding, weight loss or difficulty meeting nutrition needs.
