If a Claude citation does not support the statement beside it, treat the statement as unverified—not as proven and not automatically as false. Open the cited page, find the exact passage, compare its meaning and limits with Claude’s wording, then qualify, replace, or remove the claim. For consequential decisions, check an authoritative source or qualified professional rather than using Claude as the sole authority.
Why a citation can look persuasive and still fail
A visible link is a route to evidence, not evidence by itself. Claude may cite a real page while overstating what that page says, combining several narrow facts into a broader conclusion, missing an exception, or assigning a quotation to a source that never used those words. Anthropic says Claude can produce incorrect or misleading responses because information may be out of date and because it can generate convincing but ungrounded quotations. Its Help Center specifically advises users working with web search to review Claude’s cited sources and the original websites, where important context may be omitted from the synthesis: Anthropic Help Center guidance.
Do not infer a universal failure rate from one mismatch. A citation can be relevant but incomplete, technically correct but out of date, or attached to a claim that requires a different source. The practical question is always: Does this passage support this exact proposition, for this scope, time period, and level of certainty?

The verification workflow
1. Open the destination, not just the preview
Follow the link and confirm the destination’s publisher, title, date, and document type. Check whether it is the original authority, a primary study, an official policy, or a secondary account. A search-result snippet, cached excerpt, or headline can omit definitions and conditions. If the link redirects, record the final page you actually inspected.

2. Find the passage Claude appears to use
Search the page for distinctive terms from the answer. The evidence might be a sentence, table row, figure, footnote, or a section that defines the relevant population or product. Read enough before and after the apparent match to capture exclusions, confidence intervals, dates, jurisdiction, and “may,” “up to,” or other limiting language. Anthropic warns that original websites can contain context or details left out of Claude’s web-search synthesis.
3. Split a compound answer into individual claims
Long answers often contain several propositions under one citation. Rewrite the response as a checklist of atomic claims. For example, “The rule applies nationwide, began in 2024, and carries a mandatory penalty” contains at least three claims. Each one may need a different passage or source. Do not let a citation that supports one clause lend authority to the others.
4. Compare claim and passage on five axes

These are practical editorial checks based on Anthropic’s instruction to inspect original sources, account for omitted context, and consider source quality; they are not presented as a formal Anthropic consumer audit standard.

- Supported: the passage states the same proposition with matching scope and qualifications.
- Partly supported or missing context: the passage supports only a narrower claim, or Claude omitted a material condition.
- Contradicted: the source says the opposite or expressly rules out Claude’s wording.
- Not established by this source: the page may be relevant, but the cited material does not establish the claim.
“Not established by this source” is not the same as “false.” The claim might be true but require another source. Preserve that distinction in notes, reports, and corrections.

6. Repair the answer
Rewrite the sentence to match what the source actually establishes. Add the missing qualification, narrow the population or date, substitute a stronger source, or delete the unsupported clause. Keep the original link only if it remains useful for the narrower statement. In a consequential workflow, save the source, the passage, Claude’s original wording, and your disposition so another reviewer can reproduce the decision.
A prompt that helps Claude expose gaps
You can ask Claude to perform a claim-by-claim pass, but treat its response as a finding aid rather than independent verification:
Re-check each factual claim against the cited source. For each claim, quote the exact passage you believe supports it and explain whether it supports the full claim, only part of it, or none of it. Flag any passage you cannot locate, any missing context, and any claim that needs a different source. Do not fill gaps by guessing.After Claude responds, open the source yourself and confirm every quoted passage. Anthropic cautions that plausible-looking quotations can be ungrounded, so asking the same model to verify its own output cannot replace direct inspection.
What to do in common mismatch cases
The page exists, but the cited sentence is absent
Search for synonymous wording and check whether Claude cited a different version, section, or document. If you still cannot locate the passage, mark the quotation and the associated claim as unsupported. Do not “repair” a missing quotation by silently paraphrasing a nearby sentence.

The source supports a narrower statement
Keep the narrower statement and state its boundaries. For example, change “the treatment prevents recurrence” to “the cited study reported a lower recurrence rate in its enrolled participants during the study period,” if that is what the evidence says. Include the study’s population, timeframe, and uncertainty when they are material.
The source is outdated
Record the publication or update date and determine whether the question is time-sensitive. For laws, prices, software behavior, safety guidance, and current policies, locate a current primary source. If you cannot establish the present position, say that the cited material is historical and leave the current claim unresolved.
The citation is a secondary retelling
Use the secondary page to locate the underlying study, regulation, dataset, or announcement. Verify the important proposition against that primary source. If the original is unavailable, describe the evidence as a secondary report rather than implying direct confirmation.
Several sources disagree
Do not select the link that agrees with Claude and discard the rest. Compare methodology, date, population, jurisdiction, and authority. Explain the disagreement and the reason for your conclusion, or present the issue as unresolved when the evidence cannot be reconciled.

Web-search citations and API citations are different
Claude web-search responses
Consumer Claude responses that use web search provide links to pages Claude consulted. Anthropic’s Help Center tells users to review those cited sources and the original websites because synthesis may omit important details: read the guidance here. The link is a starting point for checking, not a guarantee that the generated sentence is entailed by the page.
Anthropic API document citations
The API citation feature works from documents supplied by the developer and can point to locations within those documents. The documentation describes page ranges for PDFs, character ranges for plain text, and custom-content block ranges; indices are zero-based as specified in the API documentation: Citations in the Claude API. Plain-text and PDF sources are chunked into sentences by default. Scanned PDFs without extractable text are not citable through this mechanism, and image citations are not supported.
A valid pointer proves that a location exists in a supplied source; it does not prove that a human would infer Claude’s claim from that location. Developers should therefore test semantic support separately from citation formatting. Anthropic’s June 23, 2025 announcement describes citations as references to the exact sentences and passages used to generate responses and says internal evaluations found its built-in capability increased recall accuracy by up to 15%. That is an Anthropic-reported internal evaluation, not an independent benchmark or a promise that every citation is correct: Introducing Citations on the Anthropic API.

The same announcement reports Endex customer results of source hallucinations and formatting issues falling from 10% to 0% and references per response increasing by 20%. Those figures are statements from Endex’s CEO as reported by Anthropic, not universal outcomes or an expected success rate for every implementation. Citation tooling improves traceability; it does not remove the need to evaluate entailment, scope, currency, context, and authority.
When the stakes are high
For medical, legal, financial, safety, employment, compliance, or security decisions, route the claim to the relevant original authority or a qualified professional. Anthropic’s Help Center states: Users should not rely on Claude as a singular source of truth and should carefully scrutinize any high-stakes advice given by Claude.
Keep a review trail showing the claim, source passage, date checked, reviewer, and final action. If no authoritative evidence supports the claim, do not publish it as fact.
A compact review checklist
- Open the cited URL and verify the final destination.
- Identify the publisher, document title, date, and whether it is primary.
- Locate and read the complete relevant passage and its surrounding context.
- Break Claude’s sentence into atomic claims.
- Check directness, scope, currency, qualifications, and authority.
- Label each claim supported, partly supported, contradicted, or not established.
- Qualify, replace, or remove unsupported wording.
- Use a different authoritative source when the claim needs one.
- For high-stakes matters, obtain human or professional review.
Bottom line
A Claude citation is a lead you must audit. The reliable response to a mismatch is to inspect the original passage, compare it with the exact claim, record the limitation honestly, and rewrite to the evidence. Better citation pointers make that process faster and more reproducible; they do not turn generated text into proof.
