Stop treating the output as trustworthy, prevent the system from taking further consequential action, and put an authorized human in control. If an unexpected result could affect safety, rights, money, employment, access to services, privacy, or security, pause or interrupt the workflow where it is safe to do so. Verify the result independently, preserve the relevant context, and use your organization’s escalation route.
The right response depends on what the system can do, how serious the possible harm is, who operates it, and which laws apply. A surprising answer from a chat tool is handled differently from an automated decision, industrial control, medical workflow, hiring system, or fraud block. The procedure below gives an immediate response for individuals, then a monitoring and governance framework for organizations.
1. Protect people and stop the system from escalating the problem
First decide whether the AI is merely displaying an odd result or is controlling, recommending, or triggering an action. If a person could be injured, exposed to an urgent hazard, denied a critical service, or financially harmed, treat the situation as an incident rather than a normal support question.

- Use the documented pause, stop, rollback, disable, or human-override procedure. In a physical environment, follow the site’s emergency and safety procedure immediately; do not wait for a vendor response.
- Prevent queued, repeated, or downstream actions. For example, hold an automated approval, quarantine a generated configuration, or remove an unverified recommendation from a workflow before it is acted on.
- Tell the responsible supervisor, operator, or safety lead what has happened and what has already been stopped.
The exact control may be a stop button, a workflow hold, a permissions change, or a manual takeover. Do not improvise a shutdown that could create a greater hazard; use the system instructions and deployment procedure for that environment.

Do not rely on the suspect result
Have an authorized person check the relevant facts using an independent source or established manual process. They should either correct or reverse the action, decide not to use the output for this case, or approve it only after the normal verification standard has been met. A plausible explanation is not evidence that an AI result is correct.
2. Classify the consequence before deciding what to test
Ask six questions. The answers determine how quickly to stop, who must be involved, and whether testing can continue:
- What happens if it is wrong? An incorrect low-stakes draft is not equivalent to a safety instruction, eligibility decision, payment, or employment action.
- Can the system act by itself? A read-only assistant has less immediate reach than a system that can send messages, alter records, execute code, move money, or control equipment.
- Can a human pause or override it? A visible, tested intervention is safer than a workflow with no practical way to interrupt an action.
- What evidence is available? Reliable timestamps, inputs, outputs, model or system version, and action logs make investigation possible.
- Who owns the decision? Identify the person authorized to approve a rollback, suspend use, notify affected people, or contact a provider.
- Which jurisdiction and classification apply? Intended purpose, deployment role, location, and system category can change the duties that follow.
| Response factor | Lower immediate exposure | Higher immediate exposure |
|---|---|---|
| Consequence of error | Revisable wording or internal brainstorming | Safety, rights, money, employment, essential services, or access decisions |
| Autonomy | Human reviews every result before action | System triggers actions or sends results directly to others |
| Intervention | Tested pause, rollback, and manual path | No clear stop, override, or safe fallback |
| Evidence | Central, accessible records | Missing, fragmented, or inaccessible logs |
| Accountability | Named owner with authority | No identified person who can suspend or correct use |
3. Apply human oversight, not automation bias
For high-risk AI systems in the European Union, Article 14 of Regulation (EU) 2024/1689 describes concrete human-oversight capabilities. The assigned overseer should understand the system’s relevant capabilities and limitations, monitor operation for anomalies and unexpected performance, interpret outputs, avoid blindly following automation, disregard or override an output, and intervene or interrupt operation through a stop button or similar procedure that brings the system to a safe halt.
“to properly understand the relevant capacities and limitations of the high-risk AI system and be able to duly monitor its operation, including in view of detecting and addressing anomalies, dysfunctions and unexpected performance”.

This sentence is from Article 14(4)(a) of Regulation (EU) 2024/1689, enacted by the European Parliament and the Council. It does not prescribe one universal button or workflow. The system’s instructions and the deployment’s operating procedure determine how the intervention is performed.
Make the override real, not theoretical
- Give the overseer the permissions, training, time, and information needed to act.
- Make the manual alternative usable during an outage or confusing output.
- Define which signals require an immediate halt versus additional review.
- Record who accepted, rejected, changed, or reversed the AI recommendation.
- Check that a person can intervene without being penalized for stopping an unsafe automation.
4. Preserve the incident context without creating a new privacy problem
Record enough information for another qualified person to reconstruct what happened. This is practical incident-recording advice, not a universal legal retention schedule.
- System name and version, if visible; model, policy, plug-in, or configuration identifiers where available.
- Date, time, time zone, and the user or service account involved.
- The task, relevant instructions, input data, and any files or tools supplied.
- The exact output, recommendation, or action, including error messages.
- What happened next: who relied on it, what was blocked, and whether anything was reversed.
- The human response, including the person who approved an override or escalation.
Store the record in an approved incident, security, safety, or quality system. Do not paste confidential, personal, regulated, or customer information into an unapproved public form simply to obtain help. Restrict access and preserve the original record before editing or redacting a working copy.
NIST identifies fragmented logging across distributed infrastructure as a monitoring barrier. If logs are split among an application, identity service, data store, and external model provider, document the gaps explicitly and ask the relevant owners to preserve their portions.

5. Escalate through accountable channels
Use the organization’s official route rather than an informal message to an individual developer. Depending on the incident, that route may include a supervisor, system owner, safety lead, IT or security operations, privacy office, compliance team, legal counsel, or the provider’s incident channel.
Include a concise escalation packet
- State the observed behavior and the potential consequence, separating confirmed facts from assumptions.
- Identify whether the system acted, recommended an action, or only generated information.
- List the containment already performed and whether people, records, funds, equipment, or services were affected.
- Attach the preserved evidence through an approved channel.
- Ask for a specific decision: suspend use, review a case, restore a prior state, notify a party, or authorize controlled testing.
If the incident is an immediate emergency, use the applicable emergency service or site-safety process first. Regulatory or vendor escalation cannot substitute for urgent protection of people.
6. Investigate only in a controlled, safe environment
Reproducing the behavior can help determine whether it is a one-off error, a configuration problem, data issue, attack, or broader performance change. Do not repeat harmful prompts or actions in production, with real customers, or around people who could be affected.
Recommended Free Tools
A safer test sequence
- Clone or isolate the relevant configuration where possible.
- Remove personal, confidential, and production credentials from test data.
- Use a minimal test case that answers one question at a time.
- Compare the result with a known-good version, baseline, or independent process.
- Stop testing if the system can escape the test boundary or cause a real-world action.
- Document the test conditions so a later result is not mistaken for the original incident.
Variation can arise from changing inputs, retrieval data, policies, tools, permissions, model versions, or external conditions. A single successful replay does not prove the original behavior is fixed.

7. What organizations should monitor after deployment
NIST’s AI 800-4 work, described on a page dated March 9, 2026, groups post-deployment monitoring into six categories. Monitoring should be proportionate to the system’s use and consequences; the source does not establish one universally correct schedule.
| Category | Question to answer | Useful signals |
|---|---|---|
| Functionality | Does the system continue to work as intended? | Output quality checks, failure rates, drift indicators, and comparison with an approved baseline |
| Operational | Does the infrastructure provide consistent service? | Availability, latency, capacity, dependency failures, and rollback readiness |
| Human factors | Are interactions transparent and usable? | Override use, operator workload, confusing explanations, training gaps, and automation-bias signals |
| Security | Is the system protected from attacks and misuse? | Unauthorized access, prompt or data attacks, abnormal tool use, and credential events |
| Compliance | Does operation meet applicable requirements? | Required notices, access controls, records, assessments, and incident handling |
| Large-scale impacts | What wider effects emerge over time? | Disparate outcomes, affected groups, systemic externalities, and cumulative harms |
NIST describes unresolved practical challenges: detecting degradation and drift, joining fragmented logs, handling policy complexity, scaling human monitoring, and recruiting and training qualified AI experts. It also identifies open questions about monitoring cadence and how automated checks should be combined with human validation.
“Given that AI systems have novel properties that introduce variability and manifest in unpredictable ways, post-deployment monitoring – from incident monitoring to field studies – is a crucial practice for confident, wide-spread AI adoption.”

8. Understand who may have legal duties
Do not assume that the person who notices an unexpected answer is responsible for regulatory reporting. Duties can differ between a provider, deployer, importer, distributor, employer, public authority, and end user. They also depend on intended purpose, system classification, location, and the facts of the incident.
European Union example
The European Commission’s AI Act FAQ says the Act can apply to public and private actors inside or outside the EU that place an AI system or general-purpose AI model on the EU market, put a system into service, or use it in the EU. High-risk status depends on intended purpose and covered use cases, not merely on a product label. Examples include certain systems used in education, employment, access to essential services, creditworthiness, law enforcement, migration, biometric applications, and safety components.
The same FAQ, displayed on October 13, 2026, lists these implementation dates: general-purpose AI obligations became applicable on August 2, 2025; high-risk AI system rules start on December 2, 2027; and requirements for AI embedded in physical products start on August 2, 2028. It also states that the Digital Omnibus entered into force on July 27, 2026. These dates and interpretations can change, so organizations should check the current Commission guidance and obtain advice for their specific deployment.

For high-risk systems, the FAQ says deployers must follow instructions, monitor operation, act on identified risks and serious incidents, and assign an appropriately equipped human overseer. When an output is used for a legally consequential decision about a natural person, the Act describes an explanation right in the circumstances it specifies, including its exceptions.
Serious incidents involving systemic-risk general-purpose models
The Commission Service Desk states that providers of general-purpose AI models with systemic risk must track and document relevant serious-incident information and possible corrective measures, then report them without undue delay to the AI Office and, where appropriate, national authorities. This is a provider duty for a defined class of models, not a general reporting instruction for every user or every unexpected answer.
9. Build a response plan before the next failure
A written plan turns a surprising output into a controlled decision instead of a debate during an incident. Include:
Quick Recap
- A risk register connecting each use case to its possible consequences and affected people.
- A named system owner, human overseer, backup, and escalation contact.
- Clear stop, rollback, access-revocation, and manual-operation procedures.
- Rules for what data may be logged, where it is stored, and who may see it.
- Predefined severity levels and response times, without pretending that one cadence fits every system.
- Provider contact details, contract obligations, and a method for preserving provider-side records.
- Training that covers limitations, anomaly recognition, automation bias, privacy, security, and intervention.
- Exercises that test the manual fallback and the ability to reconstruct an incident.
10. Common mistakes to avoid
- Continuing because the answer sounds confident: confidence and fluency do not establish accuracy.
- Testing in production: repeated attempts can expose people or systems to the same harm.
- Deleting the bad output: removal may destroy the evidence needed to diagnose or report the event.
- Sharing sensitive logs casually: an investigation must not become an unauthorized disclosure.
- Blaming the model before checking the surrounding system: permissions, prompts, retrieved data, tools, integrations, and human decisions may all contribute.
- Assuming a legal category from marketing language: classification follows the intended use and applicable rules.
- Waiting for a regulator or vendor before taking safety action: immediate containment comes first.
11. A compact decision checklist
- Could this output cause consequential harm? If yes, pause or interrupt the affected workflow safely.
- Can an authorized human verify, reject, reverse, or override it? If not, escalate before proceeding.
- What exact inputs, outputs, versions, times, actions, and responses must be preserved?
- Which owner can suspend use and approve a rollback?
- Is reproduction safe in an isolated environment, or should testing stop?
- Which privacy, security, safety, compliance, and legal specialists need to assess the case?
- Does the provider or deployer have a defined incident route, and who is responsible for any notification?
