Verify the alert against conditions at the site before treating it as a violation. Preserve the original alert and media, check the location, time, work activity and potential worker exposure, and have the site’s competent safety person document the review. If anyone may be at risk, control the hazard while the facts are being checked.

Start by separating the alert from the safety decision

An AI construction inspection alert is a prompt to investigate, not proof that a rule was broken. The system may have misread an image, lacked context about the work, or captured a condition that was safe at the time. The reverse is also possible: an alert can be incomplete while a real hazard exists.

Do not dismiss an alert solely because it appears wrong, and do not label it a confirmed violation solely because software did. Make two decisions: first, whether a person was exposed or could be exposed to a hazard; second, whether the system’s finding should be corrected or disputed through the product’s process. A software correction does not replace a safety response.

A private system alert is not, by itself, an OSHA citation. OSHA’s inspection and enforcement procedures concern the agency’s own activities; a private AI finding should be described as an alert or internal finding unless other facts establish something different. OSHA’s Field Operations Manual, Chapter 4 explains how agency inspections document potential violations and exposure.

What to do, in order

  1. Preserve the original alert and media

Save the alert text, identifier, date and time, and whatever image, video, camera context or other supporting information the system makes available. Keep an unchanged copy of the original media; if you make a crop, annotation or screenshot for discussion, retain it separately from the original. Record who retrieved the materials and when, if your site’s recordkeeping process tracks that information.

This matters because later review depends on being able to see what the system actually evaluated, not only a retouched or re-created version. OSHA inspection guidance identifies photographs, video, measurements, statements and relevant documents as types of evidence used to document exposure. See OSHA’s Chapter 4 guidance.

  2. Check the scene, time and work context

Confirm the alert’s timestamp and exact work area. Establish what task was underway, what the camera could see, whether the image matches the reported location, and whether employees were present or could reasonably have been exposed. If practical and safe, inspect the area rather than relying only on a still image or model-generated label.

Compare the apparent condition with the site safety plan, work rules, permits and the applicable standard or procedure. A picture can show a visual cue without showing whether a worker was exposed, what happened immediately before or after the frame, or whether a temporary control was in place. OSHA’s field guidance calls for documenting observed and potential exposure and reviewing pertinent documents; it does not treat one visual cue as the whole case. See Chapter 4 of the Field Operations Manual.

  3. Ask the responsible safety reviewer to assess it

Bring the alert to the site’s designated competent person or the person responsible for the relevant safety program. The reviewer can walk the area, speak with the affected worker, supervisor or witnesses, and check plans and records. Those steps help distinguish an actual hazard from a safe temporary condition, a camera artifact or a mistaken interpretation of the task.

OSHA’s 1994 construction focused-inspection guidance describes the competent person’s role and verification of plans through walk-arounds and interviews. The directive is historical guidance, not a substitute for the current rule that applies to the specific hazard. Read the August 22, 1994 OSHA construction inspection guidance.

  4. Control a credible risk while the review is under way

If the condition is real, uncertain, or could expose someone to harm, follow the site’s hazard-control process while you investigate. Keep people away from a potentially dangerous area or pause the affected task when appropriate under the site procedure; involve the responsible safety lead. Do not wait for an AI vendor to answer a ticket before acting on an immediate safety concern.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the applicable standard, safety plan and employer procedures to determine the right control. The alert alone does not establish which regulation applies, and the exact response depends on the hazard, work and jurisdiction.

  5. Write down the finding and the reasoning

Record what the system flagged, where and when the review happened, who participated, what evidence was considered, what rule or site procedure was checked, whether exposure was present or possible, the decision, and any corrective action or follow-up. If the alert is judged false, state the specific reason—for example, the timestamp did not match the task, the camera view obscured a control, or the site check found no exposure—rather than writing only “false alarm.”

OSHA’s inspection procedure directs its compliance officers to document interview statements thoroughly and accurately, including names, dates, times, locations, materials, positions of relevant articles and witnesses. That is agency procedural language, not a rule that this article says applies identically to every employer’s internal review. It is a useful model for making an internal record traceable. See OSHA’s Field Operations Manual, Chapter 3.

  6. Submit product feedback or a dispute separately

Once the safety decision is documented, use the AI product’s documented route to dispute or correct the alert, if one exists. Include the alert identifier and relevant context, following the product’s privacy and record-retention rules. Do not assume a vendor has deleted, corrected or retrained on an alert unless its process confirms that outcome.

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

No particular AI system is identified here, so its appeal route, record-retention behavior and model-feedback process cannot be specified. Keep the site’s hazard record and the product-support exchange distinct: closing or correcting an alert in software is not evidence that a hazard was controlled.

Evidence that helps resolve a disputed alert

Gather evidence that answers what happened, where, when and whether anyone was exposed. Depending on the incident and the site’s procedures, a useful record can include:

  • The unaltered image or video, its source, timestamp and available camera or view details.
  • The exact site location and work activity at the time of the alert.
  • Whether employees were present or could reasonably have been exposed, and what controls were in place.
  • Statements from the exposed worker, supervisor and witnesses, with relevant names and date, time and location details.
  • Relevant safety plans, permits, work rules, equipment manuals, program records and measurements.
  • The reviewer’s identity, decision, basis, corrective action and any follow-up.

Not every case needs every item. Preserve records that are relevant and available, and follow employer requirements for sensitive worker, project or camera data. Avoid treating a missing camera angle or lack of a measurement as proof either that a violation occurred or that it did not.

A consistent test for one alert or a pattern of alerts

When reviewing repeated alerts, use the same questions each time. Consistency makes it easier to identify whether the issue is a real recurring hazard, a camera or timestamp problem, missing work context, or an unreliable interpretation. OSHA guidance supports reviewing evidence of exposure and pertinent records; the following is a practical review framework, not an OSHA scoring system.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Review question What to establish
Image and camera coverage Is the image clear enough to assess the condition? Does the camera capture the relevant area, or do angle, obstruction or lighting limit what can be concluded?
Time and place Does the timestamp match the work activity, and does the image show the location named by the alert?
Rule and task context What requirement or site procedure could apply, and does the alert account for the task, equipment and controls actually in use?
Worker exposure Was anyone exposed, or could someone reasonably have been exposed? What evidence supports that conclusion?
Plan and applicable standard Does the observed condition align with the site plan and the applicable requirement for this hazard and jurisdiction?
Record and correction trail Can the site trace the original alert, review decision and any corrective action? Does the product provide a documented way to correct or dispute the alert?

The first five questions focus on the facts needed to assess a possible safety condition. The final question also depends on the capabilities and procedures of the unnamed product; do not assume all systems provide the same audit trail or correction route.

How OSHA guidance fits—and what it does not establish

The cited OSHA materials describe agency inspection practice and construction safety context. OSHA says its Field Operations Manual is guidance for OSHA offices and does not create enforceable duties, rights or benefits for other parties. It can inform how to organize evidence, but it is not a replacement for the applicable regulation, state-plan requirement, contract or site procedure. See OSHA’s Field Operations Manual.

For construction, OSHA’s focused-inspection directive identifies falls, struck-by, caught-in/between and electrical hazards as the four leading hazard categories and says they accounted for 90% of construction deaths and injuries. That is a statement in the agency’s directive dated August 22, 1994—not a current estimate of construction risk and not a measure of AI accuracy. The directive also discusses competent persons, safety plans, and records such as training, permits, hazard reports, inspections, uncorrected hazards, incidents and near misses. See the 1994 directive.

There is no current, named empirical statistic here for the accuracy or false-positive rate of AI construction inspection systems. An alert should therefore be assessed on its own evidence rather than discounted or accepted based on a supposed industry-wide error rate. For a specific hazard, consult the relevant standards, enforcement policies, directives and construction safety-program resources on OSHA’s Construction Industry compliance page. Requirements may differ under an OSHA state plan, local rules, contract obligations or site procedures.

Common mistakes that undermine a fair review

  • Deleting or overwriting the source media: this prevents an independent reviewer from checking what the system saw.
  • Deciding from a label alone: the alert text does not establish the work context or employee exposure.
  • Declaring the case closed because it is a false alert: verify there is no separate underlying hazard before ending the safety response.
  • Changing the image without preserving the original: annotations may help explain a decision, but should not replace source material.
  • Using “false positive” without a reason: document the evidence and applicable rule or procedure behind the decision.
  • Assuming one jurisdiction’s guidance covers every site: check the applicable federal or state-plan requirements and project obligations.

Make the review useful beyond the individual alert

After a decision, route the result to the people responsible for the safety program and any relevant system administrator. If multiple alerts point to the same location or condition, look for a recurring hazard as well as recurring visual or data problems. A camera repositioning, timestamp correction, clearer task context or revised internal review step may be appropriate, but the site should choose changes based on its own findings and product documentation.

Keep a record of the outcome in the site’s established safety process, including corrective action and follow-up where needed. That creates a defensible distinction between an alert that was reviewed and found unsupported, a genuine hazard that was corrected, and a case that remains unresolved.
