If an AI chatbot gives advice that could hurt you or someone else, stop and do not act on it. Treat the output as unverified text, not an authority. Check consequential claims with a qualified professional or reliable primary source, preserve the relevant context privately if it is safe to do so, and report the exact response through the provider’s current feedback controls. If anyone may be in immediate danger, has a plan to self-harm, or cannot stay safe, contact local emergency services or a crisis helpline and involve a trusted person first; a platform report is not emergency care.
First decide whether this is an emergency
A frightening answer is not automatically proof that a crisis exists. The key question is whether a person faces immediate danger, has a specific plan to harm themselves or someone else, or cannot reliably stay safe right now.
If there is immediate danger
- Move away from the chatbot’s instructions. Do not test, follow, or forward advice that could cause injury, expose private information, enable violence, or trigger an irreversible decision.
- Contact your local emergency service or crisis helpline now. Use the emergency number and services for your location rather than relying on an online platform’s response time.
- Tell a trusted person what is happening and ask them to stay with the person at risk, in person or by phone, if doing so is safe.
- Use calm, direct language: “I’m worried about your safety. Are you thinking about hurting yourself or someone else right now?” Listen without arguing about the chatbot’s wording, and help connect the person with professional support.
OpenAI’s guidance for trusted contacts identifies immediate danger, a plan to self-harm, an inability to stay safe, or an unsafe situation for the contact as reasons to seek emergency help. It also recommends a calm conversation and helping the person reach further support: Being someone’s trusted contact.

Pause the conversation, close the tab if necessary, and check in with the affected person. A chatbot can produce an alarming statement through misunderstanding, hallucination, role-play, or a badly handled safety refusal. Do not diagnose a crisis from the text alone, but do take concrete signs of danger seriously and ask a human professional when you are unsure.

Do not treat the answer as authoritative
Chatbots generate likely language, not verified decisions. A confident tone, a warning, a refusal, or a reassuring paragraph does not establish that the rest of the answer is safe or accurate. Safety systems can miss dangerous content and can also wrongly flag benign content. Anthropic states in its Claude safety guidance: “These features are not failsafe, and we may make mistakes through false positives or false negatives.”
For medical symptoms, medication, self-harm, weapons, physical safety, legal rights, immigration, money, or account security, separate the chatbot’s factual claims from its interpretation and tone. Check the facts against a government, hospital, regulator, court, manufacturer, or other authoritative primary source. For a personal decision, consult the appropriate licensed professional. If a claim could change treatment, legal status, finances, or physical safety, wait for that verification before acting.
Be careful with links and pasted material
Google explains that prompt injection attempts to elicit an unintended or harmful response. Material referenced in a prompt can contain instructions designed to manipulate a model. Google says suspicious shared content may be excluded, trigger a warning, or cause Gemini to block a response; that is a possible protection mechanism, not proof that any particular alarming answer was caused by prompt injection. Its guidance also advises caution with unknown shared content and untrustworthy links: How Gemini Apps help protect users from malicious content & prompt injection.
Do not open an unfamiliar link, download a file, disclose a password, or paste confidential records merely because a chatbot recommends it. Navigate to an organization’s known official website yourself when verification is needed.

Preserve the useful evidence without exposing private data
If everyone is safe and keeping a record will not increase risk, save the message and enough surrounding context to explain what prompted it. A screenshot, copied transcript, or export can help a provider understand the failure. Include the model or product name, the approximate date and time, and whether the answer came after a particular file, link, image, or multi-turn exchange.
Before sharing anything, remove names, addresses, phone numbers, account identifiers, medical records, school details, faces, and other information that is not needed to explain the safety problem. Do not repost a private conversation publicly to seek attention or prove a point. Store the original privately, and create a redacted copy for a report.
There is no universal provider rule requiring a particular evidence format. Retaining context is practical advice so the report describes the actual exchange rather than an isolated sentence.
Report the specific response through the provider
Use the control attached to the problematic message whenever it is available. Interface labels can change by product, account, region, and app version, so follow the wording currently shown on screen. Describe what the chatbot said, why it appears unsafe, what preceded it, and whether anyone was placed at risk. Do not claim certainty about the cause if you do not know it.

ChatGPT
- Find the problematic message.
- Tap or select the thumbs-down control beneath it.
- Choose Select an issue.
- Choose Safety or Legal concern.
- Follow the remaining prompts and submit the report.
OpenAI also describes reporting content through a web form in its Help Center: Reporting Content in ChatGPT and OpenAI Platforms. Use the current in-product labels if they differ from these instructions.
Gemini
Google says users can give feedback on a response that is inaccurate or feels unsafe and provides reporting instructions in Gemini Apps Help. Use the feedback option on the response, select the closest safety or accuracy reason, and supply the requested context: How Gemini Apps help protect users from malicious content & prompt injection.
Claude
Anthropic says users can send feedback to usersafety@anthropic.com. Its wellbeing guidance also says users can use thumbs reactions inside Claude.ai. Anthropic notes that this feedback shares the conversation with the company, so remove information that is not necessary before submitting it: Our Approach to User Safety.

A thumbs-down control does not imply that providers handle privacy, review, or retention in identical ways. Read the notice shown when you submit and provide only the context needed for investigation.

What to include in a useful report
- The exact harmful or alarming wording, with sensitive details redacted.
- The prompt or preceding event that caused the answer, including whether a file, image, link, or shared document was involved.
- Why the response could cause harm: for example, dangerous medical instructions, encouragement of self-harm, illegal advice, threats, exposed personal data, or a deceptive link.
- Whether the answer was a single reply or followed several turns.
- What happened afterward, such as whether anyone acted on it or remained at risk.
- The product, app, model label if shown, and approximate time.
Report facts rather than exaggerating them. Do not promise yourself or others that the provider will reply, remove the output, or change the product; the cited provider pages do not guarantee a particular outcome.
What published safety figures do—and do not—show
Anthropic publishes evaluation results for Claude wellbeing scenarios. These are company-reported tests for specified prompts, not independently verified real-world safety rates and not a comparison across providers.

Anthropic also reports low false-refusal rates for benign prompts in its own test setup. Those results do not make a particular answer safe, and they do not remove the need for human judgment, independent verification, or emergency support.

Dangerous medical or physical instructions
Stop before trying the instruction. If exposure, injury, overdose, or another urgent event may already have happened, contact emergency services or the relevant poison, medical, or safety service in your location. Otherwise, show the wording to a qualified clinician or safety professional and verify it against an authoritative source.

Advice about law, money, or accounts
Do not sign, pay, transfer funds, delete records, or surrender access based only on the response. Contact the relevant bank, regulator, lawyer, government office, or account provider using a verified channel. Preserve transaction and message records privately, then report the chatbot output.
Threats, harassment, or exposed personal information
Save a redacted copy, block further contact where appropriate, and use the affected service’s own abuse or security process if a real person or account is involved. If there is a credible physical threat, contact local authorities or emergency services rather than waiting for a chatbot company to review a report.
Self-harm or suicide content
Ask directly whether the person is in immediate danger, has a plan, or can stay safe. If the answer indicates danger or is unclear and you cannot establish safety, involve emergency services, a crisis helpline, and a trusted person. Stay focused on human connection and professional help; do not debate whether the chatbot “meant” the statement.

After you submit the report
Keep the case number or confirmation if one is provided, but do not wait for a platform response before taking safety action. Continue with the independent verification or professional appointment that the situation requires. If the output affected a child, patient, employee, or client, follow the safeguarding, clinical, workplace, or school procedure that applies to your role.
If the same failure can be reproduced without exposing private information, describe the pattern rather than repeatedly provoking the model. Repeated testing can create more harmful material and does not substitute for a provider’s review.
