If you develop possible COVID-19 symptoms, stay home and away from other people, assess whether you have an emergency warning sign, and arrange testing or medical advice. Symptoms overlap with flu and other infections, so symptoms alone cannot identify the cause. If you are at higher risk for severe illness, contact a healthcare professional promptly—even if symptoms are mild and before you have a positive test—because prescription treatment must begin within a short window.

Do these things first

  1. Move away from others. Stay home and avoid close contact with household members who are not sick when your symptoms are not better explained by another cause. Avoid sharing indoor air and postpone in-person activities while you assess what is happening.
  2. Check for emergency warning signs. Trouble breathing, persistent pain or pressure in the chest, new confusion, inability to wake or stay awake, or pale, gray, or blue lips, nail beds, or skin require emergency help. Appearance can vary with skin tone, so treat a worrying change as urgent.
  3. Get emergency help correctly. Call 911 (or your local emergency number) or call ahead to the local emergency facility. Tell the operator or facility that the person has, or may have, COVID-19 so staff can prepare.
  4. Arrange testing and clinical advice. Testing can identify the likely cause, inform treatment decisions, and help you reduce the chance of infecting others. Do not wait for a test result to seek urgent care or, if you are high risk, to call a clinician about treatment.

The emergency list is not exhaustive. Severe, rapidly worsening, or otherwise concerning symptoms deserve medical assessment even when they do not match one of the listed signs.

Recognize possible COVID-19 symptoms

COVID-19 can be mild or severe, and the combination of symptoms varies. The CDC lists:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Sore throat
  • Congestion or a runny nose
  • New loss of taste or smell
  • Fatigue
  • Muscle or body aches
  • Headache
  • Nausea or vomiting
  • Diarrhea

This is not a complete list. Variants, vaccination status, age, and individual health can affect how illness appears. Flu and COVID-19 share many of the same symptoms, so you cannot reliably distinguish them by how you feel. Testing is needed when knowing the cause would change treatment, precautions, or care decisions.

Should you test if you have symptoms?

Usually, yes. A test can help determine whether COVID-19 is a likely cause and can guide treatment and steps to limit spread. It is especially useful when you live with someone medically vulnerable, need advice about work or school, or may qualify for time-sensitive antiviral treatment.

Choose between an antigen and a molecular test

Test type What it detects and how fast it is Important limitation When it helps
At-home antigen self-test Detects viral proteins and commonly gives a result in about 15–30 minutes, according to CDC guidance published in 2025. The actual time depends on the product and service. Generally less sensitive than a molecular test. A negative result can occur even when you are infected. Convenient first check when you need a quick result at home.
Molecular test (NAAT, including PCR) Detects viral genetic material and is generally more sensitive than an antigen test. Turnaround depends on the collection site and laboratory. May take longer and may require a clinic, pharmacy, laboratory, or other testing site. Useful when a more sensitive test is important, including suspected flu and/or COVID-19 when the result can support timely treatment.

Follow the instructions supplied with the specific test, including specimen collection, timing, and reading the result. No single test detects every infection. A negative antigen result lowers neither the possibility of COVID-19 nor the need to act on serious symptoms.

Do not let testing delay care

If you may be at higher risk for severe illness, call a healthcare provider as soon as symptoms start. The CDC states that a positive test is not required for a high-risk person to discuss treatment. Ask what testing is appropriate and whether treatment assessment should begin immediately.

What if your COVID-19 test is negative?

A negative result does not always mean you are not infected. This is particularly important with an antigen test, which can miss infection more often than a molecular test. Continue to stay away from others while you are symptomatic and take the result in context with your symptoms and exposures.

Contact a clinician when symptoms are significant, worsening, or do not fit a simple explanation. Ask whether a molecular test, an evaluation for flu, or another diagnosis is appropriate. CDC guidance recommends molecular testing for symptomatic patients suspected of having flu and/or COVID-19 when it can help start treatment promptly.

If you are high risk, do not wait for a later test to make the first call. Tell the clinician when symptoms began, what test you used, the result, your medical conditions, and every medicine or supplement you take. This information helps determine whether treatment is suitable and whether interactions need to be checked.

When to contact a doctor promptly

Call a healthcare provider promptly if you have any factor that makes severe illness more likely, even when symptoms are mild. Risk is especially associated with older age—particularly age 75 or older—with risk rising substantially above 65. Other examples include being not up to date on COVID-19 vaccination, chronic lung disease, heart disease, and a weakened immune system. A clinician considers all relevant factors rather than using age or one condition alone.

Treatment decisions depend on clinical eligibility, how long you have had symptoms, other health conditions, pregnancy status where relevant, kidney or liver considerations, and interactions with current medicines. Prescription antivirals are not do-it-yourself treatments. A pharmacist or clinician must review whether they are appropriate.

Why the first call is time-sensitive

CDC public guidance says COVID-19 treatment must begin within five to seven days of symptom onset, depending on the treatment. For example, Paxlovid must be started within five days, while intravenous remdesivir must be started within seven days. These are initiation limits in CDC guidance, not reasons to wait; earlier clinical contact gives the care team more time to assess eligibility and access.

How to care for mild illness at home

Most people have mild illness and recover at home, but monitor how you are doing. Rest, drink fluids as tolerated, and keep a way to contact a healthcare professional if symptoms change. Over-the-counter acetaminophen or ibuprofen are examples the CDC lists for symptom relief. They are not suitable for everyone, and this guidance does not replace the product label or individualized advice from a clinician or pharmacist. Do not use prescription antivirals without medical assessment.

Reduce the chance of spreading infection

  • Stay home and away from people who are not sick while symptoms are not better explained by another cause.
  • Limit close, indoor contact with household members and avoid sharing personal items.
  • Tell people you need to see that you may have COVID-19 so they can make informed decisions about precautions.
  • Call a clinic before arriving if you are seeking care, and follow its instructions for entry and masking.
  • If emergency care is needed, call ahead when possible and identify the possible COVID-19 infection to emergency personnel.

Local public-health instructions can differ by country or jurisdiction. Follow the current advice where you live in addition to your healthcare provider’s instructions.

What recovery and rebound can look like

Symptoms can improve and then return, or a new positive test can appear, three to seven days after initial recovery or a negative test. CDC calls this rebound. It can occur with or without antiviral treatment and is typically mild, but it still warrants a call to a healthcare provider.

CDC clinician guidance says people with rebound should isolate to reduce transmission. Treat a return of symptoms or a new positive result as a reason to resume avoiding others and ask your clinician what to do next, especially if symptoms are worsening or you have risk factors for severe disease.

A practical decision path

  1. Symptoms begin: stay home and away from others, and note the date and time symptoms started.
  2. Emergency check: for breathing difficulty, persistent chest pain or pressure, new confusion, inability to wake or stay awake, or pale, gray, or blue lips, nail beds, or skin, call 911 or your local emergency number.
  3. Risk check: if you are older, not up to date on vaccination, immunocompromised, or have conditions such as chronic lung or heart disease, call a healthcare provider promptly. Do this even with mild symptoms and without waiting for a positive test.
  4. Test: use an available antigen self-test for a quick result or arrange a molecular test when greater sensitivity is important. Record the test type and result.
  5. Negative result: continue precautions while symptomatic and ask a clinician whether molecular testing or evaluation for flu is needed.
  6. Positive result or strong clinical suspicion: follow medical advice, ask about treatment eligibility immediately if high risk, and continue steps that reduce spread.
  7. Symptoms return after improvement: contact a healthcare provider, resume isolation, and follow rebound guidance.

Information to have ready when you call

Prepare the symptom start date, current symptoms and their severity, test type and result, age, vaccination status, medical conditions, pregnancy status if relevant, allergies, and a complete list of medicines and supplements. Mention any trouble breathing, chest symptoms, confusion, unusual sleepiness, or rapid deterioration immediately. Clear information helps the clinician decide whether you need an examination, a more sensitive test, flu testing, or time-sensitive treatment.
