If your antidepressant does not seem to be helping during cancer treatment, contact both the clinician who prescribes it and your oncology team. Do not stop, increase, or switch it on your own. Together they can review how long you have taken it, the dose and adherence, remaining symptoms, side effects, and every cancer, supportive-care, over-the-counter, herbal, and supplement product you use. If you may harm yourself or are in immediate danger, use local emergency services or a crisis service now and tell the oncology team.
Start with safety, not a medication change
Get emergency help for immediate danger
Thoughts of suicide, a plan to hurt yourself, inability to stay safe, severe confusion, or another immediate danger requires urgent help rather than waiting for the next appointment. Call your local emergency number, go to an emergency department, or contact a crisis service available where you live. Tell the cancer team what is happening. The sources for this guidance do not provide one crisis number that applies to every country, so use the emergency and crisis options in your location.
Contact your clinicians promptly when symptoms are worsening
Tell the prescribing clinician and oncology team if depression is getting worse, side effects are hard to tolerate, symptoms are interfering with cancer treatment or daily functioning, or you have had no meaningful improvement. The National Cancer Institute (NCI) patient summary identifies worsening symptoms, troublesome adverse effects, lack of improvement, and depression that interferes with cancer care as reasons to seek further help. You do not need to wait for a preset number of weeks when symptoms are severe or deteriorating.

Antidepressants are commonly started at a low dose and adjusted gradually. The NCI says that some take three to six weeks to work, but that is a general onset range, not a promise that one particular medicine will help by a certain day. Dose changes, missed doses, drug interactions, other medical problems, and the type and severity of symptoms can all affect the course.

Prepare a joint medication and symptom review
Cancer treatment often involves several medicines, and cancer or its treatment can alter absorption or metabolism. A drug that was reasonable before chemotherapy, immunotherapy, radiation, surgery, hormonal treatment, or a supportive-care change may need a new review. Make one current list and send it to both teams or bring it to the appointment.
- Record the antidepressant exactly. Include its name, strength, formulation, the time of day you take it, when you started, and every dose change. Note whether it is taken every day or only under a particular schedule.
- List all other products. Include chemotherapy, endocrine or targeted medicines, steroids, anti-nausea drugs, pain medicines, sleep medicines, patches, inhalers, medicines used only as needed, over-the-counter products, vitamins, herbals, and supplements. Include products another clinician prescribed and anything you recently stopped.
- Describe adherence without blame. Tell the clinician about missed or delayed doses, vomiting, trouble swallowing, cost, pharmacy delays, fatigue, memory problems, or a schedule that conflicts with treatment visits. These details help distinguish an ineffective treatment from a treatment that has not been taken consistently or absorbed reliably.
- Track the symptoms that matter to you. Write down mood, loss of interest, anxiety, sleep, appetite, concentration, energy, physical symptoms, and how symptoms affect appointments, work, relationships, and self-care. Note when each symptom began and whether it changed after a cancer treatment or medication change.
- Record adverse effects and possible warning signs. Include nausea, agitation, sedation, dizziness, sexual effects, unusual mood elevation, allergic symptoms, or anything that makes you consider stopping. Report severe or rapidly changing symptoms promptly.
- Bring relevant health history. Mention other medical conditions, previous antidepressants and responses, allergies, pregnancy possibility where relevant, substance use, and any prior episodes of mania or psychosis. Your clinicians decide which details change treatment choices.
- Ask for a defined follow-up. Agree on the next reassessment date, what improvement would look like, which side effects require an earlier call, and who to contact after hours.
The NCI emphasizes reviewing concomitant medicines and possible interaction or metabolism issues. A pharmacist, oncology pharmacist, psychiatrist, or primary-care clinician can participate; do not rely on an automated interaction checker as the final decision.

Check for cancer-treatment interactions
The tamoxifen example
The NCI health-professional summary identifies paroxetine and duloxetine as CYP2D6 inhibitors that are not recommended with tamoxifen because they may reduce formation of tamoxifen’s active metabolite. This is a regimen-specific concern, not evidence that every antidepressant interacts with every cancer medicine. Read the details in the NCI professional summary.
If you take tamoxifen and either of these antidepressants, ask the oncology prescriber, antidepressant prescriber, and pharmacist to review the combination. Do not abruptly stop either medicine or select a replacement yourself. The clinicians must weigh cancer treatment, psychiatric history, prior response, side effects, and other medicines before deciding whether a change is appropriate.
Why a complete list matters
Interactions can involve prescription medicines, supportive-care drugs, supplements, and changes in absorption or metabolism caused by illness or treatment. The absence of one known interaction does not prove that a combination is safe for you, and the presence of an interaction does not automatically mean a medicine must be stopped. Ask the team to check the entire regimen together and to explain which risk they are addressing.

What clinicians may reassess when benefit is limited
“Not helping” can mean no change, partial improvement, symptoms that fluctuate with treatment cycles, or side effects that outweigh benefit. The clinician should first establish what is happening rather than labeling the antidepressant a failure. The review commonly considers:

- Whether the symptoms represent depression, anxiety, adjustment to illness, medication effects, delirium, sleep disruption, pain, hormonal changes, or another condition requiring different treatment.
- Whether adverse effects, interactions, absorption, or metabolism are limiting benefit.
- What happened with previous medicines or psychological treatments.
- Safety concerns, other medical conditions, access to care, and your preferences.
For adult cancer survivors with little improvement after eight weeks despite good adherence, ASCO recommends that the treating clinician adjust the regimen. Possible adjustments can include a carefully planned dose change, changing medication, adding a psychological intervention, or combining approaches. The guideline does not name one universally best antidepressant. Its decision factors include evidence, adverse effects, tolerability, interactions, prior response, and patient preference; see the ASCO 2023 update.

Do not stop or switch abruptly
Stopping suddenly can cause discontinuation problems and can make it difficult to tell whether depression itself is worsening. The NCI advises checking with the doctor before stopping. Its professional guidance describes individualized switching and taper decisions; the pace depends on the specific antidepressant, dose, duration, other medicines, and your health.

If a change is appropriate, the prescriber may plan a gradual dose reduction, a transition to another medicine, or another strategy with monitoring. Ask for written instructions, the date of the next check-in, what to do if a dose is missed, and which symptoms require urgent contact. Do not use someone else’s medicine, double a dose to compensate for a missed dose, or add a supplement marketed for mood without telling the team.

Medication is not the only evidence-based component of care. The ASCO guideline on depression in patients with cancer describes collaborative care that can involve oncology or primary care, a care manager, psychosocial intervention, and psychiatric consultation when needed. The 2016 guideline is available at ASCO’s clinical practice guideline.
Ask whether your cancer center can arrange psycho-oncology, psychology, psychiatry, social work, counseling, or a support program. Psychological treatment may be useful alongside medication or when medication is poorly tolerated. Access varies by location and insurance; telehealth or community services may be options if your oncology center has no embedded clinician. Your preference, privacy needs, transportation, language, and treatment schedule are legitimate parts of the plan.

Questions to take to the appointment
- Have I taken this medicine long enough at the current dose for a fair assessment, and what improvement should we measure?
- Could any cancer medicine, supportive-care drug, supplement, or health condition be reducing benefit or increasing risk?
- Are my symptoms or side effects a reason to call sooner than the planned follow-up?
- Should a pharmacist, psycho-oncology clinician, psychiatrist, psychologist, or social worker join the plan?
- If a change is recommended, what is the taper or transition schedule and what should I do about a missed dose?
- Who will coordinate decisions if the oncology and mental-health prescriptions are written by different clinicians?
Bring a support person if you want help remembering the plan, and ask the team to document the current medication list and follow-up responsibility. Clear ownership reduces the chance that a side effect or interaction is missed when cancer treatment changes.
When to call again
Call promptly for worsening depression, new or increasing self-harm thoughts, severe adverse effects, inability to take medicines or keep them down, or symptoms that are disrupting cancer treatment. At every planned review, report both benefits and harms, even if the change seems small. A treatment plan can be adjusted safely only when the clinicians know what is happening and have the complete medication picture.
