Save the exact response before you do anything else, verify the disputed statement, then report it through the assistant’s feedback or legal/privacy channel. If the claim also appears on a cited webpage or search result, contact that publisher separately. A provider report is not a legal ruling, and the remedy available to you depends on the product, the information involved, your location and the harm caused.
First, preserve exactly what the assistant said
Do not rely on memory or a paraphrase. AI responses can change when the same question is asked again, and editing or deleting the conversation may remove useful evidence. Preserve the complete exchange before submitting a complaint.
- Save the full prompt and the full answer, including the specific sentence you believe is false.
- Record the date and time, product name, account or workspace used, and any model or version label shown.
- Save every citation, linked page, document name and quoted passage displayed with the answer.
- Take screenshots that show the response in context. If the service provides a conversation or share link, save that link as well.
- Keep the original, unedited copy in a safe location and create a short timeline of what happened afterward.
If the output was shown to an employer, customer, journalist or another decision-maker, record who saw it, when they saw it and any practical consequence. Do not publish an accusation against the provider or another person before checking the underlying evidence; a screenshot can show what the model generated without proving why it generated it.
Identify what is actually false
“The AI reported me” can describe several different situations, and the next step changes depending on which one occurred.

A generated statement is wrong
The assistant may have invented an event, confused you with someone else, or inaccurately described your employment, education, legal history, contact details or other personal information. Isolate the precise sentence and explain what a reliable record shows instead.
The answer repeated a false outside source
Sometimes the model summarizes a webpage, directory, post, court record or another document. Compare the assistant’s wording with the cited page. The provider may be able to review its response, but it generally cannot erase the page that supplied the claim.
The output is misleading rather than entirely fabricated
A technically true fragment can create a false impression if it omits dates, context or the fact that two people share a name. Explain the missing context and provide records that establish the complete picture.
Keep these categories separate in your notes. A report that says “the model is wrong” is less useful than one that identifies the exact sentence, the source of the statement and the evidence that contradicts it.

Report the response to the provider
Use the product’s own feedback or reporting mechanism while the conversation and its metadata are still available. Include one concise explanation for each disputed statement rather than an unrelated account of every problem you have experienced with AI.
ChatGPT
- Open the conversation containing the false statement on the web or mobile app.
- Select the thumbs-down control attached to the message.
- Choose the issue category and select Safety or Legal concern when that option describes the problem.
- Follow the prompts, identify the exact response, and explain why it is false.
- Attach or reference reliable supporting evidence and the relevant cited pages, if the form allows it.
OpenAI’s guidance says that a report can alert the company to a problematic output, but it does not promise that the response will be changed for every user. Keep the confirmation or case number with your timeline.
Microsoft Copilot
For individual Copilot, Microsoft documents several ways to flag a specific answer: select Give feedback in Settings, use the flag icon below the response on the web or mobile interface, or long-press the response in the Copilot app and choose Report. Microsoft’s individual Copilot terms also direct legal concerns to its Report a Concern route.
Check which Copilot product and account you used. Microsoft states that the individual Copilot terms do not automatically apply to Microsoft 365 Copilot, so a work or school account may have a different reporting path. In your submission, name the exact product and account context.

Google Gemini
- Open the Gemini response containing the disputed claim.
- Click the thumbs-down control.
- Describe the inaccurate statement and provide the correction or evidence requested by the feedback form.
Google’s Gemini guidance warns that answers about people can be inaccurate or inappropriate and recommends checking them. Feedback tells Google about the response; it does not mean that Google has accepted your account of the underlying facts.

Use a privacy request when the response contains your personal data
A normal thumbs-down report and a privacy request serve different purposes. A report flags an answer for product or safety review. A privacy request asks the provider to assess personal information about you under its privacy process and applicable law.
OpenAI’s current guidance allows a person to request removal of personal data from ChatGPT responses whether or not that person has a ChatGPT account. The request can address information that is inaccurate, excessive, irrelevant or no longer appropriate. OpenAI says requests are reviewed individually, balancing privacy interests with considerations such as freedom of expression and public interest; approval is not guaranteed.

Give the reviewer concrete material:
- the exact output and the conversation or screenshot showing it;
- the specific data at issue, such as your name, contact detail or a URL;
- a plain explanation of what is false, outdated, excessive or irrelevant;
- reliable records supporting your explanation; and
- the requested correction or removal, stated precisely.
OpenAI says it assesses accuracy using what the requester supplies and cannot independently investigate disputed facts. The request may be made by the person concerned or an authorized representative, and proof of authority may be requested. Its US Privacy Policy states: “For this reason, you should not rely on the factual accuracy of output from our models.”

The same policy identifies privacy.openai.com and dsar@openai.com for requests to correct or remove factually inaccurate personal information in ChatGPT output, subject to applicable law and technical capabilities. Depending on where you live, the policy also describes possible rights such as rectification and the ability to complain to a local data-protection authority. Those rights, deadlines and review standards vary by jurisdiction.
Correct the source outside the AI service
If a cited webpage, directory entry, social post, public document or search result contains the falsehood, contact its publisher or platform separately. Explain the exact passage, provide the correction and attach evidence that the publisher can verify. If a search engine reproduces a result or snippet, its process is separate from the publisher’s process.
Removing or correcting an answer in an assistant does not remove the underlying page. Likewise, correcting a webpage does not guarantee that an already generated answer will disappear from old screenshots, cached conversations or copies shared by other people. Track each request independently.
When the false claim has serious consequences
Escalate promptly if the statement has caused or may cause employment loss, denial of housing or credit, financial damage, threats to your safety, harassment or significant reputational harm. Consult a qualified lawyer in the relevant jurisdiction or the appropriate privacy regulator. Bring your preserved output, source pages, timeline, evidence of falsity and records of the consequences.

Do not assume that a false AI output automatically satisfies a defamation claim. Legal tests can depend on publication, identification, fault, privilege, actual harm, the status of the person involved and other facts. Limitation periods and available remedies also differ by location. A lawyer can assess those questions; a provider feedback form cannot.
Common mistakes to avoid
- Deleting the only copy: Save the complete exchange before testing whether the answer changes.
- Reporting a paraphrase: Quote the exact sentence and preserve the surrounding context.
- Ignoring the citation: Determine whether the model invented the claim or repeated an outside page.
- Sending unsupported accusations: Supply records that directly address the disputed fact.
- Assuming one request fixes every copy: Provider output, webpages, search results and reposts have separate owners and processes.
- Using the wrong product route: Identify whether you used an individual, workplace, education or enterprise version before filing.
- Expecting guaranteed removal: Privacy requests are reviewed under applicable law and may be declined.
Keep a follow-up record
After submitting a report or privacy request, save the confirmation, ticket number, date, route used and files supplied. Note any response and the exact action taken. If the provider asks for clarification, answer the factual question directly and avoid changing your account between submissions. If the output continues to appear, capture a new example and distinguish it from the original incident.
The most effective sequence is therefore: preserve the evidence, verify the disputed sentence, identify whether an outside source is involved, report the response through the correct product channel, use a privacy request when personal data is at issue, correct third-party sources separately and obtain local professional advice when the consequences are serious.
