Stop treating the incident as a normal dependency update. First determine whether the package was only listed, installed, executed, run in CI, or deployed. Pause builds and releases if execution is possible, preserve logs, identify credentials available to the affected process, revoke or rotate those that may have been exposed, inspect repository persistence points, then remove the affected version and rebuild from a trusted state. A clean npm install or a green npm audit result does not prove that an executed malicious package caused no further compromise.

Start by classifying what actually happened

The response depends on the package’s execution state, not simply on its presence in a file. A name in package.json is a different event from code that ran with a developer’s credentials or inside a production deployment. Record the exact package name, version or version range, first and last known appearance, source registry, and every place the dependency could have been resolved.

Exposure state What it means Initial scope
Manifest only The dependency is declared but there is no evidence it was installed or resolved. Inspect lockfiles, dependency graphs and install history before assuming execution.
Lockfile or cache A specific version was selected or downloaded, possibly on one or more machines. Identify every checkout, cache and build that used that lockfile.
Installed locally The package files exist on a workstation, but execution is not established. Preserve the machine and determine whether install scripts, tests, builds or application startup invoked it.
Executed on a developer machine Malicious code ran with that user’s environment and filesystem permissions. Investigate available tokens, source files, SSH keys and local persistence.
Executed in CI The package ran with the runner’s job token, secrets and network access. Pause affected workflows, preserve job logs and rotate secrets exposed to those jobs.
Reached production The package ran in a deployed service, image or release artifact. Contain the service, identify data and credentials reachable from its runtime, and rebuild from a trusted source.

GitHub’s incident-response guidance and CISA’s npm alert both emphasize checking direct and nested dependencies, lockfiles, code references, alerts and activity history. Do not limit the search to a direct entry in package.json.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Contain execution before investigating deeply

If a malicious version may still run, containment takes priority over convenience. Pause CI jobs, scheduled releases and automated deployments that resolve the affected dependency. If a running service contains the package, remove it from traffic or isolate its host according to your incident plan. Do not delete the only copy of a runner, container, workspace or log set before preserving evidence.

  1. Record the incident window. Note when the package first appeared, when each lockfile was generated, and the times of installs, builds, tests, deployments and application starts.
  2. Preserve evidence. Export CI logs, package-manager output, repository audit events, host logs, container or image identifiers and the relevant manifests and lockfiles. Hash or otherwise protect copies so later investigation can distinguish originals from edited files.
  3. Stop new exposure. Temporarily disable workflows or deployment stages that can install the affected version. If an emergency rollback is required, use an already verified artifact rather than rebuilding from an unreviewed workspace.
  4. Keep a written scope. List repositories, branches, workstations, runners, images, environments and owners. Mark each as confirmed, suspected or not affected.

Incident response is not a linear process, as GitHub’s official guidance notes. You may return to an earlier step when new log evidence changes the scope.

Find every path that selected the package

Inspect manifests and lockfiles

Search all package manifests and lockfiles, including workspaces and examples that have their own dependency files. Look for the exact package name and the affected version, not only a broad range. A lockfile can select a malicious transitive dependency even when no application manifest names it.

git grep -n "malicious-package" -- '**/package.json' '**/package-lock.json' '**/npm-shrinkwrap.json' '**/yarn.lock' '**/pnpm-lock.yaml'

Replace malicious-package with the real name. If your shell does not support the path pattern, search the repository with its normal text-search tool. Preserve the original files before editing them.

Trace direct and transitive owners

From each affected checkout, inspect the resolved tree and the package’s parent chain:

npm ls malicious-package --all
npm explain malicious-package

These commands help show whether the package is direct or nested and which dependency introduced it. Run them in every workspace or project that used the relevant lockfile. Compare the output with CI install logs and dependency-graph information in your source-control platform.

Check repository and build history

Review commits that changed manifests, lockfiles, workflow files, build scripts and release configuration. Search code for imports, requires, post-install hooks, shell commands, encoded payloads and newly added network destinations. Examine dependency-graph alerts, malware alerts, code-search results and repository activity. The availability and coverage of particular alerts can change, so verify what your platform currently reports.

Decide which credentials must be revoked or rotated

Should you rotate tokens after installing a compromised package? If the package executed, assume any credential readable by that process may have been exposed until logs or other evidence rules it out. If it was only declared and never installed or run, the rotation scope can be narrower, but document why execution was excluded.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Package and registry credentials: npm access tokens, automation tokens and credentials stored in package-manager configuration files.
  • Source-control access: personal access tokens, repository tokens, deploy keys, app credentials and credentials in remotes or helpers.
  • CI/CD secrets: job tokens, signing keys, artifact credentials and variables injected into the affected workflow.
  • Cloud and service credentials: cloud access keys, database passwords, API keys and service-account tokens available in the environment.
  • Host access: SSH keys, cloud-init material, local credential stores and certificates readable by the process.
  1. Identify the exact user, runner identity, container role or service account under which the package ran.
  2. List secrets available through environment variables, mounted files, credential helpers, metadata services and inherited processes.
  3. Revoke or rotate exposed credentials at their issuing systems; do not merely overwrite local copies.
  4. Review audit logs for token use, unusual IP addresses, new repositories, permission changes, package publication and cloud activity during and after the execution window.
  5. Replace secrets in CI, deployment systems and local configuration, then invalidate old values everywhere they could still be cached.

CISA specifically calls out version-control, CI/CD, cloud, npm and SSH credentials when they may have been exposed on an affected system or pipeline. Use the actual execution context to avoid both under-rotating and unnecessarily disrupting unrelated systems.

Look for persistence and unauthorized changes

Removing the package directory does not remove changes made elsewhere. Review the repository and the systems that could continue acting with stolen access.

Repository persistence

  • Inspect recent commits, branches, tags and release changes for unfamiliar authors or modifications.
  • Review workflow files, reusable workflows, action references, scripts and build hooks for newly added commands or exfiltration destinations.
  • Check webhooks, deploy keys, installed applications, machine users and repository permissions for additions or changed scopes.
  • Compare package manifests and lockfiles with a known-good revision, looking for hidden workspace or example-project changes.

Runner and host persistence

  • Inspect self-hosted runner registration, labels, startup services, scheduled tasks, shell profiles and temporary executable files.
  • Review container entrypoints, image layers, mounted volumes and deployment manifests for unexpected binaries or altered commands.
  • Check outbound network, DNS and process logs for destinations or child processes that do not belong to the build or service.

If you find evidence of persistence, expand the incident to the affected repository, runner or host rather than treating it as a single dependency cleanup.

Remove the affected dependency and restore a trusted version

Use the applicable current security advisory to identify affected versions and the first known-good version. Do not guess a safe version from a package name alone; malicious releases can be interleaved with legitimate releases, and a transitive path may continue selecting the bad one.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Change the dependency declaration. Update or remove the direct dependency, or upgrade the parent package that introduced the vulnerable transitive path. Keep the change reviewable.
  2. Regenerate the lockfile deliberately. Use the package manager and Node.js version used by the project. Confirm that the affected package and version disappear from every lockfile.
  3. Install from a trusted source. Use your approved registry, verified package metadata and a clean workspace. Avoid copying node_modules or package-manager caches from a suspected machine.
  4. Review the diff. Check manifest, lockfile, generated files, lifecycle scripts and build output. Unexpected changes are a reason to stop and investigate.
  5. Rebuild from clean infrastructure. Recreate the CI runner or container where practical, then produce a new artifact from the reviewed commit and lockfile.
rm -rf node_modules
npm ci
npm ls malicious-package --all

Use the removal command appropriate for your operating system and project policy. npm ci is intended for a clean, lockfile-driven install; it does not make an untrusted lockfile trustworthy, so inspect and approve that lockfile first.

npm audit reports dependency security issues. It is not a malware-incident containment test, and npm audit fix runs an install operation under the hood. Do not run it blindly on an evidence-preservation copy or assume a clean audit means credentials, workflows or hosts are clean.

Verify that the remediation actually worked

Verification should produce evidence, not a reassuring screenshot.

  • The affected package name and versions no longer appear in manifests, lockfiles, dependency trees, built artifacts or container layers unless an approved, known-good version is explicitly required.
  • The dependency graph shows no remaining direct or transitive path to the malicious release.
  • The rebuilt artifact comes from a clean, trusted checkout and a reviewed lockfile; its digest is recorded and promoted through the normal release process.
  • All credentials available to the executed process have been revoked or rotated, and replacements are present in the correct systems.
  • Unauthorized commits, workflow edits, webhooks, deploy keys, applications, runner registrations and binaries have been removed or formally accepted as legitimate.
  • Tests, code review and deployment checks pass without reintroducing the affected version.

Have a second person or separate security function review the evidence when the package ran with production, cloud or organization-wide access.

Monitor after the immediate fix

Continue reviewing source-control audit events, secret alerts, dependency and malware alerts, CI logs, cloud activity, registry activity and host or service telemetry for days and weeks after remediation. Search for use of revoked credentials and for repository or workflow changes made during the exposure window. Keep the incident timeline and list of affected assets current as delayed evidence arrives.

Monitoring is especially important when logs were incomplete, a runner was self-hosted, credentials had broad permissions, or the package ran in a production environment. Set an owner and review cadence rather than leaving post-incident checks as an informal promise.

Response decisions by exposure and environment

Situation Containment Credential action Recovery emphasis
Manifest entry, no install evidence Block the version and inspect all lockfiles and histories. Usually no automatic mass rotation; document why execution was ruled out. Regenerate the lockfile from a trusted revision and verify all workspaces.
Installed on a developer workstation Disconnect or preserve the machine according to your incident procedure. Rotate secrets readable by the user, process or local credential helpers. Rebuild the workspace; inspect commits, SSH access and local persistence.
Executed in CI Pause affected workflows and preserve job artifacts and logs. Rotate job, repository, registry, cloud and signing secrets injected into the job. Recreate runners or images and rebuild from a clean commit.
Executed in production Isolate the service or roll back to a verified artifact. Rotate runtime, database, cloud and deployment credentials available to the service. Investigate data access, outbound activity and all artifacts built from the compromised state.

Common mistakes that prolong an npm malware incident

  • Checking only package.json: a nested dependency and lockfile can select the malicious release.
  • Deleting the package before preserving evidence: this can erase install logs, timestamps and indicators needed to determine scope.
  • Rotating only the npm token: the process may also have read source-control, CI, cloud or SSH credentials.
  • Running npm audit fix as the response: an audit result does not establish malware containment, and the fix command performs an install.
  • Trusting a clean reinstall from the same workspace: caches, altered scripts or persistence outside node_modules may remain.
  • Stopping when the dependency disappears: unauthorized commits, webhooks, deploy keys, runner changes and stolen credentials require separate verification.
  • Assuming the first assessment is final: new log or token evidence can change the affected scope.

When to escalate

Escalate to your security or incident-response function when the package executed with production or cloud access, touched personal or regulated data, ran on a self-hosted runner, created persistence, or used credentials whose activity cannot be fully audited. Preserve the original timeline and artifacts, identify a decision owner, and coordinate credential revocation with the teams that control each system. If the package was only present in an unused manifest and you can demonstrate it never resolved or ran, record that conclusion and complete the same lockfile and repository checks without treating it as a confirmed compromise.

Final checklist

  • Exact package, version, locations and execution times recorded.
  • Direct and transitive dependency paths checked across all lockfiles and workspaces.
  • Affected builds, deployments or services paused when execution was plausible.
  • Logs, manifests, lockfiles, artifacts and repository history preserved.
  • Credentials available to the process identified, revoked or rotated, and audit logs reviewed.
  • Commits, workflows, webhooks, deploy keys, applications, runners and binaries inspected.
  • Affected versions removed and a known-good dependency selected from the current advisory.
  • Clean infrastructure used for reinstall and rebuild.
  • Dependency tree, lockfile, artifact and deployment verified.
  • Post-incident monitoring assigned and maintained.
