If a fingernail or toenail is lifting, do not pull it off or push anything underneath it. Onycholysis—the clinical term for separation of the nail plate from the nail bed—has several possible causes, including injury, repeated irritation, infection, skin disease, systemic illness and medication effects. Trim only a completely detached edge that can be clipped without pain or cutting skin, keep the nail short, protect it from water and chemicals, and arrange an examination with a clinician, especially if the cause is unclear or the change is worsening.
The detached section will not reattach. Care is aimed at preventing further injury while healthy nail grows forward and remains attached.
What does a lifting nail mean?
The nail plate is the hard visible part of the nail. It normally rests directly on the nail bed underneath. When the plate separates, air appears beneath it and the separated area often looks sharply defined and opaque white. The lifted edge may be at the tip (distal), along one side (lateral), or, less commonly, near the cuticle (proximal). One nail or several nails can be affected.

It is a sign, not a diagnosis. Looking at the color or shape alone cannot reliably identify the cause or tell you which medicine, if any, is appropriate. A separated nail can also create space where a secondary infection develops.

What to do right now
- Leave the nail in place. Do not pull, peel, bend, or force the attached portion away from the nail bed. Do not probe underneath it with a file, pin, toothpick, or other sharp object.
- Clip only the fully loose edge. If a piece is hanging free and can be cut without pain, gently trim just that detached piece with clean nail clippers. Stop if the edge is still attached, bleeding, or painful. The American Academy of Dermatology recommends this conservative approach for nail injuries.
- Keep it short. Regular, careful trimming reduces snagging on clothing, bedding, hair and equipment. Do not cut into skin or the attached nail.
- Reduce impact and bending. Avoid activities that repeatedly strike, squeeze or flex the affected finger or toe. Wear footwear with enough room for the toes and use protection appropriate to the activity.
- Limit irritants. Pause nail enamel, remover, solvents and harsh detergents while the nail is affected. For wet work, wear gloves; DermNet suggests cotton gloves beneath vinyl gloves when prolonged exposure is unavoidable.
- Arrange an assessment. A dermatologist or primary-care clinician can determine whether the change follows an injury, exposure, infection, skin disease, medicine or another condition. Do not treat the cause based solely on the appearance.
Clipping prevents catching; it does not cure onycholysis. The cause still needs to be identified and addressed.
Why a nail can separate from its bed
Several different problems can produce the same lifting appearance. Possible explanations include:
- A single injury: a blow, crush, trapped nail or other trauma can separate the plate.
- Repeated mechanical irritation: aggressive manicures, forceful cleaning under the nail, picking, biting, tight footwear or repeated impact can gradually lift it.
- Water and chemical exposure: prolonged wet work, detergents, solvents and some nail cosmetics can irritate the nail unit.
- Fungal or yeast infection: infection may occur beneath a lifted nail, but color and texture cannot confirm it.
- Psoriasis or another inflammatory skin disease: nail disease can occur with or without obvious skin plaques elsewhere.
- Systemic illness: conditions affecting the body, including thyroid disease, may contribute.
- Medication effects: some medicines can alter nail growth or make nails sensitive to light. Never stop a prescribed medicine without discussing it with the prescriber.
A green, brown, black or otherwise darkened area, pain, or inflammation around the nail can occur with injury or infection, but these clues are not specific enough to select an antibiotic, antifungal or other treatment yourself.

When should you seek urgent help?
After an injury, seek prompt professional or emergency care if any of the following applies:
- You cannot bend the injured finger or toe.
- Blood covers more than half of the nail.
- The nail is black or purple.
- The injury is particularly painful.
Arrange medical advice for an unexplained change in nail shape or color, loss of the nail, or skin around it that is sore, red, swollen or warm. A new or changing dark streak under a nail also warrants prompt dermatology assessment. Do not assume that a changing streak is harmless.
How the pattern affects the next step
These patterns do not provide a do-it-yourself diagnosis. They help determine how quickly to seek assessment and what information to tell the clinician.

| What you notice | Why it matters | Practical response |
|---|---|---|
| Recent blow, crush or repeated impact | Trauma or mechanical irritation may have separated the plate. | Protect the nail, trim only a safely detached edge, and seek prompt care if severe-injury signs are present. |
| No clear injury or exposure | Infection, inflammatory disease, systemic illness or a medicine effect remains possible. | Arrange a clinical examination rather than treating by appearance. |
| One nail affected | A local injury or irritant is possible, but this pattern does not exclude infection or another condition. | Record when it began and any recent manicure, footwear, wet work or injury for the clinician. |
| Several nails affected | A shared exposure, skin disease, systemic condition or medicine effect may be involved. | Arrange assessment, particularly if the change is persistent or progressing. |
| Pain, warmth, redness or swelling | Injury or infection may be present around the nail. | Seek medical advice; do not insert tools or self-prescribe medication. |
| New or changing dark streak | A changing pigmented nail mark needs professional examination. | Arrange prompt dermatology evaluation. |
What a clinician may do
The clinician will ask when the lifting started, whether there was trauma or repeated exposure, which nails are involved, what symptoms are present, and which medicines or skin conditions may be relevant. They will examine the nail and surrounding skin.

Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →If the cause is not apparent, DermNet states that a clinician may send nail clippings or scrapings for bacterial or fungal culture. Blood tests may be used to investigate systemic causes such as thyroid disease. These tests are selected according to the examination; they are not routine for every lifting nail.
Treatment follows the cause. It may involve removing an irritant or source of trauma, treating a confirmed infection, managing an associated skin or systemic condition, or reviewing a potentially contributing medicine with the prescriber. A clinician may recommend observation while the nail grows if no active cause requires treatment.
Should you use an antifungal or antibiotic?
Not without appropriate assessment. Fungal infection, yeast, bacterial infection, trauma and inflammatory disease can look similar, while the wrong product can irritate the nail or delay correct care. A culture or other clinician-directed test may be needed before treatment. Over-the-counter products should not be presented as a cure for unexplained onycholysis.
Will the nail grow back normally?
The detached section cannot reattach to the nail bed. Recovery means new nail grows from the nail-producing area near the cuticle and advances forward while remaining attached. Protecting that new growth and correcting the cause gives it the best chance of staying attached.
DermNet’s December 2020 onycholysis guidance gives typical full-regrowth estimates of four to six months for a fingernail and about twice as long for a toenail. These are estimates, not guarantees. The cause, how long the nail has been separated, and the condition of the nail bed affect the result. Long-standing separation is less likely to reattach normally as new growth advances.

Do not judge progress by expecting the old lifted area to stick down. Instead, watch for new nail emerging at the base and moving forward without a new area of separation. A clinician should reassess a nail that continues to worsen or fails to show expected healthy growth.

During hand washing and wet work
Dry the nail and surrounding skin carefully after washing. For cleaning, dishwashing or other prolonged wet tasks, use protective gloves; a cotton liner under a vinyl glove can reduce moisture and irritation during extended work. Replace wet gloves and avoid trapping water against the nail for long periods.
During grooming
Do not scrape under the nail, push tools into the lifted space, or aggressively remove debris. Keep trimming conservative and stop when an edge is attached. Avoid polish, remover and solvent exposure until the nail has been assessed and the irritant risk has been addressed.

For toes and footwear
Keep toenails short without cutting into the corners or skin. Choose footwear that does not repeatedly press or strike the nail, and protect the toe during activities likely to cause impact. Persistent pressure can continue the separation even after the original injury has healed.
Common mistakes to avoid
- Pulling off the nail: this can injure attached tissue and create additional trauma.
- Cleaning underneath with sharp objects: probing can enlarge the separation and damage the nail bed.
- Cutting attached nail: trimming beyond the loose edge can cause pain, bleeding or skin injury.
- Covering the problem with polish: cosmetics and removers may add irritation and make changes harder to monitor.
- Assuming a color proves infection: visual appearance alone cannot select treatment.
- Stopping a prescription medicine: discuss possible nail effects with the prescriber instead.
- Waiting despite red flags: severe injury signs, inflammation or a changing dark streak require professional attention.
Questions to bring to an appointment
Note the date you first noticed lifting, whether it followed an injury, manicure, wet work or new footwear, and whether more nails have changed. List pain, discharge, odor, color change, redness, warmth or swelling. Bring the names of medicines and describe any history of psoriasis, eczema or other skin disease. This information helps the clinician decide whether examination alone, a clipping or scraping, blood testing, or another evaluation is appropriate.
Reliable guidance
The American Academy of Dermatology advises that a dermatologist examine a lifting nail: “A dermatologist should examine any nail that’s lifting up.” DermNet’s onycholysis guidance states: “The detached portion of the nail will not reattach.” The AAD’s nail-injury advice was last updated July 24, 2023. DermNet’s onycholysis page was updated in December 2020, and the NHS advises medical advice for unexplained nail changes or inflamed skin around a nail.
