A low-grade fever after vaccination is often a short-lived immune response. The CDC says most vaccine side effects are minor and go away within a few days. A high, persistent, returning, worsening, or otherwise worrying fever needs medical advice, and a fever’s timing alone does not prove that the vaccine caused it.

Use the person’s age, measured temperature, duration, trend, and other symptoms to decide what to do. A baby younger than 3 months with a temperature of 100.4°F (38°C) or higher needs immediate contact with a clinician. For children, the American Academy of Pediatrics (AAP) advises prompt care for a temperature above 104°F (40°C). Breathing trouble, a seizure, severe allergic symptoms, unusual difficulty waking, or a child who looks very ill require urgent assessment regardless of when the vaccine was given.

First, decide whether this is an emergency

Do not wait for a fever to “run its course” if there are signs of a medical emergency. Call emergency services for life-threatening symptoms, or use your local emergency department when that is the established route.

  • Breathing difficulty or swelling: Trouble breathing, swelling of the face or throat, or other signs of a severe allergic reaction can require emergency treatment after a vaccine.
  • Seizure or inability to wake normally: A seizure, marked drowsiness, confusion, or difficulty waking needs urgent assessment.
  • Very ill appearance: A child who looks seriously ill should be assessed promptly rather than monitored at home simply because a vaccination was recent.
  • Other serious symptoms: In children, a stiff neck, unexplained rash, repeated vomiting, or breathing difficulty alongside fever are reasons to seek prompt medical advice, according to AAP fever guidance.

The presence of a red flag takes priority over the number on the thermometer. Do not delay emergency care while trying fluids, a fever reducer, or another home measure.

Temperature thresholds depend on age

There is no single post-vaccination fever cutoff that applies to every person. The strongest specific thresholds in the reviewed U.S. guidance are pediatric.

Situation What the guidance means Action
Baby younger than 3 months (12 weeks) Temperature of 100.4°F (38°C) or higher Contact the child’s doctor immediately, even if a vaccine was given recently. The AAP symptom checker advises not giving fever medicine to a baby under 12 weeks before the baby is seen.
Child of any age Temperature higher than 104°F (40°C) Seek prompt medical care. AAP general fever advice also calls for prompt advice when readings above 104°F recur.
Child with a concerning symptom Fever plus breathing difficulty, seizure, stiff neck, unusual drowsiness, unexplained rash, repeated vomiting, or a very ill appearance Obtain urgent assessment; do not wait for a duration threshold.
Adult High, persistent, worsening, or concerning fever Contact a healthcare professional for individualized advice. The reviewed CDC guidance does not establish one universal adult temperature threshold after vaccination.

These thresholds come from AAP and CDC guidance for U.S. readers. A local health service may use different care pathways, and a person’s medical history can change the safest response.

What “does not go away” means

Fever in a child lasts more than three days

The AAP immunization-reaction guidance says to contact a doctor within 24 hours if a child’s fever lasts longer than three days. This is a contact-with-the-doctor rule, not permission to wait three days when the temperature is very high or another warning sign is present.

Fever returns after improving

Contact the child’s doctor within 24 hours if the fever returns after being gone for more than 24 hours. A returning fever can have a cause unrelated to the vaccination, so the child may need a fresh examination.

Symptoms persist in an adult

For adults, CDC after-shot advice is to contact a provider when side effects are worrying or do not seem to be going away after a few days. Because there is no universal adult post-vaccine temperature cutoff in that guidance, report the actual readings, how long they have lasted, and whether they are rising or falling.

Any age: worsening is more important than the clock

A fever that is climbing, accompanied by new symptoms, or associated with a person who is getting weaker deserves medical advice sooner. Do not use a “few days” expectation to postpone care for an infant, a high reading, or an emergency symptom.

What to do while arranging advice

  1. Measure and write down the temperature. Record the number, time, thermometer method, and whether the reading is rising or falling. If a reading seems inconsistent with how the person looks, repeat it with a reliable digital thermometer and tell the clinician both results.
  2. Check the person, not just the number. Note breathing, alertness, hydration, rash, vomiting, pain, and seizure activity. A normal-looking reading does not cancel an emergency symptom.
  3. Offer fluids when the person is awake and able to drink. For children, CDC after-shot advice recommends more frequent liquids. Do not force fluids in someone who is choking, repeatedly vomiting, or too drowsy to swallow safely.
  4. Keep clothing comfortable. Avoid overheating with heavy layers or blankets. Comfort measures do not replace an evaluation when the fever is high or persistent.
  5. Ask before giving medicine to a child. CDC advises asking the child’s healthcare professional before giving a non-aspirin pain reliever after a vaccination. Dosing depends on age, weight, the product’s concentration, allergies, and medical conditions, so a universal dose is unsafe to provide here. Follow a clinician’s instructions and the product label exactly.
  6. Call the appropriate service. Use the child’s doctor, an adult’s healthcare professional, an urgent-care service, or emergency services according to the severity. Tell the service that a vaccination occurred, but do not assume it explains the fever.

How to measure temperature more reliably

A thermometer helps describe the fever but cannot identify its cause. The AAP’s temperature guidance describes several measurement methods and states that a rectal measurement gives the best reading, especially for infants younger than 3 months.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Use a digital thermometer and follow its instructions for the selected method.
  • Record the method with every reading; measurements from different sites are not interchangeable.
  • For an infant under 3 months, tell the clinician how the temperature was obtained and seek advice immediately at 100.4°F (38°C) or higher.
  • Do not let a lower reading from a less accurate method overrule a baby’s symptoms or the urgent age-based threshold.

Temperature measurement is one part of triage. It neither confirms a vaccine reaction nor rules out an infection.

Why a fever after vaccination is not always caused by the vaccine

Vaccines can produce short-lived immune symptoms, and the timing of fever differs by vaccine. It is unsafe to assume that every fever occurring afterward is a vaccine side effect. An unrelated viral or bacterial illness, another medical condition, or a coincidental infection can begin around the same time.

Consider the whole pattern: when the fever began, whether it is improving, whether it returned after resolving, and what other symptoms appeared. A fever that starts later than expected, keeps worsening, or persists beyond the usual short period deserves a clinician’s assessment rather than a causal assumption.

Different situations need different responses

Infant younger than 3 months

Take a temperature of 100.4°F (38°C) or higher seriously even after immunization. Contact the child’s doctor immediately and do not give fever medicine before the baby is seen unless a clinician specifically instructs you to do so. Emergency symptoms such as breathing difficulty, a seizure, or difficulty waking require emergency help.

Older child with a high reading

For a child above 104°F (40°C), seek prompt care. If the fever is below that level but lasts beyond three days, or returns after being absent for more than 24 hours, contact the doctor within 24 hours. Escalate sooner for a worsening child or any red flag.

Adult with a persistent or high fever

Contact a healthcare professional when the fever is high, keeps rising, lasts beyond a few days, or feels concerning. Provide the exact readings and symptoms. Since CDC guidance does not set one adult cutoff for every vaccine and situation, the clinician may need to account for the vaccine received, other illnesses, medicines, pregnancy, and examination findings.

Person with immune or other significant medical conditions

Ask the treating clinician for a lower threshold or a specific action plan. Immune status, chronic disease, pregnancy, and medicines can alter the risk from fever and the choice of treatment. Use the individualized plan rather than applying a generic internet cutoff.

Information to have ready when you call

Preparing these details helps a clinician decide whether the person needs an examination, testing, or emergency care:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Age, and weight for a child if medication has already been discussed with the clinician.
  • Vaccine name if known, date and time it was given, and injection or nasal-spray route if relevant.
  • Every temperature, with time and measurement method.
  • When the fever began, whether it ever stopped, and whether it is trending upward.
  • Breathing changes, rash, vomiting, diarrhea, pain, seizure, alertness, and ability to drink or urinate.
  • Medical conditions, immune suppression, pregnancy, allergies, and medicines already taken.

What not to do

  • Do not assume that a fever is harmless because it followed a vaccination.
  • Do not wait for a three-day mark when an infant has a qualifying temperature, a child is above 104°F (40°C), or emergency symptoms are present.
  • Do not give aspirin to a child unless a clinician has specifically prescribed it.
  • Do not use an unsupported universal medication dose; products differ in concentration and safe dosing depends on the individual.
  • Do not bundle someone in heavy clothing to “sweat out” the fever, and do not postpone medical care while trying home remedies.

How long a typical mild reaction may last

According to the CDC’s possible-side-effects guidance, most vaccine side effects are minor—such as a sore arm or low-grade fever—and go away within a few days. That statement describes a general pattern, not a guarantee for every vaccine, person, or fever.

The CDC’s after-COVID-19-vaccination guidance recommends fluids, comfortable clothing, and contacting a provider when symptoms are concerning or do not seem to be resolving after a few days. Apply that advice alongside the stricter age-based pediatric thresholds and emergency warnings above.

A practical decision rule

Measure the temperature accurately, assess the person’s appearance and symptoms, and match the situation to the person’s age. Emergency symptoms require emergency help. A baby under 3 months at 100.4°F (38°C) or higher needs immediate doctor contact; a child above 104°F (40°C) needs prompt care; a child’s fever lasting more than three days or returning after 24 hours without fever merits contact within 24 hours; and an adult with a high, persistent, worsening, or worrying fever should obtain individualized medical advice. Local guidance may differ from these U.S.-based sources, so use the care system where you live.
