Hearing that symptoms “may be psychological” can feel dismissive, but it does not mean they are imaginary or fabricated. Ask the clinician exactly what they mean, what physical assessment has been completed, what remains uncertain, and when you should follow up. Psychological support can be considered alongside medical care; new or worsening emergency symptoms still need urgent assessment.
Start by clarifying what “psychological” means
The phrase can describe several different possibilities, and it is too vague to guide your next decision on its own. A clinician might be considering whether stress, anxiety, low mood, trauma, sleep disruption or another emotional factor is affecting symptoms, how severe they feel, or how you are coping with them. They may also be using “psychological” to describe a working explanation while the cause is still uncertain.
That uncertainty is not proof that a symptom is unreal. The NHS page “Medically unexplained symptoms” states: “This doesn’t mean the symptoms are faked or ‘all in the head’ – they’re real and can affect your ability to function properly.” The same guidance reports that about 1 in 4 people who see a GP have physical symptoms that cannot be explained. That figure refers to people consulting a GP in the context of that NHS guidance; it is not a global prevalence estimate or a measure of psychological illness.
Use a calm, specific question to move from a label to a plan:

- “When you say psychological, what do you mean in my case?”
- “Are you suggesting a particular diagnosis or only a possible contributing factor?”
- “What findings support that explanation, and what is still uncertain?”
- “Could a psychological factor and a physical condition both be involved?”
You do not have to agree with an explanation before you understand it. Ask the clinician to explain unfamiliar terms in ordinary language and to record the working diagnosis in your notes if appropriate.
Find out what has—and has not—been assessed
A psychological contribution being discussed does not, by itself, establish that every physical cause has been excluded. The NHS describes a GP examination and blood tests as possible parts of an assessment, along with checking whether medication contributes, asking about anxiety or depression, and referring to a specialist such as a neurologist when indicated. Which assessments are appropriate depends on your symptoms, history, examination and existing conditions; there is no universal test list that everyone should demand.
Ask the clinician to distinguish completed checks from possible next steps:

| Question | Why it matters |
|---|---|
| “What physical examination or tests have we completed?” | It gives you a clear record instead of leaving you to guess what has been ruled out. |
| “What conditions are you considering, and which are less likely?” | It shows whether the clinician has a differential diagnosis rather than a single assumption. |
| “Could any medication, supplement or another health condition be contributing?” | Medicines and coexisting conditions can alter symptoms and may need review. |
| “What change would make you investigate further or refer me?” | You learn which developments should trigger reassessment. |
| “If we try symptom support, when will we review whether it is helping?” | A time-limited review prevents an unexamined explanation from becoming permanent. |
Bring a list of medicines, over-the-counter products and supplements, including recent changes. Tell the clinician about previous diagnoses, relevant family history, injuries, infections, menstrual or hormonal changes when relevant, sleep, substance use and any pattern you have noticed. Do not stop a prescribed medicine abruptly unless a qualified clinician tells you how to do so.

Prepare for a useful follow-up appointment
The NHS recommends describing what the symptoms feel like, when they began, what makes them better or worse, what you think may be causing them, how they limit activities and how upsetting they are. A brief record can make those details easier to communicate, especially when symptoms fluctuate.
What to record
- Date and approximate time symptoms start and stop.
- Location, quality and intensity, using words or a simple 0–10 scale consistently.
- What you were doing beforehand, including exertion, meals, sleep, work or stressful events.
- What relieved or aggravated the symptom, and how long any change lasted.
- Associated changes such as fever, weakness, numbness, fainting, breathlessness, bowel or bladder changes, rash or weight change.
- Missed work, reduced mobility, disrupted sleep, difficulty concentrating or other effects on daily function.
- Questions you want answered and what outcome you hope the appointment will address.
A phone note, calendar or optional “medical symptom journal notebook” can serve this purpose. It is an organizational aid, not a diagnostic test. Avoid recording every sensation if doing so increases distress; a short representative sample is usually easier to review.
Take support if you want it
You can ask a trusted person to accompany you, help you remember questions or take notes. NICE patient-experience recommendations say clinicians should clarify whether and how patients want family members or carers involved in important decisions. Tell the clinician what role you want your companion to have, and ask for private time if you prefer to speak alone.
Agree on a plan rather than accepting a label
Before leaving, ask for a concrete follow-up arrangement. Confirm who to contact, when to check back, which symptoms require earlier contact and what you can safely try in the meantime. Ask for the plan in writing through the service’s usual system if that is available.

- Define the working explanation. Ask whether the clinician is diagnosing a condition, proposing a possible contributor or acknowledging that the cause is currently unexplained.
- Record the safety net. Write down specific changes that should prompt a call, an urgent appointment or emergency help.
- Set a review date. A review can assess function, symptom pattern, side effects and whether further examination or referral is warranted.
- Choose one manageable goal. Examples include a gradual return to a valued activity, a consistent sleep schedule or reducing a specific limitation. Goals should be agreed with the clinician and adapted to your health.
- Check how to access help between visits. Know the service’s phone number, out-of-hours route and what to do if you cannot follow the plan.
Ask, “What will we do if this approach does not help?” A plan that includes a review and alternatives is different from being told that nothing is wrong.
Consider psychological and physical support together
The NHS lists possible support such as lifestyle goals agreed with a GP, talking therapy including cognitive behavioural therapy, specialist assessment, physiotherapy or occupational therapy. These options are not interchangeable, and none is automatically suitable for every symptom or person. The aim may be to reduce distress, improve activity, address sleep or mood, or help the nervous system and body function more effectively while medical questions remain under review.
| Option to discuss | Questions to ask before starting |
|---|---|
| Talking therapy, such as CBT | What problem is it intended to address? How will progress be measured? What happens if discussing symptoms increases distress? |
| Physiotherapy or occupational therapy | What activities are safe now? How will the programme be paced? Which warning signs mean I should stop and seek advice? |
| Lifestyle goals | Which small change is realistic with my current function, and when will we review its effect? |
| Medication | What benefit is expected, how long might it take, what adverse effects matter, and how will it be stopped or changed? |
| Specialist assessment | What question is the referral meant to answer, and what information should accompany it? |
The NHS notes that antidepressants may sometimes help symptoms even when a person is not depressed, but benefits and adverse effects need consideration. Painkillers and sedatives can carry dependence risks. Do not interpret those general statements as a recommendation to start a particular medicine; ask about your own medical history, interactions, monitoring and alternatives.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesWhen to seek another medical opinion
If you remain unsure about the suggested diagnosis or believe an important condition may have been missed, you can request another opinion. NICE recommendations say patients should know they can ask for one, and the NHS advises asking for a second opinion when you think an underlying condition may have been missed.

Access rules depend on the country, insurer and service. In the NHS mental-health assessment guidance, local arrangements may help someone obtain another professional’s view, but there is no legal right to a second opinion in that particular context. That statement should not be generalized to every NHS service or to other health systems.
How to make the request
- Tell your clinician what remains unresolved: for example, a change in pattern, a test you do not understand or a concern about a specific diagnosis.
- Ask whether another clinician in the practice, a relevant specialist or a different service is the most appropriate reviewer.
- Bring your symptom record, medication list, test results and a short timeline. Ask what information can be shared with the reviewing clinician.
- Keep an open question rather than demanding a particular test: “Could you review whether any further assessment is indicated?”
A second opinion may confirm the first explanation, identify another possibility or recommend monitoring rather than more tests. Its value is a clearer, shared plan—not a guarantee of a different diagnosis.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Do not dismiss emergency symptoms as psychological
Seek urgent help for new, severe or rapidly worsening symptoms regardless of an earlier discussion about psychological factors. In the UK, call 999 for a life-threatening emergency. If you are unsure whether immediate help is needed, NHS 111 can assess symptoms and direct you to the appropriate service. Use your local emergency number and health advice line elsewhere.
Stroke warning signs
NHS stroke guidance highlights sudden face weakness, arm weakness or speech problems. Seek emergency help even if the signs stop. Other sudden neurological changes can also be serious; do not wait for a routine appointment when you feel acutely unwell.

Heart-attack warning signs
NHS heart-attack guidance identifies chest pain that feels tight or squeezing, pain spreading to the arms, neck or jaw, severe difficulty breathing and unresponsiveness as emergency warning signs. These examples are UK-specific and not exhaustive. If symptoms are severe, sudden or unfamiliar, call emergency services.
Useful words for the conversation
If you feel dismissed, describe the impact without having to prove a cause:
- “I understand stress or mood might affect symptoms, but the symptoms are real and limiting my day. What is our plan to address both the symptoms and the uncertainty?”
- “Please tell me which causes you have assessed and what would make you reassess them.”
- “I am willing to consider psychological support. I also need to know how we will monitor physical changes.”
- “Could you write down the follow-up date, warning signs and who I should contact?”
Bring an interpreter or accessibility support if needed. Ask for extra time, written information or a quieter appointment environment when communication, hearing, vision, language or cognitive difficulties make visits harder.

By the end of the discussion, you should know the clinician’s current explanation, what has been assessed, what remains open, which treatment or self-management options are being considered, and when progress will be reviewed. You should also know the warning signs that require faster care and how to request another opinion if uncertainty persists.
Symptoms can be genuine and disabling without an identified medical cause, and emotional or psychological support can be appropriate without replacing attention to physical health. Treat the explanation as something to clarify and review, not as a reason to ignore changes in your body.
