Call the child’s healthcare professional promptly and share the complete report. The number alone is not enough: interpretation depends on the metal, units, specimen (capillary blood, venous blood, urine or hair), collection date, symptoms and likely exposure. A capillary lead screen may need venous confirmation on a specific schedule. Mercury results require identifying the chemical form and matching the specimen to the suspected exposure. Do not start chelation, “detox” products or supplements based on a result alone.

Severe symptoms, a clinician-described very high result or a suspected acute exposure require urgent medical assessment. In the United States, Poison Control is available at 1-800-222-1222; emergency symptoms such as seizures, confusion, trouble breathing or loss of consciousness require emergency services.

First steps after any lead or mercury result

  1. Obtain the full laboratory report. Record the exact value, units, specimen type, collection date, reference comments and whether the laboratory measured total mercury or identified a species.
  2. Contact the child’s clinician. Ask what the result means for this child’s age, symptoms, medical history and possible exposure. Bring the report rather than relying on a verbal number.
  3. For lead, identify the specimen. Ask whether the test was a finger-prick (capillary) screen or a venous draw. A capillary result can be affected by contamination and generally requires confirmation when it is at or above the CDC blood lead reference value.
  4. For mercury, identify the form and route. Ask whether the result is total mercury or speciated mercury, and whether the test was blood, urine or hair. The appropriate interpretation changes with the chemical form and timing of exposure.
  5. Arrange urgent care when indicated. Follow the clinician’s instructions immediately if the child is ill or the result is described as very high. Poison Control can help clinicians and families decide whether an exposure needs emergency evaluation.

Until a professional reviews the report, do not attempt home chelation, commercial “detox,” unproven supplements, laxatives or a drastic diet. Treatment decisions require an individualized assessment.

How to interpret a child’s lead result

The CDC reference value is for follow-up, not a safety boundary

The U.S. Centers for Disease Control and Prevention (CDC) uses a blood lead reference value (BLRV) of 3.5 µg/dL to identify children whose results are higher than those of most U.S. children ages 1–5. The value is based on the 97.5th percentile of the blood lead distribution in that age group, using NHANES 2015–2018 data and published by CDC in 2021. CDC lowered the BLRV from 5.0 to 3.5 µg/dL in 2021; see the current explanation at CDC’s blood lead reference-value update.

A percentile is a population comparison, not a line separating safe children from unsafe children. CDC states, “There is no safe level of lead in a child’s blood.” A result below 3.5 µg/dL does not prove that exposure did not occur or that developmental risk is impossible. CDC’s terminology and clinical actions are explained in its July 2026 guidance on the term “elevated”.

Was the first test capillary or venous?

When the initial test is a capillary finger-prick screen, CDC recommends a venous sample for confirmation. The more elevated the capillary result, the sooner the venous draw should occur. These intervals are recommendations; the child’s clinician and state or local lead program set the actual appointment and reporting plan.

What happens after a confirmed venous lead result

CDC’s recommended actions based on blood lead level use result bands to organize exposure assessment, medical review and follow-up. The bands guide care; they are not declarations that a particular concentration is harmless or toxic for every child.

Below 3.5 µg/dL Routine age-appropriate screening and prevention. Discuss ways to avoid lead sources; do not interpret the result as proof of no exposure. According to routine screening requirements According to routine screening requirements
3.5–9 µg/dL Take an exposure history; provide prevention education; assess iron status and iron/calcium intake; check development and milestones; arrange repeat testing. Environmental investigation depends on jurisdictional rules and resources. About 3 months 6–9 months
10–19 µg/dL Complete the same exposure, nutrition and developmental assessment and arrange public-health or environmental follow-up where available and required. 1–3 months 3–6 months
20–44 µg/dL Complete history and physical examination; investigate and reduce lead hazards; consult Poison Control or a Pediatric Environmental Health Specialty Unit (PEHSU). A clinician may consider abdominal imaging for swallowed paint chips or other radiopaque material. 2 weeks–1 month 1–3 months
45 µg/dL or higher Urgent clinical assessment with detailed neurologic examination. Clinicians may consider abdominal imaging or decontamination when indicated and make specialist-directed chelation decisions. Hospital care is indicated for serious symptoms and may be considered when the home is not lead-safe or exposure remains unidentified and ongoing. As soon as possible As soon as possible

Seasonal exposure can change the best interval, so a clinician or case manager should set the dates rather than relying on the table alone. Keep every result and appointment date so the care team can see the trend.

What the lead evaluation looks for

  • Exposure sources: deteriorating lead-based paint and dust, lead service lines or plumbing, contaminated soil, some foods or consumer and cultural products, folk remedies, and dust carried home from an adult’s job or hobby.
  • Nutrition: assessment for iron deficiency and discussion of adequate iron and calcium. Good nutrition supports the child but does not remove a lead source or replace repeat testing.
  • Development: review of milestones and referral to early-intervention or other supportive services when needed.
  • Environmental control: coordination with the local or state health department to determine whether a home investigation, water assessment or hazard-reduction plan is required. A filter, test kit or remediation product should be selected only when it matches the identified source and applicable local standards.

Do not disturb peeling paint or create dust by sanding, scraping or dry sweeping while waiting for advice. Ask the local health department or a qualified professional for lead-safe work instructions specific to the home.

Why a mercury result cannot be interpreted like a lead result

Mercury is a group of chemical forms with different absorption, storage and elimination patterns. A mercury number has no universal pediatric action cutoff in the materials cited here. The clinician must combine the result with the species, specimen, timing, symptoms and likely route of exposure. Do not apply the lead BLRV of 3.5 µg/dL to mercury.

Specimen and exposure route matter

Specimen What it can indicate Important limitation
Blood Can reflect recent dietary methylmercury exposure. It may be most informative during the first few days after a substantial acute exposure because blood mercury has a short half-life. Dietary sources can complicate interpretation, and total mercury does not by itself identify the chemical form.
Urine Most appropriate for assessing elemental mercury and useful for acute or chronic exposure to elemental or inorganic mercury. It is not a universal test for every mercury species or exposure route.
Hair Primarily reflects organic, especially methylmercury, exposure over time. It is not useful for assessing recent exposures and cannot substitute for a clinician-selected blood or urine test.

These distinctions are described in the Agency for Toxic Substances and Disease Registry (ATSDR) healthcare-provider resource, Evaluating Mercury Exposure, and its October 2024 Toxicological Profile for Mercury. If a report lists only “total mercury,” ask whether speciation is needed before drawing conclusions.

Questions that help identify a mercury source

  • What fish or shellfish did the child eat, in what amounts and how recently? Species-specific local advisories can change the appropriate advice; do not automatically eliminate all fish from the diet.
  • Could a thermometer, switch, fluorescent item, laboratory material or other mercury-containing product have spilled or broken in the home?
  • Are cultural products, folk remedies or imported products involved?
  • Does a parent or other household member work with mercury or use it in a hobby, potentially bringing contamination home?
  • Did symptoms begin after a particular event, and was the child indoors or in a poorly ventilated area?

These questions help the clinician decide whether to repeat a test, order a different specimen or involve a toxicologist. They do not establish a diagnosis by themselves.

When a mercury result or exposure is urgent

Call the child’s clinician or Poison Control promptly for a suspected acute exposure, concerning symptoms or uncertainty about the urgency. Go to emergency care for severe breathing difficulty, confusion, marked weakness, seizures, collapse, loss of consciousness or rapidly worsening illness. A laboratory value alone should not be used as a home triage rule: the same numerical result can mean different things depending on species, specimen and timing.

ATSDR advises that chelation decisions be made with Poison Control or a medical toxicologist experienced in chelation. Never give a chelating medicine or a commercial “detox” product without that direction.

Lead and mercury: the practical difference

Question Lead Mercury
What does the number mean? Compare a blood result with CDC’s 3.5 µg/dL reference value, while explaining that it is a population reference for follow-up, not a safety threshold. Meaning depends on chemical species, specimen, timing and exposure route; no universal child cutoff is established here.
Is confirmation standardized? Yes for a capillary screen: confirm with venous blood on the result-based schedule. No single protocol covers every form; the clinician selects the specimen, timing and possible speciation.
Main next step Confirm when needed, identify and reduce the source, assess nutrition and development, and repeat testing according to the venous level. Identify the form and likely source, interpret the exact biomarker with a clinician or toxicologist, and stop ongoing exposure safely.
Who can help? Child’s clinician, local or state lead program, PEHSU and Poison Control. Child’s clinician, Poison Control and a medical toxicologist.

How to prepare for the appointment

Bring the child’s complete laboratory report and a short exposure timeline. Include the date and amount of any fish meal, a possible spill, renovation or plumbing event, use of a folk remedy or consumer product, and any adult occupational or hobby exposure. List symptoms, medicines and supplements. Ask for a written plan that states whether another test is needed, which specimen to use, the target date, who will report results and which public-health or toxicology service is coordinating follow-up.

Guidance and contact arrangements can change. Use the current CDC and ATSDR pages linked above and follow the child’s clinician and local health department for jurisdiction-specific requirements.
