If you have fever, cough, shortness of breath, chest pain, marked weakness, or bloody or watery mucus after a plausible exposure, seek emergency medical care now. Tell the clinician exactly when and how the contact occurred and that pneumonic plague is a concern. If you have no symptoms but had close, sustained contact with a person or animal who may have pneumonic plague, promptly contact a healthcare provider and your local or state public-health department for an individualized assessment. Do not wait for test results or take leftover antibiotics.

First, decide which situation applies

Symptoms are present

Pneumonic plague can progress quickly. Fever, cough, difficulty breathing, chest pain, sudden weakness, or bloody or watery sputum after a plausible exposure require immediate medical evaluation. Call emergency services for severe breathing trouble, confusion, collapse, or rapidly worsening symptoms. Otherwise, go to urgent or emergency care without delay.

Tell the receiving service about the possible plague exposure before or as you arrive. If practical, call ahead so staff can prepare, but do not postpone emergency care while trying to arrange a call. Include the date and time of contact, when symptoms began, whether the source was coughing, and any contact with respiratory secretions, blood, other body fluids, or animal tissue.

No symptoms, but a close exposure may have occurred

Contact a healthcare provider and your local or state health department promptly, even if you feel well. Public-health personnel can determine whether the event meets a recognized exposure definition and can coordinate preventive treatment and monitoring. Do not self-diagnose based on distance alone: the circumstances, timing, source illness, secretions, animal involvement, and protective equipment all matter.

Only a brief or uncertain contact occurred

Report the event anyway if pneumonic plague is credible. A brief encounter or being in the same building does not automatically establish close exposure, but ordinary contact cannot be declared risk-free without knowing the details. A professional assessment is safer than trying to calculate your own risk.

What counts as a potentially important exposure?

According to CDC clinical guidance, examples of known exposure include close (less than 6 feet), sustained contact with a person or animal with pneumonic plague, or direct contact with infected body fluids or tissues. Respiratory spread generally occurs through droplets coughed by an infectious person or animal and usually requires direct, close contact.

When you speak with a clinician or health department, describe the event rather than labeling it “high” or “low” risk. These details are useful:

  • Exact date, time, duration, and distance of the contact.
  • Whether the person or animal was coughing or had respiratory symptoms at the time.
  • Whether contact occurred after the illness had become apparent.
  • Any droplets, sputum, blood, or other secretions that reached your eyes, nose, mouth, broken skin, or clothing.
  • Handling of an animal carcass, respiratory specimen, or other tissue.
  • Whether gloves, eye protection, a respirator, or other personal protective equipment was used and whether it stayed in place.
  • Any flea bites or handling of animals in an area where plague is known to occur.

These factors guide professional decisions; they are not a home scoring system. A flea bite or handling infected animal tissue is a different route from inhaling droplets and may lead to another form of plague. Tell the clinician about both types of contact if they occurred.

Why timing matters

CDC public symptoms guidance says inhalation incubation can be as short as one day. A CDC clinical guideline published in 2021 describes primary pneumonic-plague symptoms developing within one to three days after inhalation. These are observed ranges, not a guarantee that illness will appear on a particular schedule. Feeling well at a specific hour does not rule out disease.

Because deterioration can be rapid, do not wait for a fever, a laboratory result, or the end of a monitoring period before seeking care if symptoms begin. Tell the clinician when the exposure happened and when each symptom started; that timeline helps determine testing, treatment, and public-health measures.

Post-exposure antibiotics: who decides and how long?

Post-exposure prophylaxis (PEP) is clinician-directed medical care. CDC guidance says potentially exposed people should receive a single antimicrobial for seven days when PEP is indicated. The specific medicine and dose depend on factors such as age, pregnancy, allergies, kidney function, drug interactions, and the nature of the exposure. A clinician or public-health authority must select the regimen.

Do not use someone else’s prescription, leftover tablets, veterinary medicine, or an online dosing chart. An inappropriate drug, dose, or duration can cause harm and may not protect you. Do not delay evaluation while trying to obtain antibiotics independently; suspected illness requires prompt treatment, not a wait-and-see approach.

CDC close-contact material advises considering antibiotics for people who had close contact during the preceding seven days. That statement is a response criterion for health professionals, not permission to start medication without an assessment. Contact the appropriate authorities as soon as possible so any recommended course can begin on their schedule.

What to do while authorities arrange an assessment

Follow the monitoring plan you are given

For close contacts, CDC material advises checking temperature twice a day for seven days and obtaining medical review if a fever develops. Less-close contacts should watch for fever or cough and notify a healthcare provider and public health if either appears. Use a thermometer to record readings consistently, but remember that a temperature reading cannot diagnose or exclude plague.

  • Write down the date and time of each temperature measurement.
  • Record new cough, breathing difficulty, chest pain, weakness, or unusual sputum.
  • Keep the monitoring record available for the clinician or health department.
  • Use the duration and follow-up schedule supplied to you; do not substitute your own shorter period.

If symptoms develop during monitoring

Seek immediate medical attention and state that you are being monitored after a possible pneumonic-plague exposure. Call ahead when feasible so the facility can use its infection-control procedures, but never delay emergency treatment to make arrangements. Avoid crowded, unannounced visits when you are well enough to call first; let public health or the treating service tell you where to go.

How clinicians handle suspected illness

Plague is diagnosed using the clinical picture, exposure history, and laboratory samples. CDC states that when plague is strongly suspected, samples should be collected and antibiotic treatment begun without waiting for final laboratory results. Its clinical-care guidance similarly says, “Begin appropriate therapy as soon as plague is suspected.”

This is why a person with compatible symptoms should identify the exposure immediately. Medical teams may use respiratory precautions and other infection-control measures while evaluating you. Those procedures are directed by healthcare professionals; do not attempt to create a home isolation protocol in place of urgent care.

Respiratory, animal, and flea exposures are not interchangeable

Respiratory contact with a person or animal

Droplets from coughing by an infectious person or animal are the route most relevant to pneumonic transmission. Close, sustained contact, especially within 6 feet, is more concerning than a fleeting or distant encounter. Contact after coughing began, exposure to secretions, and absent or inadequate protective equipment can increase concern, but only a professional can classify the event.

Direct contact with fluids or tissue

Touching infected respiratory fluids, blood, or animal tissue is a separate exposure that must be reported even if there was no face-to-face conversation. Describe whether the material contacted your eyes, mouth, nose, broken skin, gloves, or clothing. Do not handle additional specimens or animal remains while waiting for instructions.

Fleas and animals

Infected flea bites and handling infected animal tissues are recognized plague routes but are not the same as person-to-person pneumonic exposure. Tell public health about recent animal illness or death, hunting or carcass handling, flea bites, and travel or residence in an area where plague occurs. They can direct appropriate evaluation and animal or environmental precautions.

What not to do

  • Do not wait for a test result before seeking care when symptoms are compatible with pneumonic plague.
  • Do not assume that being symptom-free at a particular time proves you are safe.
  • Do not take antibiotics without a prescription or public-health instruction.
  • Do not assume every contact transmits plague, or that transmission is impossible outside a familiar endemic area.
  • Do not enter a crowded waiting room unannounced when you can safely call first.
  • Do not treat a thermometer reading as a diagnosis or use monitoring as a substitute for PEP assessment.
  • Do not handle potentially infected animals, tissues, or secretions again until authorities provide instructions.

Information to prepare for the call

Having a concise timeline makes triage faster. Prepare the source person’s or animal’s name or description, location, suspected diagnosis, and whether a clinician or health department is already involved. Then list your contact distance and duration, dates, coughing or secretion exposure, protective equipment, flea or animal contact, travel, medical conditions, pregnancy status if relevant, medication allergies, and current medicines.

Ask which service will follow you, whether PEP is recommended, where to obtain it, how long to monitor, which symptoms require emergency care, and how to report a change. Follow the instructions from the authority managing your case if they differ from general public guidance.

Questions to ask when exposure happened outside the United States

The CDC material cited here is U.S.-focused. If the exposure occurred elsewhere, contact that country’s emergency medical service and public-health authority, as drug availability, reporting procedures, and clinical protocols can differ. A traveler should still describe the location and dates to a clinician in the country where care is sought.

A simple decision path

  1. Check for symptoms. Fever, cough, breathing difficulty, chest pain, severe weakness, or bloody or watery mucus means immediate medical care.
  2. Report the possible exposure. If you have no symptoms, promptly contact a healthcare provider and local or state public health.
  3. Give the complete event history. Include distance, duration, timing, cough, secretions, tissues, animals, fleas, and protective equipment.
  4. Follow the assigned plan. Take PEP only if prescribed or directed, and complete the instructed seven-day course when it is indicated.
  5. Monitor as instructed. Close-contact guidance uses twice-daily temperature checks for seven days; report fever or cough immediately.

Key points to remember

Pneumonic plague is an emergency when compatible symptoms appear, but exposure is assessed from the actual circumstances rather than from a single rule about being nearby. Close, sustained respiratory contact and direct contact with infected fluids or tissues deserve prompt professional review. Public-health and clinical teams decide whether seven-day prophylaxis is warranted, which medicine is suitable, and how monitoring should proceed. Early disclosure of the exposure allows treatment and infection-control decisions to start without waiting for final laboratory results.
