Contact the clinician or service that ordered the ECG and ask for the actual finding, its urgency, and the follow-up plan. The word “abnormal” is a label, not a diagnosis. A clinician must interpret the tracing together with your symptoms, medical history, medicines, and any earlier ECGs.
If you have acute chest pain or a chest-pain-equivalent symptom, seek emergency care rather than waiting for a routine callback. In the United States, the American Heart Association (AHA) advises calling 911; elsewhere, use your local emergency number.
1. Get the exact result and a clear explanation
Start with the professional who ordered the screening or the service that performed it. Ask for a copy of the ECG report if it is available, but do not try to diagnose yourself from the tracing or from an automated computer statement. The clinically important information is the specific finding and how it fits your situation.
The NHS puts the responsibility plainly: “The GP, nurse or specialist should talk to you about your results and explain what happens next.” Ask for that conversation even if a portal displays only a red flag or the word “abnormal.”

Questions to ask on the first call
- What exact finding was reported?
- Does it need prompt review, or is a routine appointment appropriate?
- Could my symptoms, previous ECGs, medicines, or other health conditions affect the interpretation?
- What follow-up is recommended, and who will arrange it?
- When should I expect an appointment, repeat test, or other result?
Write down the answers, the name of the person you spoke with, and the date. If the service uses a patient portal, save the report and the written plan so you can refer to the same wording later.
2. Know what an “abnormal” screening result does—and does not—tell you
An abnormal label does not identify a cause, establish how serious the finding is, or determine whether immediate treatment is needed. Those decisions require the actual ECG finding and clinical context. The same label can lead to different next steps for different people because the reason for testing, symptoms, history, and prior results differ.
Do not assume that every abnormal screening result is an emergency. Do not assume that having no chest pain proves the result is harmless either. The available guidance supports emergency escalation for acute chest pain or chest-pain-equivalent symptoms, but it does not provide a universal symptom-by-symptom triage rule for every possible ECG abnormality.

Many ECG systems display a preliminary interpretation. That text is not a substitute for a healthcare professional’s review. Ask the ordering clinician to explain which finding matters, whether it is confirmed, and what question any proposed follow-up test is intended to answer.

3. Decide whether you need emergency care or routine follow-up
Seek emergency help for acute chest symptoms
If acute chest pain or a chest-pain-equivalent symptom occurs, seek immediate medical care. In the United States, call 911 as advised by the AHA; in another country, call the local emergency number. Tell the emergency service that you recently had an abnormal ECG screening result, but do not delay the call while trying to obtain the report.
Use clinician-directed follow-up when you are otherwise stable
If you do not have acute chest symptoms, contact the ordering service promptly and follow the timeframe it gives you. The urgency cannot be assigned from the single word “abnormal.” Ask whether the clinician wants to see you within a particular number of days, at a routine appointment, or after another review of the tracing.
If new or worsening symptoms develop while you are waiting, contact a healthcare professional sooner. The appropriate response depends on the symptom and your clinical circumstances; the screening label alone cannot provide a safe triage decision.

4. Confirm whether this was screening or diagnostic testing
Ask why the ECG was performed. A screening test is done in someone without symptoms to look for a condition. A diagnostic ECG is ordered because symptoms, an examination, a known condition, or another clinical concern needs investigation. The distinction affects how the result is interpreted and what evidence applies.

The U.S. Preventive Services Task Force (USPSTF) recommends against screening with resting or exercise ECG in asymptomatic adults at low risk for coronary heart disease. For asymptomatic adults at intermediate or high risk, it found insufficient evidence to determine whether screening’s benefits outweigh its harms. This recommendation does not rule out an ECG when a clinician is investigating symptoms or another medical concern.
5. Prepare information that helps the clinician interpret the finding
Before the appointment, gather the information that can change how an ECG is read. You do not need to interpret the tracing; your job is to provide an accurate history and ask for an understandable plan.
Bring or record these details
- The date, location, and stated reason for the ECG.
- Any symptoms before, during, or after the test, including when they started and whether they are changing.
- Names, doses, and recent changes to prescription medicines, over-the-counter medicines, and supplements.
- Copies or dates of earlier ECGs and the names of clinicians who may have them.
- Relevant medical history and conditions already known to your care team.
- Any written message, portal alert, or report that used the word “abnormal.”
Ask whether an earlier ECG can be compared with the new one. A clinician may also need to verify details about medicines, symptoms, or other health conditions before deciding what the finding means.

There is no single follow-up test for every abnormal ECG. Depending on the finding and the clinical context, a clinician may recommend an appointment, repeat testing, or another investigation. Ask what uncertainty the proposed test is meant to resolve and how the result would change management.

Use these questions before agreeing to a test
- What specific question is this test intended to answer?
- Why is it appropriate for my symptoms and history?
- What are the alternatives, including watchful follow-up when clinically reasonable?
- What are the test’s important risks or burdens?
- When and how will I receive the result?
- What should I do if the result is delayed or I feel worse?
These questions are not a reason to refuse recommended care. They help you understand the purpose and timing of the next step and avoid treating a generic abnormal label as a complete diagnosis.
7. Weigh the possible downsides of a cascade of tests
Follow-up can be useful, but an abnormal screening result can also lead to tests that do not ultimately identify a problem. The USPSTF describes potential harms from downstream evaluation, including unnecessary radiation exposure and serious harms from invasive testing.
Its 2018 recommendation statement reported figures from older studies, mostly conducted in the 1980s and 1990s. Those numbers describe what happened in those studies, not the current individual risk for you.

Because these are historical study rates, do not use them as a prediction of what will happen to you. Instead, ask your clinician to explain the expected benefit, risks, and alternatives of the particular test being proposed.

8. What to do if the result is delayed
Result timing varies. NHS guidance notes that results may arrive the same day or take a few weeks, depending on the test. Follow any shorter timeframe given by your screening provider.
- Check the patient portal, voicemail, and written instructions for a stated review date.
- If the provider gave you a specific callback window, contact that service when the window passes.
- If you have heard nothing after a few weeks, contact the referring doctor, as the NHS advises.
- Tell the service if symptoms have appeared or changed, rather than waiting for the original appointment.
- Seek emergency care immediately for acute chest pain or a chest-pain-equivalent symptom.
A delayed message does not tell you whether the ECG is serious or reassuring. Only the responsible clinical team can provide that interpretation.
9. A practical script for contacting the clinic
You can say: “I was told my ECG screening result was abnormal. I would like the exact finding, an explanation in plain language, and the recommended follow-up timeframe. Was this a screening test or part of a diagnostic evaluation? Do you need information about my symptoms, medicines, prior ECGs, or other conditions? Who will contact me, and what should I do if I do not hear back?”

10. Common mistakes to avoid
- Treating “abnormal” as a diagnosis: Request the specific finding and professional interpretation.
- Waiting through acute chest symptoms: Use emergency services immediately; do not wait for a routine callback.
- Assuming a screening recommendation applies to every ECG: Screening asymptomatic adults and diagnosing symptoms are different situations.
- Choosing a consumer device to explain the result: Home monitors cannot establish the cause of an existing screening finding or replace clinical review.
- Agreeing to tests without understanding their purpose: Ask what each test is expected to clarify, its burdens, and how it could change care.
- Ignoring a missing result: Follow up with the referring doctor after the provider’s stated timeframe, or after a few weeks if none was given.
11. The decision path in one view
- Check for acute symptoms. For acute chest pain or a chest-pain-equivalent symptom, call 911 in the United States or your local emergency number elsewhere.
- Contact the ordering service. Request the actual report and the specific abnormal finding.
- Clarify the context. Confirm whether the ECG was screening in an asymptomatic adult or diagnostic testing prompted by symptoms or another concern.
- Give the clinician relevant information. Report symptoms, medicines, medical history, and earlier ECGs.
- Agree on a timeframe. Confirm whether follow-up is urgent, routine, or dependent on another review.
- Discuss proposed tests. Ask what question each test answers, its risks, alternatives, and how results will be communicated.
- Escalate if circumstances change. New or worsening symptoms require earlier clinical advice; acute chest symptoms require emergency care.
12. What a safe next step looks like
A safe response to an abnormal ECG screening result is neither panic nor dismissal. It is a documented conversation with the ordering clinician about the exact finding, the reason the ECG was obtained, your symptoms and history, and the next action with a clear timeframe. Emergency symptoms change that plan: seek immediate care instead of waiting for routine interpretation.
