A positive tuberculosis (TB) skin test or blood test means TB infection is possible. It does not by itself show whether you have inactive (latent) TB infection or active TB disease, and it does not automatically mean you are contagious. The next step is a medical evaluation with a health care provider or local TB program.
That evaluation usually includes your medical history, a physical examination and a chest X-ray. If symptoms or the X-ray raise concern, the clinician may order sputum and other laboratory tests. Do not ignore a positive result because you feel well, and do not assume it proves active TB.
What to do immediately after a positive result
- Contact a health care provider or local TB program. Tell the office whether the result came from a tuberculin skin test (TST) or an interferon-gamma release assay (IGRA) blood test, when it was performed, and where it was performed. The U.S. Centers for Disease Control and Prevention (CDC) says anyone with a positive TB test should be medically evaluated for TB disease. See the CDC clinical and laboratory diagnosis guidance.
- Report symptoms when you make the appointment. Cough, chest pain, fever, unexplained weight loss and unusual tiredness can occur with TB disease. A clinician should begin diagnostic evaluation when TB disease is suspected rather than waiting for a screening-test result. The CDC describes the symptom-based approach in its TB diagnosis guidance and clinical testing guidance.
- Bring relevant history. Tell the clinician about a known TB exposure, previous TB disease or treatment, prior positive tests, BCG vaccination, recent travel or residence in areas where TB is common, weakened immunity, pregnancy and other major medical conditions. These details affect which tests and treatment options are appropriate.
- Attend the complete evaluation. A positive screening result cannot determine the diagnosis on its own. Expect questions, an examination and usually a chest radiograph. Additional testing is selected according to your symptoms and findings.
- Ask what the result means after active disease is excluded. If the evaluation does not indicate TB disease, the clinician may diagnose latent TB infection and discuss preventive treatment.
- Keep written documentation. Save the test report, chest-X-ray result, clinician’s assessment and any treatment record. A TB test can remain positive after treatment, so documentation can prevent unnecessary repeat testing.
What a positive TB test does—and does not—tell you
Both a TST and an IGRA are screening tests for infection with TB bacteria. A positive result indicates that your immune system reacted in a way consistent with TB infection. Neither test can tell whether bacteria are inactive or actively multiplying, and neither can establish that you can spread TB to other people.

Because these conditions require different management, a positive test should be treated as a reason for evaluation, not as a diagnosis of active disease. The CDC explains that people with latent TB infection are infected with TB bacteria but do not have TB disease.

How the follow-up evaluation works
Medical history and physical examination
The clinician will ask about symptoms, possible exposure, previous testing or treatment, countries or communities where you have lived or traveled, immune-system conditions and medicines, and factors such as pregnancy. The examination helps determine whether there are signs that require an expedited work-up.
Chest X-ray
A chest X-ray is commonly used to look for findings compatible with pulmonary TB. A normal image supports—but does not by itself prove—the absence of active disease. An abnormal image can suggest TB or another condition and usually leads to additional testing. A chest X-ray alone cannot definitively diagnose TB disease.
Sputum and other laboratory tests
If you have respiratory symptoms or concerning imaging, the clinician may collect sputum. Possible tests include smear microscopy, nucleic acid amplification testing, culture and drug-susceptibility testing. The exact combination depends on the clinical situation. Culture is the microbiologic gold standard for confirming TB bacteria, while molecular and smear tests can provide other important information. Do not be surprised if you do not need every test listed above. Someone without symptoms and with an evaluation consistent with latent infection may not require sputum collection, whereas someone with symptoms or an abnormal X-ray may need a more extensive work-up.
Skin test versus blood test: why the type matters

Read the CDC’s patient information on the TB skin test, TB blood test and diagnosing TB. Do not arrange a second test on your own simply because the first was positive; ask the clinician whether another test would add useful information.
If the evaluation indicates latent TB infection
When active TB disease has been excluded and the positive test is consistent with infection, a clinician may diagnose latent TB infection. People with latent infection do not feel sick from TB and cannot transmit it, but the bacteria can become active later.

According to the CDC’s 2025 clinical overview, about 5% to 10% of people with latent TB infection develop TB disease during their lifetimes if they are not treated. CDC clinical guidance states that treatment for latent infection is about 90% effective at preventing progression to TB disease. Those figures describe population-level prevention, not a guarantee for one person.
Your clinician chooses whether treatment is appropriate and which regimen fits your age, health conditions, pregnancy status, other medicines and risk of progression. Active disease must be excluded before preventive treatment begins. Do not use leftover medicines or purchase a treatment online; TB drugs and follow-up require professional supervision.
If treatment is recommended, ask for the medication names, duration, monitoring plan, interactions with your current medicines and instructions for reporting problems. Keep proof of completion with your original test result.

Are you contagious?
Not necessarily. A positive TST or IGRA does not determine contagiousness. Latent TB infection is not contagious. Active TB disease can spread, especially when it affects the lungs or throat, and the clinician uses symptoms, imaging and laboratory evidence to assess that possibility.
Until you have been evaluated, follow the instructions given by your health care provider or public-health program. If you have symptoms, tell the scheduling staff so they can advise you about the safest way to attend. Local rules for reporting, work, school and contact investigations vary by jurisdiction; a state or local TB program can explain the requirements where you live.

BCG vaccination
BCG is a vaccine used in many countries. It can affect the interpretation of a TB skin test, whereas it does not affect a TB blood test. Tell the clinician when and where you received BCG. The history does not eliminate the need to evaluate a positive result.

A previous positive test or completed treatment
TB tests may remain positive in the future, including after treatment. Give the provider any old result or treatment-completion record. A documented prior positive result often means repeating the screening test is unnecessary; the clinician will decide what evaluation is still needed.
Recent exposure or weakened immunity
Report a known recent exposure and any condition or medicine that weakens your immune system. These circumstances can change the urgency and the testing plan. Someone with symptoms, recent exposure, pregnancy or complex medical conditions needs individualized clinical advice rather than a generic online schedule.
Keeping records and arranging help
Ask for copies of the positive test report, the date and type of test, the chest-X-ray report, the final assessment and any treatment documentation. Store them where you can retrieve them for future employers, schools, clinicians or public-health programs. A clear record helps prevent repeated testing and shows whether preventive treatment was completed.
If you cannot obtain an appointment, contact your state or local health department or TB control program. The CDC identifies these programs as sources of assistance, but referral pathways and reporting requirements differ across the United States. The guidance cited here is U.S. CDC guidance; people elsewhere should follow their own country’s health-service instructions.
When to seek prompt clinical attention
Contact a health care provider promptly if you have cough, chest pain, fever, unexplained weight loss or marked tiredness after a positive test, or if those symptoms develop while you are waiting for evaluation. Do not wait for a routine screening follow-up when TB disease is suspected. A clinician can determine whether you need same-day assessment, imaging, sputum testing or public-health support.
