When an enterprise AI pilot stalls, inspect the entire operating system around the model—not just its accuracy. Confirm the business outcome and accountable owner, trace production data and integrations, clear governance and security constraints, test realistic failure modes, and prove that people, support, monitoring and fallback procedures are ready. A successful demonstration is evidence of model capability; it is not evidence that the service can operate safely and reliably in the business.
What a stalled pilot actually tells you
A pilot that never reaches production has usually demonstrated only a narrow slice of readiness. It may have used a curated dataset, cooperative users, manual data preparation and engineering attention that will not exist after launch. Production introduces permissions, incomplete records, legacy interfaces, latency commitments, policy reviews, support queues and accountability for wrong answers.
Published conversion figures show why the transition deserves a formal diagnosis, but they are not a forecast for your company. Gartner’s 2024 AI Mandates for the Enterprise Survey, summarized on June 12, 2025, reported that an average 41% of generative-AI prototypes and 42% of non-generative-AI prototypes reached production. Those are survey averages; they do not mean that the remaining prototypes all failed for one reason, or that an individual pilot has those odds.

1. Confirm the problem, outcome and operating owner
Define the task in measurable terms
Write down the business decision or workflow step the system is supposed to improve. “Use AI to help support” is too broad. A usable statement identifies the user, input, action and measurable result—for example, reducing the time to classify a service request while keeping incorrect routing below an agreed ceiling.
- What task is in scope, and what is explicitly out of scope?
- Which metric proves value: cycle time, cost, revenue, quality, risk reduction or another measure?
- What quality threshold is acceptable for this task, and what types of errors are unacceptable?
- Who owns the outcome in the business, rather than merely owning the prototype code?
Assign post-launch accountability
A pilot can look promising while no department is willing to fund the service, accept its risks or change its workflow. Identify the person accountable for the business result, the technical service owner, the budget holder and the team that responds when the system is wrong. If those roles cannot be named, the project is still an experiment, regardless of model performance.
Ask what happens after a model, prompt, data source or policy changes. Ownership must include approval of changes, incident decisions and a decision to pause or retire the system. Treat return on investment as a hypothesis to validate with measured usage and outcomes, not as an assumed consequence of a good demo.

Compare pilot inputs with production inputs
Document every field the production workflow will need, where it comes from, who may access it, how fresh it must be and what happens when it is missing. Then compare that inventory with the pilot dataset. Curated examples often conceal incomplete records, inconsistent identifiers, stale values or manual cleaning that cannot be repeated at operating scale.
- Are the records representative of normal volume, languages, customers and edge cases?
- Do permissions allow the service account to retrieve the same information as a real user?
- Are identifiers consistent across the systems that must be joined?
- Does the design require real-time data, multiple streams or a reliable event order?
- What is the defined behavior when a source is unavailable, late or contradictory?
Inspect interfaces and failure behavior
Map the calls between the model, databases, business applications, identity systems and queues. Test timeouts, retries, rate limits, schema changes and duplicate messages. A response that is correct in isolation can still fail if it cannot be written back to the system of record, if a legacy interface rejects its format or if a downstream process cannot distinguish an automated result from a human one.

NIST’s 2026 industrial workshop report describes data standardization and interoperable infrastructure as foundational problems in some sectors. Participants identified data integration, governance, data quality and legacy integration as recurring challenges across maturity levels. The report also gives context-specific workshop assessments of 4.2 for Source and Delivery & Return processes, compared with 2.8 for Plan and 2.9 for Make; these figures are not a general enterprise benchmark.
Make lineage and permissions testable
For each production output, record the source records, transformation steps, model or prompt version and user authorization that produced it. Verify that sensitive fields are masked or excluded where required, and that logs do not become an uncontrolled copy of protected data. A pilot should not advance until data owners can explain what is collected, retained, shared and deleted.
3. Clear governance, policy, security and privacy gates
Classify the intended use and its risk
Determine whether the system recommends, generates, ranks, approves or directly executes an action. Identify the data classes involved, affected people, decision authority, retention period, third-party dependencies and required human review. A low-risk drafting assistant and an automated eligibility decision should not pass through the same approval path.
NIST’s Generative AI Profile states: “These considerations (Governance, Pre-Deployment Testing, Content Provenance, and Incident Disclosure) are relevant for voluntary use by any organization designing, developing, and using GAI and also inform the Actions to Manage GAI risks.” Translate those areas into named controls: an approval record, test evidence, provenance or labeling where appropriate, and an incident-disclosure process.
Turn policies into launch criteria
Check acceptable-use rules, privacy requirements, records-management obligations, procurement terms, licensing, data-residency restrictions and sector regulations. Define who may use the system, for which purposes, and what information must never be entered. Establish escalation for suspected leakage, harmful output, unauthorized access and material model drift.

NIST’s profile identifies auditing, change management, data protection, incident response, monitoring, risk-based controls and secure software development as governance considerations. It also notes that generative-AI deployments may require additional human review, tracking, documentation and management oversight.
A 2025 U.S. Government Accountability Office review of 12 selected federal agencies found that 10 said existing federal policies, including data-privacy policy, could present obstacles to adoption. Officials also cited technical resources and budget, current appropriate-use policies and rapid technology change. This statistic describes those selected agencies, not private enterprises or all government entities, but it illustrates how policy can block an otherwise workable prototype.
Test security abuse cases
Include prompt injection, jailbreak attempts, data poisoning, unauthorized retrieval, privilege escalation and sensitive-output leakage in the threat model. Define what the system should do when an instruction conflicts with a policy or when retrieved content is untrusted. Make security testing multidisciplinary: application security, privacy, legal, domain and operations staff should review the same evidence.
4. Make evaluation resemble real use
Build a task-specific evaluation set
Evaluate on representative production inputs, not only polished examples. Include ordinary cases, rare but costly cases, ambiguous requests, incomplete records, multiple languages or formats where relevant, and inputs containing sensitive information. Label the expected answer or acceptable range, the severity of each error and whether a human must review it.
Record separate results for quality, safety, security and usability. A single average score can hide catastrophic behavior in a small but important class of cases. Set a release threshold for each critical class and a stop condition for unacceptable failures. These thresholds must reflect the task’s consequences; no universal benchmark or percentage applies to every enterprise system.

Use red teaming and multidisciplinary review
GAO describes benchmark testing, multidisciplinary evaluation and red teaming as practices developers use before deployment. The same review notes that models can produce incorrect or biased outputs and can be vulnerable to prompt injection, jailbreaks and data poisoning. Test those weaknesses deliberately, then preserve the prompts, inputs, outputs, model version and reviewer decision as an auditable record.
Prove the human handoff
Specify when an answer is withheld, routed to a person or requires confirmation. Measure the volume and duration of reviews, the information shown to the reviewer and the rate at which reviewers override the system. If the queue would overwhelm the available staff, the pilot is not operationally ready even when its sampled accuracy is high.
5. Test production operations, not just model output
Define service expectations
Set expected latency, throughput, availability, cost per transaction and maximum recovery time. Exercise peak loads, provider throttling, network loss, dependency outages and partial responses. Verify that retries are safe and that a repeated request cannot create duplicate business actions.
Design fallback and rollback paths
Document the behavior when the model is unavailable, uncertain or outside its approved scope. Possible controls include routing to a manual queue, using a deterministic rule, returning “cannot answer,” or pausing an automated action for confirmation. A rollback must identify the last known-good model, prompt, configuration and data version, plus the person authorized to invoke it.
Monitor the complete system
NIST AI 800-4 groups post-deployment monitoring into functionality, operational, human-factors, compliance and other impact categories. Its definition is precise: “Post-deployment constitutes the period after an AI system is put into at least partial working operation in production or application.” For an AI feature embedded in a larger workflow, monitor more than model scores: permissions, data freshness, latency, queue size, overrides, harmful-output reports, drift and downstream business outcomes.

NIST also says monitoring methods and practices are still evolving, with open questions about information sharing, monitoring cadence and the balance between automated and human-validated monitoring. Choose a cadence and alert thresholds that fit the system’s risk, document why they are appropriate and revisit them after incidents or major changes; there is no single officially settled cadence or monitoring product.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.6. Check people, budget and workflow adoption
Staff the service, not only the build
List the roles required each week: data owner, product owner, platform or model engineer, security and privacy reviewer, domain reviewer, support analyst and incident lead. Confirm coverage for vacations, after-hours incidents and routine quality review. If the only person who understands the pipeline is a contractor or a pilot team member with no production allocation, schedule a knowledge-transfer and support plan before launch.
Budget the recurring work
Estimate inference or hosting charges, storage, data labeling, evaluation, security testing, support, retraining, vendor changes and compliance reviews. GAO’s selected agencies identified technical resources and budget as implementation challenges. Treat those as recurring operating costs, not one-time pilot expenses, and define who can approve an overage or scale back usage.

Show users where the AI result appears, what evidence accompanies it, how to correct it and when they must escalate. Provide role-specific training and a channel for reporting bad outputs. Measure adoption and override patterns after launch; a technically reliable tool can still fail if it adds clicks, obscures responsibility or conflicts with incentives.
How to choose the next intervention
When several fixes are possible, compare them explicitly instead of selecting the most visible model change. Score each candidate intervention against the following axes, using a documented scale that reflects your risk tolerance:
| Decision axis | Questions to answer | Evidence to collect |
|---|---|---|
| Representative, permitted data | Can the solution access complete, current data lawfully? | Data inventory, permission test, quality and freshness measures |
| Integration effort and reliability | Can it work with existing interfaces and failure handling? | Architecture test, load results, timeout and retry behavior |
| Privacy, security and policy exposure | Does the design satisfy use, retention and threat requirements? | Risk assessment, threat tests, legal and privacy approval |
| Task quality and failure handling | Does it meet critical-case thresholds and hand off safely? | Representative evaluation set, red-team results, reviewer study |
| Operational performance and ownership | Can staffed teams support it at the required service level? | Runbook, on-call roster, cost model, recovery exercise |
| Human-review burden | Is review volume feasible without creating a hidden bottleneck? | Queue simulation, review time, override and escalation rates |
| Monitoring and incident response | Can you detect, investigate and correct harmful change? | Metrics, alerts, audit trail, rollback and disclosure procedure |
This axis set synthesizes considerations identified by NIST and GAO; it is a practical decision aid, not a published comparative scoring standard.
A staged recovery plan
- Freeze the current baseline. Preserve the pilot code, prompts, model version, data snapshot, evaluation results, costs and known incidents so that changes can be compared.
- Interview the accountable owner and operators. Write the target outcome, error budget, decision authority, support coverage and funding requirement in one approved record.
- Trace one end-to-end production transaction. Follow identity, data retrieval, transformation, model call, human review, write-back, notification and audit logging. Mark every manual step and unhandled failure.
- Close the highest-risk gate first. Resolve blocked permissions, prohibited data use, unsafe outputs or missing rollback before optimizing quality or latency.
- Rebuild evaluation around real cases. Add edge cases, adversarial inputs and sensitive-data scenarios; set task-specific release and stop criteria.
- Run an operational rehearsal. Exercise peak load, dependency failure, human handoff, incident escalation and rollback with the teams that will operate the service.
- Launch narrowly with monitoring. Use a limited audience or workflow where authorized, review early outputs, and expand only when the agreed quality, safety and service evidence holds.
Signals that the pilot is not ready
- The business benefit is described as a demo impression rather than a measurable outcome.
- No named owner accepts responsibility for wrong outputs or ongoing cost.
- The test set is synthetic, hand-cleaned or materially easier than production data.
- Access to required records depends on an exception that has not been approved.
- There is no documented fallback, rollback version or incident contact.
- Reviewers cannot tell why the system produced an answer or what evidence it used.
- Support, security and privacy staff are expected to help “after launch” without allocated time.
- Monitoring covers model accuracy but not data freshness, workflow effects, human overrides or compliance.
What production readiness looks like
A production decision should be supported by a connected evidence package: an owned business outcome; a data and integration map; approved use, privacy and security controls; representative quality and abuse testing; service-level and cost results; staffed runbooks; a human-review design; and monitoring with rollback and incident procedures. The package should state unresolved risks and who accepted them.
GAO reported that inventories in 11 selected federal agencies grew from 32 generative-AI use cases in 2023 to 282 in 2024—about a nine-fold increase. That finding shows how quickly experimentation can expand, not that every use case is ready for deployment. The discipline is to make each transition deliberate: prove that the surrounding system, controls and people can sustain the capability after the pilot team steps away.
