Ask how the CDX2 stain was interpreted in your tumor, how it fits with the other stains and the tumor’s appearance, and whether it changes the working diagnosis. A positive result can support intestinal differentiation, but it does not prove that a tumor began in the colon or elsewhere in the gastrointestinal tract. A negative result does not, by itself, rule out a colorectal or other intestinal origin.
Use the questions below to guide a conversation with your pathologist or oncology team. They are prompts, not a list of tests everyone needs. A pathologist interprets the stain alongside the tissue findings, clinical history and other diagnostic information; this article cannot interpret an individual report.
What CDX2 tells the pathologist
CDX2, or caudal-type homeobox 2, is a transcription factor associated with intestinal epithelial differentiation. In immunohistochemistry (IHC), an antibody stain is used to see whether tumor cells show CDX2 immunoreactivity under the laboratory’s interpretation criteria. The stain can help characterize a tumor’s phenotype and inform consideration of its possible origin.

A stain may show that the cells have features associated with intestinal differentiation; that finding alone does not establish where the cancer started. Tumors outside the gastrointestinal tract can also express CDX2 when they show intestinal differentiation. The meaning of a result therefore depends on the overall case, not just the word “positive” or “negative” on a report.

Mayo Clinic Laboratories describes its CDX2 offering as a technical-component immunostain and states that interpretation belongs in the context of the patient’s clinical history and other diagnostic tests, by a qualified pathologist. That is a useful way to frame the conversation: ask what the stain contributes to the interpretation, rather than treating it as a stand-alone answer.
What to ask about the result itself
Is the stain positive, negative or focal?
Ask the pathologist to translate the report’s wording into plain language. Find out whether the stain is positive, negative, or present only in a focal pattern, and whether the staining is in the tumor cells. If the report uses terms you do not recognize, ask what they mean in this specific specimen.
It can also help to ask whether the amount and distribution of staining matter here. For example: “Is the staining diffuse or focal, and strong or weak? Does that pattern fit the tumor’s appearance?” The point is not to decide for yourself what a particular pattern means, but to understand how the pathologist used it in the interpretation.

What does a positive result support—and what does it not establish?
A positive CDX2 stain may support an intestinal-type phenotype. Ask which possible origins the result supports in your case, which alternatives remain, and how much weight the pathologist gives CDX2 relative to the other findings. A positive result by itself does not prove that the primary cancer is in the colon, or even establish a particular primary site.

A practical way to ask is: “What does CDX2 add to the diagnosis, and what can’t it tell us on its own?” This invites an explanation of both the useful clue and its limits without assuming the result has settled the question of origin.
What does a negative result mean in this specimen?
A negative CDX2 result does not independently exclude an intestinal or colorectal origin. Tumor heterogeneity and poor differentiation can complicate IHC patterns; a small or poorly differentiated specimen may also limit how useful the stain is. Ask whether those considerations apply to the tissue that was examined and whether other findings raise or lower suspicion despite the negative result.
You can ask: “Does the negative stain make one origin less likely, or is it not decisive here?” The answer should be tied to the specimen and the full diagnostic picture, not generalized from the result alone.
How CDX2 fits with the rest of the pathology
Ask about the complete marker pattern
CDX2 is more informative alongside the tumor’s microscopic appearance and other markers than in isolation. Ask the pathologist to explain how CDX2 fits with CK7, CK20 and any other stains performed, and which results most influenced the interpretation. If more than one origin is being considered, ask how the complete pattern compares with those possibilities rather than asking the team to rank them from CDX2 alone.

The European Society for Medical Oncology (ESMO) describes the classic colorectal immunophenotype as CK7-negative, CK20-positive and CDX2-positive; CK20 and CDX2 are usually diffuse and strong in that pattern. ESMO reports that at least 80% of colorectal cancers show this classic phenotype. That figure describes the frequency of a marker pattern among colorectal cancers. It is not the probability that an individual tumor came from the colon, and it is not proof of a primary site.
Ask how the tumor’s appearance and clinical information fit
Ask how the microscopic appearance, the IHC pattern, the specimen site, your clinical history and imaging fit together. If a particular alternative origin is under consideration, ask which findings support it and which findings do not fit as well. The sources of evidence may not all point in the same direction; the pathologist and oncology team can explain how they handle that uncertainty in the working diagnosis.
ESMO describes CDX2 as part of an initial IHC panel in selected workups of adenocarcinoma in the liver, alongside CK7, CK20 and TTF1, with other markers selected according to the differential diagnosis. This is an example of a context-dependent workup, not a standard panel that applies to everyone. The appropriate evaluation depends on the specimen site, morphology, clinical history and suspected alternatives.

Questions about whether the sample was adequate
The interpretation is only as useful as the material available to examine. Ask whether there was enough suitable tumor tissue, whether the stain controls were satisfactory, and whether sampling or tissue handling could limit confidence. These are reasonable questions when the specimen is small, the result is unexpected, or the report describes a limited or difficult-to-interpret pattern.

Ask the pathologist to distinguish between an inconclusive result and a result that is interpretable but not definitive about the primary site. Those are not the same thing: a stain can be technically interpretable while still providing only one piece of a broader diagnostic assessment. If adequacy or controls are a concern, ask what that limitation means for the conclusion drawn from this sample.

Ask what additional review or testing could clarify the origin, and whether the answer could change the working diagnosis or treatment plan. Depending on the case, the discussion may include additional stains, molecular testing, imaging or clinical evaluation. These are possibilities to discuss with the clinical team, not recommendations that every patient should pursue.
A useful follow-up is: “What uncertainty would another test address, and would its result change what happens next?” That keeps the conversation focused on the purpose of further workup. The pathologist and oncologist can explain whether unresolved questions are important for management or whether the existing evidence is sufficient for the current plan.

You can also ask who will explain how the pathology result fits the overall diagnosis and next steps. The goal is a clear account of what is known, what remains uncertain and who is coordinating the explanation.

A focused set of questions to bring
You do not need to ask every question below. Choose the ones that match what you want clarified, and bring the pathology report if you have it.
- “What does CDX2 mean in my report? Is it positive, negative or focal, and is the staining in the tumor cells?”
- “Does this result support a possible origin, or establish one? What leading alternatives remain?”
- “How do CDX2, CK7, CK20 and any other stains fit together? Which findings most influenced the interpretation?”
- “Does the amount or pattern of staining matter here? Is it diffuse or focal, strong or weak, and does it fit the tumor’s appearance?”
- “Was there enough suitable tumor tissue, and were the stain controls satisfactory? Could sampling or tissue handling limit confidence?”
- “Could any additional review or testing clarify the origin, and would it change the working diagnosis or treatment plan?”
- “Who will explain how the pathology result fits my overall diagnosis and next steps? Have the pathologist and oncologist discussed any difference between the tissue findings and clinical picture?”
How to understand the answer
A useful explanation should connect the stain to the rest of the case: what the pathologist observed, how the other markers and tumor morphology fit, whether the sample was adequate, and what uncertainty remains. If you hear that CDX2 is “consistent with” or “supports” an origin, ask whether that means it is one possibility among several or the leading interpretation in the full context.
Before the conversation ends, make sure you know whether the CDX2 result changed the working diagnosis, whether any unanswered question affects the next step, and which clinician will discuss the overall plan with you. The result is one part of tumor classification; its significance comes from the integrated interpretation, not from the stain in isolation.
