Bring a written list of questions to your oncology appointment and ask the study team to answer them for the exact vaccine, cancer, treatment history, and protocol under consideration. Focus on four decisions: what the trial is designed to learn, whether you are eligible and can wait safely, how its uncertain benefits and risks compare with other care, and what participation will require from you and your family. A trial is an option to evaluate with your clinicians—not a promise that treatment will work or outperform standard care.
First, clarify what “cancer vaccine” means
A cancer treatment vaccine is an immunotherapy for someone who already has cancer. It is different from a preventive vaccine that protects against an infection associated with cancer. Treatment vaccines are intended to help the immune system recognize targets related to a person’s cancer, but the exact target, formulation, dose, schedule, and combination drugs depend on the study protocol.
The National Cancer Institute (NCI) describes sipuleucel-T for some people with prostate cancer and talimogene laherparepvec (T-VEC, Imlygic) for some people with melanoma. These approved examples do not show that any vaccine trial is suitable for you; they illustrate that “cancer vaccine” can refer to different approaches.
| Example named by NCI | Context described by NCI | What to ask about an investigational vaccine |
|---|---|---|
| Sipuleucel-T | Some people with prostate cancer | What cancer target and treatment setting does this study address? |
| Talimogene laherparepvec (T-VEC, Imlygic) | Some people with melanoma | Is the product a vaccine alone, or is it combined with another treatment? |
Side effects vary with the vaccine type and dose, the cancer and its extent, and your overall health. Flu-like symptoms are possible with some treatment vaccines, while other products or combinations can create different or more serious problems. Do not assume that the side effects of an approved product predict those of an investigational vaccine. Ask about the precise study treatment and every drug given with it.

Prepare before the appointment
Preparation helps your oncologist and the study coordinator give protocol-specific answers instead of general information. Gather your pathology reports, imaging reports and discs, operative notes, treatment dates, medication list, allergy list, and records from other hospitals. Include prior immunotherapies, chemotherapy, radiation, targeted treatments, transplant history, autoimmune conditions, infections, and any treatment complications.
Write down your current symptoms, how they affect daily activities, and the dates of recent scans and laboratory tests. Bring the names and contact details of other clinicians who manage your heart, lungs, kidneys, immune system, or other conditions. Ask whether the trial needs tumor tissue, molecular testing, or records transferred before formal screening.
Decide who should attend the discussion. A family member or friend can take notes, and a second medical opinion may help if your oncologist does not know of a suitable study. You can ask the clinic for the consent form and patient-facing study information before deciding.

Questions about the trial’s purpose and design
Start by identifying exactly what researchers are trying to learn. Ask these questions in your own words:

- What is the primary purpose of this trial: testing safety, measuring an immune response, seeing whether the treatment controls the cancer, or answering another question?
- Why do researchers think this vaccine might help my cancer, and what are the scientific or clinical reasons it might not help?
- What phase is the study, and what does that phase mean about what is already known and what remains uncertain?
- Is the vaccine given by itself, with standard treatment, or with another investigational treatment?
- Is assignment randomized? Could I receive a comparison treatment or placebo, and how would that affect my care?
- What findings will show the team whether treatment is working, which scans or tests will be used, and when will results be discussed with me?
Ask to review the protocol summary and the patient consent materials. NCI notes that a protocol describes the treatment, possible side effects, visits, tests, and procedures. The consent discussion should make clear which parts are standard care and which are being done because you are in the study. NCI also identifies phase 2 studies as studies that assess whether a new treatment appears to work against the cancer; the study team should explain what the phase means for this particular product rather than relying on the label alone.
Questions about eligibility and timing
Eligibility is determined by the protocol, not by a preliminary phone call or a general impression that you seem healthy enough. Ask which details of your diagnosis and treatment history matter:
- Does the study require a particular cancer type, stage, measurable disease, tumor marker, or genetic change?
- Are there age limits, organ-function requirements, infection tests, performance-status rules, or restrictions related to pregnancy or contraception?
- Which previous treatments are required, allowed, or disqualifying, and how long must I wait after surgery, radiation, chemotherapy, immunotherapy, or another trial?
- Do I need fresh tumor tissue, archival tissue, genetic testing, or records from another hospital before screening?
- Which laboratory, imaging, or physical examinations are part of screening, and how long does screening usually take?
- Who makes the final eligibility decision: the local oncologist, the study investigator, or a central review team?
- Can my oncologist speak directly with the study coordinator about my records and treatment plan?
Ask how quickly you must decide and whether waiting for screening could affect a standard treatment window, surgery date, symptom control, or another trial. A study team may find that you do not meet one criterion even when the trial initially sounded appropriate. Ask what treatment can begin or continue while eligibility is being determined.

Questions about possible benefit, risk, and alternatives
Ask for information that matches your cancer and treatment history, not a general response rate from a different disease or phase. Useful questions include:

- What evidence exists for people whose cancer and prior treatments resemble mine?
- How many participants have received this exact vaccine and combination so far, and what outcomes have investigators observed?
- Which benefits are possible, and what remains unknown about the chance, duration, or timing of benefit?
- What side effects are known or suspected for this vaccine, each combination drug, and the procedures used to give or monitor treatment?
- Which symptoms require an urgent call, an emergency visit, or stopping a dose?
- Who do I call during office hours and after hours, and how quickly will someone respond?
- What monitoring, blood tests, scans, observation periods, or hospital visits might be needed to manage toxicity?
- How does this study compare with standard treatment, another available trial, active surveillance, supportive care, or no treatment in my situation?
- What could happen if I choose standard treatment, delay treatment, or decide not to have treatment now?
Every trial has a different risk profile. As NCI puts it, “Every clinical trial is different, which means risks can also differ.” A study treatment may not be better than standard treatment and can have serious side effects. Ask the investigator to distinguish effects caused by the vaccine from effects caused by accompanying drugs, biopsies, infusions, or other procedures.
Understand visits, daily life, and costs
Before consenting, map the practical workload. Ask how often you must come to the clinic during screening, treatment, and follow-up; how long a typical visit lasts; and whether any dose requires observation or an overnight hospital stay. Confirm where treatment and scans occur, whether any follow-up can be remote, and which appointments must be in person.
Discuss the financial boundaries in writing:
- Which vaccine doses, study tests, scans, procedures, and extra visits are paid by the sponsor?
- Which routine cancer care, medications, laboratory work, or treatment of side effects is billed to insurance?
- What deductibles, co-payments, co-insurance, or non-covered services could remain yours?
- Who at the study site can help you ask the insurer about coverage, and when should that conversation happen?
- Are travel, parking, lodging, meals, childcare, or time away from work likely to be needed?
Ask who coordinates your cancer care during the trial and after it ends. Clarify whether your usual oncologist remains involved, who receives scan results, and how urgent problems are communicated between the study site and your local team. Ask whether you will receive a summary of your participation or the study’s results when they become available.
Compare the trial with your other choices
Use a side-by-side worksheet with your oncologist. The goal is not to declare one option generally superior; it is to expose the trade-offs for your circumstances.

| Decision area | Questions to record for the vaccine trial | Questions to record for standard care or another option |
|---|---|---|
| Goal and evidence | What outcome is the study designed to measure, and what evidence applies to my cancer? | What outcome is expected from the alternative, and how established is it for my situation? |
| Uncertainty | What is unknown because of the study phase, small experience, or combination regimen? | What uncertainties remain about effectiveness, durability, or future treatment choices? |
| Side effects and monitoring | What symptoms, tests, observation, or hospital care could be required? | What toxicity and monitoring would the alternative involve? |
| Eligibility and timing | What criteria must I meet, how long is screening, and could waiting change my options? | When can treatment start, and what prerequisites apply? |
| Schedule and travel | How many visits, procedures, and trips are expected? | How does the alternative fit my location, work, caregiving, and energy? |
| Costs | Which expenses are paid by the sponsor, insurance, or me? | What coverage and out-of-pocket costs should I expect? |
| If treatment stops | What happens if the vaccine fails, causes toxicity, the study closes, or I withdraw? | What treatments remain available if this option is unsuccessful or intolerable? |
Ask for the names of other trials that could fit your diagnosis and for the reasons each is or is not realistic. A second opinion can be useful when the first team is unfamiliar with an appropriate study.
Consent, privacy, and your right to choose
The study team should explain the trial, answer questions, and give you time to read the consent form before you sign. Ask how your health information and tumor samples will be protected, who can access them, whether samples may be stored for future research, and what data may be shared with regulators or collaborators.
Ask these control questions:
- What alternatives do I have if I do not join?
- Can I take the consent form home and discuss it with family or another clinician?
- Can I leave the study later, and what steps are needed to stop treatment safely?
- If I withdraw, which follow-up visits or safety checks will still be requested?
- How would leaving affect my relationship with my oncologist and access to other care?
Signing consent does not remove your ability to change your mind. If you are considering withdrawal, speak with the investigator so treatment can be stopped safely and another care plan can be arranged.

- Initial discussion: Your oncologist or a coordinator checks whether the study appears relevant and explains the basic schedule.
- Record review: The team verifies pathology, scans, prior treatments, medications, and medical history against the written criteria.
- Formal screening: You may have examinations, blood tests, imaging, tumor testing, or other protocol-required procedures.
- Eligibility decision: The investigator or designated review team confirms whether every required criterion is met. A preliminary conversation is not enrollment.
- Consent and baseline assessment: You review the consent materials, ask questions, and complete baseline tests only after deciding voluntarily to participate.
- Treatment and monitoring: The protocol sets the dose, visit schedule, safety checks, scans, and rules for holding or changing treatment.
Keep a contact sheet with the coordinator’s name, clinic number, after-hours number, treatment dates, and instructions for urgent symptoms. Report new symptoms promptly rather than waiting for the next scheduled visit.

If you are not eligible or decide not to join
Ask the team which criterion was not met and whether another study, standard treatment, or a referral could be appropriate. Eligibility can change when a new trial opens or when additional tumor information becomes available, but do not delay time-sensitive care while waiting for an uncertain opening.
If you decline, ask for a written plan covering your next treatment decision, symptom management, follow-up scans, and the person to contact with questions. Choosing standard care, another trial, supportive care, or no treatment after an informed discussion is still your decision.
A concise appointment checklist
- Bring records, medication and allergy lists, and a note-taker.
- Ask what the vaccine is, what the trial is designed to learn, and what phase it is.
- Confirm eligibility criteria, screening tests, decision deadlines, and effects on current care.
- Request study-specific information about benefit, side effects, urgent symptoms, and monitoring.
- Map visits, hospital stays, travel, caregiving, work disruption, insurance, and personal costs.
- Compare the trial with standard treatment and other trials using the same decision categories.
- Read the consent form, ask about privacy and withdrawal, and take time before signing.
The most useful appointment outcome is a clear, personalized plan: what the trial could realistically offer, what it could cost in health and time, what alternatives remain, and what happens next if you join, decline, or stop.
