A targeted treatment is chosen because your cancer may have a feature—such as a gene change or protein—that the medicine is designed to affect. That label does not guarantee a match or a response. Before agreeing to treatment, ask your oncologist to connect the test result, the treatment’s evidence, expected benefits, risks, monitoring, practical demands, and alternatives to your individual situation.
Bring a written list, your complete medication and supplement list, and a trusted person if you want help taking notes. The National Cancer Institute (NCI) recommends preparing questions for cancer-treatment appointments; its treatment-question guide is a useful starting point.
Start with the treatment’s purpose
Ask first what the proposed treatment is intended to accomplish for you. The answer may be to cure the cancer, reduce the chance of recurrence after another treatment, slow or control advanced disease, shrink a tumor before surgery, relieve symptoms, or achieve another defined goal. The same medicine can have a different purpose in different cancers or stages.
Ask: “What is the goal of this treatment for me, and how will we know whether that goal has been reached?” Ask how quickly a benefit might appear and what decision would follow if the cancer remains stable, improves, or worsens. A clear goal makes later scan results and treatment decisions easier to interpret.

Your diagnosis, stage, previous treatments, overall health, organ function, and personal priorities all affect the recommendation. An answer that applies to one cancer type, biomarker, or country may not apply to yours.
Understand the biomarker and the test behind the recommendation
Biomarker testing looks for cancer features—such as genes, proteins, or other substances—that may help select treatment. Ask your oncologist to name the exact feature and test result supporting the recommendation, rather than relying on the word “targeted.”
Questions about the result
- Which biomarker did my tumor test find?
- What specimen was tested, when was it collected, and which laboratory performed the test?
- Is the result clearly actionable, or is it uncertain, benign, or not known to guide treatment?
- How strong is the evidence that this result predicts benefit from this medicine?
- Could the result differ between the original tumor and a newer biopsy or metastasis?
A matching biomarker points to a possible treatment; it does not prove that treatment will work. Other tumor changes, resistance mechanisms, the amount and location of disease, and your health can affect the outcome. Some tumors have findings for which no established treatment exists. Testing can also be limited by whether a biopsy is safe, whether there is enough tissue, access to a suitable laboratory, trial eligibility, and coverage.
Do not confuse tumor testing with inherited testing
Tumor (somatic) testing examines cancer cells. If a result raises the possibility of an inherited mutation, your clinician may recommend confirmatory germline testing and genetic counseling. An inherited result can have implications for relatives and future care, so ask what kind of test produced each result and what follow-up is needed. The NCI explains these distinctions in its biomarker-testing overview.

Clarify approval, off-label use, or a clinical trial
Ask how the proposed use is classified in your jurisdiction: Is the medicine approved for this cancer and situation, being used off-label, or offered through a clinical trial? An approved drug may still be approved for a different cancer, stage, biomarker, dose, or line of treatment. Off-label use is not automatically inappropriate, but you should understand the evidence and why it is being proposed. A trial may provide access to an investigational approach under a defined protocol, with eligibility rules and additional consent information.
Ask for the name of the study or the approved indication, the evidence supporting this use, and whether another treatment or trial would be reasonable. The NCI describes how biomarker findings can lead to an approved treatment, an off-label option, or a trial, while some findings do not guide treatment decisions.
Questions about evidence
- What studies included people with my exact cancer and biomarker?
- Were participants previously treated like me?
- What benefit was observed, over what time, and how certain is that estimate?
- Does the evidence show tumor shrinkage, longer control, symptom relief, or another outcome?
- What important limitations make the study results less applicable to me?
Do not substitute a percentage from another cancer or medicine for evidence about your treatment. Ask for information that matches your diagnosis, treatment line, and health circumstances.
Compare the proposed treatment with realistic alternatives
Ask what you could choose instead, including standard treatment, active monitoring when clinically appropriate, a clinical trial, or a second opinion. The choice is not simply “targeted therapy versus chemotherapy.” Surgery, radiation, immunotherapy, hormone treatment, chemotherapy, combinations, and supportive care may be considered depending on the cancer.

| Comparison point | Question to ask |
|---|---|
| Goal and evidence | What is each option trying to achieve, and how strong is the evidence for my cancer? |
| Expected benefit and uncertainty | How soon might we see an effect, and what is unknown? |
| Risks | What common and serious effects are most relevant to me? |
| Administration | Is it a pill, infusion, injection, or combination, and how often must I receive it? |
| Monitoring burden | How many visits, blood tests, scans, or other checks are required? |
| Daily life | How might work, travel, exercise, caregiving, sexuality, or fertility be affected? |
| Status and access | Is it approved, off-label, or investigational, and can I obtain it in my setting? |
| Timing and cost | How quickly can it start, and what coverage or financial barriers could delay it? |
Ask which trade-offs matter most given your priorities. A second opinion can help when the decision is complex, the use is off-label, or the evidence is limited.
Get the exact administration and monitoring plan
Ask how the treatment is given, where it is given, how long each visit lasts, and how long a treatment cycle continues. For an oral medicine, ask about the daily schedule, missed doses, storage, handling, and what to do if you vomit after taking it. For an infusion or injection, ask about premedication, observation, transportation, and whether a port or other access device is needed.
Ask which baseline checks must happen before the first dose—for example, blood counts, kidney or liver tests, heart testing, blood pressure, pregnancy testing, or an eye examination when relevant. Ask what tests recur during treatment and who reviews the results. Confirm how prescriptions are obtained and how refills are authorized.
Define how response will be judged
Monitoring may include visits, physical examinations, blood tests, imaging, and other tests. Ask when the first assessment will occur, which findings count as improvement or progression, and whether symptoms alone can be misleading. Ask what happens if a scan is unclear and when the plan will be reconsidered. The NCI’s targeted-therapy information explains that monitoring is used to assess both response and side effects.

Make a treatment-specific side-effect and urgent-call plan
Targeted therapies can cause serious side effects; they are not automatically gentler than other cancer treatments. The NCI states: “The side effects that you may have depends on the type of targeted therapy you receive and how your body reacts to it.” The exact drug, dose, combinations, and your medical history determine what matters.
Ask for the medication guide or prescribing information for the specific medicine and request both a written and verbal plan. Across targeted therapies, NCI lists possible effects including diarrhea, liver problems, blood-clotting or wound-healing problems, high blood pressure, fatigue, mouth sores, nail changes, hair-color changes, and skin problems. This is not a universal list or prediction for your drug.
Questions that prevent confusion
- Which effects are common during the first days or weeks?
- Which symptoms require a same-day call, an immediate call, or emergency care?
- What temperature, blood-pressure reading, bowel pattern, rash, pain, or breathing change should trigger action?
- What can prevent or treat each expected effect?
- Should I pause, reduce, or continue a dose while waiting for advice?
Ask whom to contact during office hours, overnight, and on weekends, and what to do if you cannot reach the team. Follow your own team’s emergency instructions; severe breathing trouble, chest pain, fainting, sudden confusion, uncontrolled bleeding, or other emergencies require emergency services rather than waiting for a callback.

Review every medicine, supplement, and health condition
Give the oncology team a complete list of prescriptions, over-the-counter medicines, vitamins, herbs, recreational substances, and occasional products. Include doses and how often you take them. Ask whether anything could change the targeted medicine’s absorption, blood levels, heart rhythm, blood pressure, bleeding risk, liver function, or immune response. Do not stop a prescribed medicine or supplement without guidance.

Tell the team about allergies, kidney or liver disease, heart problems, prior blood clots, uncontrolled blood pressure, current infections, dental procedures, and planned surgery. Ask whether the treatment affects contraception, pregnancy, breastfeeding, sperm or egg preservation, or timing of a procedure. If fertility matters to you, raise preservation options before treatment begins.
Plan for costs, coverage, and practical support
Ask whether your insurer covers the medicine, biomarker testing, scans, laboratory work, infusion services, and management of side effects. Coverage rules differ by country, insurer, indication, and approval status. Ask the oncology practice who handles prior authorization and what happens if coverage is denied or delayed.
Request an estimate of copayments, deductibles, travel, parking, childcare, time away from work, and supportive medicines. Ask about the hospital’s financial counselor, social worker, or patient navigator. If a clinical trial is discussed, ask which trial-related care is covered by the study and which costs remain your responsibility. Do not delay urgent care while resolving a billing question; ask the team how to proceed safely.

Ask what happens if the treatment stops working
Resistance can develop even when a treatment initially helps. Ask what signs would suggest progression, whether a repeat biopsy or new molecular test could explain resistance, and what next options might exist. Possible next steps depend on the cancer and may include another medicine, a combination, local treatment, a clinical trial, or a focus on symptom control. You are asking for contingency planning, not predicting that the treatment will fail.
Ask how decisions will be made if scans show mixed results, stable disease, or a small change. Clarify whether treatment can continue while another test is arranged and which symptoms should prompt an earlier review.
Prepare for the appointment
Write your questions in priority order, because time may be limited. Bring your pathology and molecular-test reports if you have them, a medication and supplement list, allergies, insurance information, and a record of symptoms. Ask whether you may record the visit according to local rules. A trusted person can listen, take notes, and help you remember follow-up instructions.
- Write the treatment’s name, the proposed goal, and the biomarker or test result in your notes.
- Ask whether the use is approved, off-label, or part of a trial, and request the evidence supporting it.
- Record the schedule, baseline tests, monitoring dates, and the first response assessment.
- Write down common effects, emergency symptoms, contact numbers, and after-hours instructions.
- List alternatives and compare their benefits, risks, logistics, timing, and coverage.
- Confirm who will arrange testing, prior authorization, prescriptions, and follow-up.
- Repeat the plan in your own words and ask for written instructions before leaving.
Use the NCI resources on questions to ask about cancer treatment, questions about advanced cancer, and why people participate in clinical trials to add questions relevant to your situation.
