Focused ultrasound can reduce a specific Parkinson’s symptom by creating a permanent, MRI-guided lesion in a carefully selected brain target. It is not a cure, not a home treatment, and not one single procedure: thalamotomy, pallidotomy and subthalamotomy address different symptom patterns and have different evidence, approvals and risks. Before deciding, ask your neurologist to identify the exact target, explain what is unlikely to improve, compare the option with deep brain stimulation (DBS) and medication-based therapies, and verify the current authorization that applies where you live.
Start with the exact procedure and target
“Focused ultrasound” describes a method, not a universal Parkinson’s treatment. During an MRI-guided procedure, a specialist focuses sound waves through the skull to heat a small area of brain tissue. The heating creates a lesion that interrupts part of a movement circuit. There is no incision or implanted electrode, and the patient generally remains awake rather than receiving general anesthesia. “Non-invasive” therefore means no surgical opening or implanted hardware; it does not mean that brain tissue is left unchanged.
The lesion is permanent. Unlike an implanted stimulator, it cannot be reprogrammed or switched off if your priorities change or a side effect appears. The target and the symptom it is meant to treat should be written down before you consent.

| Unilateral thalamotomy | Predominantly medication-refractory tremor | Evidence includes a small sham-controlled trial; it does not establish a general success rate for every person with Parkinson’s. |
| Unilateral pallidotomy (GPi) | Selected motor complications such as dyskinesia, “off” time, rigidity, bradykinesia, dystonia or tremor | The U.S. FDA’s 2021 Exablate Neuro supplement specified unilateral pallidotomy for adults at least 30 with advanced idiopathic Parkinson’s disease and medication-refractory moderate-to-severe motor complications, when the GPi is identifiable and accessible. This is a device- and indication-specific label. |
| Subthalamotomy (STN) | Slowness and stiffness, usually in markedly asymmetric disease | The Parkinson’s Foundation describes a higher dyskinesia risk. A 2022 European guideline recommended unilateral MR-guided focused ultrasound STN treatment only within studies or registries at that time; regulatory status must be checked for your location and device. |
Ask: “Which exact target and side are you proposing, and which of my symptoms is that target expected to change?” A treatment aimed at tremor should not be presented as if it will automatically improve balance, speech, cognition, pain or every motor fluctuation.

Find out whether you are a plausible candidate
Candidacy is an individualized specialist judgment, not a checklist you can complete online. Your movement-disorders neurologist and the treating center will consider the diagnosis, symptom pattern, medication response, imaging and general health. The center must be able to visualize and safely reach the proposed target; skull characteristics and MRI findings can affect technical feasibility.
Questions about your diagnosis and symptom pattern
- Is my diagnosis definitely idiopathic Parkinson’s disease, or could an atypical parkinsonian disorder change the expected benefit?
- Which symptom is disabling enough to justify a permanent lesion: tremor, dyskinesia, “off” periods, rigidity, slowness or dystonia?
- Does the symptom remain troublesome despite an adequate medication trial, and is it clearly measurable before and after treatment?
- Are my walking, balance, speech, swallowing, thinking or mood problems likely to remain, or could this target worsen them?
Questions about tests and medical history
- What MRI or other imaging is required, and does it show an accessible target?
- How do blood thinners, bleeding risks, seizures, prior strokes, uncontrolled blood pressure or other conditions affect safety?
- Could my skull or implanted medical hardware prevent the ultrasound from reaching the target?
- Would treatment on one side leave me with an imbalance between the two sides, and what would that mean if symptoms progress?
Eligibility rules differ by device, country, target and date. A center may discuss an investigational procedure even when a different target has a regulatory authorization. Ask the clinician to show you the current device indication that applies to your proposed treatment and to state plainly whether the procedure is routine care, part of a registry or offered in a clinical study.
Ask what improvement is realistic
Ask for a prediction tied to your priority symptom, not a broad promise that you will “feel better.” Useful questions include:

- What degree of improvement has been observed in people with my symptom pattern and disease stage?
- How soon would benefit appear, and how durable is the follow-up evidence?
- Which symptoms are unlikely to improve because they arise from circuits outside the target?
- Will I still need Parkinson’s medication, and could doses change?
- What is the plan if the untreated side or a different symptom becomes dominant later?
The best-known tremor figure comes from a specific early study, not an individual forecast. In a 2017 two-center, double-blind, sham-controlled pilot trial, 27 people with tremor-dominant Parkinson’s disease received unilateral focused-ultrasound thalamotomy or sham treatment. At three months, the median on-medication treated-hand tremor score improved 62% after treatment versus 22% after sham. The sample was small, follow-up was short, and investigators noted a substantial placebo response. Two treated participants (8%) developed mild hemiparesis related to internal-capsule heating early in the study; temperature monitoring was subsequently added in another axis. The figures cannot be converted into your personal probability of success.
Evidence for other targets is separate. A sham-controlled New England Journal of Medicine trial of subthalamotomy enrolled selected people with markedly asymmetric Parkinson’s disease and reported speech and gait disturbances, treated-side weakness and dyskinesia among adverse events. Those results should not be generalized to bilateral or broadly distributed disease.
Understand the evidence and its uncertainty
Ask your neurologist to distinguish three things: published clinical evidence, the device’s legal indication and the center’s own experience. They can overlap, but they are not interchangeable.

What the 2022 European guideline concluded
The 2022 European Academy of Neurology/Movement Disorder Society–European Section guideline judged the evidence for focused-ultrasound procedures limited. Its review described one randomized controlled trial of unilateral STN subthalamotomy with one-year data and adverse events including dyskinesia, weakness, facial asymmetry, speech disturbance, gait disequilibrium, somnolence and binge eating. At that time, it recommended unilateral MR-guided focused ultrasound of the STN only within studies or registries. The guideline was an assessment of evidence available in 2022; later publications or regulatory decisions may change practice, so ask which newer data the center relies on.
Procedure-specific uncertainty matters
NICE’s public information on MRI-guided focused-ultrasound subthalamotomy says there is not enough evidence to be sure how well it works or how safe it is. That statement concerns subthalamotomy and should not be treated as a verdict on a different target such as thalamotomy or pallidotomy. Conversely, a regulatory authorization for one device and target does not prove that every focused-ultrasound approach is established.
Ask: “How many people like me have you treated with this exact target, what outcome measure do you use, and how long do you follow patients?” Request rates for benefit and lasting complications rather than an undifferentiated “success rate.”

Discuss permanent and delayed risks
The Parkinson’s Foundation lists headache, dizziness, nausea, balance or walking problems, numbness or tingling, one-sided weakness, and speech or swallowing problems. Less common serious complications include stroke and seizure. Gait or balance problems can persist. Because the lesion cannot be adjusted, a side effect cannot be corrected by changing a programming setting. The Foundation also notes that bilateral treatment raises side-effect risk.
Make the center quantify risk for your target
Ask the treating team:
- What are your rates of lasting gait, balance, speech, swallowing, sensory and strength problems for this target?
- How do you define “temporary” and “permanent,” and at what follow-up points are complications measured?
- How do you monitor temperature near the internal capsule and other critical structures?
- What symptoms require an emergency call after discharge, and who is available overnight?
- What rehabilitation is available if weakness, imbalance or speech difficulty occurs?
Do not let the absence of an incision obscure the neurologic risk: the procedure deliberately changes brain tissue. Ask whether your baseline balance, falls, dysarthria, swallowing difficulty or cognitive impairment makes a small additional deficit especially consequential.

There is no universal winner. The right comparison depends on whether your goal is one dominant symptom or several, how important future adjustability is, and which risks you are prepared to accept.

| Decision factor | Focused ultrasound lesion | DBS | Medication or infusion-based advanced therapy |
|---|---|---|---|
| How treatment works | Creates a permanent lesion in a selected circuit. | Uses implanted electrodes and a pulse generator to modulate circuits. | Changes drug delivery or regimen to reduce fluctuations and other medication-responsive symptoms. |
| Adjustability | Cannot be reprogrammed or reversed. | Stimulation settings can be adjusted; hardware remains implanted. | Doses and delivery can be changed, stopped or combined under specialist supervision. |
| Laterality | Often unilateral; bilateral treatment may carry greater side-effect risk and depends on current authorization. | Can be configured for appropriate bilateral symptoms, with surgical and hardware considerations. | Usually treats medication-responsive symptoms more broadly, but response and adverse effects vary. |
| Follow-up demands | Pre-procedure imaging, treatment-day monitoring and neurologic follow-up; rehabilitation may be needed. | Programming visits, battery or hardware management and ongoing neurologic follow-up. | Medication titration, pump or infusion training where applicable, and monitoring for systemic effects. |
| Evidence and eligibility | Target-specific and evolving; approval differs by device and jurisdiction. | Established for selected patients but not suitable for everyone. | Depends on prior medication response, comorbidities, access and the specific therapy. |
Ask your neurologist to compare these options against the same goals: “Which would best address my tremor or dyskinesia? Which would help more than one symptom? What could remain untreated? What happens if my disease changes in five years?” The available sources support discussing DBS and pump therapies as alternatives, but they do not establish an individualized winner or settle comparative costs.
Plan the treatment day and recovery
- Confirm preparation instructions. Ask exactly which Parkinson’s medicines to take, hold or bring, and whether blood thinners or other medicines need a separate plan. Do not change medication without the treating team’s direction.
- Clarify the MRI and comfort plan. The procedure requires MRI-compatible equipment and repeated imaging. Ask how long you will be in the scanner, how discomfort, headache or nausea will be treated, and how you can communicate while awake.
- Understand intra-procedure testing. The team may ask you to move, speak or report sensations while the target is assessed. Ask what symptoms should be reported immediately.
- Arrange transport and support. Confirm whether you can go home the same day, whether an adult must stay with you, and when driving, work and exercise can resume.
- Get a written follow-up schedule. Ask when the first neurologic review occurs, which outcome measures will be repeated, and who manages rehabilitation or medication adjustments.
Verify approval, coverage and total cost
Ask whether the proposed procedure is authorized for your indication where you live, or whether it is being offered under a study or registry. Request the exact device name, target, laterality and label or study description in writing. A U.S. FDA supplement dated October 29, 2021 identified Exablate Neuro for unilateral pallidotomy in a specified adult population; that does not create a universal eligibility rule for every target, device or country.
Coverage can differ from regulatory authorization. Ask:
- What will my insurer cover, including imaging, hospital fees, anesthesia or sedation, rehabilitation and follow-up?
- What out-of-pocket amount could remain if a complication requires additional care?
- Who at the center will obtain prior authorization and provide a written estimate?
- If treatment is part of a registry or study, which costs are billed to insurance and which are covered by the study?
Take this appointment checklist
- Which exact focused-ultrasound procedure, target and side are you proposing?
- What symptom is the treatment intended to improve, and which symptoms are unlikely to change?
- How do my diagnosis, medication response, imaging and health history affect candidacy?
- What improvement is realistic for my priority symptom, and how long does the evidence follow patients?
- What are your center’s target-specific rates of lasting gait, balance, speech, swallowing, sensory or strength problems?
- What happens if a lesion causes a side effect or my disease later progresses?
- How does this compare with DBS, medication changes, infusion or pump therapies for my goals?
- Is this procedure approved for my indication here, or is it being offered in a study or registry?
- How will medicines be handled on treatment day, and what recovery and rehabilitation should I expect?
- What will insurance cover, what might I owe, and who will confirm that in writing?
These prompts prepare a discussion; they do not determine candidacy. A movement-disorders neurologist and the treating multidisciplinary team should interpret your examination, imaging, goals and risk tolerance together.
