Before ordering an aging or longevity biomarker test, ask four questions: What does it measure? Has it been validated for people like me? What meaningful health outcome does it predict? and What would we do differently because of the result? A biological-age score is an estimate, not a diagnosis. Its usefulness depends on the specimen, measurements, algorithm, comparison population and outcome for which it was developed. Current reviews describe promising biomarkers but unresolved standards, validation and clinical utility, so an aging score should not replace established screening or evaluation of a specific symptom.
Start with the decision, not the number
Tell your doctor what you hope to learn. “What is my biological age?” can mean several different things: a prediction of mortality risk, an estimate based on routine blood tests, an epigenetic-age calculation from DNA methylation, a genetic-risk result, or a proprietary composite score. Those are not interchangeable.
Ask: “What decision would this result change?” If the answer is only curiosity, the test may provide an interesting number without a proven medical benefit. If you are worried about diabetes, cardiovascular disease, cognitive symptoms, fatigue or another defined problem, a condition-specific history, examination and validated screening test may answer that question more directly.
The 2024 Nature Medicine review describes aging biomarkers as promising predictors under development, while noting unresolved validation and clinical translation (Moqri et al., “Validation of biomarkers of aging,” 2024). The 2023 Cell review similarly says that a lack of standards and consensus on reliable aging biomarkers hinders development for clinical applications (“Biomarkers of aging for the identification and evaluation of longevity interventions,” 2023).

Ask exactly what the test measures
Have the clinician identify the biological inputs and the output before you pay or provide a specimen. Request the test name, laboratory, method and a plain-language explanation of what the reported score represents.
Routine biomarkers or a clinical panel
Some services combine measurements such as blood chemistry, blood counts or other routine clinical data into a calculated “age.” A familiar laboratory value can be analytically reliable while the combined age estimate remains unproven for changing care. Ask which individual measurements are included, whether abnormal values are reported separately, and whether the calculation has been tested outside the company that sells it.
Epigenetic or DNA-methylation signals
Epigenetic tests examine chemical marks associated with DNA, commonly from blood or another specimen, and use an algorithm to estimate an age or aging rate. A 2026 review notes that direct-to-consumer epigenetic-age tests are sold online and that consumers lack a shared standard for interpreting their outputs (“An aging essential 8: closing the gap between geroscience and the public it serves,” accessed October 2026). Ask which tissue was studied, whether the algorithm was trained on that tissue, and whether a result from a different specimen can be compared at all.
Genetic variants
A genetic test may report inherited variants associated with disease risk or longevity-related traits. A variant is not the same as current disease, and a negative result does not rule out a condition. The American Heart Association states that direct-to-consumer genetic testing should not replace traditional screening and diagnostic methods (AHA scientific statement, 2025). Ask whether the finding is clinically actionable, whether a clinical laboratory should confirm it, and whether relatives could be affected.

Composite or proprietary scores
A company may combine dozens of signals into a single “biological age,” “pace of aging” or longevity score. Ask for the formula or at least the categories of inputs, the reference population, the direction of the scale and the uncertainty interval. If the method is proprietary, ask what independent evidence is available; a polished dashboard does not establish clinical validity.
Use the three-part evidence test
Ask your doctor to evaluate the test in three separate stages. The FDA-NIH BEST Resource defines the first two this way: “For biomarkers, it is critical to establish that the test measures what it was intended to measure (i.e., analytical validation) and that the biomarker (through its test) has the ability to predict or measure the relevant clinical concept (i.e., clinical validation)” (FDA-NIH BEST Resource, “Validation”).
| Evidence question | What to ask | What a reassuring answer would include |
|---|---|---|
| Analytical validity | Does the assay measure its intended molecule, signal or input accurately and reproducibly? | The specimen, method, quality controls, error limits and repeatability are documented for this exact assay. |
| Clinical validity | Does the result relate to the health concept or outcome being claimed? | Independent studies connect the result to a defined outcome in a population comparable to you, with the follow-up period stated. |
| Clinical utility | Does acting on the result improve a meaningful outcome? | A prospective trial or established care pathway shows that using the result changes decisions and benefits patients. |
A test can measure a signal consistently and still fail to improve care. The 2024 review says prospective clinical trials demonstrating improved patient outcomes are needed to establish clinical utility for aging biomarkers (Nature Medicine review). Ask your doctor to name the evidence level rather than treating “associated with longer life” as proof that an intervention based on your score will help you.

Validation is population-specific. Ask who was included in development and replication studies: age range, sex, ancestry, health status, medications, smoking status and geographic setting. Ask which outcome was measured and over what time frame. A score trained to distinguish older from younger adults may not predict an individual’s future disease, functional decline or response to an intervention.

Ask about replication and uncertainty
- Were the findings reproduced by an independent group, or only by the test developer?
- What is the expected repeat-test variation if the same person tests again?
- How much variation comes from sample collection, storage, shipment and laboratory batches?
- Is there a confidence interval or error range around the reported age?
- Can an infection, acute illness, strenuous exercise, sleep disruption, medication or recent treatment alter the measurement?
The aging-biomarker literature identifies comparability, generalizability and measurement variability as major challenges (Moqri et al., 2024). A one-year difference between two scores may be smaller than the method’s technical and biological variation; ask the clinician how large a change would be considered real.
Define what each possible result would mean
Do not order the test until you and your doctor can describe a plan for a high, low and inconclusive result. Write the plan down, including which findings would trigger a conventional clinical test.
If the score suggests “older” than your calendar age
Ask whether the result would lead to an established risk assessment, such as blood-pressure measurement, lipid evaluation, diabetes screening or another clinically indicated examination. An elevated biological-age estimate should not automatically lead to supplements, medication or an intensive program. Confirm the underlying finding first and use standard guidelines to decide on treatment.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteIf the score suggests “younger”
Ask what reassurance is justified. A favorable score does not rule out cancer, cardiovascular disease, diabetes or another condition, and it should not delay age-appropriate screening or evaluation of symptoms. The result may simply reflect the limited outcomes and population used to build the algorithm.

- Used Book in Good Condition
If the result is borderline, discordant or impossible to interpret
Ask whether repeating the assay is useful, whether a different specimen or validated clinical test is preferable, and who pays for follow-up. Do not keep ordering repeated tests until a preferred number appears; random variation can create apparent changes.
Ask whether ordinary preventive care answers the question better
Before testing, compare the proposed score with the care you already need. Ask: “Would routine preventive screening or a test for a specific health problem answer my concern more directly?” Do not postpone recommended screening, diagnosis or treatment while waiting for a biological-age result. The AHA’s statement on direct-to-consumer genetic testing explicitly warns against replacing traditional screening and diagnostic methods (AHA, 2025); the same practical caution applies when an aging score is being used to make a decision that already has an established clinical pathway.
Review the laboratory, oversight and sales model
Oversight depends on the exact test, intended use, laboratory and sales model. Do not assume that every direct-to-consumer aging or genetic test has the same regulatory status. Ask who performs the assay, where it is performed, what quality controls apply, and whether the report is intended for medical diagnosis or general information.
- Is the laboratory identified, and can your clinician contact it about methods or quality control?
- Is the assay performed in a clinical laboratory or a research setting?
- Has the exact test, not merely a similar technology, been independently evaluated?
- Does the report clearly state limitations, intended use and situations in which confirmation is required?
- Will a licensed clinician interpret the report, or are you given an automated result without medical review?
If a concerning result could change treatment, ask for confirmation through an appropriate clinical test before acting. If a company will not disclose the specimen type, method, algorithm category or limitations, that lack of transparency is itself a reason to pause.

Plan interpretation and follow-up before ordering
Identify the person responsible for explaining the result. Ask whether your primary-care clinician, a genetic counselor, a laboratory specialist or another professional has experience with the test. Clarify how quickly results arrive, whether the consultation is included, and what happens if the report is abnormal, uncertain or technically inadequate.
Ask for a written follow-up pathway: confirmation testing, referral criteria, appointment timing and emergency instructions if a separate clinical result is urgent. A biological-age report should never be the sole basis for stopping a prescription, starting a drug, taking high-dose supplements or changing treatment for a diagnosed condition.
Protect your data, specimen and future options
Read the privacy policy and consent terms before submitting a sample. Ask:

- Can you withdraw consent, delete the account and destroy the remaining specimen?
- Will a future algorithm update change your score without a new sample?
- Can you obtain a copy of raw data and the complete report?
Privacy and interpretation concerns are documented for direct-to-consumer genetic testing, while policies and follow-up for non-genetic aging tests vary by provider (AHA scientific statement, 2025). Obtain the answers in writing rather than relying on a sales-page summary.
Compare two tests without being swayed by marketing
If you and your doctor are considering multiple options, compare them on the same axes. The largest “age reversal” claim is not a substitute for evidence.
| Comparison axis | Questions to document |
|---|---|
| Measurement | What specimen and biological signals are measured? Is the method transparent? |
| Validation | What are the analytical error, clinical outcome, independent replication and performance in comparable populations? |
| Clinical utility | What decision changes, and is there evidence that the changed decision improves a meaningful outcome? |
| Actionability | What happens after high, low, borderline or inconclusive results? What confirmation is required? |
| Practical burden | What are the specimen requirements, turnaround, clinician access, privacy terms and total cost? |
Specific prices, insurance coverage and privacy terms vary by product and were not established for this guide, so request a written, test-specific quote that includes collection, interpretation, confirmation and follow-up. The review literature does not establish a universal best aging biomarker, a common biological-age scale or a generally superior test class for routine care (Nature Medicine, 2024).
A practical appointment checklist
- State the health concern or decision that prompted your interest.
- Ask the clinician to identify the exact measurement, specimen, algorithm and intended outcome.
- Request evidence of analytical validity, clinical validity and clinical utility separately.
- Ask whether independent studies included people with your age, health conditions, ancestry and medications.
- Discuss repeat-test variation, sample-handling effects and the uncertainty interval.
- Write down the action for a high, low and inconclusive result, including confirmation testing.
- Check that the test will not replace or delay recommended screening, diagnosis or treatment.
- Confirm laboratory quality controls, oversight, clinician interpretation and the sales model.
- Review storage, sharing, deletion and future-use terms for your data and specimen.
- Obtain the total cost and follow-up plan in writing before ordering.
When it is reasonable to pause
- The seller promises to “reverse” aging without naming a validated outcome.
- The report provides a precise age but no uncertainty range, method or comparison population.
- The company cannot explain what would change in your care after any result.
- The evidence comes only from the developer, a small or unrelated population, or an association study.
- The service encourages supplements or treatment without confirmation by a clinician.
- The privacy policy allows broad secondary use that you do not understand or accept.
- You are being asked to delay established screening while waiting for the score.
Pausing is not rejecting biomarker science. It is recognizing that a promising measurement becomes medically useful only when it is reliable, relevant to your situation and tied to an action that improves outcomes. As of the 2024 review, regulators such as the FDA and EMA had no recommended guidelines for standardizing the development, measurement or validation of aging biomarkers (Moqri et al., 2024). A careful conversation with your doctor helps separate an informative research measurement from a test that is ready to guide your care.
