Before you start, stop, or add any osteoarthritis (OA) medicine, ask your doctor or pharmacist to review your complete medication list, your health history, the joint being treated, and the result you want. Do not increase a dose, combine pain relievers, or stop a prescribed medicine on your own.

The most useful appointment ends with a specific plan: which medicine to try, how much to use, how long to try it, what improvement to expect, which side effects need attention, and when you will reassess or choose another approach.

Prepare a complete medication list

Bring every product you use, not only prescriptions. Include over-the-counter (OTC) pain relievers, cold and allergy products, vitamins, minerals, herbal products, topical creams, and medicines you take only occasionally. Record the product name, strength, how often you use it, and when you last took it. Photos of labels or the bottles themselves can prevent errors.

Ask: “Can we review everything I take, including nonprescription medicines and supplements, for interactions or duplicate ingredients?” Medication reconciliation matters because two products can contain the same ingredient even when their brand names differ. Supplements can also affect prescribed medicines. Your pharmacist can perform the same check if your doctor’s visit is brief.

  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

Look specifically for duplicate ingredients

  • Check whether more than one product contains acetaminophen.
  • Check whether another product contains an NSAID (nonsteroidal anti-inflammatory drug).
  • Tell the clinician about medicines prescribed by other doctors, including blood thinners and medicines for blood pressure, the heart, or the kidneys.
  • Do not assume that an OTC label means a product is risk-free or suitable with your prescription.

Start with your treatment goal

“Less pain” is useful but incomplete. Explain the activity you want to regain: walking to the shops, climbing stairs, sleeping through the night, opening jars, or exercising. Ask, “What improvement should I expect, and when should we decide whether it is working?”

A medicine may be worthwhile if it improves function even when it does not remove every ache. Agree on a practical measure, such as walking time, sleep interruptions, or a daily activity that OA currently limits. Ask when to report results and whether the trial should be changed if your target is not met.

Ask how the medicine fits your personal risks

There is no single medication that is best for every person and every OA joint. The appropriate choice depends on the joint, your medical history, your other medicines, your goals, and how you respond. Ask, “What dose and duration are safest for me, given my medical history?”

Questions about oral NSAIDs

Oral NSAIDs can reduce OA pain, but the Arthritis Foundation identifies gastrointestinal bleeding and cardiovascular events as important concerns. Ask whether your history of stomach ulcers, kidney disease, or heart disease changes the balance, and whether any of your medicines increase the risk. The Arthritis Foundation advises using the lowest effective dose for the shortest possible time; your clinician should translate that principle into an individual plan.

  • “Does my history of stomach ulcers, kidney disease, or heart disease change whether an NSAID is appropriate?”
  • “Do my blood thinners or other medicines make an NSAID riskier?”
  • “What warning symptoms should prompt me to stop and contact you or seek urgent care?”
  • “How will we review whether I still need it?”

Do not self-escalate an NSAID when pain persists. Inadequate relief may mean the dose, route, diagnosis, or treatment goal needs review rather than simply more medicine.

Questions about acetaminophen

Ask whether acetaminophen has a role for your pain and how to avoid taking it from more than one product. The Arthritis Foundation’s medication-safety guidance recommends checking labels for duplicate acetaminophen and confirming safe dosing with a clinician or pharmacist. Your doctor may need to consider liver health, alcohol use, and other products before giving personal instructions.

Ask whether a topical NSAID fits the joint

The 2019 American College of Rheumatology (ACR)/Arthritis Foundation guideline, published in 2020, states: “Topical NSAIDs are strongly recommended for patients with knee OA and conditionally recommended for patients with hand OA.” It explains that local treatment can provide less systemic exposure than an oral NSAID, so topical therapy should be considered before oral NSAIDs when appropriate.

Joint location matters. The guideline says topical NSAIDs are unlikely to work well for hip OA because the hip joint lies deep beneath the skin. That does not mean a topical product is automatically safe or useless for every person; it means the expected reach and evidence differ by joint.

Joint Guideline position on topical NSAIDs Question to ask
Knee Strongly recommended option “Could a topical NSAID meet my pain and function goal before we consider an oral NSAID?”
Hand Conditionally recommended option “Is the painful hand joint accessible enough for a topical treatment, and what result should we expect?”
Hip Unlikely to be useful because the joint is deep beneath the skin “What options are more likely to reach my hip joint?”

Topical diclofenac gel is one example. In the United States, an OTC product marketed as Voltaren Arthritis Pain is an example of topical diclofenac, but product status and labeling vary by jurisdiction and can change. Ask your clinician or pharmacist whether the specific product and label available where you live are appropriate for you. Do not treat topical use as free of systemic effects, and do not apply it more often or over a larger area than the label or your clinician directs.

Compare options using the questions that change the decision

When your clinician presents more than one option, ask for a comparison based on the factors below rather than asking which medicine is universally “strongest.”

Decision factor What to ask
Affected joint “Can this route reach the joint that hurts, especially if it is my hip, knee, or hand?”
Expected benefit “What pain or function improvement is realistic, and by when?”
Stomach and cardiovascular risk “How do my ulcer history, heart disease, blood pressure, or other risks affect this choice?”
Kidney or liver considerations “Does my kidney or liver health change the dose, duration, or medicine we should choose?”
Interactions “Could this interact with any prescription, OTC medicine, vitamin, herb, or supplement I use?”
Duplicate ingredients “Would another product I take already contain acetaminophen or an NSAID?”
Route and adherence “Which option can I use correctly and consistently, and what application or dosing schedule is required?”
Next step “If this does not help enough, what will we try next and when will we decide?”

Before adding an OTC pain reliever

Ask directly before combining an OTC product with a prescription medicine: “Can I add this OTC pain reliever to my prescription?” Show the exact package, including the active-ingredient panel. A product may contain an NSAID or acetaminophen without making that obvious in its front-of-package name.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Tell the clinician why you want to add the product and which symptoms remain uncontrolled.
  2. Show the full list of prescription medicines, OTC products, supplements, and occasional medicines.
  3. Ask whether the combination duplicates an ingredient or increases stomach, cardiovascular, kidney, or liver risk.
  4. Write down the agreed dose, timing, maximum duration, and what to do if you miss a dose.
  5. Ask which side effects require a call, a same-day assessment, or urgent care.

If a pharmacist recommends an OTC medicine, tell your prescribing doctor about the addition so the complete list stays current.

Agree on a trial and follow-up plan

A medication change should have a review point, even when the product is available without a prescription. Ask, “When should I check in, and what result would count as success?” The answer should account for the medicine, the joint, your goal, and your risks.

Clarify the instructions

  • Exact dose or amount and how often to use it.
  • Whether to take an oral medicine with food or at a particular time.
  • How long to continue before judging the effect.
  • Whether the medicine is intended for regular use or only for selected activities.
  • What to do if relief is inadequate, a dose is missed, or a side effect appears.

Track what happens

For the agreed trial period, note pain intensity, the activity you are testing, sleep, and any new symptoms. Bring the record to follow-up. This gives the clinician information about benefit and tolerability instead of relying on a single difficult day.

Know what to report promptly

Ask your clinician to name the symptoms that matter for your specific medicine. NSAID discussions should include how to respond to possible gastrointestinal bleeding or cardiovascular problems, while medicines affecting the kidneys or liver require their own monitoring advice. Do not wait for a scheduled review if your clinician has told you a symptom needs immediate attention.

Do not restart a medicine that caused a concerning reaction, and do not substitute another pain reliever without checking the ingredients and obtaining professional advice.

Use a short appointment script

You can take these questions to the visit:

  • “Here is everything I take, including supplements. Can you check for interactions and duplicate ingredients?”
  • “My goal is to ____. What improvement should I expect, and when should we decide whether it is working?”
  • “Given my stomach, kidney, heart, liver, and other health issues, what dose and duration are safest?”
  • “Would a topical NSAID be reasonable for the joint that hurts, or is the joint too deep for it to work well?”
  • “Can I use an OTC pain reliever with this prescription, and how will I avoid doubling up on acetaminophen or an NSAID?”
  • “What symptoms or side effects should make me call, stop, or seek urgent care?”
  • “If this does not help enough, what is our next step and when will we review it?”

What a safe medication conversation should produce

Before leaving, make sure you can state the plan in your own words: the medicine and route, the dose and schedule, the planned duration, the goal you will measure, the products you must avoid, the warning symptoms to report, and the date or condition that triggers follow-up. If any part is unclear, ask the doctor or pharmacist to write it down.

That process is more reliable than choosing a medicine by brand, assuming OTC means harmless, or increasing treatment because pain has not improved quickly. OA medication decisions are joint-specific and personal; a complete list, honest risk discussion, and defined review plan let your clinician adjust treatment safely.
