Ask the clinic to explain who runs its embryology laboratory, how air and equipment conditions are controlled, how eggs and embryos are identified, and what happens during an equipment, power, gas or cryostorage problem. The most useful answers are specific, current and documented; no single filter, accreditation badge or technology feature proves that a laboratory will improve your treatment outcome.
The American Society for Reproductive Medicine (ASRM) describes an embryo laboratory as a specialized clinical workspace that depends on suitable design, trained personnel, equipment and quality systems. European Society of Human Reproduction and Embryology (ESHRE) recommendations likewise address staffing, quality management, safety, traceability, cryopreservation and emergencies. Use the questions below to understand how a particular clinic applies those principles in your location.
Take this question list to your consultation
You can ask these questions verbatim or use them as prompts. Invite the clinic to describe its current process rather than offering a general assurance.
- Who directs the embryology laboratory, and what qualifications and experience does that person have?
- How is staffing matched to the laboratory’s workload, and who covers essential duties if a key staff member is absent?
- What controls are used for incoming air, volatile compounds, temperature, humidity and room pressure? How are those conditions monitored and recorded?
- Which environmental and equipment readings are checked every day? Who reviews the results, and what happens when a reading is outside the laboratory’s accepted range?
- How are incubators and other equipment maintained, calibrated and taken out of service when necessary?
- How are identity checks and traceability handled at every step involving eggs, sperm and embryos?
- How are cryostorage tanks, liquid-nitrogen reserves, alarms and backup arrangements monitored?
- What emergency procedures cover power loss, gas-supply interruption, equipment failure or a cryostorage alarm?
- Does the clinic use time-lapse imaging? If so, how does it affect embryo assessment or decisions in my care?
- Which regulator or accreditation applies to this clinic in this country, and can you explain your current status?
Ask whether the clinic can show you a sample quality log, policy summary or other non-confidential documentation. You are not asking for another patient’s records; you are checking whether the explanation is supported by a defined process.

Who is accountable for the laboratory?
Leadership and qualifications
A laboratory director should have clearly defined responsibility for embryology operations, quality management, safety and the competence of the team. Ask for the director’s professional title, relevant training, years of laboratory experience and the scope of responsibility. The answer should identify a real person or role, not only a brand name or a statement that “the doctors oversee everything.”
Staffing and continuity
Embryology work is time-sensitive, so staffing should be planned around the number and type of procedures performed. Ask how many qualified staff are normally present during retrievals, fertilization, culture, biopsy, freezing and warming, and how duties are covered during leave, illness or an unusually busy day. A credible plan identifies who can perform essential tasks and who has authority to make decisions when the usual lead is unavailable. ASRM’s minimum-standards guidance calls for contingency planning and sufficient qualified laboratory personnel.
How are air and room conditions controlled?
Incoming air and volatile compounds
Embryo laboratories need more than ordinary office ventilation. ASRM describes dedicated clean air, suitable filtration, positive air pressure and low-volatile-organic-compound (VOC) materials. ESHRE guidance also addresses HEPA filtration, VOC control and environmental monitoring. Ask what type of air-handling arrangement the clinic uses, where air is filtered, how pressure relationships are maintained and which materials or chemicals are restricted near the laboratory.
Do not treat a single HEPA filter, a “cleanroom” label or a certificate on the wall as proof of performance. The useful follow-up is how often the clinic measures particles, VOCs, pressure or other selected indicators, where results are recorded and what limits trigger corrective action. Ask whether construction, maintenance or cleaning work is isolated from the laboratory and how the clinic verifies that the environment is suitable before procedures resume.

Temperature and humidity
Room temperature and humidity can affect equipment and working conditions, while incubators and heated surfaces require their own controls. Ask which values are monitored continuously and which are checked manually, how sensors are calibrated, and whether staff review trends rather than looking only for a single pass/fail reading. The clinic should be able to explain its accepted ranges without implying that one universal number applies to every laboratory.
What is checked every day, and what happens when something is wrong?
Daily quality-control readings
ASRM lists routine checks that can include incubator gas concentrations and temperature, gas supply, heated surfaces, room temperature and humidity, and liquid-nitrogen tanks. Ask the clinic to identify the readings it checks each day, the person who records them and the person who reviews them. Find out whether checks are documented electronically or on paper, how long records are retained and whether a supervisor signs off.
Limits, escalation and corrective action
A quality system is defined as much by its response to an abnormal value as by its normal readings. Ask for an example of the escalation path, without requesting confidential incident details: who is notified, whether affected equipment is taken out of service, how samples are protected, when a supervisor or medical director is involved, and how the event is documented and reviewed. Look for a written process that distinguishes an instrument error from a genuine environmental excursion and records the resolution.

Maintenance and calibration
Ask how often incubators, gas systems, microscopes, heated stages, alarms and monitoring sensors are serviced and calibrated, and whether calibration is traceable to an appropriate standard. Clarify who performs the work, what happens when a service is overdue and how the laboratory confirms that equipment is ready before it returns to use.

Taking equipment out of service
The clinic should have a defined method for isolating a malfunctioning incubator or other device, moving material safely when necessary and documenting the decision. Ask whether backup capacity exists for critical equipment and how staff decide which procedures can proceed during a failure. A confident answer names the responsible role, the backup arrangement and the records created, rather than promising that failures never occur.
How are eggs, sperm and embryos identified and traced?
Identity checks at handoffs
ESHRE’s 2026 recommendations place patient identification and traceability of reproductive cells at the center of laboratory safety. Ask how the clinic verifies identity when specimens arrive, during processing, at insemination or injection, when dishes or tubes move between work areas, during biopsy or freezing, and when samples are warmed or prepared for transfer.
Documented custody
Ask what information links your consent, collection containers, laboratory records, storage location and final disposition. The clinic should explain how staff use identifiers, independent checks, witnessing or electronic traceability, and how corrections are handled without obscuring the original record. Ask who can access or change the record and how an audit would reconstruct the path of a specimen from collection to storage or transfer.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhat should you ask about cryostorage and emergencies?
Liquid-nitrogen monitoring
Embryos and other reproductive cells may remain in cryostorage for long periods, so ask how tank levels, temperature or other relevant indicators are monitored, how often staff inspect tanks and how alarms are tested. Clarify the clinic’s reserve of liquid nitrogen, the arrangements for replenishment and the backup storage capacity available if a tank or room becomes unusable.

Alarm response and on-call coverage
Ask who receives an alarm outside normal hours, how quickly that person is expected to respond and what happens if the first contact cannot be reached. A useful answer covers escalation to additional staff, transfer to another tank or site when appropriate, documentation of the event and communication with affected patients when a risk assessment is required.
Power, gas and other disruptions
Request an overview of contingency plans for electrical failure, interruption of medical or laboratory gases, fire, flooding, building work and equipment breakdown. ASRM guidance includes backup power and controlled access among laboratory-safety considerations, while ESHRE recommendations address emergency procedures. Ask how plans are tested, when drills were last conducted and which decisions can be made immediately without waiting for a senior clinician.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How should you interpret time-lapse imaging?
Time-lapse systems photograph developing embryos at intervals inside an incubator, allowing embryologists to review developmental timing and events without repeatedly removing embryos. ASRM’s 2022 committee opinion states that the technology has helped understanding of early embryo development and can serve as an embryo selection or deselection tool, but it is not standard practice because it has not been shown to improve pregnancy rates over standard embryo-grading techniques.
Recommended Free Tools
If a clinic offers time-lapse imaging, ask what information it adds to its established assessment, whether it changes selection decisions in your case and how embryologists are trained to interpret the images. Ask what happens when the system is unavailable and whether conventional grading remains the basis for care. Treat the explanation—not the presence of the device—as the relevant comparison.

Which regulator or accreditation applies?
Requirements differ by country and, in some places, by state or province. Ask the clinic to name the regulator, inspection scheme or accreditation body that applies to its laboratory, the scope of that oversight and the date or status of its current authorization. Ask whether the laboratory has conditions, corrective actions or a renewal in progress, and where you can verify status through an official channel.
Accreditation can show that a laboratory has been assessed against a defined standard; it does not replace questions about current staffing, daily monitoring or emergency readiness. The clinic should be willing to distinguish mandatory legal requirements from optional professional recommendations.
Compare clinics on the same axes
If you are speaking with more than one clinic, write each answer in the same worksheet. A single attractive feature should not outweigh a weak explanation in another safety-critical area.
| Comparison axis | What to record | Follow-up if unclear |
|---|---|---|
| Leadership and staffing | Responsible director, qualifications, routine coverage and absence plan | Who has decision authority when the director is unavailable? |
| Air and environment | Filtration, VOC controls, pressure, temperature, humidity and monitoring schedule | Which readings are logged, and what limits require action? |
| Quality control | Daily checks, review process, calibration and corrective-action records | How is affected equipment isolated and released back to service? |
| Identity and traceability | Identifiers, witnessing or electronic checks and custody records | Can an audit reconstruct every handoff? |
| Cryostorage and emergencies | Tank monitoring, nitrogen supply, alarms, backup capacity and on-call response | What is the first action after an overnight alarm? |
| Technology claims | Actual role of time-lapse imaging and evidence used to justify it | Would care proceed safely if the technology were unavailable? |
How to judge the quality of an answer
Signs of a useful explanation
- It names responsible roles and describes coverage, review and escalation.
- It distinguishes continuous monitoring, scheduled checks and periodic calibration.
- It explains accepted limits and the documented response to an out-of-range result.
- It describes identity controls at specific handoffs rather than saying samples are “carefully handled.”
- It separates regulatory requirements, professional guidance and the clinic’s own policies.
- It acknowledges uncertainty and avoids promising that a feature guarantees implantation, pregnancy or a live birth.
Answers that warrant follow-up
- “Everything is state of the art” without a description of readings, records or responsible staff.
- Reliance on one filter, one certification or one device as proof that the entire laboratory is safe.
- No clear person on call for a cryostorage, power or gas alarm.
- Inability to explain how specimens are identified during transfers, freezing, warming or embryo transfer.
- Technology claims framed as guaranteed outcome improvements despite the absence of evidence for that claim.
- Reluctance to state which regulator or accreditation applies in the clinic’s jurisdiction.
What this conversation can—and cannot—tell you
A detailed discussion can reveal whether the clinic has defined responsibilities, measurable controls, documented quality checks, traceability and contingency planning. It cannot independently verify the clinic’s performance, substitute for a regulator’s inspection or predict your treatment result. The professional guidance available does not establish an outcome statistic showing that a particular room feature, filter, monitoring device or laboratory technology increases an individual’s chance of success.
Use the same questions for every clinic you are considering, request clarification in writing when an answer is vague, and ask your fertility specialist how laboratory policies interact with your specific treatment plan. That approach gives you a fair basis for comparing explanations without reducing embryo-laboratory quality to a marketing label or a single piece of equipment.
