Before considering a fat-dissolving injection, ask a clinician to identify what is causing the fullness, confirm whether the product is approved for that body area, assess your anatomy and medical history, explain the complete treatment course, and outline specific warning symptoms. In the United States, FDA-approved deoxycholic acid injections are indicated for adults with moderate-to-severe fullness associated with submental fat (fat under the chin). They are not approved for fat on the stomach, arms, thighs, or other areas.

Use the consultation to make a safety decision, not simply to request that fat be dissolved. The questions below help you determine whether the proposed treatment fits your anatomy and goals, who will administer it, what recovery may involve, and what alternatives deserve comparison.

Start by confirming what is causing the fullness

A visible area under the chin is not automatically a pocket of submental fat. Ask the clinician to examine the area and explain what they believe is contributing to its appearance.

Ask whether another condition could be responsible

The U.S. prescribing information instructs clinicians to consider other causes of submental fullness, including an enlarged thyroid (thyromegaly) and enlarged cervical lymph nodes. A neck examination may therefore be more important than a quick cosmetic assessment. Ask:

  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
  • “What is causing the fullness under my chin?”
  • “Have you checked for thyroid enlargement, enlarged lymph nodes, or another medical cause?”
  • “Do I need any additional evaluation before a cosmetic injection?”

If the clinician cannot explain why the area appears full, pause before agreeing to injections. Treating an undiagnosed medical problem as cosmetic fat could delay appropriate care.

Confirm the treatment area and regulatory status

For U.S. patients, FDA says Kybella and a generic deoxycholic acid injection are the only drugs approved to improve moderate-to-severe convexity or fullness associated with submental fat in adults. The label states that Kybella is not approved for subcutaneous fat outside the submental region and that such use is not recommended.

Ask plainly: “Is this exact product FDA-approved for this exact area?” If you are considering your abdomen, upper arms, thighs, back, knees, or another body site, the answer is not the same as an approved under-chin indication. Do not let phrases such as “injection lipolysis” or “fat-dissolving” substitute for an area-specific approval check.

Questions about the clinician and the product

Only a healthcare provider should administer the approved injection. The FDA label does not establish one universal specialty, certificate, or minimum number of procedures, so ask the clinician to describe their relevant training and experience rather than assuming a title guarantees competence.

Question to ask Why it matters
“Who will perform the injections, and what training do you have with this product and the anatomy under the chin?” The label requires administration by a healthcare provider; you should know who is actually injecting you.
“Which product and concentration will you use, and is it approved for my treatment area?” Approval depends on the product and the body region, not just the phrase “deoxycholic acid.”
“How do you map injection points and protect nearby nerves, blood vessels, and other structures?” Placement near the lower face and neck has specific nerve, vascular, and tissue risks.
“How do you handle complications, and who do I contact after hours?” A clear escalation plan matters if swelling, infection, nerve symptoms, or a persistent lump develops.
“What result is realistic for my anatomy?” Individual benefit cannot be predicted from the label alone; skin laxity and neck structure affect appearance.
“What is included in the quoted fee, and what could additional visits cost?” The number of sessions varies, and the available FDA materials do not establish prices.

Ask to read the current patient information and to receive written instructions. A clinician who discourages reasonable questions, will not identify the product, or cannot describe follow-up is giving you a reason to defer treatment.

Assess candidacy, skin, anatomy, and previous procedures

Skin laxity and platysmal bands

Reducing fat does not tighten loose skin. The label tells clinicians to consider whether excessive skin laxity, prominent platysmal bands (the vertical neck muscles that can become visible), or other conditions could make the cosmetic result undesirable. Ask:

  • “Will loose skin remain or look more noticeable after fat reduction?”
  • “Are platysmal bands or my jaw and neck anatomy a larger part of the appearance than fat?”
  • “Would another approach address the main issue better?”

Medical history and medicines

Give a complete history, including allergies, bleeding or bruising problems, current medicines and supplements, previous facial or neck procedures, and any active skin or systemic illness. Do not stop a prescribed medicine on your own; ask the prescriber and treating clinician how it affects the plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Previous surgery or aesthetic treatment

Prior surgery, injectables, energy-based treatment, trauma, or scarring can alter tissue planes and landmarks. Tell the clinician exactly what was done and when. Ask how scar tissue or changed anatomy affects injection depth, placement, expected results, and complication management.

Understand the schedule before you commit

The current U.S. label permits up to six treatment sessions, with intervals of at least one month. That is a maximum allowed schedule, not a prediction that you will need six sessions or that a particular number will produce your desired result.

  1. Initial assessment: The clinician examines the fullness, reviews your history, checks for other causes, and discusses whether the under-chin indication fits.
  2. Planning: You and the clinician agree on treatment goals, injection areas, expected swelling, follow-up, and what would make you stop or change course.
  3. Session: The healthcare provider administers the injections according to the product labeling and their clinical plan.
  4. Observation and home instructions: You receive instructions for comfort, activity, wound or skin monitoring, and contacting the clinic.
  5. Review: At a later appointment, the clinician assesses healing and the visible response before deciding whether another session is appropriate.

Ask “When will we judge the result, and what finding would mean we should not repeat treatment?” A rushed series with no defined reassessment point makes it harder to recognize an unfavorable response.

Know the common reactions and the serious warnings

In clinical studies summarized in the current label, each of these local reactions occurred in more than 20% of subjects: injection-site swelling, bruising, pain, numbness, redness, and firmness. A frequency reported in studies is not a personal risk estimate; your clinician should explain how your history and injection plan may change risk.

Reaction or warning What to ask before treatment
Swelling, bruising, pain, numbness, redness, or firmness “How long are these reactions usually monitored, and what can I safely use for comfort?”
Marginal mandibular nerve injury, with possible lower-lip or facial weakness “What signs of nerve injury should make me call immediately, and how would you evaluate them?”
Difficulty swallowing (dysphagia) “What level of swallowing difficulty is urgent, and where should I go after hours?”
Bruising or hematoma “How will you distinguish expected bruising from a collection that needs treatment?”
Nodules or masses “How long should a lump last before you examine it, and what tests or treatment might be needed?”
Ulceration, skin or tissue necrosis, vascular injury, or infection “Which color change, blister, open sore, fever, spreading redness, or drainage requires same-day care?”
Injection-site hair loss (alopecia) “Is temporary hair loss possible in the treated area, and how would it be managed?”

The label also warns about tissue and vascular injury, ulceration, necrosis, infection, nodules, and masses. Ask for a written list of symptoms that require routine contact, urgent evaluation, or emergency care. Seek urgent help for trouble breathing or swallowing, rapidly worsening swelling, severe weakness of the face, signs of significant infection, or skin that becomes dusky, blistered, or open.

Put the latest FDA safety communication in context

On September 15, 2026, FDA announced additional labeling warnings after reviewing adverse-event reports and medical literature. FDA reported 129 cases of adverse events associated with use outside the approved submental region and 117 cases of unresolved injection-site nodules or masses. For the unresolved nodules or masses, the mean duration at the time of reporting was 143 days.

These are postmarket report counts, not incidence rates. They do not show that every treated person has the same probability of harm, and a report does not by itself prove that the injection caused every event. They do show why treatment outside the approved area deserves particular caution. FDA said reports involving use outside the submental region included blurred or reduced vision, involuntary eye contractions, muscle or nerve injury and weakness, numbness, and difficulty walking. Nodules and masses have been reported both with indicated under-chin use and outside that area; some may persist and require medical intervention.

Ask the clinician whether the proposed treatment is within the labeled region and what their plan is for a persistent lump, weakness, sensory change, or swallowing problem. Do not accept “off-label” as if it means “proven safe”; it means the use is outside the approved indication and requires an especially careful discussion of evidence and risk.

Avoid unapproved injections and self-injection

FDA has warned about unapproved fat-dissolving injections sold online under names including Aqualyx, Lipodissolve, Lipo Lab, and Kabelline. Products may contain phosphatidylcholine or sodium deoxycholate, and the contents, sterility, concentration, and manufacturing controls may not match an approved medicine.

Unsafe injection practices can cause scarring, skin infection, tissue damage, and other serious complications. FDA advises consumers not to purchase ingredients for unapproved fat-dissolving injections and not to inject the drugs themselves. A reusable cold pack may be discussed as an optional comfort measure because the label says ice or cold packs may be considered; it does not prevent complications, replace medical instructions, or make self-injection safe.

Compare alternatives on evidence and fit, not marketing claims

Ask for alternatives even when you remain interested in an injection. Non-invasive body-contouring devices are a separate category, with technology-specific risks and complications. FDA states that the safety and effectiveness of over-the-counter or home-use fat-freezing products have not been established.

Comparison point Questions for the clinician
Treatment area “Is the alternative intended and authorized for the area I want treated?”
Evidence and regulatory status “What evidence supports it, and what exactly is cleared or approved?”
Number of visits “How many appointments are typical, and what determines whether I need more?”
Risks “What are the common and serious complications for my anatomy?”
Recovery “What swelling, pain, activity limits, or downtime should I plan for?”
Cost “What is the total expected cost, including follow-ups or treatment of complications?”
Fit “Which concern—fat, loose skin, muscle bands, or another cause—does this option actually address?”

The available FDA information does not establish a head-to-head winner between injections and devices for an individual. A treatment that is appropriate for one person’s under-chin fat may be a poor match for another person’s loose skin, muscle bands, or medical cause of fullness.

Use this consultation checklist

  • Bring a list of medicines, supplements, allergies, bleeding issues, and prior neck or facial procedures.
  • Identify the exact body area you want treated and ask whether the product is approved for that area.
  • Ask what the clinician believes is causing the fullness and whether thyroid or lymph-node causes were considered.
  • Discuss skin laxity, platysmal bands, scars, and other anatomy that could affect the appearance.
  • Ask who will inject you, their relevant experience, and how injection points are selected.
  • Get the maximum possible number of sessions, minimum interval, reassessment timing, and total expected cost in writing.
  • Request written aftercare instructions and a 24-hour contact pathway.
  • Ask which symptoms require a phone call, same-day assessment, emergency care, or treatment by another specialist.
  • Compare at least one appropriate alternative on evidence, risks, recovery, treatment area, and cost.
  • Do not buy injectable ingredients online or attempt to inject yourself.

Make the decision only after you can answer these questions

You should be able to state, in your own words, what is being treated, why the clinician thinks it is fat, whether the proposed area falls within the U.S. approval, how many visits may be needed, what reactions are expected, and exactly how complications will be handled. If any of those answers is unclear—or if the clinician proposes treatment outside the approved under-chin region without a careful, evidence-based discussion—postpone the procedure and seek another qualified medical opinion.
