Security teams need an evidence trail that answers five questions for every consequential agent action: which agent acted, whose authority it used, what it attempted against which resource, which policy or approval allowed it, and what happened. Build that trail as structured events linked to a session or task, then protect it like other sensitive security data.
No single global schema or retention period has been finalized for agentic systems. The practical design below combines current guidance from NIST, OWASP, CISA and the NSA, while leaving the exact fields, retention and approval thresholds to your legal, regulatory and operational requirements.
Start with an accountable event trail
An agent log is useful only when an investigator can reconstruct a decision and its consequences. Capture the identity, authority, action, controls and result as related records rather than relying on a model transcript. A transcript may omit tool calls, hide delegated authority or contain secrets that should never enter a general-purpose log.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchFor each action, make these questions answerable:
- Who initiated the work? Record the human principal or calling service, the agent identity and, where applicable, the delegation that connects them.
- Which execution was this? Assign a correlation identifier for the session, task or workflow so events can be ordered across the agent, policy engine and tools.
- What was attempted? Identify the tool or capability, target resource, operation and normalized parameters.
- Which controls applied? Store authorization, policy, approval, denial and step-up decisions, including their expiry or scope.
- What happened? Record whether the request was allowed, denied, completed or failed, with enough outcome metadata to establish the result.
- Can the record be trusted safely? Apply access, integrity, classification, redaction and retention controls to the event itself.
NIST’s National Cybersecurity Center of Excellence (NCCoE) identifies agent identification, authorization, auditing and non-repudiation as central issues. Those concepts should shape your event model even though NIST has not prescribed one universal format.

Design the minimum event fields
A practical schema separates stable identity fields from action and outcome fields. Use a consistent event identifier and timestamp, preferably with a synchronized clock and an explicit time zone or UTC representation.
| Event area | What to record | Why it matters |
|---|---|---|
| Identity | Event ID; agent ID and version; human or service principal; tenant or environment; delegation or token reference | Attributes the action and supports non-repudiation |
| Execution context | Session, task or workflow correlation ID; parent event; start time; execution location | Reconstructs a sequence across services |
| Requested action | Tool or capability; operation; target resource; normalized parameters; requested scope | Shows exactly what the agent sought to do without retaining unnecessary raw content |
| Decision controls | Policy decision; authorization result; human approver or denial; approval ID; approval scope; expiry; step-up result | Connects execution to the authority that permitted it |
| Execution result | Allowed, denied, completed or failed status; reason code; external transaction ID; affected-object summary | Establishes what actually happened |
| Protection metadata | Data classification; redaction status; integrity signature or sequence marker; retention class | Helps handlers use the log without exposing or altering sensitive data |
These fields are implementation recommendations, not a mandated taxonomy. Keep a version number for your schema so parsers and investigators can distinguish a changed field definition from a changed agent behavior.
Use normalized parameters
Approval should bind to the action that will execute, not to an unconstrained natural-language request. Normalize parameters into a stable representation before policy evaluation: for example, canonicalize a resource identifier, sort object keys, standardize units and represent an allow-list as explicit values. Store a hash or approved representation when the full parameter would contain private data. OWASP’s AI Agent Security Cheat Sheet lists the actor, tool, target, normalized parameters, timestamp and expiry as elements that should be bound to an exact approval.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesKeep correlation across boundaries
Propagate the correlation ID when an agent calls a tool, a tool calls another service or one agent sends a request to another. Also record a parent event ID and the receiving service’s event ID. This lets a responder detect a mismatch between an agent’s claimed result and the tool’s authoritative record. The guidance does not prescribe a particular ID format; choose one that fits your existing tracing and security-information workflows.
Attribute both the agent and the authority behind it
Logging only a model name is insufficient. Two runs of the same model can have different tools, prompts, policies and privileges. Record a durable agent identity, deployment or version identifier and the human or service principal whose authority the agent is using.

Record delegation explicitly
Capture the delegation grant or token reference, issuer, audience, issued time, expiry and allowed scope. Do not copy a bearer token or secret into the log. A reference or cryptographic fingerprint is enough to let an authorized investigator connect the event to an identity-provider record.
Distinguish initiator, approver and executor
The person who starts a task may not be the person who approves a high-risk step, and the agent may not be the service that ultimately executes it. Use separate fields for initiator, approver, agent and executing service. This prevents a later review from treating an automated execution as a human action or assuming that a user approved every downstream call.
Handle inter-agent requests
For agent-to-agent calls, log the sending agent, receiving service, delegated principal and permission evaluated at the receiving boundary. OWASP states: “Authenticate communicating agents and enforce the sender’s permissions at the receiving service before executing a request.” Apply that rule at the service boundary and retain the authorization result with the request event.
Log the action, not a dangerous data dump
Investigators need to know what an agent attempted, but storing every prompt, retrieved document and full tool payload can create a second breach target. Record the semantic action and the minimum parameters needed to prove scope.
- Identify the capability, such as
issue_refund,rotate_keyordeploy_release, rather than storing an entire conversational transcript. - Record the canonical target, such as an account, repository, cloud resource or ticket ID.
- Store operation and scope, including read versus write and the affected object set.
- Use a content hash, encrypted reference or redacted excerpt when payload evidence is necessary but contains private data.
- Record whether the request was a dry run, simulation or live execution.
Keep raw prompts and tool payloads in a separately controlled store only when they are necessary for incident response, legal obligations or model-safety analysis. Link them by an access-controlled reference and log every retrieval.

Make approval and policy decisions independently verifiable
An agent’s plan or self-reported permission is not an authorization decision. At execution time, a trusted policy component should independently validate identity, target, scope, privilege and approval state.
Recommended Free Tools
- Create the proposal event. Record the agent’s requested capability, target and normalized parameters with a correlation ID.
- Evaluate policy. Check the delegated principal, resource sensitivity, operation, environmental conditions and least-privilege rules. Record the decision and policy version.
- Request human approval when required. Bind the approval to the exact actor, tool, target, normalized parameters, timestamp and expiry. A generic “approve this task” response is not sufficient for a high-impact operation.
- Revalidate immediately before execution. Confirm that the approval has not expired, the target and parameters have not changed, and the principal still has the required privilege.
- Execute through a constrained capability. The tool should receive only the approved scope, not unrestricted credentials.
- Write the result or failure event. Include the authoritative service response, reason code and any external transaction identifier.
OWASP recommends failing closed if policy lookup, approval validation or audit logging fails. That means the action is denied or held rather than silently proceeding when a control service or audit sink is unavailable.
Define high-risk categories
Set your own risk tiers using the potential impact of an action. Examples that commonly warrant a human checkpoint include changing production access, moving funds, deleting data, disclosing regulated information, changing security controls or deploying code to a critical service. The log should record why the tier applied and which approver satisfied it.
Capture outcomes and blocked attempts
Record both successful and unsuccessful decisions. A denied request can reveal privilege probing, prompt injection or a policy gap even when no system state changed. Use a small, documented result vocabulary such as allowed, denied, completed and failed, then add reason codes that are stable enough for detection rules.
For completed work, capture the authoritative result rather than the agent’s narrative: an API transaction ID, changed-object count, deployment revision or ticket state. For failures, distinguish policy denial, authentication failure, timeout, tool error and downstream rejection. Never treat a model-generated “success” message as proof that the operation completed.

Protect logs as sensitive security records
Agent events can contain identities, customer data, infrastructure details and fragments of secrets. OWASP advises against logging credentials and personally identifiable information in plain text and recommends data-classification and retention policies.
Minimize and classify
- Define which fields are required for accountability and omit fields that do not support an investigative or compliance purpose.
- Classify events by sensitivity, for example general audit, confidential operational data or restricted identity data.
- Redact access tokens, API keys, passwords, session cookies and private cryptographic material before the event leaves the execution boundary.
- Apply structured masking to email addresses, account numbers and other personal data; do not rely on a model to redact its own output.
Restrict access and preserve integrity
Separate write permissions from read and delete permissions. Send events to an append-oriented store, protect them with encryption in transit and at rest, and use immutable or tamper-evident controls where your environment supports them. Alert on gaps in sequence, unexpected deletion, disabled logging or a sudden change in event volume. Keep administrative access to the log itself auditable.
Choose retention by risk and obligation
There is no universally correct retention duration in the current general guidance. Sector rules, jurisdiction, contracts, litigation holds and incident-response requirements can all change the answer. Define retention classes with legal and compliance owners, document the reason for each period and enforce deletion when the period ends.
Retain a compact, redacted action record longer than raw prompts or payloads when that meets the investigative need. Place exceptional holds on relevant events during an incident, and record who created, extended or released the hold.
Monitor continuously and audit the trail
Logging is evidence; monitoring turns evidence into a control. CISA’s May 1, 2026 summary of “Careful Adoption of Agentic Artificial Intelligence (AI) Services” calls for constrained autonomy, strong identity management, layered defense, oversight, threat modeling, continuous monitoring and regular security assessment. The NSA’s April 30, 2026 summary emphasizes explicit accountability, rigorous monitoring, human oversight and incremental deployment.

Useful detection questions
- Did an agent attempt a capability outside its normal task or delegated scope?
- Did the executed target or parameters differ from the approved record?
- Did a service accept a request without authenticating the sending agent or checking its permissions?
- Did a high-risk action occur without a valid, unexpired approval?
- Did the agent make repeated denied attempts, unusual data reads or rapid privilege changes?
- Are there missing, delayed or tampered events at an agent-to-tool boundary?
Review high-impact actions
During periodic audits, sample critical actions and compare proposal, policy, approval, execution and outcome records. Verify that the acting identity was entitled to perform the operation, that the target and parameters matched approval, and that monitoring surfaced anomalous behavior. Test the fail-closed path by safely interrupting policy, approval or logging dependencies in a non-production environment.
Implement in a controlled sequence
- Inventory actions and identities. List every agent, tool, service principal, human delegation and resource class it can reach.
- Tier risk. Mark read-only, reversible, sensitive and irreversible operations; assign approval requirements to each tier.
- Define the event contract. Agree on identity, correlation, action, decision, outcome and protection fields, including schema versioning.
- Instrument boundaries first. Capture events at the policy engine and executing service, not only inside the agent runtime.
- Redact before centralization. Test masking with realistic credentials, personal data and prompt-injection content.
- Connect detection and response. Route high-risk decisions, denied bursts, scope mismatches and logging failures to the existing security-monitoring process.
- Exercise recovery. Run tabletop and technical tests for replaying a task, proving approval, identifying the principal and restoring logging after an outage.
- Reassess after change. Repeat threat modeling and control tests when models, tools, permissions or workflows change.
Common failure modes and fixes
| Failure | Why it breaks accountability | Corrective design |
|---|---|---|
| Only the model name is logged | It cannot identify the deployment, user authority or delegated scope | Record agent identity, version, principal and delegation reference |
| Approval covers a vague task | The agent can change target or parameters after approval | Bind approval to normalized actor, tool, target and parameters with expiry |
| Execution trusts model output | A prompt injection or hallucinated permission can bypass controls | Perform independent execution-time policy validation and fail closed |
| Only successful calls are retained | Reconnaissance and policy probing disappear from the trail | Record denials, failures and blocked attempts with reason codes |
| Raw payloads are copied everywhere | Secrets and personal data spread into systems with broad access | Minimize, redact, classify and reference restricted evidence separately |
| The log store can be altered silently | An attacker can erase evidence of an agent action | Use append-oriented, tamper-evident storage and audit administrative access |
Understand the standards landscape
NIST announced its AI Agent Standards Initiative on February 17, 2026, focusing on standards, open protocols and research for agent security and identity. It is an emerging initiative, not a finalized comprehensive logging standard.
On February 5, 2026, NIST’s NCCoE announced a concept paper on software-agent identity and authority covering identification, authorization, auditing, non-repudiation and prompt-injection mitigation. The public-comment period ran through April 2, 2026.
The CISA and international-partner guidance summarized on May 1, 2026, and the NSA summary published April 30, 2026, point in the same operational direction: deploy incrementally, constrain autonomy, use strong identity, maintain human oversight, monitor continuously and reassess regularly. These documents support the control objectives in this article but do not establish one mandatory event schema or retention number.
Evaluate a logging approach before deployment
When comparing an in-house event pipeline, an existing security-information platform or a vendor service, score each option against the same questions:
- Can it attribute both the agent and the human or service principal, including delegation?
- Does it capture proposal, policy, approval, execution and outcome events across boundaries?
- Can it protect integrity, restrict access and prove administrative changes?
- Does it support field-level minimization, redaction, classification and retention?
- Can responders correlate events across agents, tools and identity systems?
- Does it fit current detection, incident-response and evidence-preservation workflows?
A system that records more text but cannot prove authorization or execution is less useful than a smaller, well-controlled trail. Optimize for reconstructable decisions and trustworthy outcomes.
