Ask questions that test three things: whether the community can meet the older adult’s needs now, whether it can handle likely changes later, and what daily life and total cost will actually be. Bring the older adult into the conversation as much as they wish and are able. Use the same written checklist at every tour, request policies and fees in writing, and verify important answers with a second visit or an independent professional.

Start by defining the care and life the person needs

List present-day help

Before scheduling tours, write down what the person can do independently and where help is needed. Include bathing, dressing, toileting, eating, transferring, walking, taking medicines, managing continence, using a phone, and handling emergencies. Note diagnoses, therapy needs, allergies, communication preferences, mobility equipment, and any recent hospital or rehabilitation stay.

Ask the person’s clinician whether assisted living, memory care, skilled nursing, or another setting is medically appropriate. A community may look attractive but lack the nursing, therapy, behavioral support, or equipment the resident requires.

Plan for foreseeable changes

Ask what happens if the resident falls, needs a wheelchair, develops swallowing problems, requires two-person transfers, or begins to need frequent nursing care. Find out whether the same campus can provide a higher level of care, whether a move is required, and how much notice and additional cost that change involves.

Include personal priorities

Care is only one part of fit. Record preferences about food, wake and sleep times, religious or cultural connection, language, pets, outdoor time, hobbies, privacy, proximity to family, and transportation. Ask the older adult which compromises are acceptable and which are not.

Use one worksheet for every tour

Prepare a fresh copy for each community. Record the name and job title of every person who answers, the date and time, documents promised, fees quoted, and questions that remain open. Consistent notes make differences visible after several visits.

Topic Write down during the visit Verify afterward
Care fit Services available now, future-care policy, waiting list Assessment and admission criteria
Staffing Who is on each shift, training, leadership tenure Turnover information and written staffing descriptions
Daily life Schedules, resident choices, activities, visitor rules Activity calendar and resident handbook
Health support Clinician availability, transportation, care coordination Provider agreements and emergency procedures

For a Medicare- or Medicaid-certified nursing home, Medicare provides a printable checklist with space for notes. That checklist is designed for certified nursing homes, not as a complete regulatory checklist for every assisted living residence.

Questions about care fit and available services

What can you provide now?

  • What help with bathing, dressing, toileting, eating, mobility, medications, and other daily activities is included?
  • Do you provide nursing care, physical or occupational therapy, hospice, wound care, behavioral support, or skilled rehabilitation?
  • Which services are delivered by your employees and which come from outside providers?
  • How do you assess a new resident before move-in, and how are the resident and family involved with the resident’s permission?

What if needs increase?

  • What changes in care can be handled in the current apartment or room?
  • When would the resident have to transfer to another unit or leave the community?
  • Who decides that a higher level of care is needed, and is there an appeal or reassessment process?
  • Is there an opening or waiting list for the exact room, care unit, or service required?

Ask to see the written service description and admission or discharge criteria. A verbal assurance that “we can probably handle it” is not enough for a decision involving long-term care.

Questions about staffing and medical support

Who is present on every shift?

  • How many care providers are scheduled on day, evening, overnight, weekend, and holiday shifts?
  • What training and certifications do aides, nurses, medication staff, and supervisors have?
  • How many employees are trained to respond to medical emergencies, dementia-related distress, falls, and transfers?
  • Are agency or temporary staff used, and how are they oriented to residents’ needs?

There is no single nationwide staffing ratio that proves quality. The appropriate level depends on residents’ conditions, the services promised, the building layout, and the time of day. Treat a staffing answer as one piece of evidence rather than a guarantee.

How stable is leadership?

  • How long have the executive director, nursing director, administrator, and department heads worked here?
  • How often do key staff members leave?
  • Who is in charge overnight, and how can a family reach that person?

How is medical care coordinated?

  • Is a physician, nurse practitioner, or other clinician available on site or on call, and how often do they see residents?
  • Who notices and reports a change in condition?
  • How are prescriptions ordered, administered, and reconciled after a hospital visit?
  • How are residents transported to medical appointments, and who schedules the ride?
  • How are families notified about falls, injuries, infections, medication changes, or transfers to a hospital?

Questions about daily life, dignity, and resident choice

Ask to see an ordinary day

“What does a typical weekday and weekend look like?” should produce a schedule, not a sales description. Ask whether you may observe or join an activity, see the dining room at mealtime, and visit a common area without an escort.

  • Can residents choose when to wake, sleep, bathe, eat, and go outdoors?
  • What happens for a resident who prefers to stay in their room?
  • How do residents, including those unable to leave their rooms, choose to take part in activities?
  • Are activities adapted for hearing, vision, mobility, or cognitive limitations?
  • Can residents bring personal furniture, decorations, or a pet?

Observe how people are treated

Notice whether staff address residents by name, explain what they are doing, knock before entering, and respond patiently. Residents should appear clean, appropriately dressed, and comfortable. Look for unanswered call lights, residents left unattended during meals, rushed interactions, strong odors, clutter, or damaged equipment. One observation is not proof of a pattern, but it is a reason to ask a specific follow-up question.

Check the physical environment

  • Are hallways, bathrooms, elevators, and entrances accessible for walkers and wheelchairs?
  • Are handrails, lighting, flooring, temperature, and noise levels suitable for older adults?
  • Is outdoor space safe, shaded where appropriate, and reachable without staff assistance when residents are permitted to go independently?
  • Can you inspect a resident room, bathroom, laundry arrangement, storage, and emergency call system?
  • May you sample the food, and are menus able to accommodate allergies, cultural preferences, diabetes, swallowing limitations, and special diets?

Additional questions for dementia or Alzheimer’s care

Memory care requires a separate set of questions because supervision, environmental design, communication, and behavioral support may differ from other services.

  • Do staff receive specialized dementia training, and how often is it refreshed?
  • Is there a dedicated memory-care unit? What services and staffing are specific to that unit?
  • How many residents have Alzheimer’s disease or another dementia, and what does a typical day look like for them?
  • How do staff respond to wandering, agitation, distress, sleep changes, or refusal of care?
  • How are pain, illness, dehydration, falls, and other medical problems detected when a resident cannot explain symptoms?
  • Is there secure but usable outdoor access, and can staff help residents get outside?
  • Does memory care cost extra, and exactly which supervision, activities, supplies, and services does that fee cover?

Visit a memory-care area at more than one time of day. Meet the unit manager, watch staff-resident interactions, and ask how the community supports family involvement while respecting the resident’s privacy.

Questions about fees, payment, and the contract

Get the complete price in writing

  • What is the base monthly fee, and which room, meals, utilities, housekeeping, transportation, activities, and care are included?
  • What costs extra: medication management, personal care, incontinence supplies, therapies, special diets, escorts, laundry, transportation, or one-to-one supervision?
  • Are there application fees, community fees, deposits, move-in charges, or charges for changing rooms?
  • Are fees based on a care tier, time spent, individual tasks, or another formula?
  • How often can prices change, how much notice is provided, and can a change be made during a contract term?

Understand payment sources and financial risk

Ask the financial office which payment sources may apply to this particular service and setting, and whom the family can consult independently. Do not assume Medicare pays for a long-term residential stay. Medicare Part A does not cover long-term or custodial nursing-home care; short-term skilled-nursing coverage is a different benefit with eligibility and duration conditions.

For a Medicare- or Medicaid-certified nursing home, federal guidance says the facility must provide information about services, charges, and fees in writing before admission. Families should obtain the fee schedule, service description, refund rules, and contract, then compare total costs rather than only the advertised base rate.

Ask what happens when money or care changes

  • What happens if the resident can no longer afford the fees?
  • Is there a Medicaid-eligible bed or another lower-cost option, and what are the criteria?
  • What happens after a hospitalization or if the resident temporarily needs rehabilitation?
  • Under what circumstances can either party end the contract, and how much notice is required?
  • Are unused deposits, prepaid rent, or service fees refundable?

Visitor access, transportation, and connection with family

  • How are visitors admitted after hours, and what identification or sign-in process is required?
  • Can family stay overnight, share meals, attend care meetings, or take the resident out for a few hours or several days?
  • What are the rules during an infectious-disease outbreak?
  • How does the community support phone, video calls, mail, internet access, and language or religious practices?
  • What transportation is available for appointments, shopping, worship, and family events, and what does it cost?

How to compare communities after touring

Use the same categories and weighting for every option. A polished lobby should not outweigh an inability to provide needed care or unclear fees.

Visit several communities and, if possible, return without calling ahead at a different time or on a different day. Ask a clinician, social worker, or hospital discharge planner who knows the older adult’s needs to review the finalists.

Federal checks that apply specifically to nursing homes

Before selecting a Medicare-certified nursing home, review official inspection results, staffing information, and quality measures on Medicare’s nursing-home comparison resources. Confirm the facility’s Medicare or Medicaid certification, license, and complaint process. These federal comparison tools should not be assumed to cover every assisted living community; state licensing agencies and facility type determine which oversight rules apply.

CMS’s nursing-home checklist includes the following useful prompts: “Does the nursing home offer specialized training for staff to support residents living with dementia?”; “Will the nursing home tell me in writing about their services, charges, and fees before I move into the home?”; and “How do residents, including those who are unable to leave their rooms, choose to take part in a variety of activities?” Keep the nursing-home context when using these questions.

Follow-up steps and warning signs

  1. Send the community a written list of unanswered questions and request documents by a specific date.
  2. Compare the written response with what staff said on the tour; investigate contradictions.
  3. Ask to speak with the administrator or care director about any medical or behavioral concern that was answered vaguely.
  4. Check licensing, inspection, and complaint information with the relevant state agency for the facility type.
  5. Review the contract with a qualified adviser before paying a nonrefundable fee or signing.

Pause when staff will not provide a fee schedule, refuse to explain discharge criteria, discourage private conversations with residents, cannot describe overnight coverage, or pressure the family to sign immediately. A beautiful setting cannot compensate for missing information about care, safety, or financial obligations.
