Before a treatment that could affect fertility, ask the child’s care team for a prompt, individualized discussion with a reproductive specialist. Focus on the child’s actual diagnosis and treatment plan, whether there is time and medical safety to preserve fertility, which options suit the child’s age and development, and what each option can—and cannot—offer. Preservation may keep future possibilities open, but it cannot guarantee fertility, pregnancy, or a child.
Fertility preservation is not one procedure that fits every child. The relevant choices depend on the condition being treated, the planned medicines, radiation or surgery, the child’s health, age and pubertal development, and how soon treatment must begin. The American Society for Reproductive Medicine (ASRM) 2026 medical-indications and ethics opinions recommend prompt counseling, attention to medical urgency, and developmentally appropriate participation by minors. The questions below can help families organize that conversation; they do not replace advice from the treating clinicians.
How could the planned treatment affect my child’s future fertility?
Ask about this child’s treatment, not a general risk estimate
Ask the treating team to explain how the specific diagnosis and proposed regimen could affect the child’s gonadal function, reproductive organs, and future family-building. Invite the clinicians to address the actual medicines and doses, radiation field, or surgery under consideration, rather than relying on a broad description such as “chemotherapy” or “radiation.” The relevance of each exposure depends on the disease and regimen; ASRM notes that risks can vary and may not be fully known at the time of counseling.

Useful follow-ups include: “Which parts of the plan pose a concern, and why?” “What is known about this treatment in children like mine?” “What remains uncertain?” and “Could the treatment affect the ability to carry a pregnancy or make pregnancy medically unsafe later?” The last question matters because future family-building may depend on more than whether stored reproductive material is available.

Separate what is known from what is uncertain
Ask the team to distinguish an established concern from an uncertain possibility. If the plan changes, ask whether the fertility discussion should be updated. Do not expect one number to describe every child: the guidance considered here does not provide a single decision-relevant risk statistic that applies across diagnoses and regimens. A specialist can explain what evidence is relevant to this child and where uncertainty remains.
How quickly must we decide, and could preservation delay treatment?
Ask for a timeline alongside the treatment plan
Ask when treatment is expected to start, whether a reproductive specialist can assess the child before then, and how quickly the team can explain any clinically suitable options. ASRM recommends prompt counseling and referral when fertility may be affected, while taking the child’s health, safety, and the time available into account. If the current hospital does not offer a service, ask whether it can make a rapid referral to a program that does.
Make the trade-off explicit
Ask whether a preservation procedure could change the timing of essential treatment and, if so, what the clinical team considers the risks of that change or delay. Also ask whether the child’s current health makes a proposed procedure unsafe or unsuitable. These are questions for the treating team and reproductive specialist to answer together; a family should not assume either that there is always time to preserve fertility or that a procedure must necessarily delay treatment.

Which options fit my child’s age and pubertal development?
Ask why a method is—or is not—appropriate
There is no single best method for every child. Ask the reproductive specialist to explain which approaches are clinically suitable for this child, taking account of age, pubertal development, diagnosis, treatment plan, health, timing, and the child’s preferences. Ask the clinician to distinguish approaches considered established or principal for a particular group from investigational approaches.
ASRM’s 2026 medical-indications opinion identifies ovarian tissue cryopreservation as the principal method for cryopreserving gametes in prepubertal girls. Selected postpubertal girls may be candidates for oocyte or embryo cryopreservation when medically appropriate. In contrast, prepubertal testicular tissue cryopreservation is considered investigational. These categories do not determine an individual child’s eligibility; ask the specialist what is offered, appropriate, and supported for the child’s circumstances.
Compare only options that are actually suitable
For each option the specialist considers, ask what would be stored, what the procedure involves, what medical or procedural burdens it entails, and how stored material might be used later. Ask how mature the evidence is for that particular method and patient group, and what limitations apply. A method discussed in general terms is not necessarily available or appropriate for this child.

- Does the option fit the child’s age and pubertal development?
- Does it fit the diagnosis, planned treatment, health status, and time before treatment?
- What procedure, anesthesia, discomfort, or medical risks should the family understand?
- What is stored, and what would need to happen for it to be used in the future?
- Is the approach established for this situation or investigational?
- How would storage, future disposition, and the child’s preferences be handled?
What does each option involve, and what could it realistically accomplish?
Get a clear account of the procedure and its burden
Ask the specialist to describe the proposed procedure in language the family and child can understand, including anesthesia if relevant, expected discomfort, medical risks, and any practical steps the child would experience. Ask what is known about the chance that the stored material could later be used in the circumstances that apply to this child. If the clinician cannot give a reliable estimate, ask what is uncertain rather than treating the absence of a number as reassurance or as evidence that the option has no value.
Clarify the limits of preservation
Ask what the procedure is intended to preserve and what it cannot promise. Cryopreservation does not guarantee that stored material will be usable, that future fertility will be possible, that pregnancy will be medically safe, or that the child will have a child. The method and potential benefit depend on the patient and circumstances, and the evidence differs among approaches. Ask the specialist to explain those limits alongside any possible benefit, without presenting an investigational option as established care.
How should my child take part in the decision?
Ask how the discussion will be made developmentally appropriate
Ask the clinicians how they will explain the decision at the child’s developmental level and whether the child can understand the relevant choice. ASRM’s 2026 ethics opinion calls for developmentally appropriate assent when a minor can understand the decision, alongside parental or guardian permission based on the child’s best interests.

Confirm who gives permission
Ask the clinic to explain what permission it requires from a parent or guardian, how it will seek the child’s assent when appropriate, and how the child’s best interests will guide the decision. If the child does not want to participate or does not understand the choice, ask the clinicians how they will proceed sensitively and in keeping with applicable law and clinic policy.

Understand the storage agreement before signing
Ask where the material will be stored, who is responsible for it, what fees apply, how long storage can continue, and what happens if fees are unpaid or the family cannot be contacted. Ask whether material can be transferred to another facility and what local law or clinic policy may limit its later use. Request clear answers about the agreement rather than assuming that storage arrangements are the same everywhere.
Discuss future contingencies and adult decisions
Ask how the agreement handles death and other contingencies, and who can make decisions about stored material in each situation. ASRM’s 2026 medical-indications and ethics opinions discuss storage and disposition, including revisiting disposition agreements when the minor reaches adulthood. Ask how the clinic will involve the child in that later review and what choices the young adult will be able to revisit under the agreement and applicable rules.
These are not minor administrative details: the family is making a decision with possible consequences beyond the treatment period. A useful discussion should leave the parent or guardian understanding the storage terms, the path for future questions, and the point at which the child can reconsider the agreement as an adult.

Who will coordinate care and support the family?
Ask who is responsible for each part of the conversation
Ask whether the treating clinician and a pediatric or adolescent fertility specialist can coordinate, and whether a mental-health professional or other specialist should be involved. Different clinicians may address treatment urgency, fertility options, the child’s developmental needs, or emotional support. Ask who will serve as the family’s contact for follow-up questions so that essential information is not left between services.
Ask about referrals and practical navigation
If the hospital does not provide a relevant service, ask for a prompt referral and an explanation of what options are available elsewhere. ASRM recommends team-based counseling and says programs that lack a specific service should still explain options and refer promptly. Ask whether someone can help navigate financial questions, appointments, or coordination with the treatment schedule; do not assume that every service or form of support is available at every institution.
What other paths to family-building might remain?
Ask the specialist to explain how stored material might be used in the future, and what alternatives could be relevant if preservation is not chosen or does not lead to the hoped-for outcome. Depending on the person’s circumstances and wishes, discussion may include later fertility treatment, donor gametes or embryos, a gestational carrier when relevant, adoption, or choosing not to have children.
These paths are not interchangeable, and none should be presented as a simple substitute for a method that may or may not suit the child. Ask how the child’s future health, prognosis, treatment, and any preserved material could affect each possibility. The purpose is to understand which avenues may remain, not to predict what the child will want or be able to pursue years later.
What should families take away from the consultation?
Before ending the discussion, make sure the family can explain the decision in its own words: the fertility concerns specific to the treatment plan; how much time is available; which options are suitable and why; what each option involves and cannot guarantee; how the child will participate; and how storage and future decisions will be handled. If any of those points remains unclear, ask the team whom to contact and whether another conversation is needed before treatment begins.
ASRM’s 2026 guidance is U.S.-based professional guidance. Local law, clinic policy, access to services, and storage terms can vary, so families should ask the treating team and the relevant program how the rules apply to them. The central decision remains individualized: prompt specialist counseling can clarify choices, but the child’s immediate health and treatment needs must remain part of the discussion.
