Neanderthal shoulder injuries point to dangerous, physically demanding encounters at close range—especially hunting with hand-delivered spears—but they do not, by themselves, prove that Neanderthals routinely fought one another. The strongest evidence comes from experimentally matched spear wounds in 120,000-year-old deer at Neumark-Nord. Human injuries, including a newly described perforation in a Shanidar scapula, show severe trauma and survival, yet usually cannot distinguish a hunting accident from interpersonal violence, an animal attack or a fall.
What a shoulder injury can—and cannot—tell us
The shoulder connects the trunk to the arm, so damage there can record a violent impact, a penetrating weapon or years of altered movement. In Neanderthal fossils, however, the lesion is only one part of the interpretation. Researchers must also consider the exact bone, the shape and remodeling of the damage, the person’s age and mobility, what happened to the skeleton after death, and the range of accidents that could produce similar force.
That distinction matters because a shoulder wound is not a behavioral label. A spear entering the front of the chest during a hunt could leave a pattern similar to a thrust in an interpersonal encounter. A fall, an animal’s kick or horn, or a collision while moving across rough terrain may also injure the same region. The fossils can establish trauma more securely than they can establish motive.
Shanidar 1: major injury followed by survival
Shanidar 1, one of the best-known Neanderthal skeletons, had trauma to the right humerus and marked hypoplasia or atrophy affecting the right clavicle, scapula and humerus. Trinkaus and Zimmerman’s 1982 report documents a severe upper-limb injury that altered the growth or use of the entire shoulder region. The remodeled condition indicates that the individual survived long enough for healing and long-term impairment to develop.

This evidence is important for two reasons. First, it demonstrates that a Neanderthal could live with a badly damaged shoulder and arm, implying a degree of care or at least social tolerance while the person recovered. Second, the pattern shows the physical cost of injury without revealing its original circumstance. The 1982 findings do not identify whether the event was a hunting accident, a fall, an animal attack or violence by another Neanderthal.
Shanidar A: a perforation that resembles a spear wound
A newly described individual, Shanidar A, has a smooth-edged, roughly 15-millimeter circular hole in the scapula, with radiating fractures and displaced bone fragments. Pomeroy and colleagues (2026) interpret the morphology as consistent with a wooden spear entering from the front, probably close to the time of death.
The wording of the scientific conclusion is deliberately limited: “Whether the traumatic lesion on the scapula resulted from something like a hunting accident or from interpersonal violence cannot be known.” A spear-like shape identifies a plausible mechanism, not the social setting. The lesion may record a failed or dangerous hunt, an encounter with another person, or an event for which the archaeological evidence has not survived.
The clearest evidence: thrust-speared deer at Neumark-Nord
Human trauma is indirect evidence of behavior. The most direct evidence for close-range hunting comes from two fallow-deer skeletons recovered at Neumark-Nord in Germany. The deposits date to about 120,000 years ago and contain abundant traces of Neanderthal activity.
Gaudzinski-Windheuser and colleagues examined perforations in the deer bones with micro-computed tomography and compared them with controlled ballistic experiments. Their results showed that the lesions were made by close-range thrusting spears. The study’s conclusion states: “Detailed studies of the perforations, including micro-computed tomography imaging and ballistic experiments, demonstrate that they resulted from the close-range use of thrusting spears.”

Why the deer evidence is stronger than a human bone analogy
The deer wounds are tied to a known weapon mechanism through experimental matching. The researchers could compare fracture geometry, penetration shape and internal bone damage with injuries produced by thrusting a spear into an animal at short distance. That chain of evidence is more specific than observing a hole in a human scapula and inferring what the person was doing.
A Johannes Gutenberg University account described the Neumark-Nord result as the oldest unambiguous hunting-lesion evidence then documented. It also emphasized that this hunting method required planning, concealment and close cooperation among hunters. A thrusting hunt places people within the striking range of a frightened or wounded ungulate, making accidental injury a predictable hazard even when no interpersonal conflict is present.

A thrusting spear is delivered with the weapon in the hunter’s hands or immediately under bodily control. It is not the same as demonstrating that every Neanderthal spear was thrown, nor does the deer evidence establish a single hunting tactic everywhere. It does show that at least some Neanderthals used hand-delivered weapons close to large game roughly 120,000 years ago.
Why Neanderthal trauma was compared with rodeo injuries
Berger and Trinkaus (1995) compared Neanderthal trauma distributions with archaeological, clinical and rodeo samples. They found that the Neanderthal pattern, particularly the frequency of head and neck injuries, resembled injuries sustained by people in dangerous encounters with large ungulates. The “rodeo rider” analogy therefore linked Neanderthal trauma to repeated close contact with animals capable of throwing, striking or trampling a person.
The comparison is useful as a behavioral hypothesis, not as a diagnosis. Rodeo participants are injured in a modern activity with its own equipment, rules and exposure patterns. Neanderthal injuries accumulated under different conditions, and the fossil sample is small. The authors themselves noted that preservation and the under-representation of older people with reduced mobility could influence the apparent distribution.

Other processes that can create the same pattern
Several explanations may overlap:
- Hunting accidents: A thrust into a large animal can be deflected, causing the spear or the animal’s movement to injure the hunter.
- Animal attacks: Hooves, horns, teeth and body impact can damage the shoulder, arm, head or neck.
- Falls and terrain: A fall while pursuing game or traveling over uneven ground can produce fractures and dislocations.
- Mobility and repetitive loading: People with limited movement may be more likely to fall, while repeated strenuous activity can alter bone and injury risk.
- Interpersonal violence: A blow or thrust from another person remains a possible cause for some lesions.
- Preservation bias: Bones that survive burial and recovery are not a random sample of all injuries Neanderthals experienced.
Trinkaus (2012) argued that hunting injuries might explain only part of the observed pattern. His review added mobility, differential skeletal susceptibility, emerging evidence for effective throwing spears and interhuman violence as contributors that should be evaluated rather than excluded in advance.
Can a spear wound distinguish hunting from combat?
Usually, no. The confidence of an interpretation depends on how directly the evidence connects a lesion to an event. The following hierarchy helps separate what is established from what remains conjectural.
| Evidence | What it supports | What it cannot establish |
|---|---|---|
| Experimentally matched perforations in Neumark-Nord deer | Neanderthals used close-range thrusting spears in hunting about 120,000 years ago. | How often this tactic was used, or whether a human lesion came from hunting. |
| Shanidar A’s approximately 15-mm scapular perforation | A wooden-spear thrust is a plausible mechanism, probably near the time of death. | Whether the event was a hunting accident or interpersonal violence. |
| Shanidar 1’s remodeled humerus and shoulder-region atrophy | A severe upper-limb injury occurred and the individual survived with long-term impairment. | The weapon, activity or person responsible. |
| Population distributions of head, neck and upper-body trauma | Neanderthals regularly faced physically hazardous conditions. | A unique signature of combat, or a reliable rate of interpersonal violence. |
The distinction between hunting and combat is therefore a matter of context. A lesion accompanied by a weapon tip, a matching animal association and a secure archaeological setting would be more informative than an isolated fracture. In most fossils, those contextual clues are absent or incomplete.
What shoulder biomechanics add to the picture
Churchill and Rhodes (2006) examined muscle-attachment size and leverage at the shoulder and elbow. Their institutional summary concluded that Neanderthal upper-body strength was probably important to hunting success with close-range, hand-delivered weapons. Powerful shoulders would help stabilize and drive a spear, control a struggling animal and recover the weapon after contact.
Biomechanics supports a functional role for a strong upper body, but it does not identify the cause of an individual injury. A robust shoulder can be an adaptation to repeated labor, a response to development and activity, or simply a population characteristic. Nor can the size of a muscle attachment tell us whether a particular person was injured while hunting, fighting or falling.

Thrusting and throwing are different questions
The Neumark-Nord deer provide direct evidence for thrusting. Trinkaus’s later review also considered evidence that Neanderthals may have been effective with throwing spears. Those findings should not be merged into one generic “spear use” claim: a hand-delivered thrust and a thrown projectile impose different distances, forces and risks.
Likewise, the shape of a human wound rarely reveals the entire sequence of an encounter. A spear could be thrust, thrown at short range, deflected by bone or moved as the victim and attacker shifted position. Researchers therefore need experimental comparisons and archaeological context before assigning a precise weapon action.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Population studies weaken the idea of uniquely violent Neanderthals
A Nature study by Beier and colleagues found that Neanderthals and Upper Palaeolithic anatomically modern humans had similar overall incidences of cranial trauma. Both groups showed higher rates in males. The authors wrote: “We show that Neanderthals and early Upper Palaeolithic anatomically modern humans exhibit similar overall incidences of cranial trauma.”
This result does not make Neanderthal life safe; it places their injuries in a broader human context. Similar overall rates mean that the older image of Neanderthals as exceptionally violent is too simple. The comparison is also sensitive to preservation, age structure and how researchers classify lesions. A sample containing different proportions of young, old, male or mobile individuals can produce different apparent rates even when behavior is similar.

No population-wide percentage for Neanderthal shoulder injuries has been established from the available evidence. Counts depend on which skeletons preserve the clavicle, scapula and humerus, whether damage is visible and classifiable, and which archaeological sites are included. A percentage calculated from a small, uneven fossil sample would risk presenting preservation history as behavior.

How to evaluate a newly reported Neanderthal injury
Readers can use a consistent set of questions when encountering a claim that a shoulder lesion proves hunting or combat:
- What bone is injured? A scapular perforation, a remodeled humerus and a healed clavicle lesion are different kinds of evidence.
- Is the lesion traumatic or developmental? Atrophy and hypoplasia may record the consequences of an earlier injury rather than the original event.
- Was the mechanism experimentally tested? Micro-computed tomography and ballistic comparisons provide stronger support than visual resemblance alone.
- What was found around the skeleton? Animal remains, weapon fragments, sediment and burial context can raise or lower confidence in an interpretation.
- Could another event produce the same damage? Falls, animal attacks, accidents and interpersonal violence should be listed explicitly.
- How large and representative is the sample? Sex, age, mobility and preservation can change the apparent frequency of trauma.
- Does the claim concern an individual or a population? One dramatic injury cannot establish how common a behavior was among all Neanderthals.
What the injuries reveal overall
Taken together, the evidence supports a demanding close-range hunting system in which Neanderthals sometimes approached large game with hand-delivered spears. Strong shoulders and arms would have been useful in that setting, and the risks help explain why injuries cluster in body regions exposed during close encounters. The Neumark-Nord deer make the hunting interpretation direct for that site and period.
The human fossils provide a more cautious lesson. Shanidar 1 shows severe shoulder-region damage followed by survival and impairment. Shanidar A shows a near-fatal scapular perforation compatible with a wooden spear. Neither lesion identifies whether another person, a wounded animal or an accident delivered the blow. Population comparisons likewise do not support portraying Neanderthals as uniquely prone to violence.
Shoulder injuries are therefore evidence of hazard and physical demand, not a simple scoreboard of combat. They strengthen the case for dangerous, cooperative, close-range hunting while leaving the frequency and role of interpersonal violence unresolved.
