Neanderthal injuries do not support the simple image of an exceptionally violent species. The best population-level comparison found similar cranial-trauma incidence in Neanderthals and early Upper Palaeolithic modern humans, with men more frequently injured in both groups. At the same time, their bones and the animals they hunted show that close-range encounters with large, dangerous prey created serious risks. Healed fractures add another dimension: some people survived disabling injuries long enough to recover, implying assistance from others or living conditions that allowed sustained care.
The evidence therefore points to risk, resilience and occasional interpersonal violence—not a single explanation for every broken bone.
What a Neanderthal injury can—and cannot—tell us
Trauma is a physical event recorded in bone, not a label for its cause. A fracture may result from a fall, a hunting accident, a failed expedition, a routine mishap or an assault. Archaeologists therefore ask several questions before assigning an interpretation:
Recommended Free Tools
- When did the damage occur? Perimortem damage happened around the time of death, while remodeled edges and new bone show healing over weeks or months.
- What force and object fit the shape? A puncture, depression, crushing fracture or slicing mark can suggest different mechanisms, but few patterns identify a specific weapon with certainty.
- Where is the injury? A skull, rib, shoulder blade and lower limb each carry different implications for falls, prey encounters and interpersonal contact.
- What surrounds the skeleton? Weapons, prey remains, a burial, sediment movement and the site’s taphonomy can support—or undermine—an interpretation.
- Does the pattern recur in a population? Comparisons must account for which bones survived, how many individuals were sampled and whether preservation favors certain injuries.
These tests explain why a dramatic fracture should not automatically be called a fight wound. A convincing interpretation combines anatomy, timing, context and population evidence rather than relying on appearance alone.

Population evidence does not show uniquely violent Neanderthals
The most useful broad comparison is Beier and colleagues’ 2018 study in Nature. It compared cranial trauma in Neanderthals with early Upper Palaeolithic modern humans and reported similar overall incidence. Men had higher trauma rates than women in both groups. That sex difference is compatible with unequal exposure to dangerous activities, but it does not identify whether the danger came from hunting, travel, accidents or conflict.
The study also stressed preservation bias. Neanderthal remains are relatively few, fragmentary and unevenly preserved; skulls with visible injuries may be more likely to attract attention and publication than intact specimens. Differences in sample size, age structure and archaeological context can make one population look more injured even when the underlying risk was similar. For that reason, no reliable single percentage describes injuries for all Neanderthals, and a count from one famous site cannot be treated as a species-wide rate.
Earlier interpretations often linked Neanderthal cranial injuries to close encounters with large prey or short-range weapons. The 2018 comparison does not deny that those encounters happened. It shows instead that the overall cranial-trauma pattern does not establish a uniquely violent Neanderthal society: comparable early modern humans experienced comparable head-trauma risk.

Why hunting could produce severe injuries without combat
Neanderthals hunted animals capable of killing a human at close distance. A thrusting spear works at arm’s length, so a hunter had to approach an animal, coordinate with companions and remain within the reach of hooves, horns, antlers or claws. A miss, a charge or a fall on uneven ground could leave the hunter with a fracture that looks no different from an injury caused by another person.

The prey record gives unusually direct evidence for this danger. Russo and colleagues’ 2023 Scientific Reports study examined a cave-lion skeleton from Siegsdorf dated to about 48,000 years ago. A partial puncture in the lion was interpreted as a fatal wound from a wooden thrusting spear. Because the injury is on the prey, it documents a hunting mechanism rather than asking us to infer one from a human skeleton. It is strong evidence that Neanderthals used wooden thrusting weapons against a dangerous carnivore at close range.
The same study reported distal cave-lion phalanges from Einhornhöhle that are at least 190,000 years old. Their arrangement and surface damage were interpreted as the earliest central-European evidence of Neanderthal use of cave-lion pelts. Pelt removal brought hunters into contact with a large predator’s claws and teeth even after death, adding another route to injury.
The Natural History Museum in London summarizes the wider faunal pattern: injuries on prey such as mammoths, bison and reindeer show that Neanderthals were proficient hunters. Proficiency does not mean safety. Successful hunting can coexist with frequent accidents, especially when weapons require a close approach and when carcasses must be processed with stone tools.
Three human cases show why one explanation is not enough
St. Césaire: a case interpreted as interpersonal violence
The St. Césaire Neanderthal is the clearest cited case for interpersonal violence. Its lesion has been interpreted as the result of an assault rather than an ordinary accident, and the individual appears to have received some assistance while it healed. This is important evidence that conflict could occur, but it is a case study, not a population rate. It cannot establish that violence was common or that most Neanderthal injuries came from fighting.

Shanidar: a spear-like injury with an unresolved cause
A 2026 report on Shanidar describes severe facial trauma and a perimortem perforation of the scapula that is consistent with a wooden spear. The authors leave the cause open between a hunting accident and interpersonal violence. The lesion therefore demonstrates that a spear mechanism is plausible, not that another Neanderthal attacked the person. Its mortuary context also contributes to current debates about care during life and treatment of the dead, but context does not remove the uncertainty over how the injury happened.
The institutional caution
The Muséum national d’Histoire naturelle’s explainer, reviewed in 2023, makes the conservative point that although conflicts may have existed within or between Neanderthal populations, there are no unambiguous traces of violence at the population level. It also states that no Neanderthal fracture can currently be characterized as a deliberate combat injury rather than an accident during hunting or an expedition. This is not proof that no assault ever occurred; it is a warning that most fractures do not preserve enough information to distinguish intention.
What healed bones reveal about care and survival
A healed fracture records more than the moment of impact. Bone remodeling shows that the person lived through the acute injury and remained alive long enough for biological repair. If the injury impaired walking, gripping, seeing or feeding, survival would have been difficult without favorable circumstances and, in many cases, help from others.
The Muséum national d’Histoire naturelle describes consolidated fractures as evidence of sophisticated care and prolonged immobilization. The defensible inference is social rather than technological: companions may have supplied food, protected the injured person, helped with movement or simply remained nearby while healing occurred. The bones do not identify a caregiver, prescribe a treatment or prove that every group followed the same custom.

St. Césaire likewise has been interpreted as showing assistance during recovery. Shanidar’s severe injuries and mortuary setting keep care at the center of discussion, but the archaeological record cannot by itself separate practical nursing from broader ritual behavior. What can be said securely is that at least some severely injured individuals survived, which means their groups—or the environments they occupied—allowed a period of reduced independence.
How archaeologists weigh violence, hunting accidents and other causes
| Evidence to examine | What it can support | What it cannot prove alone |
|---|---|---|
| Perimortem puncture or perforation | A forceful event close to death; a spear mechanism may be plausible | Whether the force came from a hunter, an attacker or a nonhuman accident |
| Healed remodeling | Survival after the injury and time available for recovery | The identity of a caregiver or a particular medical technique |
| Weapon or prey association | Hunting, carcass processing or weapon use at the site | That the associated person was injured by that object |
| Location and fracture geometry | Constraints on body position, force direction and possible activity | A unique narrative for the event |
| Repeated injuries in a sample | Population-level comparison when sampling and preservation are controlled | A complete account of daily violence or behavior |
| Burial or mortuary context | Information about treatment of the dead and the site’s history | The cause of a particular wound during life |
Researchers also distinguish perimortem from postmortem damage. A break made after burial can mimic an injury until sediment pressure, weathering and bone color are examined. Conversely, a healed lesion may have disappeared from the surface except for subtle remodeling. Context and microscopy are therefore as important as the headline appearance of a bone.

Physical demands
Trauma is consistent with a life that required repeated walking over uneven terrain, carrying materials, processing carcasses and handling heavy or resisting prey. It can reveal which parts of the body were exposed to force and whether an individual remained mobile after an accident. It cannot provide a complete work schedule or tell us how often any one task occurred.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteDistance from prey
The Siegsdorf lion wound and the interpretation of wooden thrusting spears place at least some hunts within close striking distance. This helps distinguish a high-risk thrusting strategy from a purely remote projectile system. It does not mean every hunt used the same range or weapon, nor that all Neanderthal groups hunted lions.

Group survival
Healed severe injuries imply that individuals could remain part of a group—or at least survive in a setting with enough food and protection—while temporarily impaired. That is evidence for resilience and probable assistance, not a full reconstruction of kinship, leadership or social rules.
Limits of the skeleton
Injuries cannot by themselves reveal language, beliefs, household organization or a universal attitude toward violence. They illuminate particular risks and outcomes. Combining trauma with tools, animal remains, settlement evidence and mortuary context produces a stronger picture than reading an entire society from damaged bone.
Common mistakes when interpreting Neanderthal trauma
- Calling every fracture a combat wound: most fractures are mechanically nonspecific.
- Equating danger with aggression: a close-range hunt can be hazardous even when no person attacks another.
- Turning one case into a species-wide rule: St. Césaire and Shanidar are informative individual cases, not prevalence estimates.
- Ignoring preservation: the bones that survive and are recovered are not a random sample of all injuries.
- Overstating care: healing proves survival and time, while the details of assistance remain uncertain.
- Reading certainty into a recent interpretation: the 2026 Shanidar spear explanation is consistent with the injury but explicitly unresolved between accident and violence.
The most defensible conclusion
Neanderthal trauma records a dangerous, physically demanding life in which hunting large animals and traveling through difficult environments carried real costs. Population evidence does not show that Neanderthals were uniquely violent: the 2018 comparison found similar cranial-trauma incidence to early Upper Palaeolithic modern humans, with male-biased injuries in both groups. A small number of lesions, including St. Césaire, are compatible with interpersonal violence, while Shanidar remains deliberately unresolved between a hunting accident and an attack. Healed injuries show that some people survived serious impairment and likely benefited from assistance. The bones therefore reveal risk, resilience and social dependence more securely than they reveal a simple story of “violent cavemen.”
