AI can support a Medicare Advantage (MA) plan’s quality-improvement work, but it does not replace the plan’s required quality-improvement program, coverage rules, utilization-management controls, or reporting duties. The safest approach is to begin with a defined patient outcome or quality measure, assign the AI a narrow workflow role, preserve human decision authority where appropriate, and monitor whether care actually improves.
Regulatory timing matters. CMS proposed AI-specific guardrails in the CY 2026 proposed rule but says the provision was not finalized. Separately, MA organizations now have an annual obligation to submit information about internal coverage criteria used for Part C prior authorization, with the first submission covering 2026 due April 30, 2026. A CMS CY 2027 final-rule summary issued April 2, 2026 also reports changes that remove certain quality-improvement and utilization-management health-equity requirements. Treat each item as proposed, finalized, or currently applicable rather than describing every earlier proposal as a current rule.
What CMS requires before an MA plan adds AI
Every MA organization must maintain a quality-improvement (QI) program. CMS states that “The primary goal of the MA organization’s QI program is to effect sustained improvement in patient health outcomes.” AI deployment is therefore an implementation choice inside that program, not the program itself.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesThe QI structure must include a chronic care improvement program and a health information system that collects, analyzes, and reports Part C and Part D quality-performance information. CMS identifies HEDIS, HOS, and CAHPS data among the information the system must handle. An AI project should have a documented connection to those existing structures: the outcome being targeted, the population affected, the intervention the system supports, the owner responsible for acting, and the period over which results will be assessed.

Define the result, not the technology
A proposal that starts with “deploy a language model” or “add predictive analytics” is too vague for QI governance. Start by stating the problem in measurable terms, such as a chronic-condition care gap, missed follow-up, medication-adherence issue, delayed care coordination, or another defined quality-measure opportunity. Record the baseline rate, target population, intervention, expected mechanism of improvement, and measurement period before selecting a model.
Keep the plan’s authority visible
Describe exactly what the system does and who acts on its output. Examples include identifying a possible care gap for a clinician, prioritizing outreach queues, summarizing records for a care coordinator, or supporting an operational review. The documentation should identify the reviewer, the action they may take, and the person or committee that retains final authority. If an output can affect coverage, medical-necessity review, or prior authorization, it belongs in the plan’s coverage and utilization-management controls rather than an informal analytics workflow.
Use cases that can fit a QI program
Care-gap identification
An algorithm can scan available claims, encounter, laboratory, pharmacy, or clinical data to flag people who may be overdue for a service. The plan still has to define the measure, validate the data used to identify the gap, and provide a workable follow-up path. A high alert count is not a QI result unless intended enrollees are reached and the associated care changes.

Outreach prioritization
Risk or propensity scores can help staff order outreach when resources are limited. Set rules for contact attempts, language and accessibility needs, escalation, and opt-out handling. Review whether the ranking systematically leaves out groups the intervention is intended to serve.
Record summarization and coordination
Summaries may reduce the time needed to understand a complex history before a care-management interaction. Treat generated text as an aid to a trained reviewer: define which source records are in scope, how recent they must be, how conflicting information is handled, and how an error is corrected in the underlying record.

Operational review support
AI can organize cases for quality audits, identify patterns for investigation, or help staff locate relevant documentation. It should not silently convert a pattern into a coverage denial or other adverse action. The plan must preserve the applicable review process and maintain an audit trail of the information considered.
Coverage and utilization-management boundaries
AI does not create a separate coverage standard for MA. CMS’s 2024 final-rule material describes basic-benefit coverage as following applicable Traditional Medicare coverage criteria, including national and local coverage determinations, while setting boundaries for internal criteria and prior authorization. A model’s recommendation must therefore be subordinate to the plan’s approved policies and the applicable Medicare criteria.
When an AI workflow touches coverage
- Identify whether the workflow concerns a basic benefit, medical-necessity determination, internal coverage criterion, or prior-authorization process.
- State which approved policy or Medicare criterion governs the decision.
- Give the responsible reviewer access to the records and reasoning needed to apply that policy.
- Document overrides, disagreements, and corrections so the plan can investigate patterns and demonstrate oversight.
Do not describe a model as independently deciding eligibility, necessity, or authorization unless the plan has verified that the process is legally and operationally permitted. A tool that merely organizes records has a different risk profile from one that recommends an adverse coverage action.
Annual internal-criteria submission
CMS requires MA organizations to report information about internal coverage criteria used by the organization or its delegated entities for Part C prior authorization. The initial submission for coverage year 2026 was due April 30, 2026. Subsequent submissions are due February 28 each year. AI does not remove this obligation; if a delegated entity uses internal criteria in an AI-supported workflow, the plan’s oversight and submission process must account for it. Use the current CMS protocol and technical instructions when preparing each filing because operational specifications can change.
Regulatory timeline: proposed, finalized, and current
| Rule or instruction | Status and date | What an MA plan should say |
|---|---|---|
| CY 2026 proposed rule | Proposal-era language | CMS proposed language intended to promote equitable access and prevent bias when AI or automated tools are used. It was not itself a current requirement. |
| CY 2026 final-rule summary | Final disposition | CMS said it did not finalize the proposed AI guardrail provision. Do not present the proposal’s rationale as an AI-specific rule in force. |
| Part C UM annual data submission | Current operational obligation | The first 2026 coverage-year submission was due April 30, 2026; later submissions are due February 28 annually. Follow the latest CMS instructions. |
| CY 2027 final-rule summary, April 2, 2026 | Later final-rule change | CMS reported elimination of the QI requirement to include disparity-reduction activities and removal of several UM committee health-equity requirements. Confirm effective dates and applicability in the controlling regulation and instructions. |
Older descriptions may still accurately describe what CMS proposed or previously finalized, but they are not automatically current after a later rule. Date every statement about equity, AI, QI, or UM requirements and check the final regulatory text before changing a policy.

Data governance and delegated-entity oversight
An AI project is only as reliable as the definitions and data behind it. Maintain a data register that identifies each source, field definition, inclusion and exclusion rule, refresh schedule, transformation, and accountable owner. Record which version of a measure specification was used and when the data were extracted. If a source changes, assess whether historical comparisons remain valid.

Delegation does not eliminate the MA organization’s responsibility to understand what happens in the delegated workflow. Contracts and oversight procedures should identify the data supplied, the model or rules applied, the human review performed, the output retained, incident-notification timing, and the records needed for audits or CMS submissions. Include delegated internal coverage criteria in the inventory used for the annual Part C UM submission.
Access, privacy, and security controls
Limit each user and system to the minimum data needed for the stated QI task. Separate development, testing, and production data; log access and material changes; and establish retention and deletion rules consistent with the plan’s obligations. Validate that a vendor or delegate does not reuse enrollee information for an unrelated purpose without authorization. These controls support trustworthy operations even though CMS’s QI pages do not prescribe a single AI-governance standard.
How to evaluate whether AI improved quality
Prediction accuracy, summarization quality, or alert volume are intermediate measures. The QI question is whether the intended population received better care or more reliable service. Build an evaluation plan before launch.
- Set the baseline. Measure the selected outcome and relevant process rates for a defined pre-intervention period.
- Specify exposure. Record which enrollees, clinicians, care teams, or cases received the AI-supported workflow and which did not.
- Track workflow completion. Measure whether alerts were reviewed, outreach occurred, referrals were completed, and follow-up actions were documented.
- Measure the target outcome. Use the selected quality measure or clinical outcome over the declared measurement period, not just model metrics.
- Inspect errors and access. Review false positives, missed cases, turnaround time, complaints, and whether people with language, disability, geographic, or connectivity barriers were reached.
- Set intervention thresholds. Define when performance triggers retraining, rule changes, additional human review, a pause, or retirement of the workflow.
Compare results across groups relevant to the intervention’s purpose and current legal obligations. A difference does not by itself prove discrimination or improvement, but it is a signal requiring investigation. Keep a record of the population, method, dates, and limitations so leaders do not mistake an association for an effect.

A practical framework for comparing AI approaches
CMS does not publish an official vendor-scorecard for AI selection. Plans can nevertheless compare proposed approaches against the requirements their QI and UM programs already have.
| Decision axis | Questions to answer | Evidence to request |
|---|---|---|
| Outcome fit | Which patient outcome or quality measure is expected to move? | Baseline, target, population definition, intervention logic, and measurement period. |
| Workflow role | Does the system inform outreach, coordination, clinical review, or coverage work? | Process map, human owner, escalation path, and audit trail. |
| Coverage and UM exposure | Could the output affect a benefit, necessity determination, internal criterion, or prior authorization? | Applicable policy, reviewer controls, override process, and reporting impact. |
| Data and reporting | Can the plan define, monitor, and report the needed quality and coverage information, including delegated work? | Data dictionary, refresh schedule, measure version, delegate inventory, and submission mapping. |
| Oversight and evaluation | Can the plan detect poor performance and correct it in production? | Validation results, monitoring dashboard, incident process, retraining rules, and stop criteria. |
These are planning axes inferred from CMS’s QI, coverage, and reporting context, not CMS-certified criteria or proof that one product is better than another.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Implementation roadmap for an MA plan
- Choose one problem. Name the condition, care gap, coordination failure, or measure and explain why it matters to patient health.
- Assign accountable leaders. Include QI, clinical operations, compliance, utilization management, information security, data governance, and any affected delegated entity.
- Map the current workflow. Show where data originate, who reviews them, what action follows, and where coverage authority resides.
- Validate data and rules. Test definitions, exclusions, timeliness, missingness, and delegate-provided fields before judging a model.
- Run a controlled pilot. Limit scope, train reviewers, provide a manual fallback, and log every material output and override.
- Monitor outcomes and access. Use the evaluation plan, review subgroup results, and investigate complaints or unexpected workflow changes.
- Document regulatory impact. Update QI records, UM policies, internal-criteria inventories, delegated-entity oversight, and any CMS submission materials.
- Scale only after review. Obtain a recorded go/no-go decision based on outcome evidence, operational capacity, risk controls, and current rule applicability.
Common failure modes and fixes
Calling deployment “quality improvement”
Failure: The project reports that a model was launched or generated thousands of alerts. Fix: Tie the work to a defined outcome, completed intervention, and measured change.
Using a proposal as if it were law
Failure: A policy quotes the CY 2026 proposed AI guardrail as a current mandate. Fix: Label it as proposed and cite the CY 2026 final-rule disposition, then verify the rules currently applicable to the plan.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Letting an opaque score drive coverage
Failure: Staff accept a recommendation without checking the governing Medicare criterion or internal policy. Fix: Require a qualified reviewer, preserve the applicable criteria, and log the basis for the final action.

Ignoring delegated workflows
Failure: The plan inventories only its own criteria and misses a delegate’s AI-supported prior-authorization process. Fix: Include delegated criteria and data flows in oversight and the annual UM submission process.
Assuming equity language is unchanged
Failure: A policy continues to cite older QI disparity-reduction or UM committee requirements without checking later rule changes. Fix: Date the source, review the April 2, 2026 CY 2027 summary and controlling text, and update applicability statements.
How Star Ratings fit into the business case
CMS describes Star Ratings as a way for beneficiaries to compare plans and says ratings affect Quality Bonus Payments and rebates. A 2026 CMS fact sheet describes up to 43 measures for MA-PD contracts, up to 33 for MA-only contracts, and up to 12 for Part D plans, organized across five categories. Those are counts of measures, not evidence that AI improves performance. Before using a rating-related target, confirm the latest measure specifications, denominator rules, reporting periods, and contract type.
What current CMS material does not establish
The available CMS material does not provide a general AI-effectiveness statistic, adoption rate, savings estimate, comparative vendor ranking, or universal implementation cost. A plan should not claim that AI will improve a measure by a particular percentage without a study that identifies its population, method, publisher, and year. Treat local pilot results as local evidence and state the limits of what they show.
