Tuberculosis (TB) infection means TB bacteria are in the body. In inactive TB infection—also called latent TB infection—the bacteria are alive but controlled, so the person generally has no TB disease symptoms and cannot spread TB to other people. Active TB disease develops when the bacteria multiply and damage tissue; it can make someone ill and may be contagious, especially when the lungs are affected.
A positive TB blood test or skin test shows evidence of infection, but it does not by itself tell you whether the infection is inactive or active. A health professional makes that distinction using the person’s symptoms and history, an examination, chest imaging, and laboratory tests when needed.
TB infection and TB disease are not the same diagnosis
TB is caused by bacteria that can remain in the body without immediately causing illness. The term TB infection describes the presence of those bacteria. Clinicians then determine whether the infection is inactive or whether it has progressed to TB disease.

The bacteria are present but inactive, and an evaluation finds no evidence of disease. Active TB disease means the bacteria are multiplying and causing disease in one or more parts of the body. The distinction matters because it changes a person’s symptoms, ability to transmit TB, tests, urgency, and treatment.

What a positive TB test actually means
A TB blood test (an interferon-gamma release assay, or IGRA) and a TB skin test (the tuberculin skin test) detect an immune response associated with TB bacteria. As the CDC puts it, “A positive TB test result means you have TB germs in your body.” That statement establishes infection; it does not establish active disease. The quoted wording appears in CDC’s diagnosis guidance.
After a positive result, a clinician determines whether the bacteria are inactive or causing disease. A negative result also needs context: symptoms, recent exposure, immune status, and the timing of testing can affect how results are interpreted. Do not use a blood or skin test as a stand-alone way to decide that you are safe to ignore symptoms or that you are contagious.
Why follow-up is necessary
- A positive test can represent inactive infection that needs preventive treatment.
- The same positive result can occur in someone with active TB disease.
- Only a clinical evaluation can look for disease in the lungs or other organs.
- People with symptoms need prompt assessment even if they have not yet received a TB test.
How clinicians distinguish inactive infection from active disease
CDC diagnostic guidance describes a combination of medical history, physical examination, infection testing, chest X-ray, sputum testing, and drug-resistance testing when appropriate. The clinician chooses the components based on the person’s symptoms, exposure history, age, immune status, previous test results, and other findings. No single test is sufficient in every situation.

- Review history and risk. The clinician asks about a positive or previous TB test, known exposure, prior treatment, symptoms, immune-suppressing conditions or medicines, and relevant medical history.
- Perform an examination. The examination looks for signs that could indicate disease and helps identify other explanations for symptoms.
- Use chest imaging when indicated. A chest X-ray can look for evidence of TB in the lungs. An abnormal image does not by itself prove TB, and a normal image does not answer every possible question about disease outside the lungs.
- Collect respiratory samples when needed. For suspected pulmonary TB, sputum may be examined with smear and culture tests. Additional laboratory testing can identify drug resistance and help guide treatment.
- Evaluate other body sites when symptoms point there. TB can affect organs outside the lungs, so testing depends on the affected site and the clinician’s findings.
Because the work-up is individualized, a person should not order isolated tests or interpret a result without the clinician who requested it. The CDC’s diagnosis page explains the evaluation components and their role.
Symptoms: what active TB can look like
Inactive TB infection generally causes no TB symptoms. Active disease can produce symptoms that vary according to the organ involved. Pulmonary TB—the form most associated with person-to-person spread—may cause:
- A cough lasting three weeks or longer
- Chest pain
- Coughing up blood or sputum
- Fatigue
- Unintentional weight loss
- Loss of appetite
- Chills
- Fever
- Night sweats
These symptoms are not unique to TB, and not everyone with active disease has every symptom. TB outside the lungs can affect other organs and cause a different pattern of illness. A prolonged cough, coughing blood, unexplained weight loss, fever, night sweats, or a known exposure warrants medical evaluation rather than self-diagnosis. When symptoms require urgent attention
Coughing blood, chest pain, breathing difficulty, severe weakness, or rapidly worsening illness should be treated as urgent medical concerns. Call emergency services for life-threatening symptoms. For less immediate concerns, contact a health care provider promptly and mention any positive TB test or known exposure so the office can advise you about arrival and precautions.

Can inactive TB become active?
Yes. Inactive infection can progress to active TB disease at any time, although many people never develop disease. The CDC estimates that, without treatment, about 5%–10% of infected people develop TB disease over their lifetimes. This is a population estimate, not a prediction of what will happen to a particular person.
Progression is most common within the first two years after infection or when immune defenses are weakened. Risk is higher for people with weakened immune systems and certain health conditions, including HIV, diabetes, and cancer. A clinician uses the individual’s history and current health to decide whether preventive treatment is appropriate. Why preventing progression matters
Inactive infection does not make a person contagious, but untreated bacteria can later begin multiplying and cause illness. Treating inactive infection is therefore preventive care: it lowers the chance that disease will develop, rather than treating symptoms that already exist.

How treatment differs
Treatment for inactive TB infection
Preventive treatment is intended to eliminate inactive bacteria before they cause disease. CDC and the National Tuberculosis Coalition of America preferentially recommend short-course, rifamycin-based three- or four-month regimens over six- or nine-month isoniazid monotherapy when clinically appropriate. The right regimen depends on factors such as other medicines, medical conditions, possible drug interactions, pregnancy-related considerations, and whether drug resistance is suspected. A clinician must select and monitor the regimen; do not start, stop, or share TB medicines on your own.

Details on options and clinical considerations are provided in CDC’s Treating Inactive Tuberculosis guidance. Completing the prescribed course is important even when a person feels well, because inactive infection usually causes no symptoms to signal whether treatment is working.
Treatment for active TB disease
Active disease requires a clinician-directed combination of medicines, not a single drug chosen by the patient. Multiple medicines are used because TB bacteria can be difficult to eradicate and can develop resistance if treatment is incomplete or inappropriate. The regimen and duration depend on the disease site, laboratory results, drug susceptibility, prior treatment, and the person’s health. Completing the full prescribed course is essential. When pulmonary disease is potentially infectious, the health department or treating team may give specific instructions about contact with others, ventilation, masking, work or school, and follow-up testing. Those instructions are individualized; inactive TB infection does not require isolation because it cannot spread.

- Contact the ordering clinician or a TB program. Tell them the date and type of test, the result, any symptoms, and whether you have had a prior positive test or treatment.
- Do not assume the result means active disease. A positive test confirms evidence of infection but does not distinguish inactive infection from disease.
- Complete the recommended evaluation. This may include a history, examination, chest X-ray, sputum tests, and drug-resistance testing, depending on your circumstances.
- Follow the treatment decision. If evaluation finds inactive infection, discuss preventive treatment. If it finds active disease, follow the multidrug plan and infection-control instructions exactly.
- Report changes promptly. New cough, fever, night sweats, weight loss, chest pain, coughing blood or sputum, or worsening illness should be reported rather than waiting for a routine appointment.
There is no consumer product that can determine whether TB infection is inactive or active. Home supplements, generic masks, or an over-the-counter test cannot replace the clinical evaluation used to make that distinction.

Common misunderstandings
“A positive test means I have active TB.”
Not necessarily. It means TB bacteria are detected indirectly through an immune response. Active disease requires evidence from symptoms, examination, imaging, and/or laboratory testing.
“Latent TB can spread if I share dishes or sleep near someone.”
Inactive TB infection does not spread to other people. Transmission concerns arise with potentially infectious active disease, especially disease in the lungs or airways.
“I feel healthy, so I do not need follow-up.”
Inactive infection commonly causes no symptoms, and treatment is offered to prevent future disease. A positive result still needs professional evaluation even when you feel well.
“A cough automatically means TB.”
Many illnesses cause cough. A cough lasting three weeks or longer, coughing blood or sputum, or a combination of fever, night sweats, fatigue, and weight loss should prompt medical assessment, particularly after a known exposure.
Bottom line for interpreting TB terminology
TB infection means the bacteria are in the body. Inactive (latent) infection is contained, usually symptom-free, and not contagious; active TB disease is bacterial multiplication that causes illness and may be transmissible. A positive blood or skin test identifies infection but cannot make that distinction alone. Prompt clinical evaluation determines the diagnosis, and clinician-directed treatment can prevent inactive infection from progressing or cure active disease.
