Testicular tissue transplantation is an experimental fertility-restoration procedure. A surgeon removes and freezes testicular tissue before cancer treatment or another therapy that could destroy fertility. If clinical use is later considered, the tissue can be thawed and grafted back to the same person in the hope that surviving immature germ cells will resume sperm production.
A July 2026 conference abstract described the first reported human autologous transplant of immature, cryopreserved testicular tissue. The graft survived, developed a blood supply, showed focal spermatogenesis, and yielded sperm from an intratesticular graft one year later. That is an important biological observation—not proof of a reliable treatment, pregnancy, live birth, long-term safety, or routine availability.
Testicular tissue banking and transplantation are different steps
The terms are sometimes used interchangeably, but they describe separate parts of a possible fertility-preservation pathway:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors- Testicular tissue cryopreservation (banking): a biopsy is taken and stored at very low temperature before gonadotoxic treatment.
- Testicular tissue transplantation: some of that stored tissue is thawed and placed back into the patient later, usually as a graft in or near the testicle.
Banking does not mean that transplantation will occur or that fertility can definitely be restored. Tissue may remain stored for years while doctors assess whether a safe and effective clinical use has been established.

The approach is mainly being studied for prepubertal boys and some adolescents who cannot yet produce a semen sample. Before puberty, the tissue contains spermatogonial stem cells and other immature germ cells, but usually no mature sperm that can be collected and frozen. Preserving the tissue is intended to retain those cells for possible future sperm production.
For someone who can provide sperm, conventional sperm cryopreservation is an established fertility-preservation option and is generally considered first when treatment timing and the patient’s medical condition allow it.
Who might be offered tissue cryopreservation?
Potential candidates are patients facing treatment likely to impair testicular function, including some chemotherapy, radiation, or other medical interventions. Age, pubertal development, diagnosis, treatment urgency, and the ability to produce and collect sperm all affect the decision.
A child may be referred for tissue banking when delaying cancer treatment to pursue sperm collection is not medically appropriate or when the child is not producing sperm yet. The procedure requires a surgical biopsy, usually with anesthesia, so the expected experimental benefit must be weighed against that immediate burden.

Professional guidance remains cautious. The American Society for Reproductive Medicine’s 2026 committee opinion states that testicular tissue cryopreservation is investigational in prepubertal boys. The American Society of Clinical Oncology’s 2024 guideline update says testicular tissue cryopreservation in prepubertal males, and reimplantation or grafting of human testicular tissue, should be performed only in a clinical trial or an approved experimental protocol.
How the procedure is intended to work
1. Referral before fertility-damaging treatment
The process must begin quickly, before chemotherapy, radiation, or another treatment that could damage the germ cells. The oncology team refers the patient to a reproductive urologist, pediatric urologist, fertility-preservation program, or another center with an approved tissue-banking protocol.
Counselling should cover the fact that tissue can be stored experimentally without a proven future method for restoring fertility. Families should also ask whether a semen sample is possible, because sperm cryopreservation has a more established clinical role.

2. Surgical biopsy
A trained surgeon removes a small sample of testicular tissue. The exact technique and amount depend on the patient’s anatomy, age, diagnosis, and the center’s validated protocol. The tissue is divided into small fragments so that multiple portions can be stored and, if appropriate in the future, evaluated or used separately.

3. Laboratory processing and freezing
The laboratory prepares the fragments with cryoprotectants and freezes them using a validated method. ESHRE’s 2025 good-practice recommendations discuss controlled slow freezing and cryoprotectant use, but note that laboratory protocols and quality-assessment procedures are not fully standardized between centers.
Internal validation matters because freezing and thawing can injure cells. ESHRE also notes that there is no universally standardized viability assay for this tissue. A center should therefore be able to explain its processing method, quality controls, storage arrangements, consent process, and how samples are tracked.
4. Long-term storage
Stored fragments remain in a specialized cryogenic system while the patient completes treatment and grows older. Storage is not the same as a successful fertility outcome. A survey cited in ESHRE’s 2025 recommendations reported that more than 3,000 patients had undergone testicular tissue cryopreservation at participating centers. That figure counts banking procedures; it does not represent successful transplants, pregnancies, or live births.

5. Thawing and grafting, if an approved future use exists
If the tissue is considered for clinical use, selected fragments are thawed and placed back into the patient. A graft may be positioned inside the testicle, where it must reconnect with the blood supply, remain viable, and provide an environment in which immature germ cells can develop.

The intended biological sequence is:
- The thawed tissue survives the procedure.
- New blood vessels connect to the graft (vascularization).
- Immature germ cells survive and continue developing.
- Spermatogenesis occurs in at least some areas of the graft.
- Sperm are produced in quantities and locations that allow retrieval, if retrieval is technically possible.
Every step is uncertain. Graft survival does not guarantee spermatogenesis, and sperm retrieval does not guarantee that the sperm can fertilize an egg or lead to a pregnancy.
What the July 2026 human report showed
A conference abstract presented in July 2026 described one human autologous transplant of immature cryopreserved testicular tissue. “Autologous” means the tissue was returned to the same person from whom it was collected. The report described survival and vascularization of the graft, focal areas of spermatogenesis, and sperm retrieval from an intratesticular graft one year after transplantation.
The report did not establish a success rate. It did not report a pregnancy, a live birth, the long-term health of any offspring, or durable fertility over many years. A single conference abstract also cannot show how the procedure performs across different ages, cancers, treatment regimens, tissue quantities, or laboratory protocols.
These distinctions are important when reading headlines. “Sperm was retrieved” is an intermediate laboratory outcome. It is not equivalent to “fertility was restored,” “pregnancy occurred,” or “a healthy baby was born.”

|---|---|---|
| ESHRE recommendations, 2025 | Provides 44 recommendations covering counselling, eligibility, biopsy, transport, and cryopreservation. It describes testicular tissue cryopreservation as experimental and says clinical applications remained unproven when the recommendations were prepared. | Banking should occur within a carefully governed program with explicit experimental counselling. |
| ASCO guideline update, 2024 | Recommends that testicular tissue cryopreservation in prepubertal males and human tissue reimplantation or grafting be performed only in clinical trials or approved experimental protocols. | Routine clinical use is not recommended. |
| ASRM committee opinion, 2026 | Calls testicular tissue cryopreservation investigational in prepubertal boys and identifies sperm cryopreservation as an established male fertility-preservation strategy. | Sperm banking is preferred when feasible; tissue banking remains an investigational option. |
| Human Reproduction conference abstract, July 2026 | Reports one autologous intratesticular graft with vascularization, focal spermatogenesis, and sperm retrieval after one year. | Shows early human biological feasibility, not general effectiveness or reproductive success. |
| NICE research agenda, 2026 | Lists the safety and clinical and cost effectiveness of testicular tissue cryopreservation for prepubertal and peripubertal boys as a research question. | Important uncertainties remain about outcomes, safety, and value. |
The most serious safety issue: returning cancer cells
For a patient with cancer, the stored tissue may contain malignant cells. Reimplanting contaminated tissue could theoretically return cancer cells to the body. This concern is especially important in hematologic malignancies and metastatic disease.
ESHRE recommends assessing tissue for malignant infiltration before any retransplantation and avoiding retransplantation when relevant markers are positive. Testing is not perfect: the portion examined may not represent every fragment that would be grafted, and detection methods can change as new evidence becomes available.
A fertility team should explain which tests will be performed, what a positive or inconclusive result means, whether the underlying cancer changes the risk, and whether an alternative method could avoid reintroducing tissue.

- No established live-birth evidence: the evidence described here does not establish pregnancy, live birth, offspring health, or long-term reproductive safety after human grafting.
- Uncertain durability: even if sperm production begins, its duration and consistency are not known.
- Variable laboratory methods: freezing, thawing, transport, and post-thaw assessment are not fully standardized.
- Unknown individual benefit: age, diagnosis, treatment exposure, tissue quantity, and testicular condition can all affect the result.
- Procedural burden: banking requires a biopsy and usually anesthesia; later grafting is another surgical procedure.
- No reliable success or cost figures: the available sources do not establish a comparative human success rate, pregnancy rate, live-birth rate, or cost-effectiveness estimate.
How it compares with sperm cryopreservation
| Question | Sperm cryopreservation | Testicular tissue cryopreservation with possible later grafting |
|---|---|---|
| Who can use it? | Patients who can produce and collect sperm, sometimes with medical or surgical assistance. | May be considered when sperm cannot be collected, including some prepubertal boys. |
| Clinical status | Established male fertility-preservation strategy. | Investigational or experimental. |
| When is material collected? | Before gonadotoxic treatment, whenever medically possible. | Before gonadotoxic treatment, by surgical biopsy. |
| What is stored? | Mature sperm cells. | Fragments containing immature germ cells and supporting tissue. |
| Future use | Stored sperm may be used with established assisted-reproduction pathways, subject to clinical suitability. | Would require thawing, graft survival, spermatogenesis, and a method to retrieve usable sperm. |
| Major special concern | Ability to obtain an adequate sample and preserve usable sperm. | Experimental effectiveness, graft durability, and possible malignant-cell contamination. |
The appropriate choice is individualized. A patient who can provide sperm should ask whether collection can happen before treatment. A child who cannot produce sperm may be offered tissue banking only under an approved experimental program, with clear acknowledgement that the stored tissue may never provide a usable fertility treatment.
Questions to ask before agreeing to tissue banking
- Can sperm be collected now, and would that be medically safe before treatment?
- Is this center operating under a clinical trial, ethics approval, or another approved experimental protocol?
- What biopsy technique, anesthesia, freezing method, and quality-control tests will be used?
- How will the tissue be transported, identified, monitored, and stored?
- What testing will look for malignant-cell contamination, and what are the limits of that testing?
- What would happen if the tissue is damaged, lost, contaminated, or found unsuitable?
- What future uses are actually available at this center, rather than merely being studied?
- Which outcome would be considered a success: tissue survival, sperm production, sperm retrieval, pregnancy, or live birth?
- Who will provide long-term follow-up as the patient gets older?
What patients should do now
- Tell the oncology team immediately that future fertility is a concern; waiting until treatment starts can remove options.
- Request prompt specialist referral to a reproductive urologist or fertility-preservation program experienced with children or adolescents when relevant.
- Ask about established sperm banking first if the patient can provide a semen sample.
- Review the experimental consent information for tissue banking, including possible lack of future access or benefit.
- Clarify cancer-safety testing before any possible retransplantation.
- Keep expectations tied to the evidence: stored tissue is not restored fertility, and sperm retrieval is not a pregnancy or live birth.
What remains unknown
The central unanswered question is whether testicular tissue transplantation can reliably produce healthy biological parenthood for a broad group of patients. Researchers still need long-term evidence about pregnancy, live birth, offspring health, graft durability, cancer recurrence risk, and adverse effects. They also need validated ways to assess tissue viability and detect malignant cells before transplantation.
The July 2026 case moves the field beyond purely laboratory and animal evidence, but it does not resolve those questions. Until larger, carefully followed studies do so, tissue transplantation should be discussed as an investigational possibility rather than a routine fertility treatment.
