Immunotherapy is a treatment option for some people with colorectal (bowel) cancer whose tumour is MSI-high (MSI-H) or has deficient mismatch repair (dMMR). In England, current NICE recommendations cover specific adults with unresectable or metastatic disease, either before treatment or after certain chemotherapy. A biomarker result is essential, but it does not by itself decide eligibility: stage, whether surgery is possible, previous treatment, the need for rapid tumour control, general health and local funding all matter.
What immunotherapy means in bowel cancer
In this context, immunotherapy mainly means immune checkpoint inhibitor medicines. They help the immune system recognise and attack cancer cells instead of relying only on treatments that directly kill rapidly dividing cells. Pembrolizumab is an example of an immune checkpoint inhibitor; nivolumab and ipilimumab are also checkpoint-inhibitor medicines used in the colorectal-cancer pathways described by NICE.
These drugs are not a general treatment for every bowel cancer. Their use depends on features of the tumour and the clinical situation. The key tumour findings are high microsatellite instability (MSI-H) and deficient mismatch repair (dMMR). They are related but differently named laboratory findings that indicate a problem in the tumour’s DNA-repair system. The oncology team interprets the result with the cancer’s stage, location, resectability and treatment history.

“Bowel cancer” here means colorectal cancer arising from the lining of the large intestine, including the colon and rectum. The recommendations below are NHS guidance for England, not a universal rule for every country.

Why MSI-H and dMMR testing matters
NICE recommends that people with colorectal cancer are tested for mismatch-repair deficiency or microsatellite instability when the cancer is diagnosed. The result can identify people who may benefit from checkpoint inhibitors and can change the order in which treatments are considered.
What an MSI-H result says
MSI-H means the tumour has high microsatellite instability. It is a biomarker found in the cancer tissue, not a diagnosis made from symptoms alone. A report may use the term “MSI-high” or “high MSI.”
What a dMMR result says
dMMR means the tumour has deficient mismatch repair. Reports may write “MMR deficient,” “MMR deficiency” or “dMMR.” MSI-H and dMMR often identify an overlapping group, but your team should explain exactly which test was performed and what the result means for your case.
What a negative or unavailable result means
A tumour that is not MSI-H or dMMR does not meet the biomarker requirement in the NICE recommendations described here. If no result is available, the team may need to arrange testing or confirm the pathology before making a decision. A delay in obtaining a result can be clinically important when treatment needs to start quickly, so ask how the testing timeline affects your plan rather than waiting or changing treatment yourself.

Current NICE options in England
NICE’s current recommendations distinguish between untreated advanced disease and disease that has already been treated with chemotherapy. The table shows the principal differences.

On 28 May 2025, NICE stated: “Nivolumab with ipilimumab is recommended as an option for untreated unresectable or metastatic colorectal cancer with high microsatellite instability (MSI) or mismatch repair (MMR) deficiency.” That wording is deliberately specific: it does not recommend the combination for every stage of bowel cancer or for every person with an MSI-H or dMMR tumour.
Who may be eligible before any systemic treatment?
Untreated unresectable or metastatic disease
An adult in England may be considered for nivolumab plus ipilimumab when the colorectal cancer is both untreated and unresectable or metastatic, and testing shows high MSI or MMR deficiency. “Unresectable” means the disease cannot be removed safely or completely with surgery. “Metastatic” means it has spread to other parts of the body. A cancer can be metastatic and still have some lesions that surgeons can remove, so the multidisciplinary team must assess the whole pattern of disease.

When treatment may need to start quickly
NICE’s committee discussion notes that treatment choices depend on genomic test results being available. If testing is delayed, or if rapid tumour control is judged necessary, chemotherapy may be used first. That observation explains why a person with a potentially eligible biomarker does not always receive immunotherapy immediately; it is not advice to postpone care or select chemotherapy without specialist guidance.
Who may be eligible after chemotherapy?
There is a separate pathway for metastatic colorectal cancer that has already been treated with a fluoropyrimidine-based combination chemotherapy. For a tumour with high MSI or MMR deficiency, NICE recommends nivolumab with ipilimumab in this setting.
Pembrolizumab is recommended after that chemotherapy only when nivolumab with ipilimumab cannot be used. NICE specifies a maximum of two years of uninterrupted pembrolizumab, with earlier stopping if the cancer progresses. The two-year limit is a treatment rule in this recommendation, not a promise that every person will be able to continue for that long or that treatment must continue despite serious problems.
Your team will need the exact names, dates and doses of previous treatments. “I have had chemotherapy” is not enough to establish this pathway, because eligibility depends on the regimen and on whether the cancer is being treated as metastatic disease.

Why the biomarker alone does not decide eligibility
An MSI-H or dMMR result is an important gate, but it is only one part of the decision. Clinicians also consider:
- Stage and resectability: the routine NICE recommendations cited here concern unresectable or metastatic disease, not every localised tumour.
- Treatment line: an untreated cancer follows a different recommendation from one that has progressed after fluoropyrimidine-based combination chemotherapy.
- Urgency: when symptoms or tumour growth require a rapid response, the team may prioritise a regimen that can be started while testing is completed or that is judged more suitable for immediate control.
- Previous medicines: prior chemotherapy, immunotherapy or other cancer treatment can alter what is appropriate and what is funded.
- General health: other illnesses, current medicines, organ function and ability to attend monitoring can affect safety and suitability.
- Patient preferences: expected benefits, possible immune-related complications, monitoring, route and frequency of treatment, and alternatives should be discussed before consent.
- Local access: NHS commissioning rules, authorisation and availability can determine which NICE option a particular service can provide.
For these reasons, a positive report should prompt a focused conversation with the treating team, not an assumption that treatment is guaranteed.

The routine NICE recommendations discussed above are for unresectable or metastatic colorectal cancer. They should not be read as a recommendation to replace surgery or standard treatment for a cancer that can be removed. A person with localised disease may still be offered immunotherapy as part of a clinical trial, but that is a different route.

A trial may require a particular stage, a confirmed MSI-H or dMMR result, a specified previous treatment, measurable disease or particular health measurements. Meeting one trial’s criteria does not create a general right to immunotherapy outside that study. Ask the cancer team whether a relevant trial exists and what additional tests or commitments it involves.

How testing and eligibility are usually worked out
- Confirm that the diagnosis is colorectal cancer. The pathology team records whether the tumour arose in the colon or rectum and provides the tissue diagnosis.
- Check that MMR deficiency or MSI testing was requested. NICE recommends this testing at diagnosis. If it was not done, ask whether stored tumour tissue can be tested or whether a new sample is needed.
- Read the exact biomarker wording. Ask whether the result is MSI-H, dMMR, another category, or inconclusive, and whether further testing is required.
- Establish stage and resectability. Imaging and surgical review determine whether the disease is localised, removable, unresectable or metastatic.
- Map previous treatment. Bring a complete list of chemotherapy and other cancer medicines, including dates and reasons for stopping.
- Discuss the treatment objective and time pressure. The team explains whether the immediate aim is surgery, rapid disease control, long-term control or participation in a study.
- Review suitability and access. Your clinician considers other health conditions, monitoring needs, expected benefits and risks, and whether the recommended option is authorised and funded where you are treated.
Questions to take to the oncology appointment
- Has my tumour been tested for MSI and mismatch-repair deficiency?
- Is the result MSI-H, dMMR, neither, or uncertain?
- Is my cancer resectable, unresectable or metastatic?
- Am I being treated as having untreated disease or disease after previous chemotherapy?
- Does my previous regimen count as fluoropyrimidine-based combination chemotherapy?
- Which immunotherapy option, if any, fits my situation, and why?
- How quickly does treatment need to work, and could that affect the order of testing and treatment?
- What monitoring, possible immune-related effects and stopping rules should I understand?
- What alternatives are available if immunotherapy is unsuitable or cannot be funded locally?
- Would a clinical trial be relevant, and what criteria would I need to meet?
How common is the biomarker?
NICE’s 2021 committee discussion for its pembrolizumab appraisal reported that high MSI or DNA mismatch-repair deficiency occurs in around 4% of metastatic colorectal cancer. That figure applies to the metastatic population considered in that 2021 appraisal; it is not an estimate for all bowel cancers, all stages or every country. The sources used for this guidance do not establish a single current percentage for MSI-H or dMMR across all colorectal-cancer stages.
What immunotherapy can and cannot answer
Immunotherapy can provide an option when the tumour’s biology makes checkpoint inhibition relevant, particularly in the advanced settings covered by NICE. It does not make surgery unnecessary in a resectable cancer, and a biomarker does not predict the exact benefit an individual will experience. The response, side effects, scans, blood tests and overall health are reviewed during treatment. If the disease progresses or treatment cannot be tolerated, the team reassesses the plan.
Availability also depends on geography. NICE guidance describes NHS practice in England. Scotland, Wales, Northern Ireland and other countries may use different approvals, funding arrangements or treatment sequences. A clinician or local cancer service can confirm what is currently authorised where you live.
The practical bottom line for a patient
Ask for the pathology report and confirm whether MSI or MMR testing was completed. Then place that result alongside the stage, whether the cancer can be removed, the treatments already received and how urgently control is needed. In England, adults with untreated unresectable or metastatic MSI-H/dMMR colorectal cancer may be considered for nivolumab plus ipilimumab, and pembrolizumab is also listed for untreated metastatic disease. After fluoropyrimidine-based combination chemotherapy, nivolumab plus ipilimumab is the NICE option, with pembrolizumab reserved for situations in which the combination cannot be used. Only the specialist team, applying the current local rules, can determine whether one of these pathways fits an individual patient.
