“Gene-targeted therapy” usually refers to cancer targeted therapy, also called molecularly targeted therapy. These prescription medicines act on particular molecules—often proteins or molecular changes—that help a cancer cell grow, divide, survive, or spread. They are a major part of precision medicine, but they are not the same as gene therapy, which changes genetic material inside cells.

Whether targeted treatment is an option depends on the exact cancer, its stage and treatment history, the molecular feature found in the tumor, the medicine’s approved indication, and your overall health. A biomarker result can identify a possible treatment; it cannot guarantee that the treatment will work.

What “gene-targeted therapy” means in cancer care

The phrase is easy to misunderstand because it blends two different ideas. In oncology, the established terms are targeted therapy and molecularly targeted therapy. The National Cancer Institute (NCI) defines the approach this way: “Targeted therapy is a type of cancer treatment that targets proteins that control how cancer cells grow, divide, and spread.” NCI, Targeted Therapy to Treat Cancer

A targeted drug is designed to interfere with a particular molecular target or pathway. The target may be unusually active, present in excess, changed by a mutation, or otherwise important to that cancer’s survival. The same target can occur in more than one cancer type, while two tumors in the same organ can have different targets and require different treatments.

Gene therapy is different. The NCI Dictionary describes gene therapy as “An experimental treatment that adds a new gene or replaces or repairs a mutated (changed) gene inside the body’s cells to help prevent or treat certain diseases, such as cancer.” NCI Dictionary of Cancer Terms: gene therapy That definition concerns adding, replacing, or repairing genetic material. A molecularly targeted cancer drug generally does not repair a patient’s genes; it blocks or changes the activity of a selected protein or pathway.

How targeted cancer therapy works

Most targeted treatments fall into two broad groups. Their size affects where they can act and how they reach the target.

Small-molecule drugs

Small molecules are able to enter cells. That makes them useful when the relevant target is inside a cancer cell, such as an abnormal signaling protein. By inhibiting that protein or pathway, the drug can slow cell division, reduce survival signals, or trigger cancer-cell death. The exact target, dosing schedule, route, and approved use are specific to each medicine.

Monoclonal antibodies

Monoclonal antibodies are laboratory-produced proteins built to bind a specific target, often on the surface of a cancer cell or in the surrounding environment. Depending on the medicine, an antibody may:

  • Block a growth signal from reaching the cancer cell.
  • Mark the cancer cell so the immune system can destroy it.
  • Carry a cell-killing substance to the targeted cell.
  • Direct the cancer cell toward programmed death.

Some targeted medicines act on the tumor’s support system rather than directly on a cancer-cell protein. For example, a drug can interrupt signals that promote new blood-vessel formation, limiting the tumor’s blood supply. The mechanism and delivery route must be checked in the current prescribing information for the particular medicine.

Why this differs from chemotherapy

Many chemotherapy medicines damage cells that grow and divide quickly. Targeted therapy is designed to interfere with selected molecules or pathways. This selectivity can make treatment more tailored, but it does not make it harmless or automatically more effective. The NCI notes that effects depend on the patient’s health, cancer type and stage, the specific therapy, and the dose. Targeted drugs can still cause serious or uncomfortable adverse effects, and cancers can stop responding.

Who can receive a targeted therapy?

A person generally needs two things: a cancer with a relevant molecular target and a treatment that acts on that target. In some diseases, a characteristic target is common; chronic myelogenous leukemia is an example in which clinicians commonly look for a target matched to a particular drug. In other cancers, the target is present only in a subset of tumors, so testing is essential.

Testing may require a biopsy. A biopsy removes a sample of tumor tissue for laboratory analysis, and its risks vary with the sample’s size and location. If obtaining tissue is difficult or unsafe, the care team may consider another validated approach, such as a blood-based liquid biopsy, when appropriate.

How biomarker testing guides treatment

Biomarker testing examines genes, proteins, and other substances in cancer to help guide treatment. It is often called tumor profiling, molecular testing, or genomic testing. The test is performed on the cancer—not automatically on every cell in your body.

Tumor testing versus inherited genetic testing

Tumor testing looks for changes acquired by the cancer during a person’s life. These changes may help identify a drug target but are not necessarily present in the rest of the body or passed to children. Inherited genetic testing looks for changes present in the body’s normal cells that can be passed from parents. A clinician may recommend both tests, but they answer different questions.

Single-marker and broad-panel tests

A laboratory can look for one specific marker or examine many markers at once. Depending on the clinical situation, testing may use:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • A single-gene or single-protein assay when one target is strongly suspected.
  • A multigene or multiprotein panel.
  • Whole-exome sequencing, which surveys the protein-coding portions of genes.
  • Whole-genome sequencing, which surveys most of the tumor’s genetic material.
  • A liquid biopsy that analyzes cancer-related material released into the blood.

The appropriate method depends on the cancer, the available tissue, the laboratory, and the treatment decisions being considered. Ask which assay is being used, what it can detect, and whether the result will be accepted for the treatment or trial under consideration.

What a result can—and cannot—tell you

A biomarker result may support:

  • An on-label, FDA-approved treatment for that cancer and setting.
  • An off-label treatment, where a clinician uses an approved drug in a different situation based on medical judgment.
  • Eligibility for a clinical trial.

A matching biomarker does not guarantee benefit. The cancer may not depend on that target enough for the drug to work, other resistance mechanisms may be present, or the treatment may be unsuitable because of stage, prior therapy, other illnesses, or interactions with other medicines.

What cancers can targeted therapy treat?

Targeted medicines are used across multiple cancer types. The relevant question is not simply “Which organ has the tumor?” but “Which validated molecular feature does this tumor have, and is there a treatment approved or being studied for it?” The NCI’s list of targeted therapy drugs is organized by cancer type and was updated May 14, 2025. That date identifies when the page was updated; it is not a treatment-outcome statistic. NCI: List of Targeted Therapy Drugs Approved for Specific Types of Cancer

Tissue-agnostic treatment

Some approvals are based on a genomic or molecular feature rather than where the tumor began. These are called tissue-agnostic or tumor-agnostic therapies. Examples described by the NCI include pembrolizumab, larotrectinib, entrectinib, repotrectinib, selpercatinib, and the combination of dabrafenib with trametinib. Each example has its own biomarker, label, dosing, safety information, and eligibility requirements; the drug name alone is not enough to determine whether it is appropriate. Before calling a targeted drug an option, verify the current indication, the required biomarker test, the cancer setting, any prior-treatment requirements, and whether the use is approved, off-label, or limited to a trial.

A practical path from diagnosis to a targeted-treatment decision

  1. Confirm the diagnosis and setting. Ask for the cancer type, stage, disease extent, and treatment history being used to make the decision.
  2. Ask whether biomarker testing is recommended. Your oncologist can explain whether tissue is available, whether a new biopsy is needed, and whether a liquid biopsy is suitable.
  3. Review the test’s scope. Find out whether the laboratory is checking one marker, a panel, whole exome, whole genome, or another method, and what limitations apply.
  4. Interpret the report with the care team. A result may be actionable, uncertain, negative for the markers tested, or relevant only to a clinical trial. “No target found” means no target was detected by that test; it does not mean that every possible molecular feature has been ruled out.
  5. Match the result to a current treatment label. Check the exact cancer, biomarker definition, stage or setting, prior therapies, contraindications, and required confirmation tests.
  6. Compare alternatives. Discuss targeted therapy alongside surgery, radiation, chemotherapy, immunotherapy, observation, or a trial when those are medically relevant. The best choice depends on the whole clinical picture, not the biomarker in isolation.
  7. Plan monitoring. Ask how response, side effects, laboratory values, and signs of resistance will be followed, and what would trigger a dose change or a different treatment.

How to compare two targeted-treatment options

Two drugs should not be compared merely because both are called “targeted.” Use the same decision points for each option.

Decision point What to verify
Biomarker and test Which gene, protein, fusion, mutation, or other feature is required? Which assay detected it, and is confirmation needed?
Cancer and setting Does the evidence apply to this cancer type, stage, line of therapy, and measurable disease setting?
Approval and prior treatment Is the use FDA-approved for this situation, off-label, or available through a clinical trial? Are prior treatments required?
Mechanism and delivery Is it a small molecule or monoclonal antibody? Does it act inside the cell, on its surface, on a growth signal, or on blood-vessel formation? How is it delivered according to the current label?
Benefits and risks Review the current label and your clinician’s explanation for expected benefits, important adverse effects, monitoring, interactions, and reasons treatment might be stopped.

The available evidence does not support a universal ranking of targeted drugs. A medicine’s value is conditional on the matching biomarker and clinical circumstances.

Side effects, resistance, and other limitations

“Targeted” does not mean side-effect-free

Because targets can also be present in healthy cells or normal biological pathways, targeted treatment can affect normal tissues. The type and severity of effects vary with the drug, dose, patient health, cancer, and combinations with other treatments. Your oncology team should provide medicine-specific warning signs and monitoring instructions rather than relying on the word “targeted” as a safety shortcut.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A target does not guarantee a response

A biomarker is a selection tool, not a promise. Tumors are biologically diverse, and a test may not capture every cancer cell. A drug may produce an initial response and later lose effectiveness if resistant cells survive or the cancer uses another pathway. Ask how response will be measured and what alternatives exist if the disease progresses.

Tests have practical limits

Insufficient tissue, a sample taken from an older tumor site, or a test that does not cover the relevant alteration can limit the usefulness of a result. The meaning of a negative or uncertain result depends on the assay and the clinical context. A second opinion from a cancer center or a molecular tumor board may be appropriate when the report is complex or treatment options are unclear.

Questions to take to an oncology appointment

  • What molecular target, if any, has been identified in my tumor?
  • Was the result obtained from tumor tissue, blood, or another sample?
  • Was this a single-marker test or a broader panel, and what could it have missed?
  • Is the proposed treatment approved for my exact cancer and treatment setting?
  • What prior-treatment requirements, confirmation tests, or eligibility rules apply?
  • What benefit is realistic for someone with my diagnosis, and how will we measure it?
  • Which side effects require an urgent call, and what monitoring is needed?
  • If the treatment does not work or stops working, what is the next evidence-supported option?
  • Would an inherited genetic test be relevant for my family, separate from tumor testing?

Key points to remember

  • In cancer care, “gene-targeted therapy” usually means molecularly targeted therapy: a drug aimed at a cancer-related molecule or pathway.
  • It is not synonymous with gene therapy, which involves adding, replacing, or repairing genetic material.
  • Small molecules can enter cells; monoclonal antibodies bind selected targets, often on cancer cells or in their surroundings.
  • Biomarker testing can identify an approved treatment, an off-label possibility, or a clinical trial, but a match does not guarantee benefit.
  • Targeted therapies are used in many cancers, including some tissue-agnostic indications based on molecular features rather than tumor location.
  • Every option must be checked against the current label, exact biomarker, cancer setting, prior treatment, expected benefits, risks, and monitoring plan.
