An AI incident-response plan is an organization-approved, tested playbook for detecting, assessing, containing, communicating about, recovering from, and learning from incidents involving artificial-intelligence systems. It covers more than cyberattacks: faulty models, poisoned or exposed data, prompt injection, unsafe outputs, unauthorized tool actions, vendor failures, weak human oversight, and harms to people can all require response. A workable plan names who can make decisions, defines severity thresholds, preserves the right evidence, provides tested rollback or shutdown paths, and is exercised and updated as systems change.

What makes an AI incident different?

Traditional security incident response focuses on compromised accounts, malware, unauthorized access, and service outages. AI systems add failure points at every stage of the lifecycle: training and fine-tuning data, retrieval sources, system prompts, policies, model versions, agent tools, external APIs, human review, and the outputs delivered to users or downstream systems. An incident can therefore occur without a conventional breach. A model may follow an injected instruction, reveal a confidential document through retrieval, make a discriminatory decision, or execute a legitimate tool with an unsafe argument.

Response dimension Conventional cyber incident focus AI-specific questions
Detection Network, endpoint, identity, and application alerts Which prompts, outputs, model behaviors, tool calls, user reports, or evaluation results indicate harm?
Scope Hosts, accounts, data stores, and transactions Which model versions, prompts, retrieval indexes, datasets, agents, vendors, and downstream decisions are affected?
Containment Isolate systems, revoke access, block indicators Pause generation or tool use, disable a prompt or connector, switch models, restrict data retrieval, or add human approval.
Evidence Logs, alerts, forensic images, and access records Immutable logs plus model and prompt versions, input/output samples under privacy controls, tool-call traces, approvals, and timestamps.
Recovery Restore clean systems and validate security controls Revalidate safety, privacy, fairness, accuracy, and availability before returning the model to production.
Accountability Security, legal, executives, and affected customers Also include model, data, safety, privacy, responsible-AI, vendor, and business owners.

The plan should treat an AI incident as a lifecycle problem rather than only an operations-center alert. NIST’s AI Risk Management Framework (AI RMF) describes risk management as continuous and timely throughout an AI system’s lifecycle; its Core states that “Risk management should be continuous, timely, and performed throughout the AI system lifecycle dimensions.” NIST AI RMF Core

Which guidance should your plan use?

NIST provides a useful, voluntary foundation, but it does not replace sector-specific law, contractual duties, or your organization’s existing incident process. AI RMF 1.0 was released on January 26, 2023. The complete AI RMF Playbook is dated March 30, 2023, and the Generative AI Profile, NIST AI 600-1, is dated July 26, 2024. NIST SP 800-61 Revision 3, published in April 2025, integrates incident-response recommendations with Cybersecurity Framework 2.0. NIST says the AI RMF is being revised, so confirm the current status when you approve or refresh your plan.

Use the four AI RMF functions to organize ownership: Govern establishes accountability and policies; Map identifies context, intended use, affected people, and foreseeable misuse; Measure tests and monitors risk; and Manage prioritizes and treats risk. Manage 4.1 specifically calls for post-deployment monitoring with user feedback, appeal and override mechanisms, decommissioning, incident response, recovery, and change management.

What an AI incident-response plan must contain

1. Purpose, scope, and authority

State which models, applications, environments, business services, vendors, jurisdictions, and use cases are covered. Define what counts as an incident, a near miss, or a routine defect. Give named authorities the power to pause generation, disable an agent or connector, roll back a model, remove a data source, or decommission a system. Include emergency authority when waiting for normal approval would increase harm, along with a requirement to document the decision afterward.

2. Roles and contact paths

Identify an incident commander and deputies, model or AI owner, data owner, security or SOC lead, privacy officer, legal counsel, safety or responsible-AI lead, communications lead, procurement or vendor manager, business owner, and executive decision maker. List primary and alternate contacts, on-call coverage, authority limits, and the handoff rule when an incident crosses business units or jurisdictions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. System and dependency inventory

Maintain a versioned record of model weights or provider versions, system and developer prompts, policies, training and fine-tuning datasets, retrieval indexes, tools and agents, API keys, cloud accounts, monitoring, logs, human review points, downstream consumers, and third parties. Record data classifications and geographic processing locations so responders can determine quickly whether privacy or residency obligations are triggered.

4. Detection and intake

Accept signals from automated monitoring, user reports, abuse channels, red-team findings, quality evaluations, vendor notices, privacy reviews, and ordinary security alerts. Use one intake channel or ticket type with required fields: reporter, time observed, affected system and version, exact prompt or event where permissible, output or action, user impact, suspected scope, and attached evidence. Acknowledgment and escalation deadlines should be explicit.

5. Severity and activation

Rate impact across people and safety, privacy, security, bias and discrimination, financial loss, operations, legal exposure, and reputation. Define who can declare each level and how quickly specialists and executives must be engaged. The following model is a starting point; your risk committee should set the thresholds and deadlines.

Level Typical condition Activation expectation
Critical Credible risk of serious physical or societal harm, broad sensitive-data disclosure, uncontrolled external actions, or compromise of a critical service. Immediate incident command, executive notification, and consideration of shutdown or isolation.
High Material customer, privacy, security, fairness, financial, or regulatory impact affecting multiple users or a business-critical workflow. Dedicated response team, formal containment decision, and scheduled stakeholder updates.
Moderate Limited scope or reversible harm with effective compensating controls. Named owner, documented remediation, and trend review for recurrence.
Low or near miss No confirmed harm, but a control failure or unsafe behavior that could recur. Track to closure and feed the lesson into testing and monitoring.

6. Response workflow

Document the sequence responders follow, the evidence required at each gate, and the person who can advance or stop the process. Include scenario-specific branches rather than relying on one generic checklist.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

7. Scenario playbooks

Keep short, actionable procedures for the failure modes most relevant to your systems. Each should name triggers, first actions, decision points, communications, and recovery tests.

8. Evidence and records

Specify retention, access, redaction, and chain-of-custody rules. Preserve timestamps, model and prompt versions, configuration changes, input and output samples subject to privacy controls, retrieval documents, tool-call traces, approvals, analyst decisions, notifications, and hashes or immutable storage where feasible.

9. Communications and reporting

Prepare internal update templates and decision rules for notifying users, customers, affected partners, regulators, law enforcement, or the public. Legal review is jurisdiction-specific; the plan should identify who determines whether a notification duty, contractual notice, or disclosure deadline applies and who approves the wording.

How to build the plan with Govern, Map, Measure, and Manage

  1. Govern: Obtain executive sponsorship, set risk tolerance, appoint an accountable plan owner, and connect the AI process to cybersecurity, privacy, business continuity, change management, and vendor-risk procedures. Approve emergency powers before an incident occurs.
  2. Map: Inventory every AI use case and dependency. For each one, record intended use, foreseeable misuse, affected people, critical business functions, data flows, human review, external providers, and the harm that would result from wrong, biased, unavailable, or manipulated behavior.
  3. Measure: Choose indicators for model quality and drift, data integrity, prompt and retrieval abuse, security, privacy, fairness, safety, latency, and availability. Set alert thresholds, test whether logs contain the needed fields, verify that rollback and disablement work, and document limitations and residual risk.
  4. Manage: Write decision trees for the highest-priority scenarios. Pre-authorize containment such as disabling tool calls, switching to a known-good model, removing a retrieval source, enforcing human approval, rotating credentials, or taking a service offline. Define communication routes, recovery gates, and corrective-action tracking.
  5. Exercise: Run tabletop scenarios with technical and business participants. Record time to detect, time to decide, missing telemetry, authority conflicts, evidence gaps, and notification problems. Convert each finding into a dated plan change, not just meeting notes.
  6. Maintain: Review after incidents and exercises, major model or vendor changes, new tools or data sources, regulatory changes, and a scheduled cadence appropriate to risk. Manage 4.1 of the AI RMF treats monitoring, incident response, recovery, and change management as continuing lifecycle activities, so a plan that is never refreshed is not operational.

Scenario playbooks responders can use

Prompt injection or jailbreak

Freeze the triggering conversation and preserve the exact prompt, retrieved content, system instructions, model version, and tool trace. Determine whether the attack only bypassed a content policy or also exposed data or caused an external action. Isolate the affected connector or agent, add human approval, block the malicious source or session, and test the revised prompt, retrieval filters, and tool permissions against the original attack and variants before restoring automation.

Sensitive-data disclosure

Stop further generation or retrieval from the suspected source, preserve access and output logs, and identify the data owner. Revoke exposed credentials or links, limit the affected index or tenant, assess whose information was disclosed and for how long, and involve privacy and legal teams on applicable notices. Recovery requires a clean data path, a test that the information is no longer retrievable, and monitoring for copies or repeated prompts.

Poisoned or corrupted data

Quarantine the dataset, index, or pipeline stage and identify every model or report that consumed it. Compare hashes, provenance, validation results, and last-known-good versions. Roll back to verified data, retrain or re-index under controlled review, and recheck quality, fairness, and safety before releasing dependent systems.

Unsafe, biased, or materially wrong output

Preserve representative examples and the user context, then determine whether the issue is systemic, segment-specific, or tied to a prompt, retrieval source, or recent change. Add a warning or human review, restrict the affected use case, and provide a correction or remedy to impacted users where appropriate. Validate the fix with targeted evaluations and monitor for recurrence rather than relying on a single successful test.

Unauthorized agent or tool action

Immediately revoke or constrain the agent’s credentials and stop queued actions. Record the requested action, arguments, approvals, results, and external system state. Reverse transactions when possible, notify the system owner, and require explicit human confirmation until the authorization logic, tool schema, and audit trail pass testing.

Model theft or extraction

Coordinate security and model owners to preserve request patterns, account activity, rate-limit events, and repository or provider logs. Restrict suspicious access, rotate keys, and assess whether proprietary weights, prompts, training data, or personal information were exposed. Confirm that the replacement configuration does not repeat the same access weakness and document any contractual or regulatory reporting.

Denial of service

Use ordinary availability response plus AI-specific controls: rate-limit abusive prompts, disable expensive tools, fail over to a safe-capability mode, and protect queues from unbounded context or generation. Verify that fallback behavior does not silently create unsafe decisions, then restore capacity in stages while watching latency, error rates, and abuse signals.

Vendor compromise, deepfake, or harmful generated content

For a vendor event, request the provider’s incident facts, affected versions, indicators, and containment actions; isolate the integration until trust is re-established. For impersonation or deepfake content, preserve provenance and distribution evidence, coordinate with communications and legal teams, and use platform or law-enforcement channels where appropriate. Harmful generated content requires both immediate suppression and a review of safeguards, access controls, and downstream amplification.

Evidence, communications, and recovery gates

Evidence collection must balance usefulness with privacy. Restrict raw prompts and outputs to authorized responders, redact unnecessary personal information, record who accessed each item, and retain a sanitized sample for testing. Keep the original and corrected configuration versions so investigators can reproduce the behavior without altering the record.

Use a single incident timeline with observed facts, hypotheses, decisions, approvers, and timestamps. Separate confirmed impact from unverified claims. Internal updates should state what is known, what is contained, what users should do, and when the next update is due. External notices should be accurate about scope and uncertainty and should be reviewed for jurisdiction-specific obligations.

Do not declare recovery merely because service is available. Set gates such as: the vulnerable model, prompt, data source, or connector is removed or corrected; representative and adversarial tests pass; privacy and safety owners approve; rollback is still available; heightened monitoring is active; and affected users have received any required remediation or notice.

How to test and improve the plan

Tabletop exercises are the fastest way to expose missing telemetry and unclear authority. Include the incident commander, engineers, SOC, privacy, legal, safety, communications, vendor management, and the business owner. Give participants a realistic inject—for example, a prompt injection followed by a sensitive-data disclosure—and require them to make the same decisions they would make at 2 a.m., using only the documented contacts and logs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Measure time to detect, acknowledge, classify, contain, and make a recovery decision.
  • Check whether responders can identify the exact model, prompt, data, tool, and vendor versions involved.
  • Verify that emergency disablement, credential rotation, rollback, and human-approval paths actually work.
  • Test whether privacy, contractual, regulatory, and customer communications can be approved within the required window.
  • Record every missing signal, conflicting authority, and ambiguous instruction as a corrective action with an owner and due date.

Set an exercise cadence based on risk and change rate; high-impact systems may need more frequent exercises than low-risk internal experiments. Repeat a scenario after remediation to confirm that the measured gap is closed. Re-run tests after major model, prompt, data, tool, vendor, or regulatory changes, and after every real incident.

Readiness checklist

  • An accountable owner and 24-hour escalation path are named.
  • Every production model, prompt, dataset, retrieval source, tool, API, vendor, and downstream consumer has an owner and version record.
  • Severity criteria cover safety, privacy, security, fairness, financial, operational, legal, and reputational impact.
  • Logs capture prompts or event identifiers, outputs, model versions, tool calls, approvals, and timestamps with appropriate privacy controls.
  • Responders can pause generation, disable tools, rotate credentials, remove a data source, and roll back to a known-good configuration.
  • Scenario playbooks address injection, data disclosure, poisoned data, unsafe output, unauthorized action, extraction, denial of service, vendor compromise, impersonation, and harmful content as relevant to the system.
  • Recovery requires technical, safety, privacy, and business validation—not just restored uptime.
  • Exercises, incidents, major changes, and regulatory developments produce tracked updates to the plan.

The practical standard for an AI incident-response plan

A short playbook with clear authority, reliable telemetry, scenario branches, and tested shutdown and rollback paths is more valuable than a long policy that nobody can execute. Use NIST’s Govern, Map, Measure, and Manage functions to connect lifecycle risk management with incident operations, then tailor thresholds and notifications to your systems, sector, and jurisdictions. Treat every exercise and incident as a controlled change to the plan so that the next response is faster, safer, and easier to defend.
