A cyber intrusion is unauthorized access to, or compromise of, a computer, account, application, network, or cloud environment. Getting in is only one part of an incident. Attackers often try to maintain access by creating or abusing mechanisms that continue working after a restart, a password change, or another interruption. This “persistence” can involve web shells, remote-access scripts, valid accounts, exposed services, altered access controls, or modified software components. Because several access paths may coexist, removing the first malicious file or rebooting one machine does not prove that an intrusion has been fully evicted.

What a cyber intrusion means

In practical terms, an intrusion occurs when someone gains access or control without authorization. The affected asset might be a public-facing server, employee account, workstation, database, cloud resource, or an entire network. The attacker may steal information, alter systems, disrupt operations, conduct espionage, deploy malware, or use the environment as a launch point for additional activity.

An intrusion is an event or condition, not a specific piece of malware. A stolen password used to enter a remote service, exploitation of a vulnerable internet-facing application, and unauthorized changes made by an already-compromised account can all be parts of an intrusion. The visible symptom may be a suspicious process or unusual login, while the underlying compromise extends across several systems.

Initial access and persistence are different

Initial access answers, “How did the attacker get in?” Persistence answers, “How can the attacker get back in or remain present after defenders interrupt the activity?” The two goals are related but should be investigated separately.

Investigation question What it covers Why it matters
Initial access The first known path into an account, application, device, or network Closing the original weakness can stop the same entry method, but it may not remove other footholds
Persistence Changes or access paths that survive interruption and allow continued or renewed access Responders must find and remove every surviving mechanism, not just the first indicator
Lateral movement Activity that carries the intruder from one system or account to another A compromise that began on one host may require investigation across many hosts and identities

A 2025 CISA incident-response advisory illustrates the distinction: an actor exploited a public-facing GeoServer, moved to other servers, and then uploaded or attempted to upload web shells and scripts associated with remote access, persistence, command execution, and privilege escalation. That sequence is one documented case, not a fixed pattern that every intrusion follows. CISA’s incident-response advisory describes the case in detail.

How attackers try to maintain access

Persistence is best understood as an observation category for defenders, not as a recipe. Attackers can use ordinary operating-system features, valid administrative functions, altered configuration, or custom code. The specific mechanism depends on the platform, privileges obtained, and the attacker’s objective.

Web shells and remote-access scripts

A web shell is server-side code placed where a web server can execute it. It can give an intruder a way to send commands through normal web traffic, sometimes using a small file that is difficult to notice among legitimate application content. CISA-hosted MITRE ATT&CK material classifies “Server Software Component: Web Shell” as a persistence technique and describes persistence as retaining a foothold across restarts, changed credentials, or other interruptions. The technique page is available at CISA’s ATT&CK web-shell reference.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Web shells are not limited to one product or programming language. Defenders look for unexpected server-side files, unusual parent-child process relationships, web requests that trigger command execution, and access patterns that do not match the application’s normal role. Because a shell may be uploaded after the initial compromise, finding and fixing the original vulnerability is not enough by itself.

Abused accounts and remote services

An attacker who obtains valid credentials may not need to install obvious malware. Access through a remote administration service, cloud console, VPN, database interface, or other authorized channel can blend into legitimate activity. Persistence in this case may be the continued availability of the account, a newly created account, a token, a key, or another authentication method.

Evidence can include logins at unusual times, impossible travel or unfamiliar source networks, access to systems outside a person’s normal duties, new authentication factors, and administrative actions that have no corresponding change request. Investigators need to review identity systems as well as endpoint files, because disabling one suspicious process does not invalidate every credential or token that may have been exposed.

Access-control and firewall changes

CISA has reported intrusions in which actors changed access-control lists or opened services on standard or non-standard ports. A configuration change can create a durable route into a system even when no obvious malware remains. It may also expose an administrative interface to a wider network or permit traffic from an attacker-controlled location.

Responders should compare current rules with approved baselines and change records, identify who made each modification, and check whether the same rule appears on other devices. Removing an unauthorized rule is only one part of the task: the account or process that created it, and any systems reached through it, also require investigation. See CISA’s advisory on worldwide network compromises for documented examples of access-control changes and opened ports.

Startup code and altered components

Operating systems and applications provide legitimate ways to start programs automatically, load extensions, run scheduled work, and connect services. An intruder with sufficient privileges can abuse those features or replace a legitimate component with a modified one. The result may execute at boot, when a service starts, when a user logs in, or when an application handles a request.

Detection therefore requires more than scanning for a known filename. Useful evidence includes unexpected changes to startup configuration, service definitions, scheduled tasks, libraries, application modules, and deployment artifacts. File integrity records, software inventories, code-signing information, and configuration-management history help establish what changed and when. ATT&CK groups these behaviors by the adversary’s goal and technique rather than by a single universal implementation.

What a documented intrusion can look like

In the CISA 2025 incident-response case, exploitation of a public-facing GeoServer provided the entry point. The actor then moved laterally to other servers and uploaded or attempted to upload web shells and scripts. Those files were associated with remote access, persistence, command execution, and privilege escalation.

The important lesson is the investigation scope. The public-facing server was not necessarily the only affected asset, and the first exploited vulnerability was not the whole incident. A responder would need to establish which servers were reached, which accounts and privileges were used, whether the uploaded files executed, and whether similar changes remained elsewhere. The case demonstrates why an initial-access finding and a persistence finding should be tracked as separate questions.

Why a reboot or deleted file does not prove eviction

A restart can stop a running process without removing the configuration, account, service, script, or access rule that will start it again. Deleting one web shell can leave another copy on a different server, while changing one password may not revoke active sessions, application tokens, keys, or credentials stored by another service.

CISA warns that when the full scope of an intrusion is not identified and mitigated, actors may retain access and continue activity. A sound response therefore treats every indicator as a lead into a broader investigation. The question is not merely “Is the alert gone?” but “Which systems, identities, communications paths, and modifications were involved, and what evidence shows they are clean?”

How defenders investigate suspected persistence

Organizations should activate their incident-response plan and involve qualified incident responders when compromise is suspected. NICCS, a CISA resource, defines incident response and incident-response plans in its cybersecurity glossary. The following sequence keeps the investigation focused without assuming that any one security product can prove complete eviction.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Preserve evidence. Record alerts, authentication logs, endpoint telemetry, network flows, cloud-audit events, configuration history, and relevant timestamps before routine cleanup destroys context. Preserve volatile evidence when the response team’s procedures call for it.
  2. Contain carefully. Restrict compromised accounts, isolate affected hosts, block malicious destinations, or segment exposed services while avoiding changes that erase evidence or interrupt critical safety and business functions.
  3. Define the scope. Identify the first known entry point, every account used, systems contacted, privileges obtained, files or configurations changed, and time periods in which the activity could have occurred.
  4. Search for persistence across the environment. Review server-side code, startup and scheduled execution, service definitions, identity changes, remote-access settings, access-control lists, exposed ports, cloud roles, tokens, and software or deployment components. Compare findings with known-good baselines and approved changes.
  5. Remove the causes and footholds. Patch or replace the exploited component, revoke and rotate affected credentials and tokens, remove unauthorized accounts and rules, rebuild systems when trust cannot be established, and restore only from verified clean sources.
  6. Validate and monitor. Confirm that expected services work, watch for renewed authentication and command activity, and retain logs long enough to detect delayed re-entry. Document what was checked and what evidence supports the conclusion.

The correct remediation may differ by system and evidence. Reimaging a host can be appropriate when integrity is uncertain, but it does not by itself address a stolen identity, an exposed service on another host, or a cloud account that was also compromised.

Using MITRE ATT&CK without treating it as a checklist

ATT&CK gives defenders a common vocabulary for describing observed behavior. CISA explains that tactics represent adversary goals, while techniques describe the ways those goals are pursued. Mapping an event to a technique can reveal missing telemetry, guide detection engineering, and make reports easier to compare.

ATT&CK is not a fixed, left-to-right attack lifecycle. An adversary can skip tactics, revisit them, or perform activities in a different order. For example, persistence may be established before extensive lateral movement, after it, or repeatedly as new systems are reached. CISA’s Best Practices for MITRE ATT&CK Mapping explains this non-linear use and the importance of mapping evidence rather than forcing events into a predetermined sequence.

How to compare persistence patterns

No universal ranking shows that one persistence method is always more dangerous than another. The practical comparison is about what the method depends on and what evidence it leaves.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Pattern Primary dependency Evidence to examine Remediation scope
Web shell or server script Web server and writable or executable application content Unexpected files, web requests, process ancestry, server logs, code changes Application, host, credentials, and the exploited entry point
Valid account or remote service Identity, token, key, VPN, remote administration, or cloud service Authentication history, privilege changes, session data, source locations Identity systems, sessions, credentials, and every accessed asset
Access-control or port change Firewall, router, security group, ACL, or service configuration Configuration history, rule ownership, network flows, exposed listeners Network controls, the modifying account, and reachable systems
Startup or altered component Operating-system, application, service, library, or deployment feature Integrity baselines, startup entries, scheduled execution, signatures, timestamps Host or application rebuild, software supply chain, and privileged accounts

What detection gaps can teach organizations

Strong security controls reduce risk but do not guarantee that every attacker action will be detected. In a 2023 joint advisory, CISA, NSA, and the FBI described an assessment in which a team gained persistent deep access without detection of lateral movement, persistence, and command-and-control activity. That is a documented assessment example, not a prevalence rate for all organizations. The advisory is available at AA23-278A.

The defensive implication is to test assumptions. Organizations should verify that logs from identity providers, servers, network devices, cloud platforms, and endpoint tools are collected centrally; that administrators can distinguish approved changes from unauthorized ones; and that alerts are investigated in relation to one another. A single alert, product dashboard, or cleanup action cannot establish that every persistence path has been removed.

When to escalate

Escalate promptly when privileged accounts, public-facing servers, domain controllers, cloud control planes, regulated data, or multiple systems may be involved. External incident-response specialists can help preserve evidence, scope the compromise, coordinate containment, and support legal or regulatory decisions. Internal teams should follow their documented incident-response plan and avoid making unsupported claims that a system is clean simply because a file disappeared or no new alert appeared.

The central operational rule is simple: investigate the entire access chain. Identify how the intruder entered, what they changed, which identities and systems they used, and every way they could return. Only after those questions are answered and validated should an organization treat the intrusion as contained.
