A brain aneurysm is a bulge in an artery in or around the brain. Many unruptured aneurysms cause no symptoms and are discovered incidentally during imaging for another reason. Aneurysmal rupture is a different medical emergency: bleeding into the space around the brain requires immediate emergency care.

Clinicians can look for an aneurysm before rupture with CT angiography (CTA) or MR angiography (MRA). The 2015 American Heart Association/American Stroke Association (AHA/ASA) guidance specifically recommends offering screening to people with at least two affected family members and to people with autosomal dominant polycystic kidney disease (ADPKD), particularly when there is a family history of intracranial aneurysm. It does not establish routine imaging for every adult.

What a brain aneurysm is

An intracranial, or brain, aneurysm is a weakened area of an artery that balloons outward. “Unruptured” means the bulge is present but has not broken open. The term does not predict what will happen to a particular person: some aneurysms remain stable, while others may enlarge or rupture.

Most unruptured intracranial aneurysms are described as incidental findings and are typically asymptomatic. A scan obtained for headaches, trauma, hearing concerns or another medical question may reveal one even when the aneurysm was not causing the original problem. A symptom by itself cannot reliably confirm or exclude an unruptured aneurysm.

Rupture is a separate condition, usually discussed as aneurysmal subarachnoid hemorrhage. It is not simply a more noticeable version of an incidental aneurysm; it is acute bleeding that needs emergency evaluation.

Can an aneurysm be found before it ruptures?

Yes. Aneurysms can be detected before rupture when a clinician orders vascular imaging. The AHA/ASA guidance names CTA and MRA as the screening methods for relevant higher-risk groups and as options for radiographic follow-up when a known aneurysm is being observed.

Finding an aneurysm early does not mean rupture is inevitable, and screening cannot guarantee that a rupture will never occur. The value of imaging is that it gives a clinical team information for risk assessment, surveillance or a discussion of treatment rather than waiting for an emergency.

No consumer app, smartwatch, home blood-pressure device or other home test can identify, rule out or monitor a brain aneurysm. A suspected aneurysm requires clinical imaging ordered and interpreted by qualified professionals.

Who may be offered screening

The most specific recommendation in the supplied AHA/ASA unruptured intracranial aneurysm guidance is familial:

  • People with two or more family members who have had an intracranial aneurysm or subarachnoid hemorrhage should be offered aneurysmal screening.
  • People with ADPKD should be offered screening, particularly when a family history of intracranial aneurysm is present.

The guidance states that screening in these groups should use CT or MR angiography. It also identifies smoking, hypertension and female sex, along with medical or genetic conditions, as factors clinicians may consider in a higher-risk assessment. A risk factor raises a clinical question; it does not establish that an aneurysm is present.

These criteria should not be expanded into a claim that every adult needs a scan. The material cited here does not establish a universal population-screening program, nor does it settle how every person with only one affected relative should be managed. If one relative had an aneurysm or subarachnoid hemorrhage, tell your clinician exactly who was affected, what diagnosis was made and the relative’s age if known. Your clinician can place that history alongside your other risks and local guidance.

What CTA and MRA do

CTA and MRA are clinical angiography examinations that create images of blood vessels so a radiology team can look for an aneurysm. The AHA/ASA guidance names both for screening and for follow-up of an aneurysm that is being observed.

Modality What the guidance names it for How the choice is made
CT angiography (CTA) Screening in the recommended higher-risk groups and radiographic follow-up of a known unruptured aneurysm A clinician selects it according to the clinical question and the individual’s suitability
MR angiography (MRA) Screening in the recommended higher-risk groups and radiographic follow-up of a known unruptured aneurysm A clinician selects it according to the clinical question and the individual’s suitability

The cited guidance does not provide enough detail here to rank CTA against MRA for radiation exposure, contrast, implanted devices, claustrophobia, accuracy by aneurysm size or other patient-specific trade-offs. Do not choose a modality from a generic online comparison; ask the ordering clinician why a particular examination fits your situation.

What happens when imaging finds an aneurysm

An imaging finding starts a clinical evaluation; it is not a self-managed diagnosis. The treating team reviews the images, your medical history and risk context, then discusses whether the aneurysm should be observed or whether an intervention should be considered. The evidence summarized here does not provide a universal treatment threshold, because management depends on the individual clinical assessment.

If the decision is observation rather than surgery or an endovascular procedure, the AHA/ASA recommendation is regular radiographic follow-up with CTA or MRA. The same recommendation explicitly says that the ideal interval and the total duration of monitoring remain uncertain. Therefore, an article cannot responsibly prescribe a single schedule such as “scan every six months” for everyone. Keep the follow-up plan supplied by your own care team and ask what change would trigger an earlier review.

Follow-up is different from emergency assessment. A planned surveillance scan is for a person who is clinically stable and already has a management plan. New sudden neurological symptoms require emergency services even if a previous scan showed an unruptured aneurysm.

Risk reduction while an aneurysm is being observed

The European Stroke Organisation (ESO) guideline abstract recommends radiological monitoring, smoking cessation and treatment of hypertension for patients who are initially observed. These measures address modifiable risk context; they do not remove an existing aneurysm.

Smoking

If you smoke, ask a clinician for a cessation plan and support. The recommendation is directed at stopping tobacco exposure, not at replacing medical follow-up with lifestyle changes.

Blood pressure

Hypertension should be evaluated and treated with a clinician-directed plan. A home blood-pressure monitor can help record readings for that discussion, but it cannot detect an aneurysm, determine whether one has enlarged or rule out rupture. Use it only as an adjunct to medical care and follow the device and clinician instructions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Medicines

The ESO abstract specifically advises against using statins or acetylsalicylic acid (aspirin) with the indication of reducing aneurysm rupture risk. This is not an instruction to stop a medicine prescribed for another condition. Do not start, stop or change any prescription or over-the-counter drug without the clinician who manages its indication.

Recognizing a rupture emergency

An unruptured aneurysm may produce no warning symptoms, so waiting for a particular sensation is not a safe screening strategy. Conversely, a sudden severe headache or acute neurological change can signal a life-threatening event. Call your local emergency number immediately for a sudden, extremely severe headache, new weakness or numbness, trouble speaking, confusion, loss of consciousness, a seizure or sudden vision problems.

Do not drive yourself while experiencing these symptoms and do not wait for a scheduled scan. Emergency clinicians can determine whether urgent brain and vascular imaging is needed. The absence of these symptoms does not prove that an incidental aneurysm is harmless; it simply means the situation is not presenting as an obvious acute emergency at that moment.

How common are unruptured aneurysms?

Prevalence estimates depend on the population and the source. The ESO guideline from 2022 reports occurrence of unruptured intracranial aneurysms at around 3% of the population. A 2024 Chinese guideline for clinical management reports an estimate of about 7% among adults aged 35–75 in China. These figures describe different populations and should not be averaged or presented as one worldwide rate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A prevalence estimate also does not tell an individual whether an aneurysm will rupture. Personal risk assessment requires the actual clinical history, imaging findings and professional guidance.

Preparing for a screening discussion

Before an appointment, write down:

  • Which relatives had an intracranial aneurysm or subarachnoid hemorrhage, and whether more than one relative was affected.
  • Any diagnosis of ADPKD or another inherited condition.
  • Your smoking history and known blood-pressure readings or hypertension treatment.
  • Previous brain or vascular imaging and where the reports are stored.
  • Any current symptoms, including when they began and whether they were sudden.

Useful questions include: “Does my family history meet the AHA/ASA screening criterion?” “Would CTA or MRA answer the clinical question in my case?” “If an aneurysm is found, what is the follow-up plan?” and “Which symptoms should make me seek emergency help rather than wait for follow-up?”

How current the recommendations are

The familial screening recommendation described above comes from the AHA/ASA guideline on unruptured intracranial aneurysms published in 2015. The AHA/ASA guideline page addressing subarachnoid hemorrhage is dated May 22, 2023. The ESO guideline is from 2022, and the China-specific clinical management guideline is from 2024.

Guidelines can change, and practice may differ by country and health system. If you are deciding about screening now, confirm the current recommendation with a qualified clinician rather than relying on an old summary. The central distinctions remain important: an incidental unruptured aneurysm is not the same as a rupture emergency; higher-risk people may be offered CTA or MRA; and observed aneurysms require individualized radiographic follow-up.
