Use a risk-based approval policy, not a blanket “always approve” or “always automate” rule. Let AI execute only predefined, low-impact, reversible actions within a documented playbook. Require a human to confirm actions with meaningful service or data impact, uncertain scope, or material effects on users. Reserve high-consequence or difficult-to-reverse actions for an incident commander or specialist, with two-person approval when your risk assessment warrants it.
What human approval settings should you use for AI incident-response actions?
Set approval thresholds around five questions: how much harm an action could cause, how easily it can be reversed, how large and uncertain its blast radius is, whether the operator has authority and context, and whether delay would increase the damage. These criteria turn a general “human in the loop” requirement into an operating policy that responders can apply during a live incident.
There is no universal NIST action-by-action approval matrix. NIST provides risk-management outcomes and suggested controls; each organization must set thresholds for its systems, impact tolerance, contracts, laws and sector requirements. The policy should state exactly which actions AI may perform, which require confirmation, who may approve them, and how an operator can stop the system.

Automatic execution is appropriate only when the action is bounded, low impact and reliably reversible. Put every automatic action in a versioned playbook with explicit scope, evidence requirements, stop conditions and logging. Examples can include narrowly scoped enrichment, collecting diagnostic data, opening or updating an incident record, or applying a temporary control to a known, noncritical asset when the playbook defines an immediate rollback.

“Reversible” must mean operationally reversible, not merely theoretically reversible. A rollback should be tested, available to the responder and fast enough for the situation. The policy should also limit the number and type of assets the AI can touch, prevent it from expanding its own permissions, and stop execution when evidence is contradictory or confidence falls below the defined threshold.
Pre-authorized action settings
- Allow-list the exact command, API operation, asset class and maximum scope.
- Require the AI to attach the evidence and rule that triggered the action.
- Enforce time limits, rate limits and automatic expiry for temporary changes.
- Log the recommendation, execution result and rollback status.
- Give a responder a visible stop, pause or override control.
When should a human approve an AI security action?
Require confirmation when an action could materially affect service availability, users, sensitive data or business operations. Human approval is also warranted when the AI cannot establish a reliable blast radius, when evidence is incomplete, or when the proposed action crosses a delegated-authority boundary.
Human-confirmation band
Present the responder with the affected assets, supporting evidence, confidence and uncertainty, expected impact, duration, alternatives and a tested rollback path. The approval screen should identify the policy rule being applied and the identity of the person who will become accountable for the decision. Approval should expire if the underlying evidence changes materially or the incident context is updated.
Examples include isolating a production host, disabling an account used by customers, blocking traffic across a shared service, rotating credentials with downstream dependencies, or taking an application component offline. These examples illustrate policy design; an action is not automatically safe or prohibited in every environment.

Restricted or dual-control band
Require an authorized incident commander or designated specialist for broad isolation, shutdown of critical services, destructive remediation, deletion or irreversible modification of data, or any action that could affect safety or essential operations. Consider independent second-person approval when one error could create systemic outage, regulatory exposure or physical risk. If an emergency exception permits one-person action, document the exception, time limit and mandatory after-action review.
How do you decide which approval band applies?
Assess each action against the following axes and record the result in the playbook. The axes are a practical synthesis of NIST’s risk-based framework, mapped impacts, human-oversight requirements and role responsibilities.
| Decision axis | Questions for the policy owner |
|---|---|
| Impact | Could the action harm people, customers, business operations, critical services or data? What is the worst credible outcome? |
| Reversibility | Can the change be undone quickly and reliably? Is the rollback tested, and does it restore data and dependencies as well as configuration? |
| Scope and uncertainty | How many systems or users could be affected? How sensitive are they? How strong is the evidence, and could the AI be wrong about the blast radius? |
| Authority and competence | Has the organization delegated this permission? Does the responder have enough context, training and proficiency to judge the recommendation? |
| Time sensitivity | Would waiting increase harm? Does the danger of delay outweigh the risk of acting on an incorrect assessment? |
Do not let urgency silently lower the approval standard. Instead, define an emergency path in advance: who can invoke it, which limited actions are allowed, how long the authorization lasts, and who must review it afterward.
How do you keep a human in the loop during AI incident response?
1. Define roles and delegated authority
Document the human-AI configuration for every workflow. Identify the system owner, monitoring function, incident commander, approver for each action class, escalation contact and person responsible for recovery. State who may reject, pause, supersede, deactivate or override an AI recommendation. Separate the person who develops a detection rule from the person who approves a high-impact response when independence is needed.

NIST’s AI Risk Management Framework (AI RMF) Core says under Govern 3.2: “Policies and procedures are in place to define and differentiate roles and responsibilities for human-AI configurations and oversight of AI systems.” Under Map 3.5 it says: “Processes for human oversight are defined, assessed, and documented in accordance with organizational policies from the govern function.” See the NIST AI RMF Core.
2. Give approvers usable context
An approval request should show the incident timeline, affected assets, data sensitivity, detection evidence, confidence and uncertainty, proposed command or change, expected user and service impact, alternatives, rollback steps and an expiration time. Avoid a single “approve” button that hides those details. Operators need role-appropriate proficiency and training, including practice with false positives, partial outages and conflicting signals.
3. Make stopping the AI operationally real
Provide controls to reject, pause, supersede or deactivate an action and, when necessary, the AI system itself. The stop path must work even if the model is producing repeated recommendations or the surrounding automation is degraded. Keep emergency credentials and manual procedures available, and test them as part of exercises rather than assuming that a documented control will function under pressure.
4. Record every decision
Capture the recommendation, evidence shown, policy version, approver identity, decision and reason, timestamps, execution result, rollback or recovery, and any override. These fields are implementation guidance consistent with NIST’s documentation, monitoring and incident-tracking outcomes; NIST does not prescribe one mandatory field list. Protect the records from alteration and make them searchable for incident review.

How should approval settings fit the incident-response plan?
Attach AI workflows to the organization’s established incident-response plan instead of creating a parallel process. Define responsible functions and personnel for monitoring, triage, containment, eradication, recovery, communications and post-incident review. The plan should identify when an AI incident becomes a security incident, who takes command, how evidence is preserved and how affected parties are notified.
NIST’s AI RMF Manage 4.1 calls for post-deployment monitoring plans that include “mechanisms for capturing and evaluating input from users and other relevant AI actors, appeal and override, decommissioning, incident response, recovery, and change management.” The framework’s wording makes override and recovery part of ongoing operations, not optional features added after deployment.
Review both incidents and near misses. If an automatic action exceeded its intended scope, an approval request lacked critical context, or a responder could not stop the system, update the rule, playbook, training and access controls. Preserve the old policy version so investigators can reconstruct which authority applied at the time.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsExamples of approval decisions
Known malware on a disposable test endpoint
If the asset is explicitly classified as noncritical, the isolation command is allow-listed, the scope is one endpoint and the rollback is tested, automatic execution may be reasonable. The system should still log the evidence, expire the isolation when appropriate and provide a human override.

Suspected compromise of a production identity
Require human confirmation before disabling the account or rotating credentials. The approver needs to see dependent services, business ownership, recent authentication evidence and a recovery plan. A mistaken action could interrupt customers or automated jobs even if the detection is correct.
Ransomware signal across a critical service
Use restricted or dual control for broad network isolation, shutdown or destructive remediation. Escalate to the incident commander and relevant service or safety specialist. If an emergency containment step is taken before a second approver is available, limit its scope and duration and require immediate review.
What NIST guidance should inform the policy?
The NIST AI RMF Govern Playbook recommends assigning responsible functions and personnel for AI monitoring and incident response, documenting oversight and ensuring role-appropriate proficiency. The Generative AI Profile, published in July 2024, emphasizes oversight and evaluation proportional to identified risks and considers incident-response and containment roles in complex generative-AI systems.

For cyber incident handling, use current guidance: NIST finalized SP 800-61r3 on April 3, 2025, superseding SP 800-61r2. The announcement is available at NIST’s SP 800-61 revision notice. The AI RMF is voluntary guidance; applicable law, contracts, service agreements and sector rules may require stricter controls.
