For an aging large-breed dog, the core screening plan is a veterinary examination and medical workup about once or twice a year. The 2023 American Animal Hospital Association (AAHA) senior-care guidance lists a complete blood count (CBC), chemistry panel, and urinalysis every 6–12 months, with annual thyroid (T4), blood-pressure, heartworm, and tick-borne disease checks. Fecal parasite testing is recommended one to four times yearly, depending on preventive products and lifestyle.

Those are baseline guidelines, not a mandatory package for every dog. Electrocardiograms, urine protein testing, and imaging are ordered when a breed risk, symptom, physical finding, or abnormal screening result makes them useful. Large size alone does not create a universal schedule for senior X-rays.

When is a large-breed dog considered a senior?

AAHA defines a senior patient by expected lifespan: the senior period is approximately the last 25% of a dog’s projected life, continuing through the end of life. There is no single birthday that applies to every large breed. A giant breed with a shorter expected lifespan may enter this stage earlier than a somewhat smaller breed that typically lives longer.

Your veterinarian can estimate life stage using the dog’s breed or breed mix, adult size, medical history, and current health. The estimate is a planning tool, not a diagnosis. A dog can need closer monitoring before the formal senior stage if it has a chronic condition, a strong inherited risk, or a new change in appetite, thirst, movement, breathing, elimination, or behavior.

The 2023 AAHA Senior Care Guidelines emphasize that “old age is not a disease.” Aging changes the probability of certain problems, but a slower gait, weight change, lump, or altered behavior still deserves an explanation rather than being dismissed as normal aging.

The routine screening schedule

The following framework comes from AAHA’s senior-dog diagnostic frequency table. A veterinarian may shorten or extend an interval after reviewing results, medications, lifestyle, and examination findings.

Screening or assessment AAHA senior-dog frequency What it covers and when it changes
Comprehensive CBC Every 6–12 months Red and white blood cells, platelets, cell morphology, and differentials. A pathologist may review the smear when results or morphology warrant it.
Chemistry panel Every 6–12 months Common elements include total protein, albumin, ALT, glucose, BUN, creatinine, sodium, potassium, calcium, and SDMA when available.
Urinalysis Every 6–12 months Urine specific gravity, sediment, glucose, ketones, bilirubin, protein, and occult blood.
Thyroxine (T4) Annually A thyroid screen interpreted with the examination, medications, and other laboratory values.
Blood pressure Annually A noninvasive measurement that can reveal hypertension or support investigation of kidney, eye, endocrine, or neurologic disease.
Heartworm and tick-borne disease testing Annually Frequency and test selection still depend on local exposure, prevention, travel, and the veterinarian’s risk assessment.
Fecal parasite testing 1–4 times per year The interval depends on preventive products, outdoor access, contact with other animals, diet, travel, and local parasite risk.
Electrocardiogram (ECG) Annually for high-risk breeds such as Boxers and Dobermans, or when clinically indicated Used for an arrhythmia concern, an abnormal pulse, fainting, exercise intolerance, or a breed-specific cardiac risk—not automatically for every large dog.
Urine protein:creatinine ratio (UPC) When proteinuria or persistent microalbuminuria is identified A follow-up measurement that helps quantify urinary protein loss; it is not a blanket test for every senior dog.
Imaging When a region of interest is clinically indicated; breed-specific screening may be appropriate The modality and body area depend on symptoms, examination findings, breed risk, and the question the veterinarian needs to answer.

AAHA’s healthy-senior guidance describes a medical workup once or twice yearly and includes the CBC, chemistry panel, and urinalysis among possible components. The frequency table supplies the usual interval for those tests; it does not tell an owner to order tests independently or to treat a laboratory number without veterinary interpretation.

What happens at a senior wellness visit?

Laboratory work is only one part of screening. The visit should begin with a history and a physical examination that compare the dog with its previous baseline.

History questions to prepare for

  • Has appetite, body weight, water consumption, or urination changed?
  • Is the dog slower to rise, reluctant to use stairs, avoiding play, or slipping on floors?
  • Has exercise tolerance changed, or does coughing, heavy breathing, weakness, or collapse occur?
  • Are there changes in sleep, interaction, anxiety, house-training, grooming, vision, or hearing?
  • Which medications, supplements, preventives, diets, and treats are being used, and when were doses last changed?
  • Has the dog traveled, hunted, visited dog parks, eaten raw food, or had new contact with wildlife or other animals?

Short videos of the dog walking, rising, climbing a step, or turning at home can be valuable. Dogs often move differently in a clinic, and a recording can show an intermittent limp, difficulty changing position, or a change that is subtle during a brief examination.

Physical examination priorities

The veterinarian will assess body condition, muscle loss, gait, joints, spine, neurologic function, heart and lung sounds, pulses, eyes, ears, mouth, skin, lymph nodes, abdomen, and reproductive organs. Mobility findings can determine whether orthopedic examination, pain management, or imaging is appropriate. A new murmur, irregular rhythm, weak pulse, or fainting history may change the cardiac workup.

Every new or changing mass deserves attention. AAHA advises that skin masses be measured, sampled by needle aspiration when appropriate, and examined cytologically rather than assumed to be harmless. A lump that has been present for years can still change, and a small mass can be clinically important depending on its location and behavior.

What the routine laboratory tests can reveal

Complete blood count

A CBC evaluates oxygen-carrying red cells, infection- and inflammation-associated white cells, and platelets involved in clotting. Trends can be more informative than a single result. Falling red-cell values, persistent changes in white-cell populations, or platelet abnormalities may prompt a repeat sample, smear review, medication review, or investigation for bleeding, inflammation, immune disease, or bone-marrow problems.

Chemistry panel

The chemistry panel assesses organ-related and metabolic markers. BUN, creatinine, and SDMA can support kidney assessment; ALT and related values can signal hepatocellular injury; glucose can identify an abnormal blood-sugar pattern; proteins and electrolytes help interpret hydration, gastrointestinal loss, endocrine disease, and organ function. A result outside a reference interval is not automatically disease. Breed, hydration, muscle mass, fasting status, medications, and the laboratory’s own reference range matter.

Urinalysis

Urine often adds information that blood tests cannot. Specific gravity shows how concentrated the urine is, while sediment can reveal cells, crystals, casts, or organisms. Glucose, ketones, bilirubin, protein, and occult blood each have different implications and must be interpreted with the chemistry panel and clinical history. A dilute sample, a contaminated sample, or a temporary abnormality may need confirmation before a diagnosis is made.

Thyroid and blood-pressure checks

A T4 result is interpreted in context because illness and some drugs can affect thyroid values. When thyroid disease is suspected, the veterinarian may recommend additional thyroid testing rather than relying on one number.

Blood-pressure measurement is painless but can be affected by stress and technique. Repeated or carefully obtained readings help distinguish a persistent problem from a temporary clinic spike. Confirmed hypertension can damage eyes, kidneys, brain, and the cardiovascular system, so an abnormal reading may lead to an eye examination, urine assessment, medication review, or repeat measurement.

Infectious and parasite screening

Heartworm and tick-borne testing reflects exposure risk and prevention history, not simply the dog’s age. A dog that travels, lives in a region with substantial vector activity, or has missed preventive doses may need a different testing approach than a consistently protected indoor dog. Fecal testing is similarly individualized; the one-to-four-times-yearly range reflects differences in preventive products and lifestyle.

When are ECG, UPC testing, and imaging warranted?

Electrocardiogram

An ECG records the heart’s electrical activity. It is particularly relevant when a veterinarian hears an irregular rhythm, detects an unusual pulse, or learns of fainting, weakness, exercise intolerance, or episodic collapse. AAHA specifically names Boxers and Dobermans as examples of high-risk breeds for annual ECG screening. That example should not be generalized to every large breed: the decision depends on the dog’s breed, signs, examination, and known cardiac risk.

Urine protein:creatinine ratio

Dipstick protein on a urinalysis can be influenced by urine concentration and other factors. If proteinuria persists, or if microalbuminuria remains present, a UPC test can quantify protein loss and help determine whether additional kidney evaluation or monitoring is needed. It is a follow-up test triggered by a finding, not a routine replacement for urinalysis.

Imaging

Ultrasound, radiographs, echocardiography, CT, or MRI should answer a defined clinical question. The appropriate choice depends on the body region, suspected disease, urgency, and what the initial examination shows. For example, an abdominal ultrasound may be considered for unexplained weight loss or persistent laboratory abnormalities, while targeted radiographs may help evaluate a painful joint, a new cough, or a suspected mass.

The large-breed imaging caveat

Large-breed status matters, but it does not mean every older dog needs routine full-body X-rays. AAHA’s life-stage guidance says orthopedic radiographic screening may be considered in large-breed dogs as young adults. For mature adults and seniors, the guidance does not support a general imaging recommendation for all dogs; breed-specific screening may be indicated.

In a senior dog, imaging is therefore driven by a region of interest, a symptom, a physical finding, or a documented breed risk. A dog with progressive hind-limb weakness, for example, may need a focused orthopedic or neurologic evaluation. A dog with no mobility change and a normal examination does not automatically gain a health benefit from screening radiographs of every joint. Discuss the expected information, sedation requirements, cost, and how the result would change treatment before proceeding.

How breed, history, and lifestyle change the plan

There is no single test list that covers every large breed. AAHA notes that senior care may continue to screen for cancer and orthopedic, kidney, liver, heart, gland, and eye abnormalities that can be associated with breed. The relevant risk depends on the specific breed or mix, family history, prior diagnoses, and the dog’s current findings.

Give the veterinarian a complete timeline of previous illnesses, surgeries, anesthesia reactions, laboratory results, vaccination and parasite-prevention records, and all medications. A dog with chronic kidney disease, diabetes, heart disease, arthritis, cancer history, or long-term anti-inflammatory treatment may need monitoring more often than the healthy-senior framework. Conversely, a healthy dog with stable results may remain on the standard interval.

Lifestyle also changes infectious-disease and fecal testing decisions. Swimming, hunting, farm exposure, raw diets, frequent boarding, dog-park visits, international travel, and inconsistent prevention can all alter risk. Geography matters as well; heartworm and tick-borne disease exposure is not uniform across regions.

How to prepare and follow up

  1. Book a senior appointment before a problem becomes urgent. Ask whether the clinic wants fasting, a first-morning urine sample, medication timing changes, or a stool sample.
  2. Bring records and observations. List every medication and supplement, note appetite and water changes, and show videos of movement or unusual episodes.
  3. Ask what each test is intended to answer. Understanding the clinical question helps distinguish a routine baseline test from a targeted follow-up.
  4. Review results as a trend. Keep copies of CBC, chemistry, urinalysis, blood-pressure, and imaging reports so future clinicians can compare changes.
  5. Confirm the next decision. Ask whether an abnormality should be rechecked, investigated with a different test, monitored at home, or treated now.

Results should be interpreted together. Mildly abnormal kidney markers with concentrated urine, for example, mean something different from the same markers with persistently dilute urine and protein loss. The veterinarian may repeat a test under controlled conditions before labeling a chronic disease, while a marked or symptomatic abnormality may justify immediate workup.

When not to wait for the next screening interval

Routine screening is for prevention and early detection; it is not a substitute for an appointment when a dog is acutely unwell. Contact a veterinarian promptly for collapse, difficulty breathing, repeated vomiting or diarrhea, inability to urinate, sudden blindness, seizures, severe pain, a rapidly enlarging or ulcerated mass, pale or yellow gums, marked lethargy, or a sudden inability to stand or walk. New increases in drinking and urination, unexplained weight loss, persistent cough, repeated fainting, or a sustained appetite change also merit an earlier evaluation rather than waiting for the next six- to twelve-month visit.

A practical way to calendar senior care

At the beginning of the senior stage, establish a baseline examination with the routine blood and urine work listed in the AAHA table. Schedule the next visit according to the veterinarian’s interpretation and the dog’s risk; some healthy seniors are seen about twice yearly, while a stable patient may follow an annual plan for selected assessments. At each visit, compare mobility, weight, blood pressure, laboratory trends, and home observations with the previous record.

Use targeted testing when a baseline result, symptom, or breed risk points to a specific problem. A positive or persistent finding should lead to a defined follow-up plan rather than an indefinite series of tests. This approach keeps screening broad enough to find important disease while avoiding imaging or specialized testing that cannot answer a useful question.
