An AI-enabled operating room needs a layered, clinically validated system—not a single powerful computer. Plan the room around interoperable monitors and imaging, medical-grade wired networking, segmented security zones, PACS/VNA and EHR interfaces, validated AI compute, clinician displays, resilient power and storage, and a tested manual workflow for outages. Exact link speeds, GPU capacity, storage, and latency targets must come from the number and type of video or imaging streams, the model’s measured performance, retention rules, hospital standards, and a documented downtime-risk analysis.

What an AI-enabled operating room actually includes

AI is one component in a clinical information system. A practical architecture carries data from acquisition devices, through a controlled network, to inference and storage, then returns results to a display that clinicians can use without losing the ordinary surgical view. Each layer must continue to support safe care when the model, server, network, or power is unavailable.

The U.S. Food and Drug Administration (FDA) defines medical-device interoperability as the ability to “safely, securely, and effectively exchange and use information among one or more devices, products, technologies, or systems.” That definition is more demanding than simply making two products connect: the exchange must preserve meaning, security, timing, and clinical usability.

Clinical acquisition devices

  • Patient monitors, anesthesia machines, physiologic devices, and ventilators, with vendor-documented interfaces and a common time source.
  • Imaging modalities such as ultrasound, CT, MRI, fluoroscopy, or other systems used in the procedure.
  • Endoscopy and surgical-video sources, including room cameras when their use is clinically justified.
  • Microphones, environmental sensors, and positioning or tracking devices only when their purpose, privacy impact, and clinical validation are documented.

For every source, record the connector or protocol, resolution, frame rate, compression, timestamps, supported alarms, and behavior during a link loss. A device that can export a video stream is not automatically suitable for diagnostic or AI use.

Compute and inference

Inference can run on an appliance supplied for the clinical application, an on-premises GPU server, an edge server in or near the hospital, or an approved cloud endpoint. Size the platform from measured model latency, concurrent streams, image resolution, frame rate, preprocessing, and failover requirements. Consumer-GPU benchmark scores do not establish clinical suitability; validation must use the intended model, inputs, software versions, and workload.

Displays, routing, and capture

Use medical-grade displays appropriate to the diagnostic task, plus routing or matrix hardware that can send the correct source to the surgeon’s and staff’s screens. Video encoders and decoders may be required between cameras, the network, and the display system. Recording is subject to hospital policy, consent, privacy, retention, and storage capacity.

Keep a direct clinician-visible path from the source to the relevant display. The normal surgical image must remain usable when AI inference is delayed, wrong, or offline; an AI overlay should never be the only way to see the operative field.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Storage and data services

PACS/VNA should hold medical images under the institution’s retention and access rules. NIST describes PACS as a system that “centralizes functions surrounding medical imaging workflows and serves as an authoritative repository of medical image information.” AI inputs, outputs, logs, and derived files may need a separate secure file or object store, with backup copies, retention controls, and tested restoration. Decide whether AI results are stored as images, reports, structured objects, or application records before deployment.

Power and physical facilities

  • Separate electrical circuits where the hospital standard requires them, with UPS support and emergency-power coverage.
  • Rack cooling sized for the sustained compute and switching load, not only the nameplate peak.
  • Locked racks or rooms, controlled physical access, and tamper-evident procedures for removable media.
  • Clearly labeled cables and ports that tolerate the room’s cleaning agents, movement, and infection-control requirements.

How to design the network

Use a wired clinical network with capacity headroom and redundant paths. Wireless can support selected peripheral or mobility use cases, but critical imaging, video, monitoring, and inference paths should follow the hospital’s approved wired design unless a wireless link has been specifically validated for that role.

Separate traffic into security zones

Create zones according to the hospital security architecture rather than placing every device on one flat VLAN. A typical design separates:

  • Medical-device zone: monitors, anesthesia equipment, imaging consoles, and other clinical endpoints.
  • Imaging and PACS zone: modality gateways, DICOM services, archive systems, and workstations.
  • AI-compute zone: inference servers, model-management services, and approved orchestration components.
  • Building or guest zone: facilities, visitor, and nonclinical traffic.
  • Administrative zone: support, identity, patching, and security-management systems.

Firewalls and access-control lists should permit only documented flows between a source, destination, protocol, and purpose. Microsegmentation can further isolate devices that share a broader zone. Centralized logging, vulnerability management, strong authentication, encryption, and monitored privileged access are baseline controls, not optional add-ons.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Choose link capacity from measured traffic

Do not pick a switch or uplink by counting devices alone. Estimate the sustained and burst traffic for every stream, then add protocol overhead, simultaneous users, archive transfers, software updates, and a headroom margin required by the hospital. A simple planning expression is:

Required capacity = sum of concurrent stream bitrates + control and storage traffic + failover margin

For compressed video, use the encoder’s measured bitrate at the chosen resolution and frame rate; for uncompressed or lightly compressed feeds, calculate from pixels, color depth, and frames per second and then confirm the actual transport format. Repeat the calculation for the busiest plausible case, such as several cameras, an imaging modality, PACS synchronization, and an AI result being returned at the same time. No universal port speed is established, so a hospital should specify the result in its own design rather than copy a generic number.

Make time synchronization a requirement

Align clocks across monitors, cameras, imaging devices, switches, servers, PACS, and the EHR interface. Correlated events are clinically meaningful only when timestamps are trustworthy. Document the time source, synchronization protocol, expected accuracy, alert threshold, and what happens when a device loses synchronization.

Interfaces that make the data clinically usable

DICOM for imaging

DICOM is the principal interface for modality-to-PACS communication in the architecture described by NIST. Confirm each modality’s supported services, query and retrieve behavior, storage commitment, patient and study identifiers, character sets, and handling of rejected or corrected studies. An AI service should receive the correct study and series, preserve identifiers and metadata according to policy, and return a result format that downstream systems can display and archive.

HL7 for clinical-system exchange

HL7 interfaces carry clinical-system exchanges, including protected health information, between systems such as the EHR, ADT services, orders, results, and the AI application. Define message ownership, acknowledgment and retry behavior, duplicate handling, patient-identity matching, downtime queues, and reconciliation after recovery.

Other device standards

FDA-recognized standards relevant to this design include DICOM, ISO/IEEE 11073, and AAMI/ANSI/UL 2800. A standard name alone does not prove interoperability: test the exact profile, implementation, optional fields, alarms, units, and failure behavior with the specific products.

Where should the AI run: edge, on premises, or cloud?

Evaluate each architecture against latency, outage tolerance, data egress, privacy, and maintenance. The right answer can differ by use case; a live surgical-video overlay has different timing and failure consequences from an overnight image-triage job.

Architecture Strengths Risks and questions Best fit when
Edge appliance or nearby server Short path to cameras and devices, low dependence on a wide-area link, local buffering or inference during external outages Local hardware, cooling, patching, physical security, and spare capacity are the hospital’s responsibility Latency and continuity are critical, or video and images should remain on site
Central on-premises GPU server Shared capacity, institutional control, integration with local PACS and identity systems A data-center or campus-network failure can affect multiple rooms; capacity planning and failover are required Several rooms need a managed pool of validated models and the hospital operates its own data center
Approved cloud endpoint Elastic compute, managed infrastructure, and access to a service that may be difficult to operate locally Wide-area latency, data egress, identity and encryption configuration, availability commitments, retention, and outage procedures must be demonstrated The use case tolerates network dependence and institutional policy permits the data flow
Hybrid Time-critical preprocessing or fallback locally, with nonurgent workloads or centralized management elsewhere More interfaces, duplicated controls, and a larger validation and monitoring surface Different workloads have different latency, privacy, or resilience needs

Whichever location is selected, validate the complete path with real streams and expected concurrency. Measure time from acquisition to result display, not just GPU inference time, and define an alert when latency, queue depth, or data loss exceeds the clinically accepted threshold.

Integrated stack or best-of-breed components?

Decision axis Integrated stack Best-of-breed components
Interoperability Fewer interfaces may simplify initial deployment, but proprietary dependencies can limit later choices Open standards can improve choice, while every interface requires testing and ownership
Validation burden A coordinated release may reduce integration work if the supplier validates the full path Each component and the end-to-end combination need documented validation
Support ownership One primary escalation path can simplify incidents Responsibility must be assigned across device, network, archive, and AI suppliers
Upgrade control Updates may be coordinated but tied to a vendor release schedule Teams can update parts independently, increasing change-control and regression-testing work

Put the choice in a support matrix: who owns each interface, clock, certificate, firewall rule, model version, backup, and incident response step. “Works in a demonstration” is not a support model.

Storage, retention, and recovery requirements

Separate authoritative records from working data

Define whether PACS/VNA is the authoritative record for images, while a secure object or file store holds temporary frames, feature data, model outputs, and logs. Apply the minimum necessary data principle, retention schedules, legal-hold rules, and deletion workflows to each class. Ensure that an AI result can be traced to the source study, model version, input timestamp, and user or system action.

Design backup and restore before go-live

  • Keep backup copies on failure domains different from the primary system.
  • Encrypt data in transit and at rest, with managed key access and rotation.
  • Test restoration of images, worklists, AI outputs, configuration, and audit logs—not only a database snapshot.
  • Record recovery-time and recovery-point objectives for every service and test them with operating-room staff.
  • Reconcile data generated during an outage so delayed images, orders, and results are not duplicated or lost.

Cybersecurity and the AI lifecycle

FDA says AI-enabled devices require attention during development, validation, deployment, monitoring, maintenance, and modification. Maintain an inventory containing each device, software and model version, intended use, training and validation provenance, performance thresholds, approval status, and rollback package.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Controls to implement

  • Identity and access: role-based access, multifactor authentication where appropriate, least privilege, separate privileged accounts, and prompt removal of departing users.
  • Data protection: encryption or approved anonymization, authenticated data sources, integrity checks, and protection against tampered inputs.
  • Input verification: confirm patient, procedure, modality, orientation, units, and image or video quality before inference.
  • Monitoring: track uptime, latency, queue depth, rejected inputs, confidence or threshold behavior, drift indicators, and clinically significant false results.
  • Change control: approve updates, test them in a representative environment, document the decision, and retain a rapid rollback path.
  • Auditability: log who viewed, accepted, rejected, overrode, or exported an AI result and preserve the relevant model and configuration identifiers.

FDA’s 2025 cybersecurity guidance addresses secure design, labeling, and premarket documentation. FDA also notes that manufacturers and healthcare delivery organizations share responsibility for mitigating risk, so a contract cannot replace local governance.

Displays, alerts, and clinician control

Show AI status plainly: available, processing, delayed, unavailable, or requiring review. Distinguish an unprocessed image from a negative result, and display the timestamp and model or application identity when policy requires it. Avoid alert designs that obscure the operative view or encourage staff to treat a probability or suggestion as a diagnosis.

Unless a specific authorized device supports a more autonomous function, treat AI output as decision support. Require clinician review, retain an audit trail, and make it easy to dismiss or override an output. The direct source image and ordinary manual controls must remain available throughout the procedure.

Downtime: what happens when the network or AI fails?

Downtime planning is a clinical workflow, not merely an IT runbook. Write and rehearse a branch for loss of the AI service, loss of the local network, loss of PACS or EHR access, and loss of power. The team should know which alarms remain local, where cached worklists and images are available, how to document decisions, and who declares recovery.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Recommended resilience measures

  • Local buffering or edge processing when clinically justified and validated.
  • Redundant network paths and switches, with tested failover behavior.
  • UPS and emergency power for acquisition, networking, compute, and displays that must remain available.
  • Cached worklists and the minimum images or reports needed for the active case, subject to policy and privacy controls.
  • Break-glass access with strong logging and post-event review.
  • A documented procedure to revert to non-AI care without delaying surgery.

Recovery procedure

  1. Announce the affected service and switch to the approved manual workflow; do not wait for an uncertain AI status.
  2. Keep the direct clinician-visible video or imaging path active and verify patient and procedure identifiers manually if interfaces are unavailable.
  3. Record observations, decisions, and timestamps in the downtime record.
  4. After service restoration, reconcile queued images, orders, results, and audit events with PACS and the EHR.
  5. Confirm that the model version and configuration are the approved ones before returning AI output to clinical use.
  6. Review the incident with the operating-room, biomedical-engineering, informatics, and security teams, then update tests and procedures.

A commissioning checklist for the project team

  1. Define intended use: specify the procedure, users, inputs, output, review responsibility, and unacceptable failure modes.
  2. Inventory sources: list every monitor, imaging modality, camera, sensor, display, encoder, and interface, including time-synchronization support.
  3. Model the workload: measure resolution, frame rate, compression, concurrent streams, inference latency, storage growth, and peak transfer patterns.
  4. Choose placement: compare edge, on-premises, cloud, and hybrid options for latency, outage tolerance, privacy, egress, and maintenance.
  5. Engineer the network: document zones, redundant paths, firewall rules, authentication, encryption, logging, and permitted flows.
  6. Validate interoperability: test DICOM, HL7, ISO/IEEE 11073, AAMI/ANSI/UL 2800, or other required profiles with the exact products and software versions.
  7. Validate clinical behavior: test expected cases, poor-quality inputs, wrong-patient protection, delayed results, false alerts, and manual override.
  8. Test resilience: pull network paths, stop inference, fail over power, restore backups, and reconcile data with the OR team present.
  9. Operate the lifecycle: maintain the inventory, monitor performance and drift, approve changes, and keep rollback and support contacts current.

How much hardware and bandwidth should you specify?

There is no safe universal bill of materials. A single room with one compressed camera and occasional still images has a different profile from several high-resolution cameras, fluoroscopy, physiologic data, and simultaneous AI processing. Request these values from vendors and verify them in a representative test:

Requirement What to obtain Why it changes the design
Video and imaging Resolution, frame rate, codec, bitrate range, burst behavior, and number of concurrent feeds Determines switch ports, uplinks, encoder capacity, GPU preprocessing, and storage growth
AI workload Measured end-to-end latency, throughput, memory use, supported accelerators, and failover behavior Determines server or appliance size and whether local processing is necessary
Retention Days or years by data type, legal holds, compression, backup copies, and restore objectives Determines PACS/VNA, object storage, backup, and network-transfer capacity
Availability Permitted outage, recovery time, recovery point, and number of simultaneous rooms affected Determines redundancy, UPS, local buffering, spare hardware, and staffing
Security policy Required zones, identity provider, certificates, encryption, logging, and remote-support method Determines firewalls, segmentation, access controls, and operational workload

As of September 2026, the FDA reports more than 1,600 AI-enabled medical devices authorized for marketing in the United States. That figure describes authorization activity, not a standard architecture or proof that any particular device fits an operating-room workflow. Confirm the device’s intended use, region, edition, interfaces, and authorization status before procurement.

Bottom-line architecture

Specify the operating room as a resilient clinical system: interoperable acquisition devices feed a segmented, monitored wired network; DICOM and HL7 preserve meaning across PACS and clinical systems; validated compute produces measurable, reviewable results; secure storage and backups preserve the record; and redundant power, direct displays, local buffering, and a rehearsed manual workflow protect the patient when AI or connectivity fails. Procurement should approve the complete end-to-end design and its downtime tests—not just a camera, GPU, or software license.
