Short answer: Weight regain is common after semaglutide is stopped, and some improvements in blood pressure, cholesterol or other cardiometabolic measures can move back toward baseline. In the STEP 1 extension, adults without diabetes who had taken weekly semaglutide 2.4 mg for 68 weeks regained an average of about two-thirds of the weight they had lost during the following year without treatment. That is a group average, not a personal forecast. Ozempic and Wegovy contain semaglutide, but the withdrawal evidence in this study came from an obesity trial of 2.4 mg semaglutide in adults without diabetes—not from a trial of people with type 2 diabetes stopping Ozempic.
If you are considering stopping, arrange a follow-up plan with your prescriber. It should cover weight monitoring and, if you use semaglutide for diabetes, blood-glucose checks and alternative or ongoing treatment.
What usually happens after semaglutide is stopped?
The most consistent finding is recurrence of weight. The STEP 1 withdrawal extension found that people who had lost substantial weight with weekly semaglutide regained much of it after the injections and the study’s structured lifestyle program ended. The average participant still weighed less than at the start one year later, but the amount of regain was large.
Health measurements that had improved while taking semaglutide also tended to move back toward their starting values. The study authors did not report that every measure returned completely to baseline, nor that every participant followed the average pattern. Some people retained meaningful weight loss at one year.

Individual results can differ because the trial tested one dose, one treatment period, a particular population and a specific follow-up design. The result should therefore be used to plan monitoring, not to predict exactly how many pounds a particular person will regain or when that will happen.
What the STEP 1 withdrawal study actually measured
The evidence most often cited comes from the exploratory extension of the STEP 1 randomized trial, published in 2022. It enrolled 1,961 adults with obesity, or with overweight plus at least one weight-related condition. Participants did not have diabetes. They received either weekly subcutaneous semaglutide 2.4 mg or placebo, alongside lifestyle intervention, for 68 weeks.
After week 68, both the medication and the structured lifestyle intervention were discontinued. The extension analyses included 327 participants from the original trial and followed them to week 120. Because the lifestyle support stopped at the same time as the drug, the extension was not a controlled experiment isolating medication withdrawal from every other change in care.

The main numbers
| Measure | Former semaglutide group | Former placebo group | What the number means |
|---|---|---|---|
| Mean weight loss at week 68 | 17.3% of starting body weight | 2.0% | Average change after 68 weeks of assigned treatment and lifestyle intervention |
| Weight regained by week 120 | 11.6 percentage points of prior weight loss | 1.9 percentage points | Average regain during the year after treatment and structured support ended |
| Weight still below baseline at week 120 | 5.6% | 0.1% | Average difference from each group’s starting weight |
The extension authors summarized the semaglutide group as having regained about two-thirds of its previous weight loss within one year after withdrawal. That phrase describes the average across the analyzed group. It does not mean two-thirds is an inevitable outcome for every person, and it does not establish a fixed timetable for regain.
Why these results are not a personal prediction
- The participants were adults without diabetes, so the findings do not directly forecast blood-glucose changes in someone with type 2 diabetes.
- The tested dose was 2.4 mg weekly, the dose used in the obesity trial. A person may have taken a different semaglutide product, dose or treatment duration.
- Only 327 original participants contributed to the extension analyses, making it an exploratory follow-up rather than a new large randomized withdrawal trial.
- Medication and structured lifestyle intervention ended together. The design cannot determine how much of the later change was attributable to drug withdrawal, loss of support, or both.
- Group means hide a wide range of individual outcomes. Some participants maintained a substantial reduction from baseline at week 120.
Ozempic and Wegovy are related, but the evidence is not interchangeable
Both products contain semaglutide, but they are marketed for different treatment contexts. FDA review material describes Wegovy for chronic weight management and Ozempic at different marketed doses for glycemic control in adults with type 2 diabetes. The product indication, prescribed dose, accompanying treatment and reason for taking it all affect what a clinician must monitor.
| Question | What the STEP 1 evidence supports | What it does not establish |
|---|---|---|
| Who was studied? | Adults with obesity, or overweight plus a weight-related condition, without diabetes | People with type 2 diabetes stopping Ozempic |
| What treatment was withdrawn? | Weekly subcutaneous semaglutide 2.4 mg after 68 weeks | Every Ozempic or Wegovy dose, schedule or duration |
| What outcome was measured? | Body weight and several cardiometabolic measures through week 120 | An individualized blood-glucose forecast after stopping |
For someone taking Ozempic to control type 2 diabetes, the practical issue is not only possible weight regain. Blood glucose may require a new monitoring and treatment plan. The cited evidence does not provide an individualized glucose prediction, so your prescriber should decide whether another medicine, continued therapy or a different monitoring schedule is appropriate.

What can happen to cardiometabolic health?
During STEP 1, semaglutide was associated with improvements in several cardiometabolic measurements. By week 120, most measured variables had moved toward baseline in the former semaglutide group. The study does not support saying that every benefit disappeared or that every participant experienced the same change.
This matters because weight is only one part of treatment. A decision to stop should also consider the health reason the medicine was prescribed, the measurements that improved, current risk factors and whether another therapy will replace it. The American Diabetes Association’s 2026 Standards describe obesity as a chronic, relapsing disease and note that stopping chronic obesity medication often leads to weight recurrence and worsening or reemergence of cardiometabolic risks.
The ADA states: “Obesity is a chronic, relapsing disease, similar to hypertension, and typically requires continuation of pharmacotherapy after weight reduction goals are achieved to sustain weight loss and health benefits.” (American Diabetes Association, Standards of Care in Diabetes—2026.) This is guidance for shared decision-making, not a rule that every person must remain on one specific drug indefinitely.

If you are considering stopping, make a follow-up plan first
Stopping should be handled as a treatment decision rather than an unmonitored experiment. The exact plan depends on whether the medicine was being used for weight management, glucose lowering or both.
- Confirm the treatment goal. Ask whether the primary goal is chronic weight management, diabetes control, or both. That determines which measurements need to be followed and whether another treatment must start or continue.
- Review your response and tolerability. Bring your weight history and any home glucose readings, if you use them. Discuss the benefits you achieved, side effects or treatment burden, access and cost concerns, and what you would gain or risk by continuing.
- Agree on how the change will be made. The sources reviewed here do not establish a proven tapering schedule or show that tapering prevents regain. Do not invent a dose-reduction plan from internet advice; ask the prescriber whether to stop, adjust or replace treatment and what timing applies to your prescription.
- Set a weight-monitoring method. The ADA supports close weight monitoring when stopping is chosen. A home scale can be an optional way to observe trends if your clinician recommends it. It is a monitoring aid, not a treatment for regain. Decide in advance what change should prompt a call and how results will be recorded.
- Set a blood-glucose plan when relevant. If you have diabetes, ask which readings to take, how often to take them, what range requires contact, and which medication or dose will provide ongoing control. The STEP 1 withdrawal data cannot answer those questions for an individual.
- Schedule reassessment. Arrange a clinician follow-up rather than waiting until weight or glucose has changed substantially. The timing should reflect your diagnosis, other medicines and the monitoring plan your prescriber sets.
Can diet, exercise or a taper guarantee that weight will stay off?
No strategy in the cited sources has been proven to prevent regain after semaglutide cessation. The ADA notes that regular physical activity, self-monitoring and a nutrient-dense dietary pattern have been reported among successful long-term weight-loss maintainers. It also explicitly says these approaches have not been validated specifically in the post–GLP-1 therapy setting.

How to compare continuing treatment with stopping
A clinician can use the following decision axes to make the choice explicit. There is no single best option for every patient, and the cited guidance does not rank one medication or stopping strategy above all others.
| Decision axis | Questions to discuss |
|---|---|
| Purpose | Is treatment intended to maintain weight loss, control blood glucose, reduce health risk, or address more than one goal? |
| Expected benefit | What improvement has been achieved, and what benefit is likely to be lost if treatment ends? |
| Tolerability and burden | How do injections, side effects, appointments and daily treatment demands affect your willingness or ability to continue? |
| Cost and access | Is ongoing coverage or supply reliable, and what alternatives are clinically appropriate if access changes? |
| Monitoring | Which weight, glucose or other health measurements will be checked, by whom and at what point should the plan change? |
| Long-term preference | Would you prefer continued treatment at an individualized effective dose, or a supervised stop with close monitoring and a contingency plan? |
The ADA describes continued medication at an individualized effective dose or stopping with close weight monitoring as decisions to make jointly with a clinician. The choice should account for your treatment purpose and risks rather than treating the STEP 1 average as a requirement.
What the current evidence cannot tell you
- It cannot calculate exactly how much weight you will regain.
- It cannot identify the week when regain will start or end.
- It cannot prove that all cardiometabolic benefits will disappear.
- It cannot predict your blood glucose after stopping Ozempic if you have type 2 diabetes.
- It cannot show that a particular diet, exercise routine, self-weighing schedule or taper will guarantee maintenance.
- It cannot separate the effect of stopping semaglutide from stopping the trial’s structured lifestyle intervention.
For the full study details, see the STEP 1 trial extension report. Current prescribing information and product-specific instructions should be checked against the latest FDA review material and discussed with the prescriber managing your care.
