Stopping a GLP-1-based weight-management medicine commonly raises the risk that weight will return, but the amount and timing vary widely. In the SURMOUNT-4 trial, adults who switched from tirzepatide to placebo regained an average 14.0% of their week-36 body weight over the next 52 weeks, while those who continued treatment lost another 5.5%. That result is a population average—not a forecast for one person, every medicine, or every reason for treatment. Blood sugar, blood pressure, lipids, appetite, digestive symptoms, and obesity-related complications may also change, so stopping should be planned with the prescribing clinician.

First, “GLP-1 drug” can mean different medicines

People often use “GLP-1” as shorthand for several medicines used for diabetes or chronic weight management. The exact molecule, dose, indication, treatment duration, and local regulatory label matter when estimating what may happen after discontinuation.

  • Semaglutide is a GLP-1 receptor agonist used in different formulations and doses for different indications.
  • Tirzepatide activates both GIP and GLP-1 receptors. Calling it simply a GLP-1 drug is common in everyday conversation, but it is pharmacologically a dual GIP/GLP-1 agonist.
  • Indications and approved doses differ by product and jurisdiction. A medicine prescribed for type 2 diabetes raises different follow-up issues from one prescribed only for weight management.

Because withdrawal studies do not all use the same medicine or measure the same outcome, percentages should not be compared casually. A percentage of total body weight is not the same as a percentage of weight previously lost.

What the strongest withdrawal trial found

SURMOUNT-4 design

SURMOUNT-4 was a phase 3 randomized-withdrawal trial published in 2024. It enrolled adults with obesity, or overweight plus a weight-related complication, at 70 sites in Argentina, Brazil, Taiwan, and the United States. Participants did not have diabetes. Everyone first received open-label tirzepatide for 36 weeks and received lifestyle counseling throughout.

Of the 670 people who completed the lead-in and entered randomization, one group continued tirzepatide and the other switched to placebo for another 52 weeks. The trial therefore studied people who tolerated treatment and completed the initial period; it was not a test of abrupt stopping in routine practice.

The reported numbers

Measure Continued tirzepatide Switched to placebo
Average change during the initial 36-week tirzepatide period 20.9% mean weight reduction among participants who entered randomization
Mean change from week 36 to week 88 5.5% additional mean weight reduction 14.0% mean regain of week-36 body weight
Participants maintaining at least 80% of their lead-in weight loss at week 88 89.5% 16.6%

The 14.0% figure is a change from the participants’ week-36 body weight during the 52-week withdrawal period. It is not “14% of the weight lost,” and it does not mean that every participant regained all lost weight. The authors concluded that withdrawing tirzepatide led to substantial regain, whereas continued treatment maintained and extended the initial reduction.

What this trial cannot predict

  • It did not include people with type 2 diabetes, so it does not answer how stopping should be managed when glucose control is the primary concern.
  • It studied tirzepatide, not every semaglutide or other GLP-1-based regimen.
  • Participants had already completed 36 weeks of treatment and tolerated it. Their experience may differ from someone stopping after a shorter period or because of side effects.
  • The protocol included lifestyle counseling and scheduled follow-up, unlike an unsupervised medication interruption.
  • A group average cannot determine an individual’s weight trajectory, appetite, laboratory results, or symptoms.

For perspective only, if a hypothetical person weighed 100 kilograms at week 36, a 14.0% change would correspond to 14 kilograms. That arithmetic illustrates how the trial expressed the result; it is not a prediction for a particular patient.

Will weight come back after stopping?

Weight recurrence is a likely risk, not a certainty or a fixed amount. The American Diabetes Association (ADA) states in its 2025 Standards of Care in Overweight and Obesity that clinicians should continue obesity medications after treatment goals when appropriate to maintain health benefits, because discontinuation often results in weight recurrence and worsening or reemergence of cardiometabolic risk factors or obesity-related diseases and complications.

The ADA recommendation does not require every person to stay on one medicine indefinitely. It supports an individualized decision about maintenance, switching, dose changes, or stopping, with monitoring when treatment is withdrawn. A person may stop because of adverse effects, pregnancy planning, cost or access, a change in diagnosis, inadequate benefit, or a shared decision that the balance of benefits and burdens has changed. Each reason calls for a different plan.

What other health changes can occur?

Blood sugar and diabetes management

If the medicine is treating type 2 diabetes, stopping can remove part of the glucose-lowering regimen. The SURMOUNT-4 participants did not have diabetes, so its weight results do not establish a safe diabetes-discontinuation strategy. The prescriber may need to arrange glucose checks and another treatment plan before the last dose rather than waiting for readings to rise.

Blood pressure, lipids, and other cardiometabolic measures

Some improvements associated with weight loss or treatment may weaken as weight returns. The ADA specifically warns about worsening or reemergence of cardiometabolic risk factors and obesity-related disease and complications after discontinuation. That warning describes a risk at the population level, not a guarantee that an individual’s blood pressure, glucose regulation, cholesterol, or other measure will change in a particular direction or on a particular schedule.

Obesity-related complications

Conditions affected by body weight can become more difficult to control when weight recurs. Follow-up should therefore address the conditions for which treatment was prescribed, not just the number on a scale. A clinician may review symptoms, laboratory results, blood pressure, sleep-related problems, mobility, or other relevant measures according to the person’s medical history.

What happens to appetite and digestive symptoms?

People commonly ask whether hunger, fullness, nausea, or bowel symptoms change immediately after the last dose. The reviewed evidence does not quantify a reliable timetable or frequency for those changes after stopping.

The current United States prescribing information for tirzepatide (Zepbound), revised August 2026, lists nausea, diarrhea, vomiting, constipation, abdominal pain, dyspepsia, fatigue, hair loss, and gastroesophageal reflux disease among common adverse reactions during treatment. That label does not establish how quickly any of these effects resolve after discontinuation. Appetite changes after stopping are plausible concerns, but the available sources do not support promising a specific day, week, or sequence.

Report persistent or troubling symptoms to the prescribing team. Do not assume that a symptom is caused by withdrawal, and do not restart, halve, or substitute doses without medical advice.

How to plan a supervised change

There is no one-size-fits-all taper schedule supported by the evidence summarized here. The safest process is a clinician-guided decision that accounts for the drug, indication, dose, duration, other medicines, and the reason for stopping.

  1. Contact the prescriber before changing the next dose. Explain whether the goal is to stop, switch, reduce, or pause, and why.
  2. Confirm which condition is being treated. A weight-management plan may be different from a diabetes plan or treatment for another condition.
  3. Review alternatives and maintenance options. The clinician may discuss continuing treatment, changing medicines, adjusting the dose, or using another evidence-based approach.
  4. Set follow-up measurements. Agree on when to check weight trends, glucose if relevant, blood pressure, symptoms, and laboratory tests.
  5. Decide what would trigger an earlier call. Ask which changes require prompt contact rather than waiting for the next routine appointment.

A planned change is also an opportunity to review nutrition, activity, sleep, mental health, and other factors that support long-term care. These measures complement medical follow-up; they are not proof that weight regain can be prevented for everyone.

Monitoring after discontinuation

Use trends, not a single reading

An optional home scale can help show a trend if weighing is comfortable and clinically appropriate. Record measurements under reasonably similar conditions and look at the pattern over time rather than reacting to one fluctuation. A scale is a monitoring tool, not a diagnostic device or a replacement for follow-up.

Track the measures that match the indication

  • For diabetes, follow the glucose-monitoring plan supplied by the care team.
  • For hypertension or cardiovascular risk, use the blood-pressure and laboratory schedule your clinician recommends.
  • For obesity-related complications, monitor the symptoms and functional measures relevant to that condition.
  • Keep a record of appetite, gastrointestinal symptoms, medication changes, and barriers such as access or cost so the next visit can address causes rather than guessing.

What is known about semaglutide withdrawal?

The SURMOUNT-4 publication cites a 2022 extension of the STEP 1 semaglutide trial titled “Weight regain and cardiometabolic effects after withdrawal of semaglutide.” The existence of that publication supports studying withdrawal as a separate question, but the full results were not available in the evidence reviewed for this article. A numerical semaglutide-regain figure should therefore not be borrowed from secondary summaries and presented as though it were established here.

When comparing withdrawal evidence, check five details: the molecule, the treatment indication, whether participants had diabetes, how long they received treatment before withdrawal, and whether the study randomized them to continue therapy or switch to placebo. Also check whether the reported percentage refers to total body weight or to previously lost weight.

Questions to ask before your last dose

  • What is the exact medicine, dose, and indication I am stopping?
  • Could stopping affect my diabetes or another condition even if my weight remains stable?
  • Do you recommend continuing, switching, or changing the dose instead?
  • Which measurements should I record at home, and how often?
  • When should I repeat laboratory tests or schedule a follow-up visit?
  • Which symptoms or readings should prompt an earlier call?
  • If weight starts returning, what is our agreed next step?

The central evidence is consistent: stopping a medicine that has been helping with chronic weight management can allow substantial weight recurrence, while continued treatment can preserve or extend losses in people who remain on it. The size and consequences of that change depend on the individual and the indication, so a prescriber-guided plan and follow-up are more reliable than a do-it-yourself stop or taper.
