In Act I, sacrificing a creature to pay a Blood cost normally kills it and removes it from your side of the board. The sacrifice supplies Blood toward the card you are playing and normally gives you 1 Bone when the creature perishes. A creature with Worthy Sacrifice supplies 3 Blood instead of 1, while a creature with Many Lives survives the sacrifice but still supplies its Blood value.
That answer applies to the Leshy-style Blood system, not every place where Inscryption uses the word “sacrifice.” At the Bone Altar, a card is permanently removed from your deck for a boon; at a Campfire, sacrifice-like language describes the danger of survivors eating an upgraded card. The location and act therefore matter.
How a Blood sacrifice works in Act I
The familiar sacrifice happens when you play a Leshy-style card with a Blood cost of 1 or more. Blood is not paid from a permanent wallet: you create it by offering eligible creatures already on your side of the battle board.

- Choose a card with a Blood cost. The card must require at least 1 Blood. The game then asks you to select creatures that can be sacrificed.
- Select worthy creatures on your board. Only creatures already on your side of the board can be chosen for this payment. The game prevents the play if you do not have enough eligible Blood value.
- Confirm the sacrifice. Each selected creature contributes its sacrifice value. A normal eligible creature contributes 1 Blood; one with Worthy Sacrifice contributes 3.
- Resolve what happens to each creature. A normal creature perishes and is removed from the board. A creature with Many Lives remains in play instead of dying, although its Blood still counts.
- Complete the card play. The collected Blood pays the new card’s Blood cost. Any selected creature that actually perishes also gives 1 Bone.
The important distinction is between the resource and the creature. Blood goes toward the card you are playing; the sacrificed creature does not come back to your hand or deck as part of this action. For a normal creature, the board state changes because that card has died.

What counts as enough Blood
For an ordinary cost of 1, one normal eligible creature is sufficient. A cost of 2 requires two points of ordinary sacrifice value, such as two normal creatures, unless a higher-value creature supplies more. A single Worthy Sacrifice creature supplies 3 Blood, so it can cover a cost of 1, 2, or 3 by itself if the card is otherwise playable.
The game checks the total sacrifice value, not merely the number of creatures selected. If the total is too low, the play is blocked rather than allowing you to pay part of the cost.
What happens to the sacrificed card
A normal Blood sacrifice is a death, not a temporary move. The creature is removed from the board and counts as having perished. It does not return to your deck simply because it was used to pay a cost. This is why a sacrifice can both improve your immediate board position by enabling a stronger card and reduce the number of creatures you have in play.
How many Bones you receive
Each card that perishes through sacrifice grants 1 Bone. The Blood value and Bone reward are separate: Worthy Sacrifice changes the Blood supplied from 1 to 3, but it does not increase the Bone reward. A normal creature that dies gives 1 Bone, and a Worthy Sacrifice creature that dies also gives 1 Bone.

Many Lives changes survival, so it also changes the Bone result. Because that creature does not perish, it supplies its Blood without producing the 1-Bone death reward.
The two sigils that change the result
Many Lives: Blood without death
Many Lives is the key exception to the normal outcome. When a creature bearing this sigil is selected for a Blood sacrifice, it survives rather than being killed. Its sacrifice still contributes the creature’s Blood value, so you can use it to pay for the new card while keeping it on the board.
Because it survives, Many Lives does not trigger the 1 Bone awarded when a card perishes. This makes the sigil valuable when preserving board presence matters more than collecting Bones. It also means the same creature can remain available for later actions instead of being permanently lost from the current board.
Worthy Sacrifice: three Blood instead of one
Worthy Sacrifice changes the creature’s contribution from 1 Blood to 3 Blood. It does not by itself prevent death. Unless another rule such as Many Lives applies, the creature still perishes when selected and therefore grants 1 Bone.

Think of Worthy Sacrifice as a multiplier for payment, not as protection. It lets you meet a larger Blood requirement with fewer creatures, but using it still removes the creature from the board under the normal sacrifice rule.
When a creature has both relevant effects
If a creature qualifies for both the higher Blood value and survival, apply both results: it supplies 3 Blood and remains in play. Since it does not perish, it does not grant the 1-Bone death reward. The survival rule changes the creature’s fate; the Blood-value rule changes what it pays.
Worked examples
| Selected creature or group | Blood supplied | Board result | Bones from these sacrifices |
|---|---|---|---|
| One normal eligible creature | 1 | Creature perishes and leaves the board | 1 |
| Two normal eligible creatures | 2 | Both perish and leave the board | 2 |
| One Worthy Sacrifice creature | 3 | Creature normally perishes | 1 |
| One Many Lives creature | Its normal Blood value | Creature survives | 0 from that sacrifice |
| A Worthy Sacrifice creature that also survives through Many Lives | 3 | Creature survives | 0 from that sacrifice |
These values describe the payment itself. They do not imply that every card can be played after paying Blood; the card must still be legal under the rest of the encounter’s rules.

Not in the normal Act I Blood-payment interaction. A creature selected on the battle board is removed from that board. If it is a normal creature, it dies and gives 1 Bone. The sacrifice is therefore a way to convert a creature into Blood, with a Bone generated when that creature perishes, rather than a way to cycle the card back into your deck.

Many Lives is the only listed survival exception in this interaction. It prevents the death, but it does not send the card to your deck; the creature simply remains on the board after supplying Blood.
What “sacrifice” means at the Bone Altar
The Bone Altar is a different system from paying a card’s Blood cost. There, the selected card is permanently removed from the deck. It is not merely removed from the current battle board and it is not returned to the deck after the altar action.
The reward depends on what you offer:
- Black Goat: sacrificing it grants the Boon of the Bone Lord.
- Another creature: sacrificing it grants a Minor Boon.
- A Pelt: sacrificing it grants nothing.
The Bone Altar’s result should not be used to explain an ordinary Blood sacrifice. At the altar, the key cost is permanent deck removal and the key reward is a boon. The Blood and Bone values described earlier belong to the battle-board payment interaction.

Campfires offer a Power +1 or Health +2 upgrade. The first opportunity is safe, but repeated buffs can introduce a different risk: the survivors may eat and remove the card being upgraded. This is not the normal Blood-payment action and does not mean you are exchanging the card for Blood.

Some creatures can poison the survivors if they are eaten. Ring Worm, Adder, and Long Elk are the named examples associated with this danger. When survivors are poisoned, they cannot attack at later Campfires during that run.
In practical terms, the Campfire decision has two separate questions: whether the upgrade is worth taking, and whether repeated upgrades expose the card to being eaten. The initial safe opportunity and later survivor behavior are part of the Campfire event, not properties of the Blood sacrifice rules.
Which act are you playing?
The Blood-sacrifice explanation is primarily an Act I rule tied to Leshy’s cards. Inscryption changes its card-cost systems in later acts. Act III primarily uses Energy, so an explanation that treats every act as if it used Act I’s Blood-and-Bones economy will be misleading.
When looking up a sacrifice interaction, identify the act and the location first:
- Act I battle board: select eligible creatures to generate Blood; normal creatures die and usually grant 1 Bone.
- Bone Altar: permanently remove the selected card from the deck for the applicable boon, or receive nothing for a Pelt.
- Campfire: choose Power +1 or Health +2, with the possibility that repeated buffs lead survivors to eat the upgraded card.
- Later acts: do not assume the Act I Blood rule applies; their cost systems differ, with Act III primarily using Energy.
A practical decision checklist
Before confirming a sacrifice in Act I, check the following:
- Read the card’s cost. Confirm how much Blood is required.
- Add the sacrifice values. Count 1 for each ordinary eligible creature and 3 for each Worthy Sacrifice creature.
- Identify survivors. Mark any Many Lives creature so you do not expect it to die or produce a Bone.
- Predict the board after payment. Every ordinary selected creature will leave the board; only the listed survival exception remains.
- Predict the Bone gain. Add 1 Bone for each selected creature that actually perishes.
- Check the context. If the word “sacrifice” appears at the Bone Altar or Campfire, use that location’s rules instead of the Blood-payment rules.
This checklist prevents the most common mistakes: treating Blood value as Bone value, assuming a sacrificed creature returns to the deck, or carrying Act I rules into a later act.
