There is no single automatic outcome when a crewed spacecraft has a problem on the way to the International Space Station (ISS). The response depends on when the problem occurs, what systems still work, and which safe options remain. During launch or ascent, the crew may use a launch-abort system to separate from the rocket. During rendezvous, the spacecraft may stop its approach or move to a collision-free orbit while the crew and flight controllers assess the problem. If it reaches the station, the ISS may provide a temporary safe haven while teams decide whether the vehicle can return safely or another ride home is needed.

What determines the response?

NASA describes a contingency as an unusual or emergency situation during launch, ascent, return, or splashdown that requires immediate action to protect the crew and recover them safely. That definition covers several different situations, not one universal emergency procedure. A malfunction during powered ascent is different from a navigation problem near the ISS or a vehicle that becomes unsafe after docking.

Flight controllers and the crew have to assess what happened, what the spacecraft can still do, and what options are safe in its current location. Depending on the situation, those options may include continuing cautiously, stopping or retreating from an approach, entering a safe state, returning to Earth, or remaining at the station until a different transportation plan is available. These are possible responses, not a fixed sequence every crew must follow.

  • Flight phase: launch and ascent, rendezvous and docking, time aboard the ISS, or return to Earth.
  • Remaining capability: whether propulsion, navigation and control, communications, separation, and return systems are functioning well enough for a safe option.
  • Location and conditions: the spacecraft’s orbit, its distance from the station, and the weather and recovery resources available for a return.
  • Other transportation: whether another vehicle, suitable seats, trained crew, and a workable schedule are available.

The details depend on the specific spacecraft and mission. NASA’s technical analysis notes that abort strategies are shaped by a vehicle’s guidance, navigation, and control architecture and performance. The general logic can be explained publicly; it should not be mistaken for a mission’s detailed flight rules.

If the problem happens during launch or ascent

During launch, the rocket is carrying the spacecraft through a period when a major failure can leave little time to respond. A launch-abort system is designed to move the crew capsule away from the launch vehicle when an abort is needed. It is distinct from the actions a spacecraft might take later, while already in orbit and approaching the ISS.

The systems differ by spacecraft and should not be treated as interchangeable. NASA says Crew Dragon’s integrated launch-escape system uses eight SuperDraco thrusters to carry the capsule and crew away from danger during launch or ascent. NASA’s Commercial Crew reference describes Starliner as having four dedicated launch-abort engines. Those descriptions establish each vehicle’s launch-abort hardware; they do not mean the systems have identical designs or operating envelopes.

After separating from the rocket, a capsule would need to proceed through its applicable return and recovery procedures. NASA says more than 50 locations along the flight path are monitored to help ensure downrange conditions are clear in case of an ascent abort. Monitoring and prepositioned rescue resources are part of planning for recovery; the existence of those resources does not mean every anomaly triggers an abort or that every abort ends at the same site.

Vehicle Launch-abort feature described by NASA What the detail means
Crew Dragon Eight SuperDraco thrusters in an integrated escape system They can carry the capsule away from danger during launch or ascent. This is not a general-purpose solution for every problem after reaching orbit.
Starliner Four dedicated launch-abort engines This is Starliner’s launch-abort hardware as described in NASA’s program reference; it is not the same system as Dragon’s.

These are different vehicles’ launch and ascent capabilities, not a guarantee that any particular fault will lead to an abort. Whether an abort is available or appropriate depends on the flight phase, the vehicle’s condition, and the applicable mission procedures.

If the spacecraft cannot safely dock

Rendezvous is the process of bringing a spacecraft into a carefully controlled approach to the ISS. If a fault makes the approach unsafe, the priority can shift from docking to creating or maintaining separation. A spacecraft may stop the approach or maneuver away rather than continue toward the station.

NASA’s technical paper describes a possible response in which a vehicle moves into a nearby collision-free orbit and enters a safe-haven mode. In that state, the spacecraft can be kept in a safe condition while the crew and ground controllers diagnose and troubleshoot the problem. NASA characterizes safe-haven modes as independently and reliably placing a spacecraft in a safe state to allow time for diagnosis, including with ground support if a communications link is open.

A safe-haven orbit is a way to manage time and distance; it is not the same as docking, and it does not by itself repair the spacecraft or ensure that it can return to Earth. What happens next depends on which systems remain usable. Flight controllers may assess whether the vehicle can resume a controlled approach, remain safely in orbit for further work, or return without docking. If none of those choices is safe, other contingencies have to be considered.

  1. Detect and assess: The crew and flight controllers identify the anomaly and determine which functions may be affected.
  2. Protect the approach: If rendezvous safety is in doubt, the spacecraft can stop approaching or move away to reduce collision risk.
  3. Establish a safe state: When possible, the vehicle can hold in a suitable orbit while troubleshooting continues.
  4. Compare viable options: Teams evaluate continued flight, another approach, a return, or other arrangements in light of the spacecraft’s actual capability and conditions.
  5. Choose and carry out a plan: The crew and ground teams act under mission-specific procedures; the outcome is not predetermined by the word “malfunction.”

That sequence describes the decision logic, not a guaranteed checklist or a promise that every vehicle can perform every maneuver. A failure affecting propulsion, for example, raises different questions from one affecting a sensor, communications, or another system. A particular system’s failure does not have one universal consequence across all spacecraft.

How a return or recovery option is chosen

If the spacecraft must return, the return plan has to account for more than the fact that Earth is reachable. The vehicle must retain the capability needed for a controlled return, and the recovery location must be suitable for the spacecraft and circumstances. For a Dragon that cannot dock or malfunctions en route, NASA and SpaceX select a return location based on factors including the spacecraft’s orbit, weather, and proximity to rescue teams.

NASA’s launch and return operations reference also describes monitoring and recovery planning for contingencies. Downrange monitoring and prepositioned rescue resources help teams prepare for an abort or landing away from the expected location. A selected location is therefore conditional on where the spacecraft is, what its systems can do, and whether recovery forces can safely reach it—not simply the closest point on a map.

Returning immediately is not always the safest choice. If a spacecraft can remain in a safe orbit while teams assess a fault, that may preserve time to understand the issue and choose among options. Conversely, if the spacecraft cannot maintain a safe state, a return or another contingency may become more urgent. A safe return also depends on the condition of the systems needed for the full return sequence, not just the ability to leave the ISS’s vicinity.

What the 2024 Starliner test flight showed

The 2024 Starliner Crew Flight Test illustrates why a rendezvous problem does not always mean either docking failure or immediate return. During rendezvous, five thrusters were lost. Troubleshooting recovered four, which enabled the spacecraft to dock with the ISS. NASA then had time to investigate the propulsion anomalies while the spacecraft was docked and to examine its return options.

The station served as a safe haven while that analysis took place. NASA ultimately chose to return Starliner without its crew. The astronauts remained aboard the ISS and later returned to Earth on Dragon in February 2025, according to the Aerospace Safety Advisory Panel’s 2025 annual report. The panel’s account describes an extended stay but does not require a specific duration to understand the decision: reaching the station did not settle whether Starliner should carry its crew home.

This case is evidence of one contingency response, not a template for every spacecraft fault. NASA’s Commercial Crew program press kit also records the uncrewed Starliner return and the crew’s later return on Dragon. The press kit described root-cause work as continuing, so a definitive technical cause should not be inferred from the outcome alone. NASA’s Office of Inspector General reported on June 30, 2026, that Starliner remained uncertified after three flight tests had uncovered unexpected issues; that is a dated status statement, not a prediction about later certification or flight schedules.

If the vehicle becomes unsafe after docking

A problem with a spacecraft already docked at the ISS is a different contingency from an inbound vehicle that cannot complete rendezvous. The station may provide a place for the crew to stay while partners assess the vehicle and arrange another way home, but it cannot make every damaged spacecraft safe to fly.

In December 2022, the Soyuz MS-22 spacecraft suffered a coolant leak and was considered potentially unsafe for crew return. NASA’s Office of Inspector General reported that the partners arranged a replacement Soyuz. MS-22 returned uncrewed in March 2023, and the crew came home in September 2023 after a mission extended from about six months to more than a year. This was a contingency after a docked vehicle was compromised, not an example of an inbound approach failure.

The episode also exposed limits in evacuation planning. NASA OIG said NASA did not keep ready-to-launch emergency evacuation vehicles, citing cost and certification constraints. Its analysis of a proposed extra Dragon seat found that it could accommodate one additional crewmember under the circumstances analyzed—not evacuate the entire ISS crew. A different vehicle can become a ride home in some circumstances, but that depends on available seats, training, manifest arrangements, and the emergency at hand.

What the current U.S. commercial crew status tells readers

NASA OIG’s June 30, 2026 update said Crew Dragon had been certified in 2020 and had flown 12 crewed missions to and from the station. In the same update, OIG said Starliner remained uncertified after three flight tests uncovered unexpected issues. These are program-status facts reported on that date; they should not be read as a guarantee about future certification decisions, mission schedules, or the response to a particular emergency.

The Starliner and Soyuz cases show that a crew may be able to stay at the ISS while teams work through a transportation problem, and that a different spacecraft may eventually bring them home. They do not establish that a spare vehicle is always ready, that every crew can be transferred immediately, or that the station can shelter people indefinitely. The options depend on the spacecraft, the number of people involved, available seats and supplies, training, and the time needed to arrange a safe plan.

The practical answer: a problem does not dictate one outcome

When a crewed spacecraft has a problem on the way to the ISS, teams respond to the flight phase and the vehicle’s remaining capability. A serious launch or ascent emergency may call for a launch abort. A fault during rendezvous may lead the spacecraft to back away or hold in a safe orbit while teams diagnose it. If the vehicle reaches the station, the ISS may provide time to evaluate whether it can return safely or whether the crew needs another ride. A return location and recovery plan must also fit the spacecraft’s position, conditions, and available rescue resources.

The central distinction is between an anomaly and the safe options that remain after it. “Problem” alone does not say whether the crew must abort, can continue, should wait, or needs a different return vehicle. Each contingency is planned around the spacecraft and scenario, and the outcome depends on what is still safe and possible.
