Menopause is a multi-year transition, not a single switch. A large new study nevertheless found that several laboratory measures changed abruptly around the final menstrual period (FMP), leading its authors to describe a step-like physiological “cliff.” That statistical pattern suggests menopause can concentrate changes across multiple body systems, but it does not mean every person experiences a sudden medical crisis or needs broad laboratory testing.

What the new “biological cliff” study found

The Nature Portfolio summary of a Nature Medicine study published in October 2026 says researchers used a computational method to reconstruct physiological changes relative to the FMP. They analyzed more than 300 million laboratory tests from over one million women in United States and Israeli datasets covering 1999–2024.

Study detail What was reported
Data volume More than 300 million laboratory tests
Participants Over one million women
Data period and countries United States and Israel, 1999–2024
United States findings 52 of 90 laboratory tests showed a statistically significant abrupt change
Measures affected Sex hormones, bone health, liver and kidney function, blood chemistry and blood fats
Cross-sex comparison The publisher reported no comparable shifts in male individuals

The Nature Portfolio summary says the United States and Israeli datasets showed concordant abrupt changes. “The authors describe the changes as a step-like physiological ‘cliff,’” the summary states. Here, “cliff” describes a population-level curve reconstructed from existing records. It is not a diagnosis, an individual risk score or proof that everyone’s body changes on the same day.

Hormone changes did not all occur at the final period

The study’s timing estimates place some hormone changes years before the FMP. Anti-Müllerian hormone declined more rapidly around 15 years before the FMP, estradiol around 12 years before it, and follicle-stimulating hormone rose more rapidly around 14 years beforehand. Those are averages from a temporal reconstruction; they cannot predict the exact timing of menopause for one person.

Important limits of the evidence

The analysis used retrospective laboratory data and self-reported menopause information. That design can reveal associations and timing patterns, but it cannot establish why each test changed, whether a change caused a later health outcome, or whether treatment would prevent it. The publisher also says apparent associations involving menopausal hormone therapy require confirmation in future studies.

What menopause is—and how perimenopause fits

The World Health Organization defines natural menopause retrospectively as 12 consecutive months without menstruation when there is no other obvious physiological or pathological cause and no clinical intervention. Natural menopause generally occurs between ages 45 and 55 worldwide. Perimenopause is the transition that begins with signs such as changing cycles and continues until one year after the final menstrual period; it can last several years. Surgery, chemotherapy and other medical treatments can cause an induced menopause.

Which health systems can change around menopause?

Area What health authorities describe How to interpret it
Bones Bone loss increases in the first two years after menopause; reduced density contributes to osteoporosis and fractures. Risk also depends on age, genetics, medications, nutrition, activity and other conditions.
Heart and blood vessels Menopause is associated with changes in cardiovascular risk and blood lipids. An associated risk is not an inevitable outcome and is not a reason to treat a laboratory number in isolation.
Pelvic and urinary health Pelvic support, vaginal tissues and urinary function may change. Symptoms are treatable, but the appropriate option depends on medical history and preferences.
Body composition Fat distribution and muscle mass can change during midlife. The International Menopause Society’s 2025 recommendations say chronological aging drives most midlife weight gain, while declining estrogen is a major contributor to increased abdominal fat.

Menopause and aging overlap but are not identical. The International Menopause Society says menopausal hormone therapy can manage menopause symptoms; it is not indicated for weight management or improving body composition.

Does the study mean you should get hormone or “menopause” blood tests?

No. For otherwise healthy people aged 45 or older with typical symptoms, NICE recommends identifying perimenopause or menopause from symptoms and menstrual history without laboratory tests. The National Institutes of Health likewise says hormone tests are usually unnecessary because levels fluctuate substantially during the transition.

Testing may be considered in specific circumstances, such as suspected menopause at an unusually young age, an atypical presentation or a medical condition that needs investigation. The millions of tests analyzed in the study were used to reconstruct population patterns; they do not turn a routine panel into a menopause diagnostic test or justify blanket screening.

What to do if symptoms affect daily life

  1. Record patterns. Note cycle dates, bleeding changes, hot flushes, sleep, mood, pain and urinary or sexual symptoms. This record can help organize a clinical conversation, but it cannot diagnose menopause or detect the study’s laboratory changes.
  2. Arrange an individualized review. WHO and NIH advise discussing bothersome symptoms, medical history, treatment goals and personal preferences with a health-care professional.
  3. Ask about bone and cardiovascular health. Your clinician can consider age, fractures, family history, medications, blood pressure, lipids and other risk factors rather than relying on the study’s population averages.
  4. Discuss management choices. Depending on the symptom and your history, options can include lifestyle measures, non-hormonal medicines or menopausal hormone therapy. Benefits, risks, contraindications, route and treatment burden differ between individuals; the study does not identify a single best option.
  5. Maintain strength and function. Regular physical activity that preserves muscle mass and strength is a practical part of midlife health planning. A clinician can tailor activity to fitness, injury, bone density and other conditions.

What the “cliff” does—and does not—change

The study makes menopause look more system-wide and temporally concentrated than a simple story of slow, uniform aging. It does not show that every individual has an abrupt clinical event, that routine blood panels will reveal when menopause occurred, or that treating one reported laboratory shift prevents a later disease. Symptoms, examination, medical history and preferences remain the basis for personal care.
