CVSS is a standardized way to describe the severity of a software or hardware vulnerability. It scores the technical conditions that make exploitation possible and the direct confidentiality, integrity and availability consequences of a successful exploit. CVSS v4.0 can also record changing threat conditions and the importance of a vulnerability in a particular environment.
That is not the same as measuring business risk. A CVSS Base score—whether 4.3 or 9.8—is an intrinsic severity assessment, not a probability of exploitation, a patch deadline or a complete remediation priority. Deciding what to fix first requires CVSS plus evidence about exposure, active attacks, asset criticality, controls, cost and your organization’s risk tolerance.
CVSS measures vulnerability severity, not complete risk
The Common Vulnerability Scoring System (CVSS) provides a vendor- and platform-agnostic vocabulary for describing how serious a vulnerability is. Its central question is technical: If this weakness is exploited, how difficult is exploitation and what direct damage can result?
CVSS v4.0 separates that assessment into metric groups. The Base group describes characteristics that are intrinsic to the vulnerability. Threat metrics can reflect conditions that change over time, while Environmental metrics adapt the assessment to a particular deployment. Supplemental metrics can carry additional information without changing the core severity calculation.

Most public databases and assessment providers, including NVD, commonly publish Base scores. The organization using the affected product is responsible for adding local context. Consequently, a public “CVSS 9.8” usually tells you about the vulnerability’s technical severity in the abstract, not whether that vulnerability is the most urgent issue in your own environment.
What the CVSS Base metrics actually measure
Exploitability: the conditions an attacker must satisfy
CVSS Base exploitability metrics describe the technical path to a successful attack. In v4.0, that description includes:
- Attack Vector: how the attacker reaches the vulnerable component, such as across a network, from an adjacent network, locally or physically.
- Attack Complexity: whether exploitation requires unusual, repeatable conditions beyond the attacker’s control.
- Attack Requirements: whether a particular state or prerequisite must exist in the target environment for exploitation to work.
- Privileges Required: the level of access an attacker needs before attempting the exploit.
- User Interaction: whether another person must perform an action, such as opening content or approving a prompt.
These metrics describe the means and prerequisites of exploitation. They do not say that exploitation will occur, how many attackers are trying, or how often a vulnerable asset is exposed. A vulnerability that is technically easy to exploit can still be a low operational priority if no attacker can reach the affected system; a harder exploit can deserve immediate attention when it affects a public, business-critical service.
Impact: the direct consequence on the vulnerable system
CVSS expresses impact through the familiar security objectives of confidentiality, integrity and availability:
- Confidentiality concerns unauthorized access to information.
- Integrity concerns unauthorized modification or destruction of information or system behavior.
- Availability concerns loss or degradation of access to the system or its services.
FIRST defines the impact metrics as the direct consequence of a successful exploit to the things that suffer the impact. The Base assessment therefore asks what the vulnerable system itself can lose, not what every connected business process might eventually cost.

Impact on subsequent systems
CVSS v4.0 also allows an assessment to represent impact on subsequent systems—other systems affected after the initially vulnerable component is compromised. This helps describe attack chains in which the first system is only a stepping stone. It still describes technical consequences, however; it does not turn downstream effects into a financial loss estimate or an enterprise risk score.
How CVSS v4.0 outputs are organized
The letters in a CVSS output identify which metric groups were used. They are useful because two scores with the same number may contain very different amounts of context.

| CVSS-B | Base | Intrinsic exploitability and direct impact, including relevant subsequent-system impact | Whether the issue is being exploited, how exposed your asset is, or what your business considers most important |
| CVSS-BT | Base + Threat | Base severity adjusted with time-varying exploit conditions | Whether a particular asset is reachable, critical to your business, or protected by effective controls |
| CVSS-BE | Base + Environmental | Base characteristics interpreted using local security requirements and modified characteristics | All operational evidence, such as incident activity, remediation effort or risk appetite |
| CVSS-BTE | Base + Threat + Environmental | The most context-rich CVSS combination, joining changing threat conditions with deployment-specific context | A complete enterprise-risk calculation or an automatic patch order |
Every CVSS assessment produces a numerical score and a qualitative severity rating. The number is a compact representation of the selected metrics; it is not a percentage, a count of affected users or a forecast of attack frequency.
What a Base score leaves out
Current exploitation and attacker activity
A Base score is independent of factors associated with threat. It does not tell you whether proof-of-concept code exists, whether criminal groups are exploiting the vulnerability, whether exploitation has been observed in your sector or whether an attack campaign is accelerating. Those facts can change the urgency without changing the vulnerability’s intrinsic characteristics.
Threat metrics are intended to represent time-varying exploit conditions. They are useful when maintained with current intelligence, but a Threat-enhanced score is still a contextual CVSS assessment rather than a complete incident or risk judgment.

Exposure of your particular asset
CVSS does not inventory your systems or determine whether a vulnerable component is reachable. It cannot know that one instance is internet-facing while another is isolated behind filtering, that a service is disabled, or that a vulnerable library is present only in a test image. Exposure management, network data and accurate asset inventories must supply that information.

Business criticality and dependency chains
The impact metrics describe technical consequences. They do not know that one server supports payroll, emergency communications or a regulatory reporting process while another identical server is disposable. Nor do they automatically capture contractual obligations, safety implications, customer concentration or the effect of an outage on a wider dependency chain. Environmental metrics let a consumer express local security requirements and modified characteristics, but the organization still has to identify and value the affected assets.
Compensating and preventive controls
A Base score does not inspect your firewall rules, endpoint protections, application isolation, authentication policy, monitoring coverage, backups or segmentation. Such controls can reduce the likelihood or consequence of exploitation in your environment without changing the vulnerability’s Base metrics. Environmental scoring can reflect modified characteristics and local requirements, while operational validation is needed to determine whether a control is actually deployed and effective.
Remediation cost and feasibility
CVSS does not estimate engineering effort, licensing cost, downtime, testing time, change-window availability or the risk that a rushed fix will break a critical service. Two vulnerabilities with identical scores may require very different work: one may be corrected by a routine update, while another may require a redesigned integration. Those trade-offs belong in the remediation plan, not in the Base score.
Risk appetite and governance
Organizations choose different tolerances for confidentiality loss, service interruption and operational change. A hospital, a small internal lab and a public payment service may make different decisions about the same technical severity. CVSS supplies a common severity language; policy, legal obligations and leadership-approved risk appetite determine how that language becomes a decision.

Why a CVSS 9.8 is not automatically your top priority
A high Base score indicates severe intrinsic characteristics, but prioritization depends on the intersection of severity, threat and context. Consider three illustrative cases:
| Situation | What the Base score captures | Additional evidence that can change priority |
|---|---|---|
| A remotely reachable flaw with broad confidentiality, integrity and availability impact | The attack path is technically favorable and the direct impact is high | Whether the service is actually exposed, whether exploitation is active, the asset’s business role and available mitigations |
| A high-scoring flaw in a disconnected test system | The vulnerability remains severe if its technical prerequisites are met | Isolation, lack of sensitive data, absence of production dependencies and the planned retirement date |
| A lower-scoring flaw on an internet-facing, mission-critical service under active attack | The direct technical consequence may be narrower | Observed exploitation, unavoidable exposure, regulatory obligations and the cost of service compromise |
The examples do not contradict CVSS. They show why severity and risk answer different questions. “How bad could exploitation be under the defined conditions?” is a CVSS question. “What should we do first, for which asset, and by when?” is a risk-management question.
Using CVSS with threat and asset context
A practical workflow keeps the score intact while adding the evidence it cannot provide.
- Start with the CVSS vector and metric group. Record whether the source provides CVSS-B, CVSS-BT, CVSS-BE or CVSS-BTE. Read the individual exploitability and impact metrics instead of treating the number as a standalone verdict.
- Confirm affected assets. Use inventory, dependency data and configuration records to identify which versions are installed, where they run and whether the vulnerable function is enabled.
- Determine exposure. Establish whether each instance is reachable from the internet, an internal network, a privileged segment or no attacker-controlled network at all. Document segmentation and other compensating controls.
- Check current threat evidence. Look for credible indications of exploitation, attack tooling, incident telemetry or activity in the environments you protect. A separate exploit-probability signal such as EPSS can complement CVSS, but it should not replace asset and exposure analysis.
- Apply Environmental metrics when you own the deployment context. Set local security requirements and modified characteristics so that the assessment reflects the importance and configuration of your systems. Use the resulting CVSS-BE or CVSS-BTE label accurately.
- Estimate response options. Compare patching, configuration changes, temporary isolation, access restrictions, monitoring and compensating controls. Include testing, downtime, rollback and residual risk.
- Make and record the decision. State why an asset is being fixed now, accepted temporarily, mitigated or deferred; identify the owner, due date and evidence that will trigger a reassessment.
This workflow prevents a common error: using a public Base score as if it already contained your inventory, threat intelligence and business priorities.
Severity, contextual severity and risk: a precise comparison
| Question | CVSS Base | Threat and Environmental CVSS | Broader risk decision |
|---|---|---|---|
| What technical conditions enable exploitation? | Yes | Yes | Uses the CVSS result |
| What direct CIA impact can follow? | Yes, including relevant subsequent systems in v4.0 | Yes, interpreted in context | Uses the CVSS result plus business analysis |
| Is exploitation happening now? | No | Threat metrics can represent changing exploit conditions | Requires current intelligence and incident evidence |
| Is my asset exposed and important? | No | Environmental metrics can reflect local requirements and modified characteristics | Requires inventory, exposure and criticality data |
| What will remediation cost and when should it occur? | No | No | Requires engineering, operational and governance judgment |
Common misreadings to avoid
“The score is the probability of exploitation.”
It is not. Exploitability metrics describe technical prerequisites and attack conditions; they do not predict the chance that an attacker will choose or successfully attempt the vulnerability in the wild.

“A 9.8 means patch within a fixed number of days.”
CVSS does not encode a patch deadline. Your policy may map severity and context to service-level targets, but that mapping is an organizational rule, not a property of the score.
“The public score already includes my environment.”
Usually it does not. Public providers commonly publish Base assessments. The consumer must add Environmental and, where appropriate, Threat context.
“CVSS replaces a vulnerability-management program.”
It does not replace asset inventory, exposure management, exploit-activity intelligence, incident response or a remediation plan. It is a consistent measurement within that larger process.

Use CVSS to communicate and compare intrinsic vulnerability severity. Read the exploitability metrics to understand the attack path, read the CIA and subsequent-system impact metrics to understand direct consequences, and identify whether the result is CVSS-B, CVSS-BT, CVSS-BE or CVSS-BTE. Then add the facts CVSS cannot know: current exploitation, asset exposure, business criticality, effective controls, remediation cost and risk appetite. The resulting decision is more defensible precisely because it does not confuse a technical severity score with complete organizational risk.
