A CDX2 test usually uses immunohistochemistry (IHC) on tumor tissue to check whether cancer cells express the CDX2 protein. A positive stain can support intestinal differentiation and help pathologists characterize a tumor. Reduced or absent staining can occur in colorectal cancer, but it does not rule out a colorectal origin. CDX2 also does not determine stage, identify Lynch syndrome, or replace direct mismatch-repair (MMR) or microsatellite-instability (MSI) testing. Its meaning depends on the tumor’s appearance, clinical history, other pathology results, and molecular findings.
What a CDX2 test measures
CDX2 is a protein involved in the development and maintenance of intestinal cells. In cancer pathology, a CDX2 test generally means immunohistochemistry: a laboratory applies an antibody to a tissue sample and looks for staining in tumor-cell nuclei. The sample may come from a biopsy or a surgical resection. Studies of colorectal tumors commonly describe testing formalin-fixed, paraffin-embedded tissue.
The test measures whether the sampled tumor cells show CDX2 protein expression. A report may call staining positive or present, reduced, or lost or negative. The result is not a count of cancer cells in the body and does not measure how far cancer has spread. It also does not directly assess a gene mutation: IHC detects protein expression in the tissue under the laboratory’s assay and interpretation criteria.
There is no single scoring threshold established by the cited sources that applies to every laboratory. Antibody methods, scoring systems, and the amount or pattern of staining considered significant can vary. If a report gives a percentage, intensity, or specific score, the pathologist’s interpretation and the laboratory’s criteria are the context for that number.

What a positive CDX2 result can show
A positive stain means the tested tumor cells express CDX2 according to the laboratory’s criteria. In an appropriate clinical and microscopic setting, that finding supports intestinal differentiation. It can help a pathologist characterize a tumor as having an intestinal phenotype, including when considering whether a tumor is colorectal in origin.
CDX2 is one part of the diagnostic picture, not a complete diagnosis by itself. The pathologist may interpret it alongside the tumor’s morphology, where it was found, the patient’s cancer history, and other stains. Depending on the question, a broader panel can include SATB2 and cytokeratins such as CK7 and CK20. No single marker establishes the origin of every tumor in every circumstance.
A positive result does not establish the cancer’s stage or prove that it has spread. It does not identify a hereditary syndrome, determine whether a cancer is MSI-high, or select treatment. Those questions depend on other clinical, pathology, and molecular information.
What it means if CDX2 is negative or reduced
A negative or lost result means the sampled tumor did not show the expected CDX2 expression under the laboratory’s interpretation criteria. Reduced staining describes less expression than expected, but the precise meaning depends on the report and the assay. Neither result, by itself, rules out colorectal cancer.

Most colorectal cancers express CDX2, but expression is not universal and varies across tumor types. Some histologic subtypes show lower expression. A tumor may also have lost or reduced CDX2 while still being colorectal cancer; the pathologist weighs that result against the tumor’s appearance, location, history, and other markers.
In some cohorts, CDX2 loss has been associated with right-sided tumors, deficient MMR (dMMR), and BRAF mutation. These are associations observed across groups, not rules for diagnosing an individual tumor. A negative CDX2 stain does not show that a tumor is right-sided, dMMR, or BRAF-mutated; those features require their own assessment.
How often colorectal cancers express CDX2
A 2022 meta-analysis by Hong Bae Choi and colleagues combined findings from 38 articles. It estimated CDX2 expression in 88.2% of primary colorectal cancers and 89.3% of metastatic colorectal cancers in the included studies. These pooled estimates describe the studies’ populations; they are not a prediction of what any one patient’s test will show.
The same meta-analysis reported expression in 88.6% of adenocarcinomas and 43.6% of medullary carcinomas. The difference illustrates why tumor histology matters when interpreting a result: a negative stain is more expected in some subtypes than others, and an overall colorectal-cancer rate does not apply equally to every subtype.

A separate cohort study of 449 patients reported CDX2-negative expression in 18 of 441 evaluable cases, or 4.1%. The meta-analysis and cohort figures should not be treated as competing universal rates. Study populations, tumor histology, antibody methods, and scoring approaches can differ; the 2022 cohort reported a substantially lower proportion of negative tumors than some earlier cohorts. The appropriate takeaway is that most colorectal tumors express CDX2, while a minority do not, and the observed rate depends on the study and methods.
What CDX2 can and cannot say about prognosis
Several studies have associated loss of CDX2 expression with less favorable outcomes, and the 2022 meta-analysis found better overall and cancer-specific survival among patients whose tumors expressed CDX2. This is population-level evidence. It does not mean that a positive result guarantees a favorable outcome or that a negative result predicts what will happen to a particular person.
In the 449-patient cohort, among patients with stage I–III disease, CDX2 loss was associated with worse five-year overall and disease-free survival in univariate analyses. In multivariable analysis, loss remained associated with worse overall survival: hazard ratio 2.4, with a 95% confidence interval of 1.12–5.11. These are estimates from that cohort, not an individual survival calculation. A hazard ratio describes a relative association in the study’s analysis; it is not a percentage chance of survival for a patient.

A separate study of stage II–III disease reported worse cancer-specific survival for pMMR/CDX2-negative cases than for the comparison groups, while the interpretation differed for dMMR/CDX2-negative tumors. The result cautions against treating CDX2 loss as one uniform prognostic category across patients.

For an individual prognosis, the oncology team considers the full clinical picture, including stage and other relevant tumor features. CDX2 may be one finding in that picture, but the available evidence does not establish it as a stand-alone prediction tool or treatment-selection test.
CDX2 is different from MMR and MSI testing
CDX2 IHC, MMR IHC, and MSI testing answer different questions. CDX2 IHC looks for a differentiation-associated protein. MMR IHC looks for mismatch-repair proteins. MSI testing assesses microsatellite instability, a molecular feature that can result when mismatch repair is deficient. Although CDX2 loss has been associated with dMMR in some studies, the association is not exclusive and cannot establish MMR or MSI status.

The College of American Pathologists’ colorectal protocol text describes universal testing for MSI and/or MMR enzyme status by IHC as recommended by EGAPP guidelines. It also says that MSI testing in stage II cases can help inform decisions about adjuvant chemotherapy. That guidance concerns MMR/MSI assessment, not a universal mandate to order CDX2. The protocol states: “Since MSI-H cancers have a favorable prognosis, MSI testing for stage II cases can help in making decisions regarding adjuvant chemotherapy.” The statement is about MSI testing and treatment decisions, not what a CDX2 stain means.
If a report mentions CDX2 loss and you need to know whether the tumor is dMMR or MSI-high, look for the separate MMR or MSI result or ask the care team whether that testing was performed. CDX2 cannot answer that question by proxy.

How to read the result in a pathology report
Read the CDX2 finding as one component of the pathology interpretation rather than as a stand-alone verdict. The wording can vary between laboratories, so focus on the result as reported and how the pathologist connects it to the diagnosis.
- Locate the result and specimen. Check which biopsy or resection is described and whether the result refers to the primary tumor or a sampled metastatic site. The stain speaks to the tissue tested.
- Note the reported expression. Identify whether the report calls CDX2 positive, reduced, or lost/negative. If it provides an extent or intensity, interpret it according to the laboratory’s report rather than applying a universal cutoff.
- Read the diagnosis and other stains together. The final pathology interpretation integrates morphology, CDX2, other markers such as SATB2 or CK7/CK20 when used, and clinical information.
- Check for separate MMR/MSI and molecular results. These tests address questions CDX2 does not. Do not infer an MSI, MMR, or BRAF result from CDX2 alone.
- Ask what the result changes in your case. A pathologist or oncologist can explain whether CDX2 helped characterize the tumor, how it fits with the complete report, and whether any separate testing is relevant.
Useful questions include: “Does this stain support the tumor’s suspected origin, or were other findings more important?” “Were MMR IHC or MSI testing done separately?” and “Does this CDX2 result change the interpretation or management in light of my stage and other results?” The answers depend on the full case, not the stain in isolation.
What the result does not mean
- Positive does not mean the cancer is early or localized. CDX2 is not a staging test.
- Negative does not rule out colorectal cancer. Some colorectal cancers, including certain histologic subtypes, show reduced or absent expression.
- Neither result diagnoses Lynch syndrome. Lynch syndrome evaluation involves appropriate clinical assessment and hereditary or tumor testing; CDX2 is not that test.
- Neither result substitutes for MMR/MSI testing. Those assays directly assess mismatch-repair proteins or microsatellite instability.
- A prognostic association is not a personal forecast. Cohort results do not determine an individual patient’s outcome.
- CDX2 alone does not choose treatment. Treatment decisions require the complete clinical and tumor assessment.
Why results and interpretations can vary
Published rates are not identical because studies do not necessarily examine the same patients, tumor subtypes, tissue samples, antibodies, or scoring rules. A meta-analysis summarizes the studies it includes; it does not erase those differences. Cohort outcome findings are also influenced by the patients studied, disease stage, and the other tumor characteristics considered in the analysis.
