Atypical memory B cells are linked to malaria exposure, but finding them does not show that someone is either protected or failing to mount an immune response. Studies have found both dampened B-cell signaling in some atypical cells and the ability to produce antibodies that act against malaria parasites. Their overall contribution to lasting protection from illness remains uncertain.
What “atypical B cells” means
In malaria research, the term usually refers to atypical memory B cells (atMBCs, also written aMBCs): antigen-experienced B cells identified by a pattern of surface and other markers. Many studies classify them as having reduced or absent CD21 and CD27. Some reported subsets also express T-bet, a protein encoded by TBX21, or markers such as CD11c, CXCR3, and FcRL5.
Researchers often compare them with classical memory B cells, which commonly express CD21 and CD27. Both groups have encountered antigen, but the label “atypical” describes a phenotype, not a definitive functional diagnosis. It does not by itself mean that a cell is protective, exhausted, or inactive.

There is no single marker panel used in every study. Differences in the markers researchers measure, the populations studied, the malaria parasite or antigen involved, and the laboratory tests used make results harder to compare. A 2020 review highlighted this heterogeneity as a major reason the cells’ role has been difficult to interpret.

This matters when reading a study or comparing headlines. One paper may define a population using CD21 and CD27, while another may also focus on T-bet or other markers. A result about cells selected with one panel cannot automatically be treated as a result about every cell described as “atypical” elsewhere. The term groups together cells with some shared features; it does not establish that they all have the same developmental history or behavior.
In practical terms, the marker pattern answers “which cells did this study classify as atypical?” It does not answer “are these cells helping or harming immunity?” That second question requires functional tests, and even a laboratory function test does not by itself show whether a person is protected from illness.
Why atypical memory B cells are associated with malaria
Repeated exposure to Plasmodium parasites and infection-related immune activation are associated with expansion of atypical memory B-cell populations. That association does not, on its own, establish why an individual cell develops or what it will do.
In a study of malaria-exposed children, researchers observed T-bet-high B cells with markers associated with atypical memory cells. The cells showed reduced phosphorylation of B-cell receptor (BCR) signaling molecules after the researchers cross-linked the BCR—a laboratory method for triggering receptor signaling. The experiments implicated malaria-induced interferon-gamma (IFN-γ), together with BCR stimulation, in inducing T-bet.
These findings connect an infection-associated immune environment with a particular cell feature and a measured signaling response. They support one proposed mechanism, not a complete account of how every atypical memory B cell forms. In particular, the observations do not establish that IFN-γ and BCR stimulation are the only influences or that every cell expressing T-bet follows the same route.
This offers evidence for one route by which malaria-associated immune conditions may shape B-cell features; it is not a complete account of how these cells develop. Researchers have considered whether atypical cells arise from activated naïve B cells, memory cells, or more than one route. The available findings do not establish a single developmental pathway.

That uncertainty is important because a cell’s origin could help explain why two atypical populations behave differently. The available findings do not settle whether atypical cells represent one lineage, several routes into a similar phenotype, or cells that change state over time. It is therefore safer to describe the observed markers and tests than to assume one universal pathway.
Why some studies describe impaired responses
Some atypical B-cell populations express inhibitory receptors and show reduced BCR signaling. Under particular experimental conditions, some have also shown less proliferation or antibody secretion in vitro. These results support the idea that certain subsets or states may respond less strongly to stimulation.
A reduced response in a laboratory test is meaningful evidence about the cells under those test conditions, but it is not identical to an assessment of the person’s whole immune response. The assay may measure signaling after receptor cross-linking, cell proliferation, or antibody secretion; each is a different readout. A finding in one readout should not be silently substituted for a conclusion about all functions.
“Exhausted” is sometimes used to interpret such features, but applying it to all atypical memory B cells overstates the evidence. A marker pattern or a reduced response in one laboratory assay does not prove that every cell in the group is dysfunctional, nor does it establish how the cells affect a person’s clinical protection.
The distinction also helps reconcile apparently conflicting reports. A subset with lower BCR signaling can coexist with a subset capable of making parasite-directed antibodies. The studies may be measuring different cells, activation states, antigens, parasite species, or functions. Those results need not cancel each other out, and neither alone establishes the net effect on protection over time.

Evidence that atypical cells can make useful antibodies
Findings in P. falciparum
A 2013 human study reported that both classical and atypical memory B cells formed after natural Plasmodium falciparum infection produced broadly neutralizing antibodies against blood-stage parasites. Both cell groups contributed to anti-P. falciparum serum IgG, and atypical cells showed signs of active antibody secretion.

This demonstrates that atypical cells can have parasite-directed antibody activity. It does not prove that atypical cells alone provide durable protection, or that a laboratory neutralization result translates directly into reduced risk of malaria illness.
The comparison with classical memory B cells is useful: the reported antibody activity was not exclusive to atypical cells. Both groups contributed to the measured anti-parasite IgG. The finding therefore supports the capacity of atypical cells to participate in an antibody response, rather than the claim that they are the only, or necessarily the dominant, source of protective antibodies.
Findings in P. vivax
A 2025 human study of natural Plasmodium vivax infection identified antigen-specific atypical memory B cells and reported antibodies that inhibited erythrocyte binding across the tested variants of PvDBPII, a parasite antigen involved in red blood cell binding.
In this case, the measured function was inhibition of the tested antigen’s erythrocyte-binding activity. That is a specific laboratory result, not a general demonstration of immunity to every stage or species of malaria. It also does not show how long the response lasts or how it changes with repeated exposure.
This is functional evidence in a particular species and antigen context. The study also leaves questions about how durable these responses are and how repeated exposure affects them. The findings should not be generalized to every malaria species, antigen, or atypical B-cell population.
How to interpret what the evidence shows
The most defensible interpretation is that atypical memory B cells are associated with malaria exposure and can be biologically active, but are not a validated measure of protection. Reduced signaling and inhibitory features in some experiments can coexist with parasite-directed antibody activity in others. The findings concern different cell subsets, activation states, antigens, parasite species, and assays.

It is especially important to distinguish a laboratory result from a clinical outcome. Antibody neutralization or inhibition measured in vitro is not the same as demonstrating that a person is less likely to develop malaria or will remain protected for longer. Existing evidence does not establish that a higher proportion of atypical B cells reliably predicts an individual’s malaria risk or lasting protection.

For example, a report of antibody inhibition against tested PvDBPII variants in P. vivax is not interchangeable with a report of neutralization against blood-stage P. falciparum. Both may be useful laboratory findings, but they address different biological contexts.
Likewise, “more atypical cells” is not a stand-alone interpretation. A frequency measurement says how common a marker-defined population was in the sample. Without evidence connecting that measurement to later disease or durable protection, it cannot be used as an individual immunity score. The current research supports describing an association with exposure and investigating cell function; it does not support diagnosing protection from a blood measurement.

The scale of malaria makes the distinction between exposure, laboratory activity, and clinical protection consequential, but burden statistics should not be confused with evidence about these cells. For historical context, a 2020 review cited World Health Organization estimates of 228 million malaria cases and 405,000 malaria deaths in 2018. These are historical estimates reported by WHO in 2019, not current burden figures, and they do not show that atypical B cells predict individual outcomes.
What atypical B cells do—and do not—tell you
- They can indicate a history of malaria-associated immune exposure at the population level. Their expansion has been associated with repeated Plasmodium exposure.
- They do not establish that a person is immune to malaria. Cell frequency has not been established as an individual protection test.
- They are not automatically evidence of immune failure. Some subsets show reduced signaling, while studies have also found parasite-directed antibody function.
- Their lasting clinical contribution remains an open question. Current functional findings do not settle whether, or how much, these cells protect people from malaria disease over time.
Bottom line
Atypical memory B cells reflect a varied set of immune-cell features associated with malaria exposure—not a simple score of protection or dysfunction. Some show dampened signaling, while others can produce antibodies that inhibit or neutralize parasites in laboratory studies. Their net effect on durable clinical immunity is still unresolved, so their presence alone cannot tell whether an individual is protected.

Are atypical B cells protective or dysfunctional in malaria?
The evidence supports neither a single label for all atypical memory B cells nor a settled answer about their net effect. Some subsets show inhibitory features or reduced signaling in particular assays, while human studies have found parasite-neutralizing or parasite-inhibiting antibodies from atypical cells. Their contribution to durable clinical protection remains unresolved.

Do atypical B cells mean someone is immune to malaria?
No. Atypical memory B-cell frequency is not an established individual protection test. A cell or antibody result in a laboratory assay is not the same as proving that someone is less likely to develop malaria or will stay protected over time.
How do researchers identify atypical memory B cells?
